STATEMENT OF WORK
Statement of Work Dry Ice Delivery
Non-Personal Services for 20th Medical Group Shaw AFB, South Carolina
1. DESCRIPTION OF SERVICES: The contractor shall furnish parts, material, labor, equipment, transportation and supervision to deliver 200 lbs of dry ice every Monday morning (between 0730 AM-1000 AM EST) to the 20th Medical Group Laboratory located at 420 Polifka Dr. (Bldg# 1042) Shaw AFB, SC 29152
2. QUALIFICATIONS: Experience in making dry ice and delivering to Medical Facilities
3. WORK HOURS/DAYS: Delivery must be made on Monday of each week, between the hours (0730-1000 hours). If a Holiday or Down Day falls on a Monday, delivery will be made on a Tuesday. Delivery will be made on the first actual work day of each week, not including Holidays or Down Days.

4. GUIDELINES: Delivery person must deliver 200 lbs of dry ice pellets each week to the Medical Group and continue delivering it to the Laboratory for medical tests that must be shipped out weekly. Dry Ice is delivered in packaging (special dry ice packaging) and will be delivered to the Lab into their dry ice container/equipment. If lab is closed for any reason, the dry ice will be delivered to the Medical Logistics warehouse to store in the freezer until delivery to lab is made by Medical Logistics.
6. SECURITY REQUIREMENTS FOR UNCLASSIFIED CONTRACT:
a. Listing of employees. The contractor shall maintain a current listing of employees to include employee’s name, social security number, and date of birth. The list shall be provided to the Program Manager and sponsoring agencies Security Manager. An updated listing shall be provided when an employee’s status or information changes.
b. Pass and Identification Items. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees and non-government owned vehicles.
c. Retrieving Identification Media. The contractor shall retrieve all identification media, including vehicle passes from employees who depart for any reason before the contract expires: e.g. terminated for cause, retirement, etc.
d. Traffic Laws. The contractor and its employees shall comply with base traffic regulations.
e. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicle or privately-owned vehicle while on Shaw Air Force Base, SC.
f. Reporting Requirements. Contractor personnel shall report to an appropriate authority and information or circumstances of which they are aware may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified or unclassified defense information.
Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment.
g. Physical Security. The contractor shall be responsible for safeguarding all government property and controlled forms provided for contractor use.
h. Controlled/Restricted Area (IF APPLICABLE). The contractor shall implement local base procedures for entry to Air Force, controlled/restricted areas where contractor personnel will deliver. An AF Form 2586, Unescorted Entry Authorization Certificate, must be completed and signed by the sponsoring agencies Security Manager before a Restricted Area Badge will be issued. Contractor employees must have a completed National Agency Check (NAC) investigation before receiving a Restricted Area Badge.
7. SPECIAL REQUIREMENTS. None
8. MATERIALS AND EQUIPMENT. The contractor will provide all materials and equipment required.
9. SPECIFIC TASKS. None
