1.0. DESCRIPTION OF SERVICES. The Contractor shall provide all personnel, labor, tools, parts, supervision and transportation to perform routine inspections, preventative maintenance, and repairs of cardio and strength equipment as defined in this Statement of Work (SOW) Contractor will be experienced in performing electrical, electronic and mechanical maintenance on many different brands of cardio and strength fitness equipment listed in Appendix 1 and 2.
The Government may increase or decrease equipment to be maintained. New equipment shall not be the reason for price adjustment, any requests for equitable adjustment in contract terms shall be coordinated through the Contracting Officer/CO.
1.1. LOCATION: Service will be performed at the Main Fitness Center, 428 Shaw Drive, Building 806 and the Annex Fitness Center, 5 Lasano Drive, Building 1955 (both) on Shaw AFB, SC 29152.
1.2. PERSONNEL/QUALIFICATIONS: All technicians must possess experience on the repair and maintenance of commercial brand name fitness equipment Life Fitness, Body Masters, Hammer Strength, Cyber, Concept 2, Sicfit, Matrix and Nautilus fitness exercise equipment. The contractor must be a manufacturer’s designated service agency authorized to perform preventive maintenance, repairs, factory modifications and warranty work for the attached listing of gym equipment. (Appendix 1 Main Fitness Center Inventory)
1.3. HOURS OF OPERATION: All work will be performed between the hours of 0600-1300 Monday through Friday except, holidays observed at the Shaw Fitness Center. Work schedule will be approved by the Fitness Center Director (FCD)/Fitness Center Section Chief or Fitness Center Manger to avoid interference with customer fitness programs. The Contractor shall complete work at the Annex Fitness Center on Tuesday and Thursdays between the hours of 0600-1100. The Contractor shall complete work at the Main Fitness Center on Monday, Wednesday, and Fridays between the hours of 0600-1300.
1.4. PREVENTIVE MAINTENANCE INSPECTIONS:
1.4.1. Routine inspection and preventive maintenance schedules shall be provided by the contractor and will be reviewed and approved by the FCD or designated representative. The Contractor’s preventive maintenance schedule shall meet or exceed the manufacturer’s guidelines for the specific pieces of equipment. Schedule may change to accommodate fitness program changes and to minimize equipment downtime during peak fitness hours. The contractor will be notified within 24 hours of these changes to the schedule.
1.4.2. Contractor shall maintain computerized maintenance log for all fitness equipment where all preventive maintenance, warranty services, and repairs will be documented. The Government will provide the Appendices 2 and 3 in an electronic format. A list of cardio and strength equipment is provided at Appendix 2 and 3, Fitness Center Equipment Inventory. The list will be updated as required and provided to the contractor via email.
1.4.3. Work performed shall be in accordance with (IAW) manufacturer’s commercial practice and shall include, but not limited to, cleaning, oiling, waxing, adjusting, testing electronic boards, and replacing parts where needed and authorized. All equipment will be maintained in good serviceable condition. All torn seat/machine vinyl cover will be recovered/replaced.
1.4.4. Contractor shall request approval from FCD or representative prior to ordering replacement parts or requesting manufacturer’s warranty service.
Warranty replacement parts will be ordered the same day approval is received to avoid additional equipment down time.
1.4.5. Contractor shall be responsible for validating all warranty or manufacturer’s invoices to ensure times of service, replacement parts, and equipment service is documented correctly. Contractor shall ensure the manufacturer’s representative performs all necessary warranty maintenance on all equipment made by the manufacturer during each service call.
1.4.6. Contractor shall be responsible for maintaining an up-to-date library of manufacturer’s manuals for all fitness center equipment. Manuals shall be ordered for update and/or replaced by the Contractor. Perform a variety of repair and maintenance tasks quarterly on all assigned fitness/exercise equipment, including but not limited to cardiovascular, weightlifting, and other related equipment. Contractor shall provide all logs to FCD or representative.
1.5. CONTRACTOR PERSONNEL
1.5.1. IDENTIFICATION. Contractor personnel shall be easily recognizable while on the installation in conjunction with this SOW. This shall be accomplished through the wear of distinctive clothing, overcoats, or hats, bearing the company name or logo. The coloring or design of the items selected should identify personnel easily and quickly for reasons of safety and personal protection. During meetings with Government personnel, Contractor personnel shall introduce themselves as “Contractor personnel” and display distinguishing badges or other visible identification. In addition, they shall appropriately identify themselves as “Contractor personnel” in telephone conversations and in formal and informal written correspondence.
1.5.2. STANDARDS. The Government is authorized to restrict employment under this contract of any Contractor personnel or prospective Contractor personnel, who is identified as a potential threat to the health, safety, security, general wellbeing, or the operational mission of the installation and its population.
A determination may also remove Contractor personnel for, but is not limited to, incidents involving most immediately identifiable types of misconduct or delinquency such as: disorderly conduct, use of abusive or offensive language, intimidation by words or actions, fighting, participation in disruptive activities which interferes with the normal operations or brings discredit to the Government, theft, vandalism, sexual harassment, or any criminal actions. This also includes but is not limited to the selling, consuming, or being under the influence of intoxicants, drugs, or substances which produce similar effects while in or on Federally-controlled property. Contractor personnel shall only smoke and/or use tobacco products in designated and approved smoking/tobacco use areas. If the CO directs removal of Contractor personnel for violating the above provisions, the Contractor shall immediately remove the individual(s) at no cost to the Government. Removal of employees for misconduct does not alleviate the Contractor from providing services. Per Shaw AFB Plan 31-101, the installation commander has authority to bar any individual from the installation who commits a crime or offense prejudicial to good order and discipline. All disputes to the CO’s decision must be filed within ten (10) days of the decision to the Base Operations Support Flight Chief. The removed Contractor personnel may not resume work unless/until a decision is made by the Government CO overturning the removal decision.
1.5.3. EMPLOYING GOVERNMENT PERSONNEL. The Contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest. Further, the Contractor shall not employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval in accordance with DODD 5500.7R, Joint Ethics Regulation. In addition, the Contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies contained in AFI 64-106, Industrial Labor Relations Activities. All questions of “conflict of interest” shall be referred to the CO. The Contractor is cautioned that off duty active military personnel hired under this contract are always subject to Permanent Change of Station (PCS), change in duty hours or deployment. Military Reservists and National Guard members may be subject to recall to active duty. The abrupt absence of these personnel could adversely affect the Contractor's ability to perform. Their absence at any time shall not constitute an excuse for nonperformance under this contract.
1.5.4. COMPLIANCE WITH LABOR LAWS. The Contractor shall comply with all applicable laws pertaining to wages, worker’s compensation, equal opportunity, Service Contract Act, and so forth, as implemented by Air Force directives and required by law.
1.6. BASE ACCESS REQUIREMENTS. The Contractor shall comply with all requirements and procedures IAW AFFARS 5352.242-9000, Contractor Access to Air Force Installations, and shall ensure the pass and identification items required for contract performance are obtained for all personnel. All documentation shall be submitted for base passes or base identification per Base Security Requirements. Contractor personnel shall maintain base passes on their person at all times in performance of this contract. Contractor personnel who have personal DoD retiree or dependent ID cards shall not use those personal ID cards for base access when performing contract work. All Contractor personnel who operate a vehicle in performance of this contract shall possess a valid driver’s license and current registration and insurance. Failure to turn in a base credential when requested can result in a non-recommend from Security Forces for future employment. The Contractor is responsible for employee inability to obtain base access required for contract performance.
1.7. CONTRACT MANPOWER REPORTING. The Contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract via secure data collection site. The contractor is required to completely fill in all required data fields at the Air Force Contractor Manpower Reporting Application (CMRA) link https://afcmra.hqda.pentagon.mil/Login.aspx. Reporting inputs will be for the labor executed during the period of performance for each Government Fiscal Year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the CMRA help desk.
1.7.1. Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.
1.7.2. User Manuals: Data for Air Force service requirements must be input at the Air Force CMRA link. However, user manuals for Government personnel and contractors are available at the Army CMRA link at http://www.ecmra.mil.
1.8. UNSCHEDULED SERVICE CALLS. Unscheduled service calls are defined as additional visits and services requested at any time other than bi-monthly visits. Unscheduled calls (routine and emergency) shall include determining the cause of trouble and making repairs or adjustments which require parts, supplies and labor hours. The contractor must notify the customer of all projected downtime and estimated time for repair. The contractor shall prepare and submit a written report within two business days after repairs are completed. The report shall include the date and time of the service call, the location of equipment, the repairs performed, and the name of the technician performing the repairs. When a service call is placed against this contract by the Contracting Officer or FD/POC the contractor shall perform in accordance with the following paragraphs.
1.8.1. Routine Calls. The contractor shall commence repair work within twenty-four (24) hours after receipt of a service call. All work will be accomplished during the regular business hours 0600-1300, Monday - Friday. All repair work shall be in accordance with normal commercial practices using parts specified by the fitness equipment manufacturer or items of equal or better quality. Equipment shall be repaired and functioning normally within 72 hours after receipt of service calls.
1.8.2. Emergency Calls. The contractor shall commence repair work within eight (8) hours after receipt of a service call. All work will be accomplished during the regular business hours 0600-1300, Monday - Friday. All repair work shall be in accordance with normal commercial practices using parts specified by the fitness equipment manufacturer or items of equal or better quality. Equipment shall be repaired and functioning normally within 48 hours after receipt of service calls.
1.8.3. Callbacks. Callbacks shall be performed at no additional cost to the government.
1.9. PARTS AND MATERIALS FOR SERVICE CALLS. Parts required for repairing equipment shall be coordinated with the government representative for approval prior to ordering. The contractor shall acquire parts and materials required for the performance of this contract. The contractor shall purchase parts and materials at the lowest price obtainable and secure reasonable competition for such purchases when available.

1.9.1. All parts and materials shall be genuine replacement parts, as manufactured or recommended by the manufacturer. All parts shall be U.L.
approved/Listed. Only new standard parts shall be used in completed repairs. All replaced parts obtained by the contractor shall be invoiced at contractor’s cost.
Additional cost/mark-up on materials of work performed by sub-contractor will not be accepted. The contractor shall provide a copy of the original supplier’s invoice, tickets, sales slips or other documents. All documents shall be available for POCcontracting officer review and audit.
1.9.2. The contractor shall supply at their own expense, (no cost to the government) all materials, test equipment, supplies of the craft, trade, profession, or regular line of business of work as it relates to the Statement of Work (SOW) i.e. lubricants, rags, cleaning items, solvents, tools, test equipment, and software upgrade, etc).
1.9.3. The contractor shall provide to the FCD or representative a service report within two business days itemizing all parts and materials used, upon completion of each service call. Both the contractor and the designated representative must sign the report before the service call can be officially closed. In addition to the required signatures, the report shall include the date and time of service call, location of equipment, repairs performed, date and time repairs completed, description parts/materials replaced and actual cost, contract number, and the name of the technician performing the repairs.
2.0. LABOR FOR SERVICES CALLS ONLY.
2.1. The government will reimburse the contractor for all labor hours used in repairing the equipment maintained under this contract during routine and emergency calls. All labor shall be billed under the hourly rates as specified in the contract.
2.2. Labor charges will not apply for time spent by the contractor personnel before arrival at the repair site, awaiting arrival of additional contractor personnel and/or delivery of parts after service has commenced.
2.3. All service calls, once started, shall be worked until equipment is back in service or, as a minimum, renders the equipment safe until parts are received and work can commence. After repairs have been completed the contractor shall prepare and leave a copy of a service report, which shall be signed by a government representative.
3.0. MISCELLANEOUS REQUIREMENTS.
3.1. RECORDS: The Contractor shall be responsible for creating, maintaining, and disposing of only those Government required records that are specifically cited in this SOW. When requested by the Government, the Contractor shall provide the original record or a reproducible copy of any such record within five (5) working days of receipt of the request.
3.2. INFORMATION MANAGEMENT: All records, files, documents, regardless of media (e.g., paper, electronic, etc.) as described in the SOW that are the responsibility of or generated by the Contractor, are the property of the Government, and shall remain so upon termination or completion of the contract. The Contractor shall keep these items current, maintain and dispose of them in accordance with the requirements established in Air Force Instruction (AFI) 33-322, Records Management Program, paragraph 10, Air Force Manual (AFMAN) 33-363, paragraph 6.4, AFI 33-364, paragraph 2.10, and the Air Force Records Disposition Schedule available on the Air Force Portal link. Records shall be turned over to the Government upon completion of the contract unless otherwise stated. All records are subject to the Freedom of Information and Privacy Act. The Shaw AFB Records Management Office (20 CS/SCOKR), 895-1630, will provide assistance and training to the Contractor to ensure compliance with Federal record keeping requirements.
3.3. QUALITY ASSURANCE. The Government shall monitor and evaluate the Contractor’s performance to ensure services are received IAW requirements set forth in this SOW. The POC will periodically inspect by watching actual task performance, physically checking an attribute of the completed task, checking a management information report, investigating customer complaints, or otherwise inspecting the task or its result to determine whether or not the performance meets the standards contained in this SOW. The POC may also inspect tasks not specifically mentioned in the Services Summary (Para 2.0 above) but as required in paragraph 1.0 of this SOW. The POC will document the results of the surveillance and this documentation will become the official Air Force (AF) record of the Contractor’s performance. The POC will conduct, document, track, and analyze Contractor performance via monthly scheduled surveillances.
When a performance threshold has not been met or Contractor performance has not been accomplished, the POC will initiate and provide the CO a Corrective Action Report (CAR) for issuance to the Contractor. The Contractor shall respond to the CAR IAW instructions provided and return it to the CO within ten (10) calendar days of receipt. Surveillance results and CARs will be used to develop metrics to track Contractor’s overall compliance with contract terms and conditions. The surveillance results shall be maintained as past performance information.
The Government will neither supervise contractor employees nor control the method by which the contractor performs the required tasks. It shall be the responsibility of the contractor to manage its employees and to guard against any actions that are of the nature of personal services, or give the perception of personal services. If the contractor believes that any actions constitute, or are perceived to constitute personal services, it shall be the contractor's responsibility to notify the Contracting Officer immediately.
3.4. PERIODIC PROGRESS MEETINGS. The CO, POC, and other Government personnel, as appropriate, will meet periodically with the Contractor to review the Contractor's performance. At these meetings the CO will apprise the Contractor of how the Government views the Contractor's performance and the Contractor will apprise the Government of problems, if any, being experienced. Other topics may include (as applicable): opportunities to improve the contract, any modifications required of the contract, unsatisfactory performance inspections results (CAR), and the results of corrective and preventive action measures. The Contractor will also notify the CO in writing, of work being requested or performed, if any, that the Contractor considers over and above the requirements of the contract. Appropriate action shall be taken to resolve outstanding issues.
The minutes of these meetings will be documented in writing, signed by the CO and any other signatures as deemed appropriate, and distributed to the functional area and the Contractor. Should the Contractor not concur with the minutes, the Contractor will set out in writing to the CO any areas of non-concurrence. These meetings shall be held as needed, but not less than semi-annually. All documents shall be sent to the CO, Contractor, and the POC.
4.0. GOVERNMENT FURNISHED PROPERTY AND SERVICES. The Government will furnish or otherwise make available the supplies and/or facilities identified below. The Contractor shall use and abide by all applicable current Air Force guidelines. Any changes in scope of utilization, mission and personnel requirements by the Contractor are properly requested and approved by the Government. These supplies and/or facilities shall only be used for performance of this contract.
4.1. Equipment: The government will furnish equipment for the performance of the contract. Government will retain accountability / ownership
4.1.1. All fitness equipment to be serviced by the contractor in accordance with this SOW.
4.1.2. Electronic equipment listing in Microsoft Excel.
4.2. Contractor Responsibility for Government Property:
4.2.1. The contractor assumes full responsibility and shall compensate the Government for any and all loss and damages resulting in whole or part from acts of neglect or omissions by the contractor, any subcontractor or employee, agent or representative of the contractor or subcontractor, to any government property while in their custody and care for storage, repairs or services to be performed under terms of this contract.
4.2.2. The FCD shall be notified by the contractor prior to the removal of any government owned property from Shaw AFB SC.
4.2.3. All equipment removed from the installation will be accounted for on an AF Form 1297 (hand receipt), with a copy provided to the FCD or his representative.
5.0. SPECIFIC REQUIREMENTS
5.1 INSTALLATION OPERATIONS
5.1.2. RECOGNIZED HOLIDAYS: The Government will not provide base support or facility access for the days below, except when services are required by the Customer and with prior approval in writing from the CO. Any of the below holidays falling on a Saturday will be observed on the preceding Friday;
holidays falling on a Sunday will be observed on the following Monday:

5.2. SECURITY REQUIREMENTS
5.2.1 Listing of Employees: The Contractor shall maintain a current listing of employees. The list shall include the employee's name, social security number and type of investigation if contract work involves unescorted entry to Air Force restricted areas or other sensitive areas designated by the installation commander. The list shall be provided to the Program Manager/POC and sponsoring agency’s Security Manager. An updated listing shall be provided when an employee's status or information changes.
5.2.2 Security Training: The Contractor ensures all Contractor employees receive initial and recurring security education training from the sponsoring agency’s security manager. Training must be conducted IAW DOD 5200.1-M, Information Security Program, Volume 3, and AFI 31-401, "Information Security Program Management". Contractor personnel who work in Air Force controlled/restricted areas must be trained IAW AFI 31- 101, Integrated Defense.
5.2.3 Pass and Identification Items: The Contractor shall ensure the pass and identification items required for contract performance are obtained for employees.
5.2.4 Background Checks: All Contractor employees shall submit to a criminal background check accomplished by Shaw Air Force Base Security Forces prior to the employee being issued base access credentials. Each Contractor personnel must submit their Social Security Number, Date of Birth, and Driver’s License state and number (if applicable). Failure to submit required information can result in denial or delay of issuance of credentials. Background checks are only good for the company that requested them. Processing criminal background checks takes, at a minimum, 7-10 days from time of submission.
5.2.5 Reporting Requirements: Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, Contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment.
5.2.6 Physical Security: The Contractor shall be responsible for safeguarding all Government property and controlled forms provided for Contractor use. At the end of each work period, all Government facilities, equipment, and materials shall be secured.
5.2.7 Photography Restrictions: No one is authorized to take any pictures or video recordings of Air Force operational aircraft or restricted areas without the approval of 20 FW/CVN regardless of location while on Shaw AFB. This includes, but is not limited to, photographs taken from personal electronic devices such as blackberries, iPads, iPods, cell phones, or any other devices with digital recording capabilities.
5.3 SAFETY REQUIREMENTS.
5.3.1. Safety Requirements and Reports: The Contractor shall perform work in a safe manner as required by OSHA Safety and Health Requirements and applicable Department of Labor, Department of Defense and Air Force safety standards. The Contractor shall provide a verbal report to the CO as soon as possible following an occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. Provide a completed copy of the required Accident Investigation Report to the CO within five (5) calendar days of each occurrence. The Accident Investigation Report shall be prepared and submitted IAW AFI 91-204.
5.3.2. Mishaps: In the event of an accident/mishap, the Contractor shall take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the CO.
5.3.3. Investigations: If the Government elects to conduct an investigation of the accident/mishap, the Contractor shall cooperate fully and assist Government personnel in the conduct of investigation until the investigation is completed.
