STATEMENT OF WORK

QUARTERLY RANGE CLEANING AND HAZARDOUS WASTE REMOVAL

SHAW AIR FORCE BASE

SUMTER, SOUTH CAROLINA

The government’s requirement is for the quarterly range cleaning and hazardous waste removal and annual filter replacement at the 20th Security Forces Squadron Combat Arms Weapons Firing Range located at 904 Patrol Road - Building 1833, Shaw AFB, SC 29152.

1. SCOPE:

1.1 WORK TO BE PERFORMED.

1.1.2 The work to be performed shall include the furnishing of all labor, materials, tools, transportation, supplies, and equipment necessary to perform lead and hazardous waste clean-up, removal, and disposal from 20th Security Forces Combat Arms Weapons Firing Range.

1.1.3 The contractor must be regularly employed and when applicable, licensed, and certified in the handling and proper disposal of hazardous materials.

1.1.4 The hazardous waste clean-up, removal, and disposal shall be in accordance with all applicable federal, state, and local, to include: Air Force, Occupatinal Safety and Health Administration (OSHA), Enviornmental Protection Agency (EPA), and South Carolina Department of Health & Enviornmental Control (SCDHEC), standards.

2. DESCRIPTION OF WORK:

The following shall be completed on a quarterly/annually/as needed basis:

2.1 QUARTERLY:

2.1.1 The contractor shall coordinate quarterly scheduling with Combat Arms at 803-895- 0290 and remove, package, and replace 55 gal drums from bullet trap containing bullet/lead residue. Contractor shall be notified when 55 gallon drums are at 75% capacity and remove, package, and replace within three days notification by Combat Arms.

2.1.2 The contractor shall provide replacement drums.

2.1.3 The contractor shall coordinate quarterly scheduling with Combat Arms; remove, package, and replace the 15-24”x24”x2 pre filters from each of the three Filter Housings located outside on rear of range. Contractor shall be notified when pre filters are at 1.0” final pressure, for filter replacement; remove, package, and replace within three days notification by Combat Arms.

2.1.4 The 20 CES shall forecast for and provide replacement 24” x24” x2” pre filters.

2.1.5 The contractor shall remove lead dust/debris from the range floor, bullet trap and clean/clear all water drains. This area includes all concrete floors, three bays from the “Red Line” to include the bullet trap, and the area under the horizontal auger located outside on rear of range.

2.1.6 The contractor shall collect all lead dust using High Efficiency Particulate Air (HEPA) vacuum and shall comply with all applicable federal, state, and local, to include:

EPA, OSHA, SCDHEC, and Air Force, standards.

2.1.7 The contractor shall remove lead dust/debris using HEPA vacuuming from the back side of the range; pressure wash immediately after HEPA vacuuming. This area includes all scrolls, horizontal auger, and area under the horizontal auger located outside on rear of range.

2.1.8 The contractor shall package all debris and hazardous waste as required and dispose of in an approved manner in accordance with all applicable federal, state, and local, to include: EPA, OSHA, SCDHEC, and Air Force, standards.

2.1.9 The contractor shall collect all hazardous sludge and waste from the bullet trap overflow system and dispose of in accordance with all applicable federal, state, and local, to include: EPA, OSHA, SCDHEC, and Air Force, standards.

2.1.10 This task shall be completed within 3 days of notification by Combat Arms at 803- 895-0290 in addition to being accomplished quarterly.

2.2 ANNUALLY:

2.2.1 The contractor shall coordinate annual scheduling with Combat Arms; remove, package, and replace the 15- 24” x24” x12” HEPA filters from each of the three Filter Housings located outside on rear of range. Contractor shall be notified when HEPA filters are at 2.5-3.0” final pressure, for filter replacement; remove, package, and replace within three days notification by Combat Arms.

2.2.2The 20 CES shall forecast for and provide replacement 24” x24” x12” HEPA filters.

2.2 UNSCHEDULED:

2.2.1 Contractor shall provide two unscheduled quarterly services on an annual basis as described throughout the Statement of Work.

2.3 ADDITIONAL REQUIREMENTS:

2.3.1 The contractor shall provide all services, necessary for the disposal of hazardous waste in accordance with all applicable federal, state, and local, to include: EPA, OSHA, SCDHEC, and Air Force, standards.

2.3.2 Containerized debris (sweepings, bullet residue, and personal protective equipment) shall be palletized and transported to an off-base, approved, commercial disposal or recycling location. The contractor shall be responsible for generating the shipping documents in accordance with all applicable federal, state, and local, to include: EPA, OSHA, SCDHEC, and Air Force, standards. The contractor shall ensure that the off-base, approved, commercial disposal or recycling location are certified and/or authorized to do so.

2.3.3 The contractor shall obtain and provide all documentation to the Non-Commisioned Officer In Charge (NCOIC) of Combat Arms at 803-895-0290, who shall then turn in copies of the documentation to the 20th Contracting Squadron to prove that the final and proper disposal of all items has been accomplished in accordance with all applicable federal, state, and local, to include: EPA, OSHA, SCDHEC, and Air Force, standards.

2.3.4 A final inspection shall be conducted with the contractor and the NCOIC of Combat Arms.

3. GENERAL INFORMATION:

3.1 SECURITY REQUIREMENTS

3.1.1. Listing of Employees. The contractor shall maintain a current listing of employees. The list shall include the employee's name, social security number and type of investigation if contract work involves unescorted entry to Air Force restricted areas or other sensitive areas designated by the installation commander. The list shall be provided to the Program Manager and sponsoring agency’s Security Manager. An updated listing shall be provided when an employee's status or information changes.

3.1.2. Security Training. The contractor ensures all contractor employees receive initial and recurring security education training from the sponsoring agency’s security manager.

Training must be conducted IAW DOD 5200.1-M, Information Security Program, Volumes 3, and AFI 31-401, "Information Security Program Management". Contractor personnel who work in Air Force controlled/restricted areas must be trained IAW AFI 31- 101, Integrated Defense.

3.1.3. Pass And Identification Items. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees.

3.1.4. Background Checks. All contractor employees shall submit to a criminal background check accomplished by Shaw Air Force Base Security Forces prior to the employee being issued base access credentials. Failure to submit requested Contractor Listing of Employees as indicated can result in denial or delay of issuing base access credentials. Processing criminal background checks takes, at a minimum, 7-10 days from time of submission.

3.1.5. Federal Investigations Requirements. (If Applicable) Contractor employees requiring initial issuance of a Common Access Card (CAC), Access to Restricted Areas, Use of Government Automated Information Systems/Networks, Child Care positions, or designated Positions of Trust shall complete submission of a National Agency Check with Inquiries (NACI) to the Office of Personnel Management (OPM), or a DoD-determined equivalent investigation. When required, the government (20 FW Information Protection Office) shall submit NACI investigations to OPM at no cost to the contractor.

3.1.6. Retrieving Identification Media. The contractor shall retrieve all identification media from employees who depart for any reason before the contract expires; e.g.

terminated for cause, retirement, etc.

3.1.7. Traffic Laws. The contractor and its employees shall comply with base traffic regulations.

3.1.8. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicle or privately-owned vehicle while on Shaw AFB SC.

3.1.9. For Official Use Only (FOUO). The contractor shall comply with DoD 5400.7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program, requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding For Official Use Only(FOUO) material.

3.1.10. Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment.

3.1.11. Physical Security. The contractor shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.

3.1.12. Controlled/Restricted Areas. (If Applicable) The contractor shall implement local base procedures for entry to Air Force controlled/restricted areas where contractor personnel shall work. An AF Form 2586, Unescorted Entry Authorization Certificate, must be completed and signed by the sponsoring agencies Security Manager before a Restricted Area Badge shall be issued. Contractor employees must have a completed

National Agency Check with Written Inquires (NACI) investigation before receiving a Restricted Area Badge. Interim access can be granted IAW AFI 31-501, Personnel Security Program Management.

3.1.13. Key Control. The contractor shall establish and implement methods of making sure all keys issued to the contractor by the Government are not lost or misplaced and are not used by unauthorized persons. The contractor shall not duplicate any keys issued by the government.

3.1.13.1. The contractor shall immediately report to the Program Manager or NCOIC of Combat Arms, any occurrences of lost or duplicated keys.

3.1.13.2. In the event keys, other than master keys, are lost or duplicated, the contractor may be required, upon written direction of the contracting officer, to re-key or replace the affected lock or locks without cost to the government. The government may, however, at its option, replace the affected lock or locks or perform re-keying and deduct the cost of such from the monthly payment due the contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the government and the total cost deducted from the monthly payment due the contractor.

3.1. 13.3. The contractor shall prohibit the use of keys, issued by the government, by any persons other than the contractor’s employees and the opening of locked areas by contractor employees to permit entrance of persons other than contractor employees engaged in performance of contract work requirements in those areas.

3.1.14. Photography Restrictions. No one is authorized to take any pictures or video recordings of Air Force operational aircraft or restricted areas without the approval of 20 FW/CVN regardless of location while on Shaw AFB. This includes, but is not limited to, personal electronic devices such as blackberries, iPADs, iPODs, cell phones, or any other devices with digital recording capabilities.

3.1.14.1. Any contractor requiring pictures or videotape of Shaw’s airfield to support their contract must request and possess written authorization prior entering the airfield area.

3.1.14.2. Written authorization letters to photograph or video of the flightline must be signed by the respective (55th, 77th or 79th) Maintenance Squadron Commander; a Fighter Squadron Top 3, Director of Operations, or Commander; Logistics Readiness Squadron Commander or Deputy; the Operations Group Commander or Deputy; 20 Operations Support Squadron Commander or Director of Operations or 20 FW/CVN.

3.2 CONSERVATION OF UTILITIES

The contractor shall encourage employees on utilities conservation practices. The contractor shall be responsible for operating under conditions which prevent the waste of utilities which include the following:

3.2.1. Lights shall be used only in areas where and when work is actually being performed.

3.2.2. Mechanical equipment controls for heating, ventilation, and air conditioning systems shall not be adjusted by the contractor or by contractor employees.

3.2.3. Water faucets or valves shall be turned off after the required use has been accomplished.

3.2.4. Government telephones shall be used only for official government business not for personal use.

3.3 RECORDS

3.3.1. The contractor shall be responsible for creating, maintaining, and disposing of only those government required medical and personnel records that are specifically cited in this SOW or required by the provisions of a mandatory directive listed in Air Force Instruction 44-102, Medical Care Management, Air Force Instruction 41-210, Patient Administrative Functions, and Air Force Instruction 44-119, Medical Quality Operations.

If requested by the Government, the contractor shall provide the original record or a reproducible copy of any such record within five working days of receipt of the request.

3.4 ENVIRONMENTAL CONTROLS

3.4.1. Compliance with Laws and Regulations. The contractor shall be knowledgeable of and comply with all applicable requirements regarding environmental protection. In the event environmental laws/regulations change during the term of this contract, the contractor is required to comply as such laws come into effect. If there is an increase or decrease in cost as a result of the change, the contractor shall inform the Contracting Officer pursuant to notice requirements and negotiate a modification to the contract.

3.4.2. Notification of Environmental Spills. If the contractor spills or releases any substance contained in 40 CFR 302 into the environment, the contractor or its agent shall immediately report the incident to Shaw AFB Fire Dept at 911. The liability for the spill or release of such substances rests solely with the contractor and its agent.

3.4.3. Material Storage and Use. The contractor shall follow manufacturer’s guidelines and professional recommendations for control of humidity, temperature, cleanliness, and materials handling. This includes hazardous materials.

3.5 SAFETY REQUIREMENTS

3.5.1. Conform to the safety requirements contained in the contract for all activities related to the accomplishment of the work.

3.5.2. Perform work in a safe manner as required by OSHA 2206, General Industry, Occupational Safety and Health Standards (29 CFR 1910).

3.5.3 Provide protection to government property to prevent damage during the period of time the property is under the control or in possession of the contractor.

3.5.4 Record and report promptly (within one hour) to the designated government representative (GR), all available facts relating to each instance of damage to government property or injury to either contractor or government personnel.

3.5.5 In the event of an accident/mishap, take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the Contracting Officer.

3.5.6. If the government elects to conduct an investigation of the accident/mishap, the contractor shall cooperate fully and assist government personnel in the conduct of investigation until the investigation is completed.

3.6 CONTRACTOR PERSONNEL

3.6.1. Contract Manager: the contractor shall provide a contract manager and alternate contract manager (responsible in the absence of the contract manager) who shall be responsible for the performance of the services. The names of the contract manager and alternate(s) shall be provided to the Contracting Officer in writing prior to the beginning of the first performance period. The contract manager and alternate(s) must be able to read, write, speak, and understand English.

3.6.2. The Contract Manager and alternate shall have full authority to act for the contractor on all contract matters relating to daily operations of this contract.

3.6.3. The Contract Manager or alternate shall be available during normal duty hours within two (2) hours to meet on the installation with government personnel or by phone (designated by the Contracting Officer) to discuss problems. After normal duty hours the manager or alternate shall be available within four (4) hours.

3.6.4. Contractor Employees: the contractor shall not employ persons for work on this contract if such employee is identified to the contractor by the Contracting Officer as a potential threat to the Health, Safety, Security, General well being or operational mission of the installation and its population. Based on criminal background results or established disciplinary channels.

3.6.5. Contractor personnel shall present a neat appearance and be easily recognized as contractor employees. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges, which contain the company name and employee name in English.

3.6.6. The contractor shall not employ any person who is an employee of the U.S.

Government if employing that person would create a conflict of interest. Additionally, the contractor shall not employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval according to DOD 5500-7-R Joint Ethics Regulation. The contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies in AFI 64-106.
