STATEMENT OF WORK
FOR
UNIFORMS & SHOP RAGS
SHAW AFB
17 August 2014
Uniform and Shop Rags Performance Work Statement
1.0 DESCRIPTION OF SERVICES. The contractor shall furnish all management, labor, tools, equipment, transportation, delivery, and materials necessary to provide fire retardant uniforms and shops rags service. Contractor shall provide a weekly cleaning service that includes picking up dirty uniforms and soiled shop rags from locations on Shaw Air Force Base (AFB), South Carolina. Uniforms shall be cleaned a manner required to maintain appropriate serviceability.
1.1 Uniforms and Shop Rags For Electric Shop. The contractor shall provide 30 sets of rental Armorex fire retardant (FR) uniforms (shirts/pants) that meet the minimum Occupational Safety and Health Administration
(OSHA) requirements under final rule 1910.269, National Environmental Policy Act (NEPA)70E, and American
Society for Testing and Materials (ASTM) F 1506-2 for a three person electric shop. The contractor shall provide 6 each rental bomber jackets. The rental bomber jackets shall be considered part of the uniform for all associated cleaning, repair, and serviceability requirements. The contractor shall perform initial sizing and fitting of personnel for uniforms and additional sizing as needed. The contractor shall provide a weekly cleaning service that includes picking up dirty uniforms from building 308, cleaning them in a manner required to maintain appropriate serviceability, and returning uniforms to building 308.
Additionally, the contractor shall provide 500 new red shop rags. The rag size shall be twelve inches by twelve inches. Contractor shall provide weekly pick up of dirty shop rags from building 302.
1.1.2. Repair/Replace. Contractor shall have the capability to make repairs to the uniforms and shall provide that service as needed. All repairs shall be made in a manner that shall ensure FR rating is maintained and meets current industry and OSHA standards. Contractor shall replace uniforms that are beyond repair or no longer fit individuals properly.
1.2. Uniforms for Heating Ventilation Air-Conditioning - Refrigeration (HVAC/R) Shop. The contractor shall furnish all management, labor, tools, equipment, transportation, and materials necessary to provide 60 sets of rental
Tecasafe FR uniforms (shirts/pants) for a 6 person HVAC/R shop located in building 251. Contractor shall provide
12 each rental bomber jackets. The jackets shall be considered part of the uniform for all associated cleaning, repair, and serviceability requirements. The contractor shall perform initial sizing and fitting of personnel for uniforms and additional sizing as needed. Contractor shall replace uniform items that are beyond serviceability or no longer fit individuals properly. All uniforms shall meet Arc-Flash standards. Contractor shall provide appropriate storage containers and provide a weekly service that includes picking up dirty uniforms and cleaning them in a manner required to maintain FR rating and returning uniforms to buildings 251 Akin Street, Shaw AFB S.C.
1.2.1 Repair/Replace. Repair and replacement of these uniforms shall be the same as stated in Para. 1.1.2.
2. SERVICE SUMMARY.

3. SERVICE REQUIREMENTS ELECTRIC SHOP. The contractor shall provide 30 sets of various sizes of
Armorex Fire Retardant Uniforms. Uniform sets shall consist of the following;
a. 15 each navy blue Long sleeve shirts w/two chest pockets, left pocket has pen/pencil slot.
b. 15 each tan long sleeves shirts w/two chest pockets, left pocket has pen/pencil.
c. 30 each navy blue long work pants standard industry design and made of the same materials as shirts.
d. 6 each bomber jackets.
e. 500 each 12” x 12” shop rags, red in color.
4. SERVICE REQUIREMENTS HVAC/R SHOPS. The contractor shall provide 60 sets of various sizes of
Tecasafe uniforms, 12 each jackets and 6 belts. Uniform sets shall consist of the following;
a. 30 each Long sleeve Brown shirts.
b. 30 each Long sleeve Blue shirts.
c. 60 each Navy Blue long work pants, standard industry design and made of the same materials as shirts.
d. 12 each bomber jackets.
e. 6 each belts (one time purchase).
5. GENERAL INFORMATION.
5.1. Government Furnished Property and Services. Containers will be provided for the storage of dirty shop rags.
Contractor pick up of rags from provided container.
5.2. Hours of Operation. Hours of operations are 7:30am thru 4:30pm, Monday through Friday Excluding
Recognized Federal Holidays.

If these holidays fall on Saturday, the preceding Friday will be observed. If these holidays fall on Sunday, the following Monday will be observed. If a holiday falls on a scheduled service day, the contractor will be responsible for rescheduling services for the first day after the holiday observance.
5.4. Base Closures. Work scheduled but not accomplished because of base closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening the base.
6. SECURITY REQUIREMENTS.
6.1 Contractor/Employee Base Pass and Identification, Special Clearances and Vehicle Passes. The contractor shall comply with all requirements and procedures in accordance with (IAW) Federal Acquisition Regulation (FAR)
5353.242-9000, Contractor Access to Air Force Installations. Contractor will submit names of drivers and labors that will be servicing Shaw AFB up to 30 days prior to the first servicing to the Government Representative for Personnel
Back-Ground checks for the issuing of passes. All documentation shall be submitted at the pre-performance conference.
7.0. SHAW AFB ENVIRONMENTAL COMPLIANCE
7.1. Environmental Requirements. Offerors are reminded of the environmental requirements contained in the solicitation and to comply with all applicable federal, state and local environmental statutes and regulations. In addition to the scrutiny of civil environmental regulatory and enforcement agencies, environmental compliance incidental to Air Force (AF) activities, or on AF property, is subject to AF scrutiny. The AF routinely investigates environmental violations and cooperates fully with civil authorities to secure remedies and discourage future violations. Failure to comply with environmental statutes and regulations can subject the contractor to criminal and civil penalties and/or contractual sanctions including, but not limited to, termination, suspension, debarment, and adverse past performance assessment.
7.2. Shaw AFB Environmental Management System. All contract personnel must be aware of the Shaw AFB
Environmental Management System (EMS) and their personal responsibility to protect the environment. The following link to Shaw AFB EMS (30 minute) training module must be viewed by contract personnel and certified by contractor no later than: 1 June 2009

8.0. SHAW AFB SECURITY REQUIREMENTS FOR UNCLASSIFIED CONTRACTS

8.1. Shaw AFB Security Requirements. The following security requirements are recommended for placements in
Statement of Works (SOW’s) and Performance Work Statements (PWS’s) when contract work does not involve access to classified information.
8.1.1. Listing of Employees. The contractor shall maintain a current listing of employees. The list shall include the employee's name, social security number and type of investigation if contract work involves unescorted entry to Air
Force restricted areas or other sensitive areas designated by the installation commander. The list shall be provided to the Program Manager and sponsoring agencies Security Manager. An updated listing shall be provided when an employee's status or information changes.
8.1.2. Security Training. The contractor ensures all contractor employees receive initial and recurring security education training from the sponsoring agencies security manager. Training must be conducted IAW DOD 5200.1-
R, Information Security Program Regulation, and Air Force Instruction (AFI) 31-401, "Information Security
Program Management". Contractor personnel who work in Air Force controlled/restricted areas must be trained
IAW AFI 31- 101, The Air Force Installation Security Program.
8.1.3. Pass and Identification Items. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees. Contractor cannot sponsor other contractors on base. Only government employees (military or civilian) are authorized to sponsor contractors.
NOTICE: All contract employees shall submit to a criminal background check accomplished by Shaw Air Force
Security Forces prior to the employee being issued a contractor badge, Common Access Card or short-term pass.
8.1.3.1. Obtaining Contractor Badges. All contract employees who work on Shaw Air Force Base must obtain a
Contractor Badge. Before starting work on base, the contractor must complete the Shaw Form 54 from the contracting office. Security Forces will conduct the background check using the Shaw Form 54. Contractor badges will be issued from the Security Forces Pass & Registration Office in Building 1118, Shaw Air Force Base, SC
29152. Contractor badges will not exceed one (1) year in duration.
8.1.3.3. Short-Term Passes. Contractor requiring brief periods of access on base will be an AF
Form 75. Sponsors can contact the Visitor Center or Pass and Registration to obtain these passes.
8.1.3.4. Contractor vehicles do not require passes.
8.2. Retrieving Identification Media. The contractor is responsible for retrieving all identification media, including vehicle passes from employees who depart for any reason before the contract expires; e.g. terminated for cause, retirement, etc. The contractor shall return retrieved items to the Security Forces Pass and Registration
Office.
8.3. Traffic Laws. The contractor and its employees shall comply with base traffic regulations.
8.4. Weapons, Firearms, and Ammunition. Contract employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicle or privately-owned vehicle while on
Shaw Air Force Base.
8.5. For Official Use Only (FOUO). The contractor shall comply with DoD 5400-7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program, requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding
FOUO material.
8.6. Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment.
8.7. Physical Security. The contractor shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.
8.8. Controlled/Restricted Areas. (IF APPLICABLE) The contractor shall implement local base procedures for entry to Air Force controlled/restricted areas where contractor personnel will work. An AF Form 2586, Unescorted
Entry Authorization Certificate, must be completed and signed by the sponsoring agencies Security Manager before a Restricted Area Badge will be issued. Contractor employees must have a completed National Agency Check, Local Agency Check, and Credit Check (NACLC) investigation before receiving a Restricted Area Badge.
8.9. Key Control. The contractor shall establish and implement methods of making sure all keys issued to the contractor by the Government are not lost or misplaced and are not used by unauthorized persons. The contractor shall not duplicate any keys issued by the government.
8.9.1. Lost/Duplicate Key Reporting. The contractor shall immediately report to the Quality Assurance Evaluator
(QAE) or Program Manager any occurrences of lost or duplicated keys.
8.9.1.1. In the event keys, other than master keys, are lost or duplicated, the contractor may be required, upon written direction of the contracting officer, to re-key or replace the affected lock or locks without cost to the government. The government may, however, at its option, replace the affected lock or locks or perform re-keying and deduct the cost of such from the monthly payment due the contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the government and the total cost deducted from the monthly payment due the contractor.
8.9.1.2. The contractor shall prohibit the use of keys, issued by the government, by any persons other than the contractor’s employees and the opening of locked areas by contract employees to permit entrance of persons other than contract employees engaged in performance of contract work requirements in those areas.
8.10. Lock Combinations. The contractor shall control access to all government provided lock combinations to preclude unauthorized entry. The contractor is not authorized to record lock combinations without written approval by the Government Program Manager. Records with written combinations to authorized secure storage containers, secure storage rooms, or certified vaults, shall be marked and safeguarded at the highest classification level as the classified material maintained inside the approved containers.
