1.0 DESCRIPTION OF SERVICES. The Contractor shall provide a service to prepare an application for water sample analysis at Shaw Air Force Base (AFB), Poinsett Range, and Wateree Air Force Recreational Area. The Contractor shall provide all personnel, equipment, tools, materials, supervision, travel to and from Shaw AFB facilities (to include Shaw AFB, Poinsett Range, and the Wateree Recreational Area), and coordination to be escorted onto Shaw AFB facilities, sample collection and other items and services necessary to perform analysis of water samples as defined in this Performance Work Statement (PWS). The Contractor shall perform to the standards in this contract and comply with any and all Department of Defense (DOD), Environmental Protection Agency (EPA), Occupational Safety and Health Administration (OSHA), and Department of Health and Environmental Control (DHEC) rules and regulations.
The Contractor shall provide all customer requested documents in this PWS with delivery to the 20th Medical Group Bioenvironmental Engineering (BE) Contracting Officer’s Representative (COR) at the following address:

1.1 ROUTINE ANALYSIS. The Contractor shall provide Bioenvironmental Engineering with a user’s guide. This guide shall include specific instructions regarding how to request specific analysis, how to ensure samples are processed in the most efficient manner, points of contact for sample tracking and technical inquiries, field sampling preparations, mailing instructions for laboratory analysis, and emergency contact information. This guide shall be provided to the 20th Medical Group Bioenvironmental Engineering COR prior to initial sampling service requests.
The user's guide shall be provided to the Contracting Officer (CO) with 30 days of contract award.
1.1.1. Bacteriological Analysis. Bacteriological samples shall be analyzed in a state certified laboratory, using an EPA approved method for drinking water. Testing equipment should have quality control procedures in place for cross contamination of drinking/waste water testing. The Contractor shall analyze the number of samples for total coliform and fecal/E-coli confirmation. The Contractor shall provide chain of custody forms, and any other necessary processing documents required. In the case of an emergency or special requirement the Government may request for additional samples to be taken in an expeditious fashion.
1.1.1.1 The Contractor shall collect water samples for all testing requirements. The Contractor is responsible for transportation between sampling locations. Any preparations needed prior to these samplings being performed by the Contractor shall be addressed in the User Guide and changes given to the COR no less than 24 hours in advance.
1.1.1.2 In the event the Shaw AFB Bioenvironmental Engineering office possesses DHEC certification to perform bacteriological testing, the bacteriological testing portion of this contract will be terminated.
1.1.1.3 Examination of samples shall begin promptly upon receipt at the laboratory or in accordance with hold time for various samples. Samples shall be cultured the day of delivery and read after 24 hours of incubation in accordance to applicable regulations and laws. The Contractor shall not allow the time elapsing between sample collection and examination to exceed 30 hours or in accordance to applicable regulations and laws for each sample tested. ***Total Coliform, Fecal/E-coli, and Heterotrophic Plate Count samples have a specific 6 hour hold time and must be done accordingly.***
1.1.1.4 The Contractor shall telephone Bioenvironmental Engineering if any laboratory actions compromise the validity of the sample, or result in sample destruction immediately following the incident. The Contractor shall provide a repeat analysis at no charge to Shaw AFB. BE may be contacted at (803) 895-6196 from 0730 to 1630 Monday through Friday, or at any time using the BE On-call Emergency Phone number at (803) 468-7679.
1.1.2 Chemical Analysis. Chemical analysis for all water contaminants shall be accomplished in accordance with DoD, OSHA, DHEC, State, Federal, and EPA regulations. This is to include drinking water samples accomplished in accordance with the Safe Drinking Water Act (SDWA) or other DHEC requirements, using approved analytical methods. Residual chlorine and pH samples have a short 15 minute hold time and must be analyzed in the field by technicians who are certified by the State of South Carolina on these field methods and shall use calibrated field analysis equipment provided by the Contractor. All other samples shall be analyzed in a laboratory certified by the State of South Carolina.
1.1.2.1 If samples are able to be taken by BE then the Contractor shall provide clean sample containers (e.g., free of debris and residue, acid-washed) and arrange for delivery and return of these containers. Sample containers will contain applicable preservatives. In the event that it is not acceptable for sample containers to contain preservatives, the preservatives will not be provided. Sample preservation methods must be approved by the EPA. In addition, the Contractor shall provide chain of custody forms and any other necessary processing documents as required. If samples are required to be iced, the Contractor shall supply iced packs and coolers for shipping. Instructions for these types of samples must be provided to Bioenvironmental more than 24 hours in advance of sampling.
1.1.2.2 The Contractor shall be responsible for all mailing costs of these items both to and from Shaw AFB. The Contractor shall deliver the containers and necessary documents to 20 AMDS/SGPB-Bioenvironmental Engineering within 24 hours of request.
1.1.2.3 The Contractor shall analyze the samples within the legal holding time as specified in the EPA approved analytical method and prescribed in Rules for South Carolina Safe Drinking Water and all other federal and state regulations.
1.1.2.4 The Contractor shall accomplish analysis of a trip blank, and a spiked matrix for each analysis requested.
The laboratory will analyze these quality assurance samples and the results of the blank and the percentage recovered in the spiked sample shall be reported to Bioenvironmental Engineering with the sample results.
1.1.2.5 The Contractor shall telephone BE if any laboratory actions compromise the validity of the sample, or result in sample destruction immediately following the incident. The Contractor shall provide a repeat analysis at no charge to Shaw AFB. BE may be contacted at (803) 895-6196 from 0730 to 1630 Monday through Friday, or at any time using the Bioenvironmental Engineering On-call Emergency Phone number at (803) 468-7679.

1.1.3 Sampling Schedule. The Contractor shall coordinate with the COR to obtain a schedule for both bacteriological and chemical analysis for water sampling. This schedule will be dictated by DHEC requirements and the requirements of the Shaw AFB Drinking Water Sampling, Analysis, and Monitoring (DWSAM) Plan.
1.2 EMERGENCY ANALYSIS. The Government may request additional samples to the Contractor in the event of an emergency (e.g., suspected contamination that threatens public health). The Contractor shall collect and analyze the samples in accordance with the State, Federal, EPA and DHEC rules and regulations. The Contractor shall analyze the samples within the legal holding time as specified in the analytical methods. Results must be expedited to the COR within 24 hours.
1.3 REPORTING REQUIREMENTS.
1.3.1 Bacteriological Samples.
1.3.1.1 The Contractor shall provide sample results for positive, special, or emergency samples to Shaw AFB immediately after the incubation period, via telephone by calling Bioenvironmental Engineering immediately following the incident. BE may be contacted at (803) 895-6196 from 0730 to 1630 Monday through Friday, or at any time using the Bioenvironmental Engineering On-call Emergency Phone number at (803) 468-7679. At the time of notification the Contractor will be given an e-mail address to submit official results.
1.3.1.2 The Contractor shall, within five (5) working days of completing analysis of a compliance sample, submit the required reporting form(s). Non-compliance results must be reported immediately to BE via telephone at (803) 895-6196. If no one answers the primary office phone number, the Contractor shall call the Bioenvironmental Engineering On-call Emergency Phone (803) 468-7679. The Contractor shall mail all applicable copies of Reporting Form(s) to 20 AMDS/SGPB - Bioenvironmental Engineering.
If e-mail is not available, the Contractor shall fax a copy of the sample analysis report with the Reporting Form(s) to Bioenvironmental Engineering at (803) 895-6066.
1.3.2 Chemical Analytical Samples (SDWA).
1.3.2.1 The Contractor shall transcribe all results for samples identified as compliance samples onto all required (if applicable) Reporting Form(s). Sample and system identification data and other required data shall be transcribed from the chain of custody onto Reporting Form(s). The Contractor shall, within five (5) working days of completing routine analysis, submit a Reporting Form(s) (if applicable) to Bioenvironmental. The Contractor shall e-mail and mail copies of the Reporting Form(s) to 20 AMDS/SGPB - Bioenvironmental Engineering.
If e-mail is not available, the Contractor shall fax a copy of the sample analysis report with the Reporting Form(s) to Bioenvironmental Engineering at (803) 895-6066.
1.3.2.2 Screen all sample results for noncompliance EPA Maximum Contaminant Level (MCLs). If a sample result exceeds an EPA MCL, or secondary MC as applicable, the Contractor shall telephone Bioenvironmental Engineering at (803) 895-6196 within 24 hours. If no one answers the primary office phone number, the Contractor shall call the Bioenvironmental Engineering On-call Emergency Phone (803) 468-7679.
1.3.2.3 Laboratory detection limits and the number of significant digits included in reported results must comply with those specified in 40 CFR 141, 40 CFR 143, South Carolina Rules for Safe Drinking Water and all other Federal and State regulations. The Contractor shall ensure that all minimum reporting limits for analysis are less than the MCL and, if applicable, the secondary maximum contaminant level. In addition to the Reporting Form(s) (if applicable), the Contractor shall ensure all reports contain as a minimum: the base sample number, control laboratory sample number, sample site identifier, analysis method, date sampled, date analyzed, sample result, and the respective MCL. This report must be signed by the chemist performing the analysis, and the laboratory director reviewing the analysis results.
1.3.3 Special Event or Emergency Services.
1.3.3.1 In the event of a positive bacteriological sample or a chemical analysis sample that exceeds an MCL, additional services will be required. The Contractor shall collect samples immediately following a notification of exceedence. This will require the Contractor to provide laboratory services within 24 hours of notification.
1.3.3.2 The Government may request additional samples to the Contractor in the event of an emergency (e.g., suspected contamination that threatens public health). This will require the Contractor to provide services within 24 hours of notification.
1.3.4 Proof of State Certification. The Contractor shall submit proof of laboratory certification by the State of South Carolina. The Contractor shall maintain and provide current proof of annual certification by the State of
South Carolina with the Contracting Officer (CO) and Bioenvironmental Engineering prior to execution of any option period. The Contractor shall immediately notify the CO and Bioenvironmental Engineering COR if this certification is subject to expiration or termination.
1.3.5 Requirement for Valid Sampling/Analysis. For all sampling covered in this document, the Contractor shall telephone Bioenvironmental Engineering at 803-895-6196 if any laboratory actions (or lack thereof) compromise the validity of the sample destruction, immediately following the incident. The Contractor shall provide a repeat analysis at no charge to Shaw AFB. If no one answers the primary office phone number, the Contractor shall call the On-Call Emergency Phone at 803-468-7679. The Contractor shall invoice the customer only after valid sample results are reported.
1.4 QUALIFICATIONS The Contractor is solely responsible for ensuring sufficient personnel are assigned to this contract and all personnel are qualified and certified to perform the requirements listed herein, including those qualifications and certifications (if required) by any original equipment manufacturer (OEM) requirements to maintain, install, or operate equipment covered by this contract. The Contractor shall also ensure personnel meet the criterion stated in its proposal for this requirement. This includes all documented changes or updates made during the discussion process.
1.5 IDENTIFICATION. Contractor personnel shall be easily recognizable while on the installation in conjunction with this contract. This shall be accomplished through the wear of distinctive clothing, overcoats, or hats, bearing the company name or logo. The coloring or design of the items selected should identify personnel easily and quickly for reasons of safety and personal protection. During meetings with Government personnel, Contractor personnel shall introduce themselves as “Contractor personnel” and display distinguishing badges or other visible identification. In addition, they shall appropriately identify themselves as “Contractor personnel” in telephone conversations and in formal and informal written correspondence.
1.6 STANDARDS. The Government is authorized to restrict employment under this contract of any Contractor personnel or prospective Contractor personnel, who is identified as a potential threat to the health, safety, security, general well being, or the operational mission of the installation (to include annexes such as Poinsett and Wateree) and its population. A determination may also remove Contractor personnel for, but is not limited to, incidents involving most immediately identifiable types of misconduct or delinquency such as: disorderly conduct, use of abusive or offensive language, intimidation by words or actions, fighting, participation in disruptive activities which interferes with the normal operations or brings discredit to the Government, theft, vandalism, sexual harassment, or any criminal actions. This also includes selling, consuming, or being under the influence of intoxicants, drugs, or substances which produce similar effects while in or on Federally-controlled property. Contractor personnel shall only smoke and/or use tobacco products in designated and approved smoking/tobacco use areas. If the CO directs removal of Contractor personnel for violating the above provisions, the Contractor shall immediately remove the individual(s) at no cost to the Government. Removal of employees for misconduct does not alleviate the Contractor from providing services. Per Shaw AFB Plan 31-101, the installation commander has authority to debar any individual from the installation who commits a crime or offense prejudicial to good order and discipline. All disputes to the CO’s decision must be filed within ten (10) days of the decision to the Base Operations Support Flight Chief. The removed Contractor personnel may not resume work unless/until a decision is made by the Government CO overturning the removal decision.
1.7 CONTRACT MANPOWER REPORTING. The Contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract via secure data collection site. The Contractor is required to completely fill in all required data fields at the Air Force CMRA link https://afcmra.hqda.pentagon.mil/Login.aspx. Reporting inputs will be for the labor executed during the period of performance for each Government Fiscal Year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year.
Contractors may direct questions to the CMRA help desk.
1.7.1 Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.
1.7.2 User Manuals: Data for Air Force service requirements must be input at the Air Force CMRA link.
However, user manuals for Government personnel and contractors are available at the Army CMRA link at http://www.ecmra.mil.
1.8 BASE ACCESS REQUIREMENTS. The Contractor shall comply with all requirements and procedures in accordance with (IAW) AFFARS 5352.242-9000, Contractor Access to Air Force Installations, and shall ensure the pass and identification items required for contract performance are obtained for all personnel. All documentation shall be submitted for base passes or base identification per Base Security Requirements. Contractor personnel shall maintain base passes on their person at all times in performance of this contract. Contractor personnel shall obtain and use specific Contractor base passes. Contractor personnel who have personal DoD retiree or dependent ID cards shall not use those personal ID cards for base access when performing contract work. All Contractor personnel who operate a vehicle in performance of this contract shall possess a valid driver’s license and current registration and insurance. Failure to turn in a base credential when requested can result in a non-recommend from Security Forces for future employment. The Contractor is responsible for employee inability to obtain base access required for contract performance.
1.9 COMPLIANCE WITH LABOR LAWS. The Contractor shall comply with all applicable laws pertaining to wages, worker’s compensation, equal opportunity, Service Contract Act, and so forth, as implemented by Air Force directives and required by law.
1.10 MISCELLANEOUS REQUIREMENTS
1.10.1 Contractor: The Contractor shall provide telephone number(s) where surveillance results and complaints can be reported. The Contractor shall also provide the names and telephone numbers of Contractor POCs for after business hours including nights, weekends, and holidays. This information will be kept updated by the Contractor whenever personnel changes occur. The Contractor, or alternate, shall have full authority to act for the Contractor on all contract matters relating to the daily operations.
1.10.2 RECORDS The Contractor shall be responsible for creating, maintaining, and disposing of only those government required records that are specifically cited in this performance of work statement in accordance with SC EPD and Federal EPA. If requested by the Government, the Contractor shall provide the original record or a reproducible copy of any record within five (5) working days of receipt of the request.
1.10.3 Records Management. The Contractor shall ensure all records (according to Title 44, United States Code, Public Printing and Documents, Section 3301), to include all books, papers, maps, photographs, machine readable materials, or other documentary materials, regardless of physical form or characteristics), files, documents and working papers provided by the Government, and/or generated for the Government in the performance of this contract, remain Government property and are maintained in accordance with established Federal Records Act (Public Law 81-754), Freedom of Information Act (Public Law 90-23), Privacy Act of 1974 (Public Law 93-529), (DoD), and Air Force directives listed in this SOW.
1.10.4 Information Management: All records, files, documents, regardless of media (e.g., paper, electronic, etc.) as described in the PWS that are the responsibility of or generated by the Contractor, are the property of the Government, and shall remain so upon termination or completion of the contract. The Contractor shall keep these items current and maintain and dispose of them in accordance with the requirements established in Air Force Instruction (AFI) 33-322, Records Management Program, paragraph 10, Air Force Manual (AFMAN) 33-363, paragraph 6.4, AFI 33-364, paragraph 2.10, and the Air Force Records Disposition Schedule available on the Air Force Portal link. Records shall be turned over to the Government upon completion of the contract unless otherwise stated. All records are subject to the Freedom of Information and Privacy Act. The Shaw AFB Records Management Office (20 CS/SCOKR), 895-1630, will provide assistance and training to the Contractor to ensure compliance with Federal record keeping requirements.
2.0 SERVICES SUMMARY (SS). The Government will use the services summary which includes critical objectives and standards to determine contractor performance against mission essential items. Performance objectives define the desired outcomes for mission essential tasks. Performance thresholds define the minimum acceptable level of performance required to meet the standard to successfully meet the performance objective.

2.1. QUALITY CONTROL. The Contractor shall establish, operate and maintain a highly effective Quality Control Program (QCP) to ensure all services are performed in accordance with this Performance Work Statement (PWS).
The Contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. As a minimum, the Contractor shall develop quality control procedures that address the areas identified in paragraph 2.0, Services Summary. The Contractor’s QCP shall be provided to the contracting officer for acceptance not later than the post award conference, and shall remain in effect during the term of this contract (including any option periods if exercised). A copy of the QCP shall be maintained on-site and be available to all members of the on-site staff as well as the Contracting Officer and COR.
The Contractor shall establish processes and assign appropriate resources to effectively administer the requirement.
The Contractor shall respond to Government requests for contractual actions in a timely fashion. The Contractor shall have a single point of contact between the Government and Contractor personnel assigned to support contracts.
The Contractor shall assign work effort and maintain proper and accurate time keeping records of personnel assigned to work on the requirement.
2.2. QUALITY ASSURANCE. The Government shall monitor and evaluate the Contractor’s performance to ensure services are received IAW requirements set forth in this PWS. The COR will periodically inspect by watching actual task performance, physically checking an attribute of the completed task, checking a management information report, investigating customer complaints, or otherwise inspecting the task or its result to determine whether or not the performance meets the standards contained in this PWS. The COR may also inspect tasks not specifically mentioned in the Services Summary (Para 2.0 above) but as required in paragraph 1.0 of this PWS. The COR will document the results of the surveillance and this documentation will become the official Air Force (AF) record of the Contractor’s performance. The COR shall conduct, document, track, and analyze Contractor performance via monthly scheduled surveillances. When a performance threshold has not been met or Contractor performance has not been accomplished, the COR will initiate and provide the CO a Corrective Action Report (CAR) for issuance to the Contractor. The Contractor shall respond to the CAR IAW instructions provided and return it to the CO within ten (10) calendar days of receipt. Surveillance results and CARs will be used to develop metrics to track Contractor’s overall compliance with contract terms and conditions. The surveillance results shall be maintained as past performance information.
The Government will neither supervise contractor employees nor control the method by which the contractor performs the required tasks. It shall be the responsibility of the Contractor to manage its employees and to guard against any actions that are of the nature of personal services, or give the perception of personal services. If the Contractor believes that any actions constitute, or are perceived to constitute personal services, it shall be the contractor's responsibility to notify the Contracting Officer immediately.
2.3. PERIODIC PROGRESS MEETINGS. The CO, COR, and other Government personnel, as appropriate, will meet periodically with the Contractor to review the Contractor's performance. At these meetings the CO will apprise the Contractor of how the Government views the Contractor's performance and the Contractor will apprise the Government of problems, if any, being experienced. Other topics may include (as applicable): opportunities to improve the contract, any modifications required of the contract, unsatisfactory performance inspections results (CAR), and valid customer complaints against each performance objective observed, and the results of corrective and preventive action measures. The Contractor will also notify the CO in writing, of work being requested or performed, if any, that the Contractor considers over and above the requirements of the contract. Appropriate action shall be taken to resolve outstanding issues.
The minutes of these meetings will be documented in writing, signed by the CO and any other signatures as deemed appropriate, and distributed to the functional area and the Contractor. Should the Contractor not concur with the minutes, the Contractor will set out in writing to the CO any areas of non-concurrence. These meetings shall be held as needed, but not less than quarterly. All meetings require meeting minutes. All documents shall be sent to the CO, Contractor, and the COR.
3.0 GOVERNMENT FURNISHED PROPERTY AND SERVICES. None provided
4.0 SPECIFIC REQUIREMENTS
4.1 INSTALLATION OPERATIONS
4.1.1 HOURS OF OPERATION. The Bioenvironmental Engineering Office hours of operation are Monday through Friday, 7:30 a.m. - 4:30 p.m. An on-call technician is available 24 hours a day, 7 days per week at (803) 468-7679.
Federal holidays or unique military activities, such as down or family days, may interfere with hours of operation. If so, the on-call technician is available as stated above.
4.1.2 Recognized Holidays: The Government shall not provide base support or facility access for the days below, except when services are required by the Customer and with prior approval in writing from the CO. Any of the below holidays falling on a Saturday will be observed on the preceding Friday; holidays falling on a Sunday will be observed on the following Monday:

4.1.3 Base Closure: The Government will notify the contractor of anticipated closure of the facility due to inclement weather or any other authorized closure (i.e. ACC Family Days). In the event of an unplanned closure of the facility the contractor will be notified using the contact information provided. The Contractor is not authorized to bill the Government for those hours unless the Contractor can perform productive duties from an alternate worksite. Work scheduled but not accomplished because of base closure, will be accomplished as soon as possible after reopening the base.
4.2 SECURITY REQUIREMENTS
4.2.1 Listing Of Employees: The Contractor shall maintain a current listing of employees. The list shall include the employee's name, social security number and type of investigation if contract work involves unescorted entry to Air Force restricted areas or other sensitive areas designated by the installation commander. The list shall be provided to the Program Manager/COR and sponsoring agency’s Security Manager. An updated listing shall be provided when an employee's status or information changes.
4.2.2 Security Training: The Contractor ensures all Contractor employees receive initial and recurring security education training from the sponsoring agency’s security manager. Training must be conducted IAW DOD 5200.1- M, Information Security Program, Volume 3, and AFI 31-401, "Information Security Program Management".
Contractor personnel who work in Air Force controlled/restricted areas must be trained IAW AFI 31- 101, Integrated Defense.
4.2.3 Pass And Identification Items: The Contractor shall ensure the pass and identification items required for contract performance are obtained for employees.
4.2.4 Background Checks: All Contractor employees shall submit to a criminal background check accomplished by Shaw Air Force Base Security Forces prior to the employee being issued base access credentials. Each Contractor personnel must submit their Social Security Number, Date of Birth, and Driver’s License state and number (if applicable). Failure to submit required information can result in denial or delay of issuance of credentials.
Background checks are only good for the company that requested them. Processing criminal background checks takes, at a minimum, 7-10 days from time of submission.
4.2.5 Retrieving Identification Media: The Contractor shall retrieve all identification media from employees who depart for any reason before the contract expires; e.g. terminated for cause, retirement, etc.
4.2.6 Traffic Laws: The Contractor and its employees shall comply with base traffic regulations.
4.2.7 Weapons, Firearms, and Ammunition: Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicle or privately-owned vehicle while on Shaw AFB SC.
4.2.8 For Official Use Only (FOUO): The Contractor shall comply with DoD 5400.7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program, requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.
4.2.9 Reporting Requirements: Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, Contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment.
4.2.10 Physical Security: The Contractor shall be responsible for safeguarding all Government property and controlled forms provided for Contractor use. At the end of each work period, all Government facilities, equipment, and materials shall be secured.
4.2.11 Key Control: The Contractor shall establish and implement methods of making sure all keys issued to the Contractor by the Government are not lost or misplaced and are not used by unauthorized persons. The Contractor shall not duplicate any keys issued by the Government.
4.2.11.1 The Contractor shall immediately report to the COR or Program Manager any occurrences of lost or duplicated keys.
4.2.11.2 In the event keys, other than master keys, are lost or duplicated, the Contractor may be required, upon written direction of the CO, to re-key or replace the affected lock or locks without cost to the Government. The
Government may, however, at its option, replace the affected lock or locks or perform re-keying and deduct the cost of such from the monthly payment due the Contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the Government and the total cost deducted from the monthly payment due the Contractor.
4.2.11.3 The Contractor shall prohibit the use of keys, issued by the Government, by any persons other than the Contractor’s employees and the opening of locked areas by Contractor employees to permit entrance of persons other than Contractor employees engaged in performance of contract work requirements in those areas.
4.2.12 Photography Restrictions: No one is authorized to take any pictures or video recordings of Air Force operational aircraft or restricted areas without the approval of 20 FW/CVN regardless of location while on Shaw AFB. This includes, but is not limited to, photographs taken from personal electronic devices such as blackberries, iPADs, iPODs, cell phones, or any other devices with digital recording capabilities.
4.2.12.1 Any Contractor requiring pictures or videotape of Shaw’s airfield to support their contract must request and possess written authorization prior entering the airfield area.
4.2.12.2 Written authorization letters to photograph or video of the flight line must be signed by one of the following: 20 Maintenance Group Commander or Deputy; Aircraft Maintenance Squadron Commander; a Fighter Squadron Top 3, Director of Operations, or Commander; 20 Operations Group Commander or Deputy; 20 Operations Support Squadron Commander or Director of Operations; or 20 FW/CVN.
4.3 SAFETY REQUIREMENTS.
4.3.1 Safety Requirements and Reports: The Contractor shall perform work in a safe manner as required by OSHA Safety and Health Requirements and applicable Department of Labor, Department of Defense and Air Force safety standards. The Contractor shall provide a verbal report to the CO as soon as possible following an occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. Provide a completed copy of the required Accident Investigation Report to the CO within five (5) calendar days of each occurrence. The Accident Investigation Report shall be prepared and submitted IAW
AFI 91-204.
4.3.1.1 Mishaps: In the event of an accident/mishap, the Contractor shall take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the CO.
4.3.1.2 Investigations: If the Government elects to conduct an investigation of the accident/mishap, the Contractor shall cooperate fully and assist Government personnel in the conduct of investigation until the investigation is completed.
