1. DESCRIPTION OF WORK/SERVICES: The Contractor shall provide all management, tools, supplies, equipment, and labor necessary test, maintain and repair the elevators at Grand Forks AFB in a manner that will ensure continuous and safe operation. The contractor shall furnish all labor, brushes, lubricants, tools and materials necessary to perform the monthly, semi-annual, annual, and five year tests as required, by ANSI Standards 17.1 and 17.2. Five year tests shall be accomplished during the last month of the contract’s last option year. The contractor shall also respond to request for service in these elevators or lifts when contacted by the Contracting Officer Representative (COR) or the Contracting Officer (CO). Following is a list of elevators and their locations.

1.1. MAINTENANCE SERVICES:

1.1.1. MAINTENANCE: All maintenance will be scheduled and accomplished during normal working hours, 0745 – 1630, Monday through Friday. Elevators in buildings 109, 541, 542, 629, 631 and 699 are considered high use and shall be inspected, tested and maintained monthly. The lifts in buildings 308 and 418 shall be inspected, tested and maintained annually. Maintenance shall include but is not limited to, cleaning, lubricating, adjust and align, furnish supplies and provide reports necessary to comply with ANSI Standards.

1.1.2. INSPECTIONS: Monthly and annual inspections shall include but not limited to detailed visual examination of the elevator/lift system, operators, related mounting structure, and hardware during operation. The contractor shall provide maintenance, as part of the inspection in accordance with ANSI Standards 17.1 and 17.2, any abnormal operation, worn, broken or damaged parts requiring repair and or replacement discovered during the inspection shall be analyzed and repaired or replaced immediately, to make the system fully operational.

1.1.3. REPLACEMENT PARTS: When any part of the elevator or lift fails to perform properly the contractor shall replace the part, charging the Government the actual supplier’s price. All labor required to replace the defective parts shall be provided as part of this contract. Request for payment for parts shall be submitted with a copy of the supplier’s receipt of purchase or invoice. All replacement parts in excess of $1,000.00 must have pre-approval of the CO.

1.1.4. VANDALISM, ABUSE AND MISUSE: In the event the contractor determines equipment repair or adjustment is needed, due to vandalism or misuse of equipment, the contractor shall be entitled to compensation for this work. The contractor shall not proceed with the work until they have received authorization from the CO. Any work done without the contracting officer’s prior authorization shall be done at no additional expense to the government. The contractor shall furnish any requested estimates or proposals for this work as requested by the COR.

1.2 REPAIR/SERVICE CALLS:

1.2.1. ROUTINE CALLS: The contracting officer or his designated representative will notify the contractor of repairs to be made from the inspection report or service call and shall respond within four hours after receiving a call to repair an out of service elevator. The contractor shall report to the work location, survey the repair, and provide to base contracting an estimated time to repair and cost of repairs not covered in the basic service contract. The contractor shall commence repair work after notification from the contracting office. All repair work shall be in accordance with normal commercial practices using parts specified by the elevator manufacturer or items of equal or better quality. Downtime of the elevator will be kept to an absolute minimum.

1.2.2. EMERGENCY CALLS: The contractor shall be available for service call 24 hours, 7 days a week, to include Saturdays, Sundays, and Federal holidays and shall respond to emergency repair calls within two hours after notification. The contractor shall prepare and submit a written report within two business days after the emergency repair. The report shall include the date and time of the service call, the location of the elevator, the repairs performed, and the name of the technician performing the repairs. Only the COR/CO may request service calls outside of the routine service cycle. Historical data indicates there has been no emergency calls orders placed for 10+ years, it’s not anticipated that there will be emergency services required.

1.3. REQUIRED REPORTS, RECORDS/LOGS: The contractor shall prepare and submit a written report to the COR/CO within two business days after the routine repairs are completed. The report shall include the date and time of the service call, the location of the elevator, the repairs performed, and the name of the technician performing the repairs. The contractor shall furnish to the COR/CO a copy of the log to be maintained on file. The log shall list each inspection and test called for in the ANSI standard 17.1 and 17.2, as they apply to the installation.

1.3.1. RECORDS MANAGEMENT: The Contractor shall make available in a timely manner, any permits, reports, or general performance data required in the PWS.

The contractor shall create, handle and maintain records for the Air Force, regardless of medium, (in a pre agreed medium that can be used by the Air Force ) in accordance with the requirements established in AFRIMS Records Disposition Schedule (RDS), AFI 33-322, Records Management program, AFI 33-364, Records Disposition Procedures and Responsibilities, and AFMAN 33-363, Management of Records. 1.4. SPECIAL QUALIFICATIONS: Technicians certified in the trade of inspection, testing, maintenance, service and repair of elevators and lifts shall accomplish all work. When required, personnel clearance access or escorted entry shall be coordinated with the COR. The contractor shall provide a list to the CO of all personnel authorized to repair the elevator at building 699, Control Tower. This list will include the person’s social security number and if necessary, other approved identification of certificates of training and a picture I.D. will be required to gain entry.

2. GOVERNMENT FURNISHED PROPERTY AND SERVICES: The government shall provide without cost to the contractor:

2.1. UTILITIES: The government will furnish existing electrical power, water, and sewer service to the contractor during performance of this contract. The contractor shall conserve all furnished utilities.

2.2. SECURITY FORCES AND FIRE PROTECTION: The government will furnish Security Forces and Fire Protection when required.

3. GENERAL INFORMATION:

3.1. STORAGE OF MATERIALS: The contractor shall not store material and equipment in the elevator mechanical rooms without approval and coordination with the COR.

3.2. DISPOSITION OF MATERIALS: All debris and spent material, such as oils, greases, brushes, and belts, shall be disposed of at an off-base location by the Contractor. The contractor shall dispose of all material in an environmentally sound manner at no additional cost to the government.

3.2. BASE PASSES: The contractor shall obtain paperwork to obtain a base access pass from the Contract Administrator or CO. Failure to obtain the required documents or obtain the required passes will result in the contractor experiencing access problems and require an escort while on the installation.

3.3. QUALITY CONTROL: The contractor shall develop and maintain a quality control plan to ensure services are performed in accordance with the requirement of this Performance Work Statement. The contractor shall develop and implement procedures to identify, prevent and ensure non-recurrence of defective systems.

3.4. QUALITY ASSURANCE: The government shall evaluate the contractor’s performance under this contract in accordance with the Quality of Assurance Surveillance Plan.

3.5. HOURS OF OPERATION: The contractor shall normally perform the services required under this Performance Work Statement during 7:30A.M.-4:30PM., Monday through Friday. No routine work shall be performed on federal holidays without COR/CO approval.

4. Performance of Services during Crisis Declared by the National Command Authority or Overseas Combatant Commander. According to Department of Defense Instruction (DoDI) 3020.37, Continuation of Essential DoD Contractor Services During Crises, and the Air Force implementation thereof, unless otherwise directed by an authorized government representative, it is determined that Elevator services are requirement under this PWS are essential to be performed during a crisis.
