STATEMENT OF WORK
29 August 2023
INSTALL FLOORING
IN BUILDING 471 HVAC SHOP
MALMSTROM AFB, MT
1. GENERAL
1.1. Summary of work: The contractor shall provide all labor, equipment, tools, accessories, and materials to properly prepare existing flooring for overlaying and install approximately 3303 square feet of hard surface flooring (plank flooring) to include approximately 800 linear feet of 4” rubber cove base using the direct glue down method of installation in building 471 on Malmstrom AFB, MT.
1.2. Anti-Terrorism Awareness Training: In accordance with DoDI O-2000.16v1_AFI 10-245-O, Antiterrorism (AT) Program Implementation, ensure Level I - AT Awareness Training is completed by all contractors within 30 days of requiring access, and annually thereafter. The training will be provided by the government.
The objective of the training is to provide personnel with requisite knowledge necessary to remain vigilant and enable employment of AT tactics, techniques, and procedures.
1.3. Force Protection: Contractor personnel are required to abide by all real-world Force Protection Condition (FPCON) measures, random anti-terrorism measures (RAMs), shelter-in-place orders, evacuations and lock-down notices as directed. Examples include mandatory badge or ID checks, facility single points-of-entry and searches of hand carried items.
2. MATERIALS
2.1. The hard surface flooring shall be Shaw LVT Style Solitude 0648V, color 48760 Woodland (or equal). Cove base shall be 4” rubber of a complementary color (color must be approved by Facility Manager prior to purchase).
3. EXECUTION
3.1. The performance period will be 30 calendar days after flooring is received on site.
Working hours will be from 7:00 AM to 4:30 PM, Monday through Friday.
3.2. The contractor shall coordinate all his activities with Facility Manager, this facility will be in use during installation.
3.3. The contractor shall install the flooring as recommended by the manufacturer, using the direct glue down method. The contractor shall provide all materials required to clean, patch, level, and prepare substrate for the new flooring. Flooring shall be installed in accordance with attached floor plan. The contractor’s performance must meet or exceed The Carpet and Rug Institute’s Standard for Installation of Commercial Carpet, CRI 104- 2002. Installation must be performed in strict compliance with all local, state and federal regulations. The Occupational Safety and Health administration Hazard Communication Standard must be followed.
3.4. The government will inspect and accept the areas as the contractor completes them.
If any areas are found to be unacceptable when inspected by the government, the contractor shall re-perform until the newly installed area meets standard commercial practices.
3.5. Cove Base, Edge Guards and Transition Strips. The contractor shall provide and install aluminum bend-down type edge guard and rubber /vinyl transition strips for direct glue down flooring where required.
4. CLEAN UP
4.1. Clean-up of the job site shall be the responsibility of the installation contractor. The contractor will perform a clean-up (sweep or vacuum) of all replaced surfaces when completed.
4.2 Waste generated by the Contractor in performance of the services specified with this SOW shall be disposed of by contractor off base in compliance with State and Local ordinances.
