STATEMENT OF WORK (SOW)
FOR
REPAIR OUTDOOR POOL
LINER
MALMSTROM AIR FORCE BASE
MONTANA
25 February 2018
SECTION 1
DESCRIPTION OF SERVICE
1. SUMMARY OF EXPECTATIONS
1.1. The Contractor shall provide all labor, hardware, equipment, and incidentals necessary to replace the Malmstrom Air Force Base pool liner. Currently, the pool liner is in poor condition.
Also, the concrete below the liner on the shallow end is in poor condition and cracking/crumbling. In order for the pool to open, the surface and liner need repaired/replaced.
The place of performance is Building 1020 (Base Pool) located on Rec Drive, Malmstrom AFB, Montana.
1.2. Work Requirements:
341 CES Structural Repair Shop will remove old unserviceable pool liner, ladders, drain covers, light rings and any other hardware prior to Contractor starting any repair/replacement services.
1. Contractor will remove old loose concrete in floor area of pool.
2. Contractor will blast clean all pool surfaces and clean up blasting material (sand, beads etc.).
3. Contractor will repair all cracks, low spots, voids and float unsmooth areas on low end of swimming pool (approx. 2000 sq ft) with a water resistant polymer in accordance with industry standards.
4. Contractor will apply a primer/sealer to entire pool surface (Approx. 6000 sq ft) prior to final top coat application.
5. Contractor will apply a poly iso elastomer coating on entire pool surface (Approx. 6000 Sq Ft).
6. Contractor will apply a top coat to entire pool surface (Approx. 6000 sq ft) in the color of swimming pool blue.
7. Contractor will clean up all debris and leave pool in manner it was found prior to repair/replacement of liner.
NOTE: Contractor will fabricate enclosure over entire pool in the event of inclement weather during pool liner installation.
1.3. Period of Performance: Work must be completed as soon as possible to allow for filling and chlorination of the pool for opening.
1.4. Warranty: Liner installer shall provide a 3-year full coverage warranty independent of standard manufacturer warranty. Liner installer shall respond and perform repairs within 3 days of contact from the government.
2. HOURS OF OPERATION
2.1. Normal Hours. The contractor shall perform all the routine services required under this contract during the following hours: 0700 through 1700 Monday through Friday, with the exception of Federal Holidays, or when the base is officially closed by the base commander.
2.2. Recognized Holidays. The contractor is not required to provide routine service on the following days: New Years Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. NOTE: Normally, if the holiday falls on Saturday, it is observed on Friday. If the holiday falls on Sunday, it is observed on Monday.
3. CONTRACTOR EMPLOYEES
3.1. Employee Certifications. Contractor shall have valid Contractor’s license with the State of Montana. General Contractors are required to have license or hire Sub-Contractors with valid Contractors license with the State of Montana Contractors Licensing Board.
4. ENVIRONMENTAL, SAFETY, AND HEALTH
4.1. Comply with the highest degree of environmental protection or safety practices where any disagreements between requirements exist.
4.2. Specific Environmental Requirements
4.3. Contractor Liability. The contractor shall be liable and responsible for all costs resulting from non-compliance, whether accidental or intended.
4.3.1. Hazardous Chemical Discharge. Any discharge or spills resulting from contractor negligence.
4.3.2. Non-Compliance with Environmental Regulations. Failure to follow any Federal, State, or local laws or regulations.
4.4. Fines and Corrective Actions. Any fines or corrective actions assessed by any regulatory or governing agency due to the actions or omissions of the contractor in carrying out his duties.
5. MISHAP NOTIFICATION AND REPORTING. The contractor shall immediately report any and all injuries and damage involving contractor, Air Force civilian or military personnel to the QAE and the Wing Safety Office.
5.1. Cooperate fully and assist government personnel in any government investigation of the mishap, until released by the mishap investigative authority through the CO. Safety personnel shall have the right, at any time, to investigate and report all mishaps involving Air Force personnel and property. The CO and contractor Safety representative may accompany Air Force mishap investigation personnel but may not release information regarding the mishap without written approval of the AF Safety investigating official or senior installation commander.
5.2. Maintain records of contractor employee’s occupational injuries, illnesses, and other related activities as required.
6. FIRE PREVENTION. Establish and implement procedures to ensure careful handling and storage of flammable and combustible products IAW National Fire Protection Association (NFPA) Standard 30 and National Fire Codes.
7. HOUSEKEEPING AND CUSTODIAL SERVICES. The contractor must maintain the contractor assigned areas in a clean, safe, and sanitary condition. Remove trash from all the work areas and store all flammables in an approved flammable storage container and ensure the area is left in an orderly manner at the end of the duty day.
7.1. Sanitary Facilities. The Contractor’s employees may use the nearest sanitary facilities.
The Contractor shall not use facilities in controlled or restricted areas IAW security requirements.
7.2. Refuse Collection. Waste generated by the Contractor in performance of the services specified with this PWS shall be disposed of by contractor off base in compliance with State and Local ordinances, including hazardous materials in which proper disposal instructions shall be complied with. All trash, debris, and refuse caused by the job shall be removed and disposed and shall be deposited in an approved receptacle or container furnished by the Contractor. The job site shall be cleaned at the end of each workday.
8. ADMINISTRATIVE RECORDS. All records, documents, and associated papers provided by the government and generated during the period of this contract become government property and will remain in place upon contract termination or completion. During the period of the contract, records, documents, and associated papers shall be available for review.
9. INSPECTION. The 341st Civil Engineering Squadron will provide a Quality Assurance Evaluator (QAE) to verify accomplishment of work. The QAE will be the spokesman for compliance with base standards. Controversies between the QAE and the Contractor will be resolved by the Contracting Officer. If, for some reason, a change in the primary or alternate QAE is required, the Contractor will be notified in writing by the Contracting Officer. The Contractor shall coordinate Contractor activities with the 341st Civil Engineering Squadron.
9.1. Final Inspection. The Contractor will notify the QAE of their estimated work completion date at least 3 days in advance, to verify accomplishment of work and receive contractor work report. Any deficiencies found shall be rectified by the Contractor and the Contractor shall inform the QAE upon completion. Final acceptance will be accomplished when all requirements are met and punch list items are completed, if necessary.
