SAMPLES TESTING BLANKET PURCHASE AGREEMENT (BPA)

WHITEMAN AIR FORCE BASE, MO

STATEMENT OF WORK

1 April 2014

1. DESCRIPTION OF AGREEMENT:

SERVICES NON-PERSONAL: The Contractor shall furnish all labor, equipment, tools, materials and facilities to analyze government-furnished soil, water, non-hazardous liquids and solids and hazardous waste samples for Whiteman Air Force Base, Missouri provided to the

Contractor for the period 15 June 2014 through 14 June 2019.

2. SCOPE:

a. All plant, labor, and equipment necessary to accomplish analysis on soil, water, non-hazardous liquids/solids and hazardous waste. This includes transporting, testing, recording and reporting. All samples are collected from the Government owned and operated Waste Water

Treatment Plant (WWTP) and Water Treatment Plant (WTP).

b. Laboratory shall follow the EPA Contract Laboratory Program protocol. Standard testing methods shall be followed as outlines in current applicable EPA publications.

c. Contractor shall provide all sample containers, shipping containers, instructions and chain of custody forms. Contractor shall provide pre-paid shipping labels. Contractor shall provide courier on-site pickup and delivery of sample(s) when/as required for the specific test sample. The Contractor shall provide local pickup for the samples with a short holding time.

Use of commercial carrier for all other samples is acceptable. Sample containers must be compatible with the sample and analysis to be performed.

d. Sample supplies may be provided in advance with prior coordination/approval of the

WWTP Manager.

e. Laboratory shall retain sample materials for a period of 30 days and then dispose of the samples properly.

f. Reporting shall follow standard laboratory reporting format unless otherwise agreed upon by both parties. Request all results be provided via electronic mail, with written report to follow. E-mail address will be provided upon award. All written reports shall be sent to:

3. GOVERNMENT:

a. Will perform all collecting and gathering of samples.

b. Will seal and package sample in the Contractor provided containers.

4. EXTENT OF OBLIGATION: The Government is obligated only to the extent of an authorized call actually placed against the Blanket Purchase Agreement (BPA).

5. PRICING:

a. Prices to the Government shall be as low as those charged to the Contractor’s most favored customer for comparable quantities under similar conditions, in addition to any discounts for prompt payment.

b. The Contractor shall furnish price lists at the time of acceptance with beginning effective dates to the Contracting Officer.

c. The Contractor shall furnish price lists whenever requested by the Contracting Officer.

d. The BPA number shall be shown on all price lists.

e. Price lists are to remain relatively firm with consideration given to market and labor trends.

f. All price lists will be approved by the Contracting Officer prior to being utilized with this BPA.

g. If a test is not listed on the “Approved” price list, the Contractor shall submit an updated price list to the Contracting Officer for approval before that test is accomplished.

6. CALL LIMITATION: No individual call under this agreement shall exceed $2,500.00.

7. NOTICE OF INDIVIDUAL AUTHORIZED TO PLACE CALLS: A separate letter will be issued listing the positions and/or individuals authorized to place calls under this agreement.

8. DELIVERY TICKET: All services accepted under this agreement shall be accompanied by two copies of the sales ticket which shall contain the following minimum information:

a. Name of Supplier

b. BPA Number

c. Date of Call

d. Call Number

e. Name of Individual Placing the Call

f. Itemized List of Services Furnished

g. Quantity, Unit Price and Extension of Each Item

h. Date Service was Furnished

9. INVOICES: Summary itemized invoices shall be submitted at least monthly for all services furnished during a billing period and for which payment has not been received. The invoice shall identify the delivery tickets (call number) covered, stating their total value and supported by receipted copies of the delivery tickets.

10. PAYMENTS: All payments will be made by via the Government Purchase Card (GPC), a

VISA card.

11. TERMINATION OF AGREEMENT: The BPA may be unilaterally terminated at any time with a 30 days notice by either party.

12. SUBCONTRACTING: Sub-contractor for this requirement is acceptable. The use of a sub-contractor shall be identified in advance and the Contractor shall not subcontract without prior written approval of the Contracting Officer. The facilitates of any approved subcontractor shall meet the minimum standards required by this BPA.

13. NUMBERING OF BPA CALLS: Calls will be numbered consecutively beginning with

0001 and continue in numerical sequence throughout the term of this agreement.

14. TRANSPORTATION COSTS: All transportation costs will be the responsibility of the

Contractor.

15. REQUIRED TESTS: Frequency and quantities of tests are estimated and subject to change.
