SUMMARY OF CHANGES

The purpose of this Amendment is to provide answers to the following questions:

Q1: If this is not a brand-new contract, what is the previous contract number?

A1: This solicitation is to reprocure mortuary services. The current contract is FA462019DA003. A new contract resulting from this solicitation will be awarded.

Q2: Is there a distance preference of funeral home that you require? Is there a distance restriction?

30, 60 miles out?

A2: There is no distance restriction. There is no distance preference for funeral home. The families will be choosing the location, which could be overseas for burial.

Q3: Is there an estimated amount of services for the year?

A3: Actual number of services needed is unknown. We estimate to utilize services for one deceased member annually. This estimate is the average of services ordered/paid for in the past 5 years.

Q4: For the emblems, will the government provide these or is it our responsibility to find someone to create the emblems?

A4: Emblem will be provided by contractor. See PWS, Section 10.7.

Q5: What is the preferred method of the emblems? Engraving or an actual piece?

A5: The emblem is an actual piece. The emblem shall be affixed to the face. See PWS Section 10.7 and figure 10.7.

Q6: In the pricing sheet, what does LOT mean? Is that for the year, all services, or per person? Can you please clarify what LOT includes?

A6: LOT is in reference to mortuary services performed for one deceased member. The pricing shchedule is pre-calculated to auto populate the areas highlighted in blue i.e the areas with Unit=LOT. Fill out the areas highlighted in yellow ONLY. The areas highlighted in blue will auto-populate.

Q7: Are we able to have a special fee that’s applied when the decedent is able to be viewed, but through intense restorations? When multiple gunshot wounds, or explosion wounds/amputations are present it is still possible for an open casket viewing or private viewing with family only. Though that isn’t always the case and a closed casket may be heavily suggested, certain cases can have a good outcome to view, but only with hours of work that come with an extra fee.

A7: No. The contract will be a fixed firm priced (FFP). Thus, no additional fee will be paid outside the fixed firm rates. All additional costs should be included in the unit prices you put on the pricing schedule. After your completed price schedule is evaluated and awarded, prices will be fixed and shall not be subject to adjustment based on contractor’s performance cost.

Q8: Regarding transportation, will we be responsible for getting the deceased from place of passing overseas or anywhere that isn’t in Washington? Will we be picking up directly from the military base and transporting to the mortuary where the preparations are held?

A8: Section 6.5 states, “Remains will be transported in a funeral coach or other appropriate vehicle authorized by the Military Service Casualty and Mortuary Affairs Office to a place designated by the PADD (Person Authorized Direct Disposition) or PAED (Person with Authority to Effect Disposition).” This may include delivery directly to a common carrier, the receiving funeral home, or a government or nongovernment cemetery.” Funeral home would not be responsible for getting deceased member from overseas location. Funeral home would be responsible for transportation of remains from Spokane County Coroner for processing at funeral home, followed by working overland transportation to location designated by PADD or PAED via common carrier.

Q9: Will services for the deceased be held in Washington only, or would they be held where the decedents family is or the hometown where they came from originally? I didn’t really see anything regarding a visitation, funeral service or graveside. Would we strictly be handling the preparations, viewing if able, and cremation if that’s what chosen?

A9: Contractor will not be responsible for visitation, funeral service or graveside events. Contract is for mortuary services only. Section 6.2.1 states, “The preparing contracted facility representative will obtain the necessary permits, licenses, and documentation, issued by the appropriate authority, to remove inter, cremate, or transport remains (e.g., burial/transit permits, a medical examiner cremation authorization certificate, or a cremation certificate)”.

Q10: The emblem dimension is noted as 3 inches. We found the bronze urn that has the 3-inch emblem. However, the emblem on the wooden urn is only 2.5 inches. Would that be okay?

A10: No. Emblem must be 3 inches. Section 10 states, “Commercial Description of Items. This establishes minimum standards for caskets. Shipping containers, urns and emblems. They will meet or exceed the American Funeral Industry’s standards with the following specific requirements for the American Armed Forces”.

Q11: Will there be a full service or family viewing in Washington, or will that be in the soldier's hometown?

A11: See response A9 above.

Q12: Can I get clarification on the numerous ID #'s associated with this quote request. (The main ID ending in 301, then there's one ending in 300, and finally on the pricing schedule ending in 108)?

A12: The request for quotation (RFQ) PIID is FA462024QA301. The sources sought (market research) PIID associated to this RFQ in SAM.gov is FA462024QA300. PIID FA462024QA108 is internal and was used during the requirement development phase. Sorry for any confusion. The RFQ number, FA462024QA301, shall be used to reference any written quote provided under this RFQ.

Q13: Will the contractor be utilizing their own facility, the government's facility or both?

A13: All work associated within the contract shall take place at the contractor’s facility. Fairchild AFB does not have a mortuary facility.

Fill out blocks 15a, 15b, and 15c and provide a copy with your proposal.
