STATEMENT OF WORK
1. GENERAL
1.1. SCOPE OF WORK. The Contractor shall provide services necessary to collect, package, and transport medical waste from the 92d Medical Group, Fairchild AFB, Washington to a treatment and disposal facility licensed under appropriate state law to accept medical waste.
1.2. PERSONEL.
1.2.1. SECURITY REQUIREMENTS. Personnel shall obtain a Fairchild Form 225 (Request for Issuance of Base Pass) from the Medical Contracts Office (Building 9000, Room 039A) and shall comply with all base regulations regarding operation of a motor vehicle.
1.3. HOURS OF OPERATIONS. The contractor shall pick up medical waste 1 time per week at a time mutually agreed upon by both parties.
2. GOVERNMENT FURNISHED PROPERTY & SERVICES
2.1. The government will furnish, without cost, building 9000, room 035 (located in the basement) to store contractor furnished containers.
2.2. The 92d Medical Group’s contract housekeeping personnel will package all medical waste materials in accordance with the contractor’s procedures.
3. CONTRACTOR FURNISHED ITEMS AND SERVICES
3.1. GENERAL. The contractor shall furnish all necessary containers, bags and labels required for the accumulation of medical waste products. The contractor shall also provide shipping manifest documents as outlined in Paragraph 4.
3.2. CONTAINER REQUIREMENTS. The Contractor provided containers/bags must be, leak resistant, and impervious to moisture and have sufficient strength to prevent tearing or bursting under normal conditions of use and handling. Rigid containers shall be lined with, at a minimum, three (3) mil red bag for an inner liner. The Contractor shall provide only 40-gallon containers and appropriate sized bags.
3.3. The Contractor shall provide the state of Washington Utilities and Transportation Commission certification “For the Operation of Motor Propelled Vehicles” stating the Contractor has authority to haul Medical Waste for the purpose of disposal.
3.4. The Contractor shall provide a business license from the City of Airway Heights and the City of Spokane, Washington.
3.5. The Contractor shall provide certification of liability insurance with a minimum of $1,000,000.00 combined single limit (CSL).
4. SPECIFICS TASKS
4.1. The Contractor shall provide for the pick up, transportation and disposal of medical waste from the 92d Medical Group, Fairchild AFB, WA. The Contractor shall remove the Contractor provided containers from the 92d Medical Group, place them into a licensed transport vehicle and transport the waste to a treatment facility holding an operating license. The point of collection of medical waste shall be Fairchild AFB, WA, building 9000 (Medical Clinic), room 035 (basement).
4.2. The Contractor shall complete a shipping manifest for each shipment of Medical Waste. The shipping manifest shall indicate the following as a minimum:
a. Transfer manifest number
b. Total number of 40 gallon (tubs/containers) picked up
c. Facility number
d. Account number
e. Location of pick up
f. Certification to verify that the above-named materials are properly classified, described, packaged, marked, labeled, and in proper condition for transportation according to applicable regulations of the state of Washington Utilities and Transportation Commission
g. Address and phone number of transporter
h. Signature of both the contract employee (driver) and the housekeeping
Contract Manager or his/her designee verifying the total number of 40 gallon containers
4.3. The Contractor shall provide a copy of the manifest to the housekeeper who verified the manifest upon pick up.
4.4. The Contractor shall return the original manifest to the 92d Medical Group/SGSM, 701 Hospital Loop, Suite 039A, Fairchild AFB, WA 99011-8701 upon completion of the disposal to verify destruction of the transported material.
4.5. The Contractor shall transport the medical waste to an approved medical waste treatment facility and treat it to make it noninfectious and/or unrecognizable, as appropriate. Treatment shall include any method certified by the appropriate state/local environmental regulatory agency to render the waste noninfectious.
4.6. The Contractor shall send a summary monthly invoice to the address in paragraph 4.4.
4.7. The Contractor shall maintain copies of all official invoices, shipping manifest, and related documentation. Copies of any documentation relating to the 92d Medical Group shall be made available to the 92d Medical Group upon request within 72 hours.
5. PAYMENT
5.1. The 92d Medical Group will make payment for services provided on a per container (40 gallon tub) via Wide Area Workflow (WAWF). This payment will constitute full compensation for all operations covered by the statement of work (SOW) including disposal fees at the disposal facility.
