STATEMENT OF WORK
FITNESS CENTER SECURITY & SURVEILLANCE EQUIPMENT
MAINTENANCE SERVICE
1. DESCRIPTION OF SERVICES. The contractor shall perform preventive maintenance, repair and replacement of parts on an as-needed basis for the Fitness & Sports Center, 300 W. Castle Street, Bldg.
2379, Fairchild AFB WA, 99011. The contractor must meet certification and licensure requirements deemed necessary by standard commercial practice, and submit qualifying documentation proving they are qualified.
2. GENERAL INFORMATION:
2.1. HOURS OF OPERATION. Fairchild AFB normal hours of operation is Monday-Friday, 7:00 a.m. to 8:30 p.m., excluding Federal Holidays and Family Days where the hours are 7:00 a.m.-2:30 p.m. The following federal holidays are observed (note: Unstaffed on Thanksgiving and Christmas):

3. SCOPE OF WORK DEFINED. Preventive maintenance, repair or replacement of systems or parts will be performed in accordance with manufacturer guidelines and recommendations on a mutually agreeable schedule. Service calls for repairs will begin within 24 hours after receipt of a call.
3.1. All work shall be performed on-site unless prior approval is received from the Fitness & Sports Manager (FSM). If work has to be performed, Contractor shall remove all materials and debris and maintain the work area(s) clean and orderly.
3.2. Contractor will provide all tools and equipment necessary to perform the required maintenance and repairs.
3.3. Contractor shall provide all labor, materials, and transportation necessary to correct defective programs, applications including any reprogramming, replacement, and/or change out as may be necessary.
3.3.1. The equipment list is as follows:

4. MAINTENANCE SERVICE: Shall include, but is not limited to:
4.1. PARTS:
4.1.1. All materials and equipment incorporated into any work shall be new and, where not specified, of the most suitable grade of their respective kinds for their intended use, and all workmanship shall be acceptable.
4.1.2. Repair or replace damaged, worn, non- or improperly functioning, or missing parts or systems as necessary to ensure continuous function.
4.1.3. When the Contractor determines that materials, parts, or supplies are required they shall notify the FSM of the vendor/source, part number or description quantity and unit cost.
4.2. LABOR:
4.2.1. Travel and labor should be based in accordance with commercial practices and applicable fees and charges. All cost presentations provided by the contractor shall also include Overhead Charges, General and Administrative Charges and Fees.
4.3. BI-ANNUAL INSPECTION:
4.3.1. All servers, workstations and peripherals, cameras, computer/servers, recording devices shall receive preventive maintenance servicing and cleaning twice per year comprised of equipment inspection, cleaning, lubricating, calibrating, refocusing, adjusting zoom levels, and such other tasks as required by the equipment manufacturer.
4.3.2. Check operation/inspect door system monitoring and administrative functions, door lock mechanism function, connectivity/wiring, and surveillance cameras and recording devices and function as necessary to ensure continuous function.
4.4. SERVICE CALLS
4.4.1. HelpDesk support will be available for the life of the contract.
4.4.2. Provide up to six on-site service calls per year. Calls called against this Statement of Work will only be placed between 9:00 a.m. – 5:00 p.m. weekly. Contractor must be available to receive service calls during these hours.
4.4. WARRANTY
4.4.1. Labor Warranty: Contractor shall provide Standard Labor Warranty for completed work materials for twelve (12) months from the Project Completion Date.
4.4.2. Materials / Manufacturer’s Warranty: Contractor shall transfer applicable manufacturer’s warranty in full for the supplied materials.
5. CERTIFICATION. Contractor shall provide only certified/competent service technicians to perform any work in accordance with the manufacture’s guidelines and best commercial practices. In addition to standard commercial licensure and qualifications, the contractor shall be certified to perform work on Open Eye brand products and software.
6. CONTINUATION OF MISSION ESSENTIAL FUNCTIONS. IAW DoDI 1100.22 (Policy and Procedures for Determining Workforce Mix), and any applicable Air Force implementation, unless otherwise directed by an authorized Government representative, it is determined that services provided under this contract are not mission essential for performance during a crisis.
7. CONTRACTOR MANPOWER REPORTING. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for Recycling Service via a secure data collection site. The contractor is required to completely fill in all required data fields at http://www.ecmra.mil. Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the CMRA help desk.
7.1. Uses and Safeguarding of Information: Information from the secure web site is considered to be Proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.
7.2. User Manuals. Data for Air Force service requirements must be input at the Air Force CMRA link.
