SUBJECT: FA4608-19-R-A003 COCESS Solicitation, Past Performance Questionnaire

1. The 2nd Contracting Squadron, Barksdale Air Force Base, Louisiana, Air Force Global Strike

Command is in the process of selecting a contractor for a Contractor Operated Civil Engineer

Supply Store (COCESS) contract.

2. One of the considerations in proposal evaluation is the verification of the offeror’s past and present performance on contracts, which reflect the offeror's ability to perform on the proposed effort. We depend on information received from agencies such as yours, which have had firsthand experience with an offeror, for the evaluation of the offeror's performance on those contracts.

a. Our areas of interest in the offeror are summarized in the enclosed questionnaire. Our schedule is extremely tight and we need your written response no later than 20 March 2019 at 3 p.m. CST to this office. This schedule will allow us sufficient time to analyze the data prior to evaluation.

b. To assist you in preparing your response and expediting your reply, the questionnaire may be filled out by hand and faxed to 318-456-2629 to the attention of MSgt Ryan Haselhuhn or email to ryan.haselhuhn@us.af.mil.

c. Please contact the above POC at (318) 456-1353 prior to transmission or if you have any questions. Your completed questionnaire will become a part of the official Source Selection records.

3. Your prompt attention to this questionnaire will be greatly appreciated and will be one of the keys to the successful and timely completion of this Source Selection.

CODE PERFORMANCE LEVEL

6 BLUE/EXCEPTIONAL - During the contract period, contractor performance is meeting (or met) all contract requirements and consistently exceeding (or exceeded) many. Very few, if any, minor problems encountered. Contractor took immediate and effective corrective action.

5 PURPLE/VERY GOOD - During the contract period, contractor is meeting (or met) all contract requirements and consistently exceeding (or exceeded) some. Some minor problems encountered. Contractor took timely corrective action.

4 GREEN/SATISFACTORY – During the contract period, contractor performance is meeting (or met) all contract requirements. For any problems encountered, contractor took effective corrective action.

3 YELLOW/MARGINAL – During the contract period, contractor performance is not meeting (or did not meet) some contract requirements. For problems encountered, corrective action appeared only marginally effective, not effective, or not fully implemented. Customer involvement was required.

2 RED/UNSATISFACTORY – During the contract period, contractor performance is failing (or failed) to meet most contract requirements. Serious problems encountered. Corrective actions were either ineffective or non- existent. Extensive Customer oversight and involvement was required.

1 WHITE/NOT APPLICABLE - Unable to provide a rating. Contract did not include performance for this aspect. Do not know.
