PERFORMANCE WORK STATEMENT (PWS)

FOR

Grease Traps Barksdale AFB

8/7/2017

1. DESCRIPTION OF SERVICES

1.1. Scope: This is a performance-based services contract for non-personal services. None of the functions to be performed are inherently governmental, in accordance with (IAW) FAR 7.503. The contractor shall provide all management, tools, supplies, equipment, and labor necessary to maintain grease traps located at Barksdale AFB. Work shall include using vacuum pumping and recharging traps. Work shall comply with government and commercial standards.

1.2. Place of Performance: Barksdale AFB, LA. See Attachment 1 for building numbers.

1.3. Period of Performance: The period of performance for the required services shall be one base year plus four (4) one-year option periods.

1.4. Standards: The following list of federal regulations pertaining to liquid wastes handling and disposal.

· Resource Conservation and Recovery Act (as amended by the Hazardous and Solid Waste amendments of 1984), subtitle C, “Hazardous Waste Management.”

· Oil Pollution Act of 1990, Section 1001.

· The Clean Water Act, Title 3, “Standards and Enforcement”, and Title 4, “Permits and Licenses”.

· Title 40 CFR 260-282, “Hazardous Waste”

· Title 40 CFR Parts 104-149 “Water Programs”.

· Title 40 CFR Part 110, “Discharge of Oil”.

· Title 40 CFR Part 112, “Oil Pollution Prevention”.

· Title 40 CFR Part 401-471, “Effluent Guidelines and Standards”.

1.5. SPECIFIC REQUIREMENTS:

1.5.1. COR will email pump order for current month services to Contractor no later than 5th day of each month (Attachment 3).

1.5.1.1 Contractor shall email a schedule of service to COR within 5 business days of receipt of Pump Order.

1.5.2. The COR with the approval from CO can add or delete Grease Traps based on facility occupation or lack of, construction/renovation and/or Government needs.

1.5.3. Pump out all grease and residues from trap; remove waste build up from trap or pit walls and all associated equipment within the pit.

1.5.4. Power wash walls with water and pump out remaining residue as required by the unique configuration of the grease trap.

1.5.4.1. Obstructions must be cleared at the inlet and outlet pipes to ensure unrestricted flow. When finished, recharge the unit with clean water prior to receiving wastewater. This level should be at least as high as the grease baffle or effluent invert.

1.5.4.2. The Contractor shall be responsible for any leaks or spills from equipment while cleaning the trap.

1.5.4.3. In the event of a spill, the Contractor shall immediately call 911, ask for Barksdale AFB Fire Department and give all pertinent information. Thereafter, the contractor shall notify the COR.

1.5.5. Transport all materials from grease traps to an approved off-base facility for disposal according to City, State and Federal Regulations

1.5.5.1. The contractor shall comply with all applicable city, state, and federal ordinances and laws.

1.5.5.2. The Contractor shall email documentation of proper disposal of waste within 5 days of service to the COR.

2.0. GENERAL INFORMATION

2.1. Hours of Operation: Normal service hours are Monday through Friday from 0730 to 1630 or as facility needs dictates below.

2.1.1. Red River Dining Facility shall only be cleaned between the hours of 0900 – 1100 or 1400-1600. Operating outside of these hours is unacceptable.

2.1.2. The contractor is not required to provide services on Federal Holidays or base down days (e.g. AFGSC Family Days). The contractor shall not be required to perform services when the base is delayed or closed due to inclement weather.

2.1.3. Federal Holidays/AFGSC Family Days: The contractor is not required to provide services on Federal Holidays or designated AFGSC Family Days. Those listed with an asterisk (*) may also have an additional corresponding AFGSC Family Day:

- *New Year’s Day- *Labor Day
- Martin Luther King Jr.’s Birthday- Columbus Day
- President’s Day- Veterans Day
- *Memorial Day- *Thanksgiving Day
- *Independence Day- *Christmas Day

2.2. Emergency Services: On occasion, services may be required to support an activation or exercise of contingency plans outside the normal duty hours described above. The contractor will be notified if this becomes necessary.

2.3. Contracting Officer’s Representative: A Contracting Officers Representative (COR) will be assigned to this contract. The COR does not have the authority to obligate the government; only the Contracting Officer can do so. The COR will inspect and accept the services rendered under this contract.

2.4. Base Access Cards: All contractor personnel shall obtain appropriate base access cards for entrance onto the base. If the requirements call for Air Force computer or network access, a Common Access Card (CAC) is necessary. ID card/CAC processing will be done by the unit Trusted Agent.

2.5. Contractor Identification: All contractor personnel shall identify appropriately in accordance with DFARS 211.106. This includes visible identification, identification in workspace (i.e. name placard), identification on telephone conversations and in formal and informal written correspondence (i.e email signature blocks).

2.6. Contractor Vehicles: Each contractor vehicle shall have the company name/logo prominently displayed on each side of panel of the vehicle whenever vehicle is on Barksdale AFB property.

3.0. RECORDS MANAGEMENT. The Contractor shall manage all data created for Government use or legally controlled by the Government, in support of the functional activity or required by AF publication, IAW with the AF records management procedures in Air Force Instruction (AFI) 33 - 322, Records Management Program, paragraph 10, Air Force Manual (AFMAN) 37-123,paragraph 7.4, Air Force Manual (AFMAN) 37-138, paragraph 2.13, and the Air Force Records Disposition Schedule (AF RDS) located at https://afrims.amc.af.mil/rds_series.cfm or http://www.amc.af.mil/shared/media/document/AFD-060803-003.pdf “

4.0. AIR FORCE ANTITERRORISM (AT) PROGRAM

The AT program seeks to deter or limit the effects of terrorist acts against the AF by giving guidance on collecting and disseminating timely threat information, providing training to all AF members, developing comprehensive plans to deter and counter terrorist incidents, allocating funds and personnel and implementing AT measures. In accordance with AFI 10-245 paragraphs 2.18 and paragraph 2.25, DOD contractors and sub-contractor personnel shall be offered Level 1 - AT Awareness Training. AT Awareness Training may be accomplished by either a Level I - qualified instructor, or by completing the "Force Protection" computer based training (CBT) course on the Advanced Distributed Learning System (ADLS) https://golearn.csd.disa.mil/kc/login/login.asp or at https://jkodirect.jten.mil for individuals without access to NIPR, possessing a CAC card, nor enrolled in DEERS. Compliance with the training will be monitored by the contracting officers representative.

5.0. CONTRACTOR FULL-TIME EQUIVALENT REPORTING.

The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the [NAMED COMPONENT] via a secure data collection site. The contractor is required to completely fill in all required data fields at https://afcmra.dmdc.osd.mil/default.aspx?ReturnUrl=%2f.

Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the CMRA help desk.

5.1. Reporting Period: Contractors are required to input data by 31 October of each year.

5.2. Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.

5.3. User Manuals: Data for Air Force service requirements must be input at the Air Force CMRA link. However, user manuals for government personnel and contractors are available at the Army CMRA link at http://www.ecmra.mil.

6.0. SECURITY REQUIREMENTS

6.1. For either the site visit or submission of proposals, all personnel requesting access to Barksdale Air Force Base must possess a valid State or Government picture identification card. Furthermore, individuals presenting identification cards from a state that is non-compliant with the REAL ID Act will require additional documentation to gain unescorted base access. Additional documentation include a valid U.S. or foreign government issued passport, an employment authorization document that contains a photograph, or identification cards issued by federal state or local government agencies that include a photo and biographic information. A full list of REAL ID Act compliant and non-complaint states can be found at https://www.dhs.gov/current-status-states-territories. Personnel requesting vehicle access to Barksdale Air Force Base must provide a valid driver's license, current vehicle registration, and valid vehicle insurance.

6.2. Physical Security: The contractor shall be responsible for safeguarding any government property provided for use.

6.3. Pass and Identification Items: The contractor shall ensure that pass and identification items required for contract performance are obtained for contractor employees and non-Government owned vehicles (as applicable.)

6.4. Traffic Regulations: Contractor employees shall comply with all base traffic regulations, and are subject to random vehicle speed control checks. Contractor personnel cited for traffic violations on the installation may suffer loss of base driving privileges, debarment from the base, or other administrative action. The use of cell phones, except hands-free phones, is prohibited while driving on military bases.

6.4.1. Random Personnel and Vehicle Searches: Contractor personnel are subject to random personnel and vehicle searches. If contractor personnel refuse to be searched, they will be denied entry to the base, and may result in loss of base driving privileges, debarment from the base, or other administrative action.

6.4.2. Weapons, Firearms, And Ammunition: Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within contractor-owned vehicles or privately owned vehicles while on military bases.

6.5. Reporting Requirements: Contractor personnel shall immediately report any information or circumstances of which they are aware may pose a threat to the security of Department of Defense personnel, contractor personnel, resources, and classified or unclassified defense information to an appropriate Government authority.

6.6. Cell Phone Usage: The use of cell phones will be in accordance with posted directives on the government installation.

6.7. Security Clearance Requirements: Reserved

6.8. Installation Perimeter Access Control: Contractors shall return all local access credentials to the issuing office when the contract is completed or when a contractor employee no longer requires access to the installation (e.g., quits or the contract is terminated). Immediately send notification to Security Forces for an appropriate update in Defense Biometric Identification System (DBIDS) when a contractor refuses to return issued access media. If a local access credential is not returned, the Installation Commander may consider debarment. If debarment occurs, promptly notify the applicable contracting officer and/or civilian personnel office.

6.9. Surveillance Methods: Surveillance will be accomplished by the COR on a monthly basis using the methods in accordance with (IAW) the Service Summary found in the QASP.

8.0. DEFINITIONS

Contracting Officer (CO): A person with the authority to enter into, administer, and/or terminate contracts and make related determinations and findings. The term includes certain authorized representatives of the contracting officer acting within the limits of their authority as delegated by the contracting officer.

Contracting Officer Representative (COR): An individual, including a contracting officer’s technical representative (COTR), designated and authorized in writing by the contracting officer to perform specific technical or administrative functions.
