FOR

AIRMAN AND FAMILY READINESS CENTER

INFORMATION AND REFERRAL ASSISTANT

Dated 12 August 2010

1.0 SCOPE OF WORK: The Contractor shall provide program support, information and referral services for the Barksdale AFB Airman and Family Readiness Center(A&FRC). This project requires knowledge of military family support programming and extensive knowledge, skills, and abilities associated with this Statement of Work. The contractor shall supply all services contained in this Statement of Work, but not limited to only those areas identified.

2.0 HOURS OF WORK: Services will be performed Monday through Friday from 0730 – 1630 under the supervision of the Airman and Family Readiness Center Superintendent. Hours may be adjusted with prior approval from supervising staff in advance of work performance at which time there will be an adjustment to the duty day.

3.0 CONTRACTOR PERSONNEL

The Contractor shall provide a qualified employee with widespread knowledge of Airman and Family Readiness Center services and activities, customer service, and communication skills appropriate in a social service environment. Contractor must become familiar with all available services; and be able to provide Air Force and commercial social service program referral information to clients as applicable in a manner which in no way offends any member of the A&FRC clientele. Recognize and acknowledge clientele with a cognizance of hierarchy and protocol with reference to military rank, civilian grades, and Wing and Tenant Unit organizations. Contractor must be able to work with a diverse clientele.

4.0 DESCRIPTION OF SERVICES:

4.1 Contractor shall develop and update quarterly events calendar based on programs and classes offered at the A&FRC. Publicize activities/events weekly using established venues identified by the AFRC Director and/or Quality Assurance Personnel (QAP)

4.2 Contractor shall utilize Air Force Family Integrated Results Statistics Tracking System (AFFIRST) to maintain workshop schedules and register participants for all A&FRC classes, events and briefings and briefings.

4.3 Contractor shall schedule and maintain appointment calendars for eight A&FRC program activity areas: PFMP, AFAS, Relocation, Transition, Employment, Personal/Work life, Volunteer Resources, Family Readiness, and Community Events.

4.4 Contractor shall compile program binders and critiques in support of Airman and Family Readiness Center’s workshops and seminars. Distribute Personal Readiness Inventory forms to all customers in the absence of the Discovery Resource Center (DRC) Manager.

4.5 Contractor shall perform typing duties as required by A&FRC staff

4.6 Contractor shall attend any required meetings and take notes during the entire session.

4.7 Contractor personnel shall maintain control and distribute mail for all building occupants as directed by the A&FRC Director.

4.8 Contractor shall maintain accountability statistics for the Information and Referral services Document in AFFIRST statistical data for walk in customer, phone calls, emails and other statistical data relating to the eight core service areas. The contractor shall use this data to develop charts, logs, and summaries as directed by A&FRC Director/QAP.

4.9 Contractor shall greet and assess the needs of incoming clients/customers.

4.10 Contractor shall provide information and sign up sheets to clients pertaining to A&FRC Programs, local services and resources

4.11 Contractor must be familiar with the AFFIRST computer program.

4.12 Contractor shall respond to routine telephone requests; refers calls and visitors/clients/customers to Community Readiness Technicians, Consultants or other staff members as appropriate. In the event the Emergency Family Assistance Control Center (EFACC) or the Unit Control Centers (UCC) are activated (Exercise or Real World) the contractor shall post FPCON signs and track all matters related to the situation in the events log book.

4.13 Contractor must have a minimum of at least one year of related experience in a customer service environment, or 30 college accredited semester hours, or a combination of education and direct work experience.

4.14 Contractor must have practical knowledge of human resource programs, requirements and procedures.

4.15 Contractor must be familiar with using a personal computer and proficient with Microsoft computer software applications.

4.16 Contractor must have basic understanding of Human Resources, and the ability to recognize military rank structure and the ability to work effectively with diverse groups.

4.17 Contractor shall have knowledge of various office automation software programs, tools and techniques to support program support operations and produce a variety of documents, such as letters, reports, spreadsheets, databases and graphs.

4.18 Contractor shall have knowledge of correct grammar, spelling, punctuation, capitalization, and format to accurately prepare and edit written correspondence and reports. Knowledge of automated and/or physical filing systems and procedures and the subject matter content of the materials being processed.

4.19 Contractor shall have knowledge of format and clerical procedures to arrange a variety of materials from different sources. Skill in typing and working with a variety of office equipment is required. Ability to locate, assemble and compose information for routine reports inquiries and non-technical correspondence.

4.20 Contractor shall communicate effectively, both orally and in writing, using tact and courtesy. Must be able to read, write, speak, and understand English.

4.21 Contractor shall have the ability to plan, organize work and meet deadlines.

4.22 Contractor shall maintain work area in a neat and orderly manner; ensuring the area is conducive to a customer friendly environment.

4.23 Contractor shall provide augmentation to the Discovery Resource Manager assisting customers by making all A&FRC resources (pamphlets, internet sites, and books, magazine and A&FRC computer programs) available.

4.24 Contractor shall contact all workshop/seminar participants prior to class start date to confirm attendance.

4.25 Contractor shall assist in reproducing training/educational materials, sign-in sheets, critiques and building folders for the eight core program areas; PFMP, AFAS, Relocation, Transition, Employment, Personal/Work Life, Volunteer Resources, Family Readiness and Community Events.

4.26 Contractor shall assist in ensuring that all Random Access Measures (RAMS) are completed in accordance with monthly schedule.

4.27 The Contractor shall serve as the Functional Area Records Manager (FARM) for the A&FRC. The FARM serves as the point of contact for and maintaining the records management program of the A&FRC IAW AFI 33-364, AFI 33-311 and AFMAN 33.363. The FARM serves a liaison with the base Records Manager and guides/assists A&FRC staff in maintaining and disposing of records. The contractor shall prepare file maintenance and disposition plans for all A&FRC staff. The FARM will ensure A&FRC receives a SAV at least once every 24 months. Follow up with the Chief of the Office of Records (COR) on corrective actions for discrepancies noted during SAV. The FARM will ensure office of records promptly retire eligible records and maintain accountability of such records. The FARM shall schedule training for all personnel who create, maintain and dispose of records. The Farm will assist the RM in providing guidance and training to A&FRC personnel on using document imaging systems and electronic storage media for records. Evaluates the effect of document imaging and electronic storage on creating, transferring storage, retrieving and disposing of office records, Reports problems to the COR and RM.

5.0 DELIVERABLES

5.1 Provide A&FRC Manager a monthly client usage report in the set format not later than the fifth working day of the next month.

6.0

SECURITY REQUIREMENTS

6.1 Physical Security: Contractor shall be responsible for safeguarding all Government property provided for use. At the end of each workday, ensure Government facility, equipment and materials are secure.

6.2 Pass and Identification: Contractor shall obtain and display all Government and contractor identification as required. Contractor shall return all Government issued identification and vehicle passes at contract expiration or termination thereof.

6.3 Traffic Laws, Weapons, Firearms and Ammunition: Contractor shall comply with all base traffic regulations. Contractor employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their contractor-owned or privately owned vehicle while on Barksdale AFB.

6.4 Key Control: Contractor shall safeguard and ensure Government furnished keys are not lost, misplaced or used by unauthorized individuals. Contractor shall not duplicate any Government furnished key and shall report immediately to the A&FRC security monitor the loss thereof not later than the next workday. In the event keys other than master keys are lost or duplicated, the contractor may be required by written direction of the Contracting Officer, to re-key or replace the affected lock or locks at no cost to the Government. The Government may however, replace any affected lock(s) or perform re-keying and deduct such cost from the monthly payment due the contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the Government, and the total cost of replacement shall be deducted from the next monthly invoice submitted for payment by the contractor.

7.0

OTHER SIGNIFICANT QUALIFICATIONS

7.1 Contractor shall be competent in operating personal computer systems and Microsoft Office Products. Basic communication skills are essential for dealing with Airman and Family Readiness Center patrons.

7.2 Experience of a family separation due to deployment is highly desirable.

7.3 Contractor should have a basic understanding of military protocol, support group organizations, understanding of family systems in a military setting, and poses the ability to communicate and work effectively with a diverse population.

7.4 Contractor is required to dress appropriately for a professional atmosphere.

8.0

GOVERNMENT FURNISHED EQUIPMENT AND SERVICES

8.1 . The Government shall supply/furnish the necessary workspace, desk, personal computer, associated equipment and office supplies necessary for normal daily office functions. The contractor shall be responsible for safeguarding all government property provided for contractor use.

8.2 The Government shall provide telephone service for official use only. Government furnished phones will not be used for personal business. .

9.0

PRIVACY ACT:

9.1 Work on this contract may require that Contractor have access to Privacy Information. Contractor Personnel shall adhere to the Privacy Act, Title 5 of the U.S. Code, Section 552a, and applicable agency rules and guidance.

10.0

PERFORMANCE EVALUATION MEETING:

10.1 The contract manager may be required to meet with the Quality Assurance Personnel (QAP) and the contracting officer. Meetings shall be as often as necessary as determined by the contracting officer.

11.0

PERIOD OF PERFORMANCE:

11.1 The Government shall have the unilateral right to exercise option periods as specified in the Contract Schedule.

11.2 The Government may extend the term of this contract by written notice to the contractor within 30 days of the expiration of the existing period of performance, provided that the Government shall give the contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.

12.0

LOCATION:

12.1 Services shall be performed in the office of the Airman and Family Readiness Center, Barksdale Air Force Base, LA.

13.0

HOURS OF OPERATION:

13.1 NORMAL HOURS: The contractor shall perform the services required under this contract during the following hours: Monday through Friday, 0730 to 1630. Contractor will work a maximum of 40 hours per week. The contactor will not be paid and is not required to perform service on federal holidays, closure days for the A&FRC, and other planned closure days such as AFGSC Family Days.

13.2 FEDERAL HOLIDAYS: The contractor is not required to provide service on the following days: New Year’s Day, Martin Luther King, Jr. Day, President’s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran’s Day, Thanksgiving Day, and Christmas Day.

14.0

DEFINITIONS:

14.1 CLIENT: Any person including military, civilian, reservists, and retirees or their family members seeking services from the Airman and Family Readiness Center.

14.2 CONTRACTING OFFICER: A person with authority to enter into, administer, and/or terminate contracts, in addition to making related determinations and findings on behalf of the United States Government.

14.3 QUALITY ASSURANCE PERSONNEL (QAP): A person assigned by the Government to inspect and accept services.

52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. 252.232-7003 ELECTRONIC SUBMISSION OF PAYMENT REQUESTS AND RECEIVING REPORTS (MAR 2008)

(a) Definitions. As used in this clause--

(1) Contract financing payment and invoice payment have the meanings given in section 32.001 of the Federal Acquisition Regulation.

(2) Electronic form means any automated system that transmits information electronically from the initiating system to all affected systems. Facsimile, e-mail, and scanned documents are not acceptable electronic forms for submission of payment requests. However, scanned documents are acceptable when they are part of a submission of a payment request made using Wide Area WorkFlow (WAWF) or another electronic form authorized by the Contracting Officer.

(3) Payment request means any request for contract financing payment or invoice payment submitted by the Contractor under this contract.

(b) Except as provided in paragraph (c) of this clause, the Contractor shall submit payment requests and receiving reports using WAWF, in one of the following electronic formats that WAWF accepts: Electronic Data Interchange, Secure File Transfer Protocol, or World Wide Web input. Information regarding WAWF is available on the Internet at https://wawf.eb.mil/.

(c) The Contractor may submit a payment request and receiving report using other than WAWF only when--

(1) The Contracting Officer authorizes use of another electronic form. With such an authorization, the Contractor and the Contracting Officer shall agree to a plan, which shall include a timeline, specifying when the Contractor will transfer to WAWF;

(2) DoD is unable to receive a payment request or provide acceptance in electronic form;

(3) The Contracting Officer administering the contract for payment has determined, in writing, that electronic submission would be unduly burdensome to the Contractor. In such cases, the Contractor shall include a copy of the Contracting Officer's determination with each request for payment; or

(4) DoD makes payment for commercial transportation services provided under a Government rate tender or a contract for transportation services using a DoD-approved electronic third party payment system or other exempted vendor payment/invoicing system (e.g., PowerTrack, Transportation Financial Management System, and Cargo and Billing System).

(d) The Contractor shall submit any non-electronic payment requests using the method or methods specified in Section G of the contract.

(e) In addition to the requirements of this clause, the Contractor shall meet the requirements of the appropriate payment clauses in this contract when submitting payments requests.

OMBUDSMAN (AUG 2005)

(a) An ombudsman has been appointed to consider and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. The ombudsman may not serve as an advocate for any party.

(b) Before consulting with an ombudsman, concerned parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).

(d) The ombudsman has no authority to render a decision that binds the agency.

(e) Do not contact the ombudsman to request copies of the solicitation, verify offer due date, or clarify technical requirements. Such inquiries shall be directed to the Contracting Officer.

(End of clause) 5352.242-9000 CONTRACTOR ACCESS TO AIR FORCE INSTALLATIONS (Aug 2007)

(a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.

(b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or security police for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver’s license, current vehicle registration, valid vehicle insurance certificate, and Contractor Access Request Form to obtain a vehicle pass.

(c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.

(d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with local policy/Air Force Instructions as applicable citing the appropriate paragraphs as applicable.

(e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office.

(f) Failure to comply with these requirements may result in withholding of final payment.

5352.242-9001 COMMON ACCESS CARDS (CACs) FOR CONTRACTOR PERSONNEL (AUG 2004)

(a) For installation(s)/location(s) cited in the contract, contractors shall ensure Common Access Cards (CACs) are obtained by all contract or subcontract personnel who meet one or both of the following criteria:

(1) Require logical access to Department of Defense computer networks and systems in either:

(i) the unclassified environment; or

(ii) the classified environment where authorized by governing security directives.

(2) Perform work which requires the use of a CAC for installation entry control or physical access to facilities and buildings.

(b) Contractors and their personnel shall use the following procedures to obtain CACs:

(1) Contractors shall provide a listing of personnel authorized a CAC to the contracting officer. The contracting officer will provide a copy of the listing to the government representative in the local organization designated to authorize issuance of contractor CACs (i.e., “authorizing official”).

(2) Contractor personnel on the listing shall each complete and submit a DD Form 1172-2 or other authorized DoD electronic form to the authorizing official. The authorizing official will verify the applicant’s name against the contractor’s listing and return the DD Form 1172-2 to the contractor personnel.

(3) Contractor personnel will proceed to the nearest CAC issuance workstation (usually the local Military Personnel Flight (MPF) with the DD Form 1172-2 and appropriate documentation to support their identification and/or citizenship. The CAC issuance workstation will then issue the CAC.

(c) While visiting or performing work on installation(s)/location(s), contractor personnel shall wear or prominently display the CAC as required by the governing local policy.

(d) During the performance period of the contract, the contractor shall:

(1) Within 7 working days of any changes to the listing of the contract personnel authorized a CAC, provide an updated listing to the contracting officer who will provide the updated listing to the authorizing official;

(2) Return CACs in accordance with local policy/directives within 7 working days of a change in status for contractor personnel who no longer require logical or physical access;

(3) Return CACs in accordance with local policy/directives within 7 working days following a CACs expiration date; and

(4) Report lost or stolen CACs in accordance with local policy/directives.

(e) Within 7 working days following completion/termination of the contract, the contractor shall return all CACs issued to their personnel to the issuing office or the location specified by local policy/directives.

(f) Failure to comply with these requirements may result in withholding of final payment.

BOSSIER CITY -- PARISH SALES AND USE TAX

Bossier City and Bossier Parish levy a use tax defined by state and local sales tax law. As such, these sales are taxable at the point of delivery to the "contractor" (consumer). If the sales or use tax is not collected by the vendor, tax is payable directly to the jurisdiction in which delivery is made. Contractors and subcontractors are each individually liable for sales and use tax on the basis of their individual purchase transactions. This tax may extend to equipment leased or rented to the Government. It may also extend to supplies and equipment purchased elsewhere, but brought into Bossier Parish for final use. All contractors are cautioned to assure themselves of any liability for this tax. The local telephone number for questions concerning this tax is A/C 318 741-8551.

CONTRACTOR/VENDOR ACCESS TO BARKSDALE AFB

All commercial vehicles are required to enter Barksdale AFB through the East Gate, just off I-20 and Industrial Drive. This includes construction, food, supply, FedEX, and UPS deliveries, as well as contractors entering with company trucks. Contractors entering in their privately owned vehicles with an approved pass will be allowed entry at any gate. Hours of operation for the East Gate are 0500-2200 Monday through Friday, Saturday 0600-1200.

CELL PHONE POLICY

The use of cell phones while operating a moving vehicle on Barksdale AFB is prohibited, unless the phone is operated in a “hands free” mode. “Use of cell phones” includes placing calls, retrieving stored information from the phone, programming the phone, talking on the phone, and receiving calls, regardless of the source. Violators may be issued a citation and assessed 3 points on their base driving record. Additional points may be assessed if cell phone use is found to be a contributing factor in a motor vehicle accident.

HEALTH & WELFARE: $3.50 per hour or $140.00 per week or $606.67 per month

VACATION: 2 weeks paid vacation after 1 year of service with a contractor or successor; 3 weeks after 8 years, and 4 weeks after 15 years. Length of service includes the whole span of continuous service with the present contractor or successor, wherever employed, and with the predecessor contractors in the performance of similar work at the same Federal facility. HOLIDAYS: A minimum of ten paid holidays per year, New Year's Day, Martin Luther

King Jr's Birthday, Washington's Birthday, Memorial Day, Independence Day, Labor

Day, Columbus Day, Veterans' Day, Thanksgiving Day, and Christmas Day. (A contractor may substitute for any of the named holidays another day off with pay in accordance with a plan communicated to the employees involved.) (See 29 CFR 4174)

THE OCCUPATIONS WHICH HAVE NUMBERED FOOTNOTES IN PARENTHESES RECEIVE THE FOLLOWING:

1) COMPUTER EMPLOYEES: Under the SCA at section 8(b), this wage determination does not apply to any employee who individually qualifies as a bona fide executive, administrative, or professional employee as defined in 29 C.F.R. Part 541. Because most Computer System Analysts and Computer Programmers who are compensated at a rate not less than $27.63 (or on a salary or fee basis at a rate not less than $455 per week) an hour would likely qualify as exempt computer professionals, (29 C.F.R. 541.

400) wage rates may not be listed on this wage determination for all occupations within those job families. In addition, because this wage determination may not list a wage rate for some or all occupations within those job families if the survey data indicates that the prevailing wage rate for the occupation equals or exceeds

For example, if an individual employee is nonexempt but nevertheless performs duties within the scope of one of the Computer Systems Analyst or Computer Programmer occupations for which this wage determination does not specify an SCA wage rate, then the wage rate for that employee must be conformed in accordance with the conformance procedures described in the conformance note included on this wage determination.
