ADDENDUM TO FAR 52.212-1

ADDENDUM TO FAR 52.212-1, INSTRUCTIONS TO OFFERORS – COMMERCIAL ITEMS

1.0 PROPOSAL PREPARATION INSTRUCTIONS

To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein.

Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements. Failure to meet a requirement may result in an offer being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. Volume I – Price Proposal, Volume II – Technical Factor

2.0 Volume 1 – PRICE PROPOSAL

The Contracting Officer has determined there is a high probability of adequate price competition in this acquisition.

Upon examination of the initial offers, the Contracting Officer will review this determination and if, in the Contracting Officer’s opinion, adequate price competition exists no additional cost information will be requested.

However, if at any time during the evaluation process the Contracting Officer determines that adequate price competition no longer exists; offerors may be required to submit information to the extent necessary for the Contracting Officer to determine the reasonableness of price. Volume I shall contain the following:

2.1 SECTION B (BID SCHEDULE)

The offeror shall complete Attachment #4 (Bid Schedule) AND on section B starting on page 3 of this RFQ. For each performance period, which includes the base period plus four option periods, a unit price, rounded to two decimal places, shall be proposed and inserted for the Firm Fixed Price (FFP) Contract Line Item Numbers (CLINs). The extended amount for each CLIN must equal the unit price multiplied by the quantity of units.

2.2. SF 1449, REQUEST FOR QUOTES (RFQ)

Complete 17a, 26, 30a, 30b, 30c of the RFQ, SF 1449. In doing so, the offeror accedes to the contract terms and conditions as written in the RFQ. These sections constitute the model contract. Complete all necessary clause fill-ins in Representations, Certifications and Other Statements of Offerors. Ensure compliance with RFQ requirements using the annual representations and certifications electronically via the System of Award Management (SAM) website at http://www.sam.gov/portal/public/SAM.

3.0 Volume II – TECHNICAL FACTOR

Volume II shall be prepared in accordance with these instructions and will be evaluated in accordance with the evaluation criteria in FAR 52.212-2, Commercial Evaluations and its accompanying addendum. All Technical Factors will be evaluated on a pass/fail basis, assigning ratings of Acceptable or Unacceptable. The offeror’s proposal submission represents that the firm will perform all the requirements specified in the RFQ. The Technical Proposal shall not exceed 10 pages as defined in paragraph 5. Volume II shall contain the following:

3.1 SUBFACTOR – EXPERIENCE DEMONSTRATION (Acceptable/Unacceptable)

Provide documentation of AT LEAST FIVE (5) YEARS of experience in the following:

Commercial/Industrial Automation and Controls (Building automation, Energy usage Monitoring, Industrial Ventilation, Lighting Control, Access Control, System integration….)

Commercial/Industrial Electrical (Generator Installation/maintenance, UPS installation/Maintenance, Industrial Process Installation/Integration…..)

4.0 General Information – INFORMATION REGARDING SUBMISSION OF PROPOSAL

Proposals are due by 21 January 2016 @ 2:30PM CST. Send one electronic copy to SSgt San ha Guerrero at san_ha.guerrero@us.af.mil AND TSgt Joshua Malyemezian at joshua.malyemezian@us.af.mil. The entire proposal size is limited to 10MB.

The technical proposal shall be in either Microsoft Word or PDF format. Page limitations shall be treated as maximums. If exceeded, the excess pages will not be read or considered in the evaluation of the proposal and (for paper copies) will not be returned to the offeror. Excess pages will be destroyed along with all unsuccessful proposals. The RFQ, SF 1449, Bid Schedule, and any fill-in provisions/clauses will not be counted towards the overall page count. Each page shall be counted except cover pages, tables of contents, cross reference matrix, tabs, and glossaries.

Format size shall be letter paper, so if printed each page will print on an 8.5”x11” piece of paper. Pages shall be single-spaced. Except for the reproduced sections of the solicitation document, the text size shall be no less than 10 point. Use at least 1-inch margins on all sides of each page. Pages shall be numbered sequentially by volume.

**SITE VISIT INFORMATION**

1. An organized site visit has been scheduled for :

14 January 2016 at 09:00 AM CST Participants will meet at:

Kenny Gate Visitor Center 101 Nelson Drive Offutt AFB, NE 68113 (The address above is for the inside of Offutt Air Force Base, so please park outside in the Kenny’s Gate Visitor Center’s parking lot to await further transportation.)

(a) All participants must bring their own high visibility vests to wear during the site visit. Any participant that does not bring their own high visibility vest to the site visit will not be allowed to attend.

(b) All participants are required to meet and sign in. All participants will board transportation provided by the Government regardless of base access privileges.

(c) Please have all site visit information to SSgt San ha Guerrero by 03:00PM CST on 12 January 2016.

(d) Photographs are not authorized to be taken by participants. The Government will provide a camera and assign a representative to take photographs at the request of participants.

(e) Any person wanting to attend this site visit SHALL RSVP with SSgt San ha Guerrero by email to san_ha.guerrero@us.af.mil l. Any person who fails to RSVP will be excluded from the site visit. If there are any questions contact SSgt Guerrero by the same means listed above.

(f) Below is the required information to participate in the site visit. Contractors with current base access are not required to provide the below information, but must meet at the designated place and sign in.

(g) All questions generated during the site visit must be submitted in writing to the contracting specialist for an official response. Address, in writing, technical and contractual questions to the contract specialists, joshua.malyemezian@us.af.mil and san_ha.guerrero@us.af.mil by close of business (COB) no later than three (3) days after the site visit. If questions are received after that date, the government may or may not answer them.
