PERFORMANCE WORK STATEMENT FRONT END LOADER
Winter Rental Services December 2018
INTRODUCTION
The 91st Operations Group (91 OG) at Minot Air Force Base (MAFB) North Dakota requires the lease of sixteen
(16) front end loaders to be used to supplement the snow removal program for the MAFB Missile Complex (complex consists of multiple sites) and the 91 OG building on MAFB. The Contractor shall provide supplemental equipment for rental during the winter season based on the specification of items detailed below. Equipment operators are not required in the performance of this contract. Rental equipment shall be provided with an average expected use of 50 hours per month. The Government reserves the right to reduce said numbers of loaders when said original numbers are no longer needed during the performance period.
SCOPE
LEASE OF WHEELED FRONT END LOADER – Preferred during base period of the contract only (Mandatory for Option Years 1 and 2)
The Contractor shall provide sixteen (16) front end loaders weighing approximately 13,000 pounds with a lift capacity of approximately 8,000 pounds and with a compatible bolt-on edge snow bucket of between 84 and 101 inches or between 1.5 to 3 cubic yards. Snow bucket shall have a footprint wider than the loader and a cutting front edge for dealing with compacted ice and snow.
LEASE OF WHEELED FRONT END LOADER – Acceptable during base period of the contract only
The Contractor shall provide sixteen (16) front end loaders weighing up to approximately 26,000 pounds with a lift capacity up to approximately 16,000 pounds and with a compatible bolt on edge snow bucket between 84 and 101 inches or between 1.5 to 3 cubic yards. Snow bucket shall have a footprint wider than the loader and a cutting front edge for dealing with compacted ice and snow.
All loaders and buckets shall be of the same size, and with the following characteristics:
OPERATOR ENVIRONMENT:
· Cab with heat and air conditioning
· Key start
· Articulated power steering
· Fully adjustable, suspension seat
· Foot throttle
· External rearview mirrors
· Defroster
· Side window, partial/fully open
· Dome light
· Wipers, rear and intermittent front
· Windshield washers
· Horn
· Warning lights and gauges: parking brake, coolant temperature gauge, hydraulic oil temperature gauge, transmission oil temperature gauge, air filter, transmission filter, alternator, low fuel indicator, etc.
· Backup alarm
DRIVETRAIN:
· 4-wheel drive
Each wheeled loader shall be delivered in a clean condition and with a full tank of diesel fuel, all fluids topped off, and all safety markers and beacons required for snow removal applied. Equipment shall be winterized to operate in sub-zero weather down to -40 degrees Fahrenheit with long periods of inactivity (days to weeks). The Contractor shall perform all delivery and recovery actions when on-loading or off-loading the loaders from Contractor-controlled trailer equipment. The Contractor shall provide all equipment needed in the transport of the loaders to and from the missile complex and Minot AFB to include restraints used to secure the equipment to the trailers.
Period of Performance: The period of performance shall be for one (1) base year and two (2) option years. The base year and option year periods shall consist of up to a five (5) month lease period between 1 November and 30 April. Period of Performance reads as follows:
Base Year (3-5 months, dates to be determined at time of contract award) Option Year 1 (3-5 months, dates to be determined at time of exercise of option) Option Year 2 (3-5 months, dates to be determined at time of exercise of option)
PERFORMANCE REQUIREMENTS
The Contractor shall deliver the loaders to each missile alert facility within the complex to be in place by the date determined at time of contract award or exercise of option. The Contractor shall remove the loaders within 15 calendar days after the removal date determined at time of contract award or exercise of option.
The Contractor shall be responsible for all scheduled routine maintenance on leased equipment throughout the term of the lease at no additional cost to the Government.
On-site maintenance and repair shall be provided by the Contractor as part of the lease contract. If the equipment becomes inoperable during the lease period, a replacement shall be delivered within 48 hours.
The Contractor shall be on site within 72 hours of notification from the Government for all unscheduled maintenance and repairs.
At the close of the contract year, the Government will ensure that diesel fuel and diesel exhaust fluid are replaced, and front end loaders are cleaned to the same level as received. The Government shall not be responsible for any additional fuel or cleaning costs.
WELDING. When any welding, cutting, brazing or any open flame device is to be used, an AF Form 592 permit shall be obtained from the Base Fire Department, 896 Flightline Drive, prior to beginning such work. The Fire Department can be reached at PH # 723-2461. During such work, a fully charged and operable fire extinguisher, appropriate for the task, shall be provided by the Contractor.
CONTRACTOR’S WORK HOURS. Contractors shall have access to the MAFs Monday through Friday between the hours of 7:30 a.m. and 6:00 p.m., excluding Federal Holidays or any other no work period as designated by the Government.
WORK STOPPAGES. In the event of on-site priority maintenance involving Contractor occupied sites, the Contractor shall be required to vacate the area of all personnel and vehicles for an indefinite period when requested by the Contracting Officer (CO) or Government representative. All Government-caused work stoppages are documented by Government escort personnel at the time of occurrence and reported to the CO within 48 hours of occurrence.
SECURITY DELAYS. Processing onto and off of each site may require delays of up to thirty (30) minutes both ways and is included in the contractor’s mobilization fee. Delays in site access/departure processing beyond thirty (30) minutes are considered security delays. Security situations that require the Contractor to stop work and evacuate the work area after the Contractor personnel/vehicles are already on site are considered work stoppage or evacuation of the work area.
TRANSPORTATION OF EQUIPMENT
The Contractor shall deliver the leased front end loaders to the following facilities; One (1) each at Missile Alert Facilities (MAF) A-01, B-01, C-01, D-01, E-01, F-01, G-01, H-01, I-01, J-01, K-01, L-01, M-01, N-01 and O-01.
Locations of the MAFs are in the following counties: Bottineau, Burke, Renville, McLean, Mountrail, and Ward. One (1) loader shall be delivered to the 91st Operations Group, 544 Tanker Trail, Minot AFB, ND 58701.
TRAINING
The Contractor shall provide a training syllabus or User’s Manual to educate Government personnel on how to properly change out attachments for the leased equipment, safe operating of equipment and warning indicators to look for throughout operations.
The Contractor shall perform initial operator training with Government personnel at the time of delivery of the leased equipment.
CONTRACT MANAGER. The Contractor shall designate a Contract Manager and an Alternate Contract Manager. The Contract Manager or Alternate shall be responsible for management and coordination of the contract, and shall be the point of contact with Government representatives. The Contractor shall notify the CO in writing prior to contract start date of the names, e-mail addresses, and telephone numbers of these personnel.
CONTRACTOR EMPLOYEES. All contractor employees requiring access to missile alert facility grounds for the purpose of delivery or training are subject to background checks.
ENTRY AND SITE REGULATIONS
REAL ID ACT. All personnel requesting access to Minot Air Force Base must possess a valid State or Government picture identification card. Furthermore, individuals presenting identification cards from a state that is non-compliant with the REAL ID Act will require additional documentation to gain unescorted base access.
Additional documentation includes a valid U.S. or foreign government issued passport, an employment authorization document that contains a photograph, or identification cards issued by federal state or local government agencies that include a photo and biographic information. Additional information to include a full list of REAL ID Act compliant and non-complaint states can be found at https://www.dhs.gov/current-status-states- territories. Personnel requesting vehicle access to Minot Air Force Base must provide a valid driver's license (compliant with REAL ID Act), current vehicle registration, and proof of insurance.
ENTRY. In order to gain entry onto a Missile Alert Facility, VENDOR personnel must be listed on an Entry Authority List (EAL), identified, and physically escorted on-site by authorized Government inspector/escort.
Twenty (20) working days prior to the initial delivery of equipment, the VENDOR must submit to the Contracting Officer a summarized list of employees and of the vehicle information. A legible photocopy of each employee’s US government issued credential photo ID must be submitted with this submittal. This summarized list of employees and vehicles along with the legible photocopies of the VENDOR’s employee’s US government issued credential photo IDs will be used to prepare the VENDOR EAL referenced above.
The VENDOR’s summarized list of employees information must show each employee’s:
· Full name exactly as it appears on the credentialed photo identification (ID) submitted for use in conjunction with the VENDOR’s EAL
· Date of birth (DOB)
· Full Social Security Account Number (SSAN)
· The government issuing agency name and the control number (exactly as it appears on the credentialed phot ID) of the government issued credentialed photo ID submitted for use in conjunction with the VENDOR’s EAL
· Provide a legible copy of the individual’s US government issued credential phot ID. These must have a picture of the individual, individual’s DOB and a control number. The following are acceptable government issued credentialed photo IDs: US Federal (i.e. US passport or US passport card), or state, commonwealth, territory, protectorate, the District of Columbia, US sovereign insular area or a tribal organization recognized by the US Department of the Interior (i.e. state issued driver’s license, state issued photo identification or Tribal ID card). State issued driver’s license must be from a state authorized to operate in federal bases or sites.
The VENDOR’s summarized list of vehicle information must include:
· Vehicle Make
· Vehicle Model
· Vehicle Year
· Vehicle Primary Color
· Vehicle License Plate # (State of Issue and Number). If no license plate, provide the vehicle identification number (VIN) or Serial Number
Vehicles include: mission essential company owned/leased vehicles driven into restricted areas. Vehicles must be listed as either owned or leased.
Mission essential equipment (any equipment being transported on trailers) and trailers need not be listed on the EAL. If mission essential equipment is driven to a Missile Alert Facility, then it must be listed as a vehicle on the EAL with information listed above.
No privately owned vehicles (POVs) will be permitted on the Missile Alert Facilities.
Each employee must have their identity proofed and their access vetted through the National Crime Information Center database and the Security Forces Information Management System using their name, DOB and SSAN. The employees DOBs and SSANs will only be used for this proofing and vetting process and are not used on the EAL used in the field for access.
Deletions to the EAL may be made without preparing a new EAL summarized list, but must be requested by the VENDOR in writing. However, for additions of personnel or changes to EAL, a new EAL employee summarized list and a legible photocopy of each employee’s credentialed photo ID must be submitted by the VENDOR, in writing, twenty (20 working days prior to any additions or changes to the EAL.
Upon arrival at the MAF, VENDOR personnel shall contact the Flight Security Controller (FSC) to inform the FSC of their presence and purpose at the site. At the MAF you may use the MAF intercom telephone located by the site gate or cellphone. If the Government inspector/escort is not at the site upon the VENDOR’s arrival at the site, VENDOR personnel must remain 25 feet outside of the security fence and in or near their vehicle(s) until the arrival of the Government inspector/escort. VENDOR shall not impede flow or traffic on and off site. Government will provide FSC phone number at start of project.
Upon arrival, the Government inspector/escort will identify and search all VENDOR personnel. All VENDOR personnel will provide the Government inspector/escort with the government issued credentialed photo ID as listed above, stating the employee’s full name, issuing agency and control number. This government issued credentialed photo ID shall be the same one listed on the EAL. The VENDOR’s vehicles will be checked / searched prior to site entry. Only VENDOR owned / leased vehicles and equipment needed to support mission activities (accomplish the contracted work) on site shall be permitted on MAFs. No privately owned vehicles (POVs) will be permitted on MAFs.
If VENDOR personnel or vehicles are not listed on the EAL, or those who are listed cannot provide proper identification, they will not be permitted on the site. Delays caused by lack of proper EAL data or proper identification shall be at no cost to the Government.
Restricted Area. The work to be performed is in a restricted area. Therefore, the VENDOR shall conform to all requirements of DoD 5210.41M AFMAN 31-108 AFGSCSUPV3 and the Minot AFB IDP 31 Volume 1. Workers will be required to have Government inspector/escort with them at all times.
