Performance Work Statement

(PWS)

for

5 FSS

Group Fitness Instructor at

Minot AFB, Minot

1. SCOPE OF WORK:

1.1. Contractor shall conduct and administer all aspects of the exercise portion of the Fitness Improvement Programs (FIP) Fitness Enhancement Programs, and High Intensity Strength and Conditioning Classes held at Minot AF Base Fitness Center (Bldg 455) ND.

2. REQUIREMENTS:

2.1. Program shall consist of an Aerobics Contract Manager (ACM) (whom may be the contractor) and instructors certified in the following aerobic and strength type workouts to include but not limited to: Fit2Fight, Zumba, TRX, water aerobics, pre-natal instruction, yoga, Indoor Group Cycling and High Intensity Strength and Conditioning Program Also, at least one instructor in this program will be certified as a USAW (USA Weightlifting) Sports Performance Level 1 Coach.

2.2. Classes will be performed based on the needs/requirements of our customers, providing the maximum participation for the classes. Class times will be coordinated with the Fitness Center Director and the Lead Aerobics Instructor. Instructors must arrive fifteen (15) minutes before scheduled class to allow for prep time.

Total number of classes – 800 not to exceed 1040 annually

The following schedule will be used for Holidays, Military Down & Training Days/Family Days:

Christmas Day and New Year’s Day the Fitness Center will be closed, no classes will be offered.

Federal Holidays (not mentioned above): NO classes

The contractor does not have to provide instructors on other days as identified by the Fitness Center due to closure or special circumstances when notification is given 24 hours prior. However, due to inclement weather shutdown notification will be made as soon as possible.

Contractor will be notified 30 days in advance of any changes in class times.

3. PERSONNEL:

3.1. The contractor will ensure that all instructors (to include substitutes and/or (ACM)) possess and maintain:

A) Aerobics and Fitness Association of America (AFAA)/ American Council of Exercise (ACE) Primary Group Exercise Certification OR similar certification from another nationally recognized certifying agency.

B) A current Cardiopulmonary Resuscitation (CPR) card with automated external defibrillator (AED)

C) Specialty classes require specific certifications such as Johnny G/Mad Dogg Spinning, Yoga Fit, CrossFit, and Tactical Fitness High Intensity Interval Certification.

All certifications must be valid prior to employment, and copies presented to the fitness center before providing instruction. Copies of the certification(s) will be maintained on file at the fitness center. If a certification expires on any instructor, instruction will cease until certification is renewed.

3.2. The Contractor and/or ACM must also have at least three (3) years fitness teaching experience and be able to personally mentor new instructors on safety guidelines and methods of instruction. If the contract is awarded to an agency outside the greater Minot area, the contractor must provide a qualified ACM.

3.3. The contractor shall designate in writing to the Fitness Center Director, the names and telephone numbers of the ACM and an alternate (when ACM is not available). These designated individuals shall meet with Government personnel to discuss immediate problems, should they arise.

3.4. The contractor and/or ACM work under the authority and direction of the Fitness Director. Complaints and request for all issues are to be addressed utilizing the chain of command.

3.5. The Contractor is reminded that off-duty, active military personnel hired may be subject to permanent change of station (PCS), change in duty hours, recall and deployments. Their absence does not constitute a valid excuse for non-performance under this agreement.

3.6. The Contractor and/or ACM shall be financially responsible for any damage caused to Fitness Center equipment while it is under their control (i.e. broken keys/locks, stereo equipment, microphone/headset, etc.) to include the repair/replacement, maintenance, shipping and handling of damaged equipment.

3.7. Contractor personnel must present a neat appearance and be easily recognizable while in the Fitness Center, which should be accomplished by wearing distinctive athletic attire.

4. ACM SPECIFIC DUTIES

4.1. Provide the Fitness Center director within thirty (30) days after award of contract, and on or before the 15th of the month prior to the next effected month. (EX. Jan 15 you will have completed/submitted the Feb Schedule.):

A) Complete roster of instructors, to include telephone numbers and accreditation expiration dates.

B) Master program schedule, in a monthly calendar format, depicting aerobics instructor name for the designated type of class. Limit the number of classes taught by one instructor to no more than two classes per day.

4.2. Be responsible for:

A) Conduct interviews with prospective instructors and verify certifications are valid and current.

B) Train instructors on proper usage of stereo equipment prior to equipment being used.

C) Ensure each instructor incorporates standardized and safe warm up, stretching and exercise technique, as well as a safe cool down period to conclude the cardio segment of each class.

D) Attend monthly meeting with the Fitness Center staff to receive any changes and class scheduling.

E) Conduct a monthly meeting with all instructors to keep them informed of facility/base policy changes or any other information needed to ensure a quality program.

F) Ensure an instructor is present for scheduled classes. If for some reason a scheduled instructor cannot make a scheduled class, the ACM must provide a qualified substitute to teach at scheduled time.

G) Verify instructors return all the equipment their class used to the rightful area, report any damage to the Fitness Center Director immediately, and enforce the Air Force Fitness Center Age Policy per AFI34-266

Permitted in all areas of the FSC without a qualified adult*. (T-1)
Youth 13 - 15 years of age
Permitted in cardiovascular, weight equipment, group exercise rooms, locker rooms, saunas, etc., only under qualified adult interactive supervision. Interactive supervision requires qualified adult and child to participate in/perform activity together at all times.

Permitted in other activity areas (i.e., basketball court, racquetball court, running track) only when a qualified adult is present in the facility at all times. Permitted to be a participant (participating in age appropriate programming) or spectator in a sport or special event when a qualified adult is present in the facility at all times. For FSC pools, use is permitted during instructional programs, or when a qualified adult is present in the facility at all times. (T-1)

Youth 6 - 12 years of age
Not permitted in cardiovascular, sauna, weight equipment rooms, or group exercise rooms during scheduled adult exercise classes.

Permitted in other activity areas (i.e., basketball court, racquetball courts, running track, locker room, Parent/Youth Area (PYA)) only under qualified adult interactive supervision.

Interactive supervision requires qualified adult and child to participate in/perform activity together at all times. Permitted to be a participant (participating in age appropriate programming) or spectator in a sport or special event only under qualified adult interactive supervision. For FSC pools, use is permitted during instructional programs, or when under qualified adult interactive supervision. (T-1)

Children under 6 years of age
Not permitted in FSCs except as follows. Permitted in FSC Parent (legal guardian)/Child Areas (PCAs) for supervised play. Permitted to be a participant (participating in age appropriate programming) or spectator in a sports activity or special event under qualified adult interactive supervision. Interactive supervision requires qualified adult and child to participate in/perform activity together at all times. For

H) Ensure instructors notify on-duty fitness center staff of any injuries or accidents associated with aerobic patrons. Should ambulatory services be needed, the fitness center shall notify the proper agency for assistance. The instructor is required to assist the fitness center staff in the completion of an accident report after the conclusion of the incident.

I) Contacting the Contracting Officer to obtain the necessary information required to request issuance of a base pass to gain entry onto the base during contract period of performance.

5. INSTRUCTORS SPECIFIC DUTIES

5.1. Instructor shall:

A) Arrive 10-15 minutes prior to the scheduled class start time in order to sign out keys, headset and equipment for each class. In addition, instructors will provide their own music, by MP3s for each class.

B) Use stereo equipment provided in each room and ensure all equipment is in good working order before and after use. Ensure the exercise area is clean after the class and keys are returned to the front desk.

C) Report any equipment malfunction or problems with the class to the Fitness Director or representative by the end of the class.

D) Record an accurate head count for each class performed using attendance forms provided by the Fitness Center.

E) Provide specialized instruction for students demonstrating high and low impact options as well as beginning, intermediate, advanced techniques in a progressive building format.

6. SURVICE SUMMARY (SS): The SS below highlights the most critical services required by the contract effort. The SS defines the minimum, satisfactory performance thresholds for each service. The absence of any contract requirement from the SS shall not limit its enforceability nor the rights or remedies of the Government under any provisions of the contract.

7. CONTRACTORS FAILURE TO PERFOM: If contractor fails to perform required services within the specific limitations, the government may:

a. Accept the services at a later date; or

b. Refuse to accept the service at a later date (in which case the government will bear no cost and make no payment for the missed service); or

c. Procure the missed service, upon such terms and conditions as the Contracting Officer deems appropriate, from an alternate, responsible firm and the contractor shall be liable to the government for any excess costs incurred and to continue the performance as require thereafter.

8. GOVERNMENT FURNISHED PROPERTY:

8.1. The Government shall provide, without cost to the contractor, the utilities necessary to accomplish the tasks specified in the Performance Work Statement, including electric, fire protection and 911 services.

8.2. The Government shall provide a designated area suitable for aerobics instruction. In addition, the Government shall provide steps, mats, weights, bands, balls, spinning cycles and a sound system.

9. GOVERNMENT RESPONSIBILITES:

9.1. The Fitness Director will train the contractor and/or ACM on proper usage of stereo equipment prior to being used.

9.2. The Government will monitor the quality of instruction provided by all instructors as well as to ensure instructors are eligible to participate under this performance work statement.

9.3. The Fitness Director may cancel or add aerobic classes (upon the availability of funds) due to mission requirements, insufficient class attendance or substandard contractor performance. The Director will provide the contractor and eight (8) hour notice prior to cancellation, at a minimum.

10. INVOICES AND PAYMENTS:

10.1 Contractor will invoice in Wide Area Workflow (WAWF) on a monthly basis.
