Statement of Work

Kitchen Equipment at 15 Missile Alert Facility

27 MAY 2017

1. GENERAL: The contractor shall provide all labor, materials and equipment to install and remove the old freezers, Drop-in-Fryer and Built-in-Plate and at 15 Missile Alert Facilities at Minot North Dakota. The installation shall be in accordance with manufacturer’s instructions, attached schematics and requirements of the Statement of Work (SOW) as stated in the following paragraphs.

2. PERFORMANCE PERIOD: This contract shall have 30 days performance period and a total of one day at each Missile Alert Facility. Coordinate work area with Contract Management and Using Agency. Work should begin within 60 days after award of contract.

3. GENERAL SCOPE OF WORK: Work includes but is not limited to: removing the existing freezer and placing in the garage on site and Drop-in-Fryer and Built-in-Plate and install a new Drop-in-Fryer and Built-in-Plate. This will include all electrical and wiring if necessary. Old equipment will be moved to Missile Operation Building, building 695, placed on pallets and strapped down. All transportation, equipment, tools, supervision, labor, materials and performing the work in accordance with the specifications, attached equipment standards, standard industry practice, National Fire Prevention Association (NFPA) codes, the National Electrical Code (NEC), Uniform Mechanical Code (UMC), Uniform Plumbing Code (UPC), Uniform Building Code (UBC), manufacturer’s recommendations and terms of the contract. Contractor will coordinate deliver of the new equipment with the shipping company so that it will be delivered no more than three days prior to work starting. Delivery will be made to the Missile Operation Building, building 695. Contractor will notify the Using Agency of the exact delivery date. Contractor will ensure equipment/personnel necessary to unload the truck is in place at the time of delivery. If equipment arrives more than 3 days prior to the start of work. Contractor is responsible for securing a delivery point, coordinating storage of the equipment and ensuring the equipment is at the Missile Operation Building, building 695 facility when work starts.

4. NOTIFICATION: Contractor shall provide written notification to the Contracting Officer two weeks prior to commencement of work. The Contracting Officer shall notify the Project Inspector who in turn will notify the Using Agency.

5. DEBRIS: Contractor shall remove all construction rubble from the work site to a Contracted Authorized onsite Construction Dumpster or remove from Missile Alert Facility on a daily basis. Contractor is responsible for the security of tools and equipment being used at the project site.

6. SUBMITTALS: Contractor will submit complete list of materials with cost by line item required to complete the project. Contractor shall provide product samples if selecting material sources other than those recommended for work.

7. FACILITY ACCESS: Coordination with Missile Alert Facility Staff as soon as contract is granted to process paperwork to give access to Missile Alert Facilities access to Contractor. Contractor will contact Missile Alert Facility Staff one day prior to heading out the Missile Alert Facilities.

8. SITE DAMAGE: Contractor shall be responsible for all damage incurred during this project and shall return damaged areas to original condition at no additional cost to the government.

9. CONTRACTOR WORK HOURS: Recommended working hours for the Contractor will be Monday - Sunday 0800-2000, weekends will need pre-arrangement with the Missile Alert Facility Chef Manager
