PERFORMANCE WORK STATEMENT
FOR
Wi-Fi Service and Installation of HDTV Devices
| 1. | DESCRIPTION OF SERVICE |
| 1.1 | Scope of Work: The Contractor agrees to provide Wi-Fi service for 1 dayroom located in building 723 and HDTV cable service for 29 dayrooms in 11 dormitories listed in table 2 PARA 2.1. Contractor may not mount any satellite dishes on the buildings. |
| 1.2 | Contractor: The Contractor will be responsible for providing Wi-Fi and cable service; as well as all required equipment to ensure service is offered with limited to no down time. Satellite Dish may not be used. If a satellite dish has to be used the provider must submit a work order (332) approved thru the, 19 Civil Engineer Squadron if they plan to use satellites to furnish and serve. They also must acquire a Franchise Agreement/ Cable TV Real Estate license through the, 19 Contracting Squadron prior to providing the service. |
| 1.3 | Certification Contractor must meet all certified technician requirements. Contractor must provide copies of certifications upon request of the Government. |
| 1.4 | Service Calls: Government will notify contractor of any interruptions involving all service to Wi-Fi and HDTV cable. The contractor in turn will address the concerns/complaints to the reporting official and provide the Government with documentation that this has been accomplished. Additionally, the contractor will provide the Government with a remedy to the issue within 24 hours and will attempt to respond promptly and effectively to calls for service. |
| 1.5 | Labor: The Government will not pay for newly required installation, repair, and maintenance. Existing coax wall jacks were previously installed, and are Sudden Link Wi-Fi compatible. |
| 1.6 | Limitations of Liability: Contractor will provide its own private insurance. In no event shall the Government be liable for any loss of or damage of equipment owned by the contractor, or for any other consequential, incidental, indirect or economic damages incurred or suffered by the Contractor. Contractor further agrees that the total liability shall indemnify and hold the Government harmless against any claims by third parties, including all costs, expenses and attorney’s fees incurred by the Contractor therein, arising out of or in conjunction with Contractor’s performance under or breach of this contract. |
| 1.7. | Changes: Contractor shall notify the Government (a list of dormitory manager’s phone numbers will be provided) of any scheduled outages, equipment upgrades or changes to the cable channels. |
| 2. | GOVERNMENT FURNISHED PROPERTY: |
| 2.1 | Government: During initial installation and future repair service the Government shall provide a personnel to accompany the technician along with a clean and safe environment for the contractor to complete services needed in accordance to the statement of work. |

| 2.2 | Contractor. Contractor shall be responsible to provide all tools, materials, and equipment necessary to ensure limited or no uninterrupted service in accordance with this Performance Work Statement. If possible Wi-Fi router must be secured in place to prevent relocation or tampering. |
| 3. | GENERAL INFORMATION |
| 3.1 | In Case of Base Exercise: In this instance, The LRAFB Dormitory will notify the contractor of the possibility of traffic congestion entering the base during these exercises. Dorm Manager will set schedule to avoid announced exercise. |
| 3.2 | Federal Holidays: Install or repairs will not be conducted on the days shown in Table 3. |
| 3.3 | Hours of Operations: LRAFB Dormitory hours of operation are Monday-Friday, 0700-1600. The Contractor will perform maintenance or service during duty hours of operation. |

| 4. | INVOICING |
| 4.1 | The contractor will use Wide Area Work Flow (WAWF) to submit all invoices to the Government by the fifth day of the month. The Government will verify that the services have been performed satisfactorily, and approve the invoice for payment by the tenth day of the month. |
| 4.2 | If totals are incorrect the Government will notify the contractor of any discrepancies. Once the discrepancy has been verified/corrected the contractor will re-submit the invoice in WAWF by the next workday. |
