PERFORMANCE WORK STATEMENT

FOR

CABLE TELEVISION

1 Oct 2014

1. DESCRIPTION OF SERVICES.

1.1. SCOPE OF WORK. Contractor shall provide/furnish all labor, tools, equipment, uniforms, material, supervision and transportation necessary to provide Cable Television services, at the Fitness Center at Little Rock Air Force Base, Arkansas.

1.2. EVALUATION CRITERIA. Contractor shall have experience in providing cable television services. Contractor shall have retained ability to provide services for the time equal to the period of the contract.

1.3. AREAS OF RESPONSIBILITY

1.3.1. Cable television shall be made available throughout the Fitness Center facility.

1.3.2. The contractor should obtain access via the 19th Communications Squadron for the communications closet at the facility.

2. GOVERNMENT FURNISHED PROPERTY AND SERVICES. The government shall provide the facilities, fixtures, equipment and services listed here.

2.1. Property.

2.1.1. Facilities. The contractor will maintain all cable run to the facility. No alterations shall be made to furnished facilities without specific written permission from the Contracting Officer.

2.1.2 Equipment. The government will furnish the equipment required and all equipment shall be returned to the Fitness Center upon completion of the contract or as directed by the Little Rock AFB Fitness Director. The government will provide the maintenance and repair of all televisions. Contractor is required to maintain all equipment provided for making cable television viewable.

3. CONTRACTOR FURNISHED ITEMS AND SERVICES.

3.1. Except for those items or services specifically stated in section 2 as government furnished, the contractor shall furnish everything needed to perform this contract according to all its terms and conditions.

4. GENERAL INFORMATION.

4.1. CONTRACTOR PERSONNEL. The contractor shall provide a contract manager and alternate(s) that shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. The contract manager and alternate(s), who shall act for the contractor when the manager is absent, shall be designated in writing to the Contracting Officer prior to the start of the contract.

4.1.1. The contract manager or alternate shall be available within 60 minutes during normal duty hours on the installation or by telephone with government personnel designated by the Contracting Officer, to discuss problem areas.

4.1.2. Telephone numbers of the contract manager or alternate shall always be updated and provided to the Fitness Center.

4.1.3. The contract manager and all employees shall be able to read, write, speak and understand English. The government reserves the right to restrict employment of any contractor employee, or prospective contractor employee, who is identified as a potential threat to the health, safety, security, general wellbeing, or operational mission of Little Rock AFB and its population.

4.1.4. Contractor shall maintain a professional attitude and wear appropriate professional attire. Contractor personnel shall present a neat appearance and be easily recognized. This may be accomplished by wearing distinctive clothing item bearing the name of the company and employee name.

4.1.5. The Contracting Officer may, in writing, require that the contractor personnel be removed from the work site, area or base due to objectionable behavior.

Objectionable behavior includes, but is not limited to, wearing inappropriate attire, using profane language, and abusing or disobeying base rules or regulations. No official or score keeper hall be allowed to work a game if it is apparent he/she has been consuming alcoholic beverages.. At no time shall an official consume alcoholic beverages or use tobacco products in, on or around the playing area or use cell phones while performing officiating duties during games.

4.2. HOURS OF OPERATION.

4.2.1. Fitness Center hours of operation are as follows:

4.2.1.1. Monday – Friday: 0500 – 2200

4.2.1.2. Saturday and Sunday: 0900 – 1800

4.2.1.3. Holidays. Contractor is required to provide services on all Federal Holiday, defined as New Year’s Day, Martin Luther King Day, Washington’s Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran’s Day, Thanksgiving Day and Christmas Day.

4.2.2. Contractor may be required to work on weekends as schedule in advance.

Cancelled or postponed classes may also be added to the end of the schedule, extending the schedule. Classes will not be scheduled on Federal Holidays.

4.3. QUALITY ASSURANCE.

4.3.1. The government will periodically evaluate the contractor’s performance.

4.3.2. The government is responsible for the inspection and acceptance of services authorized. All work shall be accomplished in a timely and professional manner. Work, which does not conform to the specifications contained herein, will be justification for rejection and non-payment after due process in accordance with the Federal Acquisition Regulation by the Contracting Officer. Correction of any deficiency shall be at no cost to the United States government or the Little Rock AFB Fitness Center.

4.3.3. The contractor will respond to written concerns or complaints received by the contracting office. (Note: Response is an action designated to correct or clarify a situation under question.) The contractor will provide the Contracting Officer a written response within 5 working days after receipt of the concern or complaint has been received by the contractor.

4.3.4. GOVERNMENT REMEDIES. The Contracting Officer shall follow FAR 52.212.4, Contract Terms and Conditions – Commercial Items , for contractor’s failure to perform satisfactory services or failure to correct non-conforming services.

4.3.5. Payment will be made monthly upon receipt of a properly certified invoice for services rendered under this order. Contractor will only be paid for officials provided in accordance with the contract. The Fitness Center Director, Fitness Director (or designated representative, in their absence) and Contracting Officer Representative (COR) will be the only persons authorized to certify this pay document.

4.4. SECURITY REQUIREMENTS.

4.4.1. All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the government installation shall abide by the security regulations of the installation.

4.4.2. The contractor shall be responsible for safeguarding all government property provided for contractor use. At the end of each work period, all government equipment and materials shall be secured as directed by the Fitness Center Director after use.
