PERFORMANCE WORK STATEMENT

Section A

PERFORMANCE WORK STATEMENT

LAUNDRY AND DRY CLEANING SERVICES

LITTLE ROCK AFB, AR 72099

1. Description of Services

1.1 The contractor shall provide all personnel, equipment, tools, materials, supervision and other items and services necessary to perform the laundry services as defined in this Performance Work Statement (PWS) and Annual Workload Estimates. The contractor shall perform, at a minimum, to the standards in this contract. Performance shall be done in a manner that will provide clean laundry in accordance with local, county, state, federal and professional industry standards. This service includes pick-up and delivery of linen to and from Little Rock Air Force Base.

1.2 In accordance with the infection control recommendations (Linen and Laundry subsection) of the Center of Disease Control (CDC) and the Healthcare Infection Control Practices Advisory Committee (HICPAC), Contractor is responsible as a minimum for properly handling contaminated laundry in accordance with these recommendations.

1.3 The contractor’s facility will be subject to inspection of sanitary conditions by the Government at least annually and at no notice to the contractor.

2. Pickup and Delivery. The pickup and delivery schedule is listed in Appendix 3. The contractor and the government representative shall reconcile weights at the time of the pickup and delivery. Any discrepancies concerning cleanliness and serviceability shall be addressed at the time of the delivery. The government shall identify any items that are damaged or stained to the contractor prior to pickup. The contractor shall be responsible for all items after the pickup until items are returned.

3. Loss or Damages

3.1. Weight loss. All linen will be weighed at pickup and delivery. Any disagreement on weight shall be resolved by the contracting officer. The permissible variation in weight (maximum weight loss) shall not exceed 8 percent. Any weight loss in excess of the maximum 8% will be subject to the loss provision in DFARS 252.237-7015. The weights of the missing items will not rollover to the new month. At the end of the month the total weight will be calculated to determine the amount payable to the government due to weight loss.

3.2. Damaged or destroyed items. Any items determined to be damaged by the contractor will be returned to the contractor. The Contractor will pay the replacement cost identified in Appendix 2 for damaged or destroyed items while in the Contractor’s possession.

3.3. Returns. The customer will ensure unsalvageable items are identified and not sent out for laundering. If the contractor identifies unsalvageable items they will bundle the items separately and clearly mark the bundle as unsalvageable. These identified items shall be accepted by the Quality Assurance Personnel. If the contractor causes any of the items to be unsalvageable, the contractor shall reimburse the government for replacement of the item(s).

4. Repairs. The contractor shall repair, at its expense, all damages to individual garments that occur while in the custody of the contractor, to include rips, tears, holes, open seams, and replacement of buttons and repair or replacement of broken zippers. Thread, material, zippers and buttons of the same type, texture, manufacturer quality, color and color fastness as that used in the manufacture of the garment shall be used for repair and replacement. Any item with a rip or tear in excess of 4 inches long is considered damaged beyond repair or unserviceable. The unserviceable condition of each individual garment shall be so noted on that laundry ticket. Contractor shall ensure that unserviceable items are segregated from other laundry items, and are readily identifiable upon return to the government as unserviceable.

5. Delivery Vehicles. All laundry delivered under this contact shall be transported in clean, closed vehicles, provided by the contractor. Soiled items and clean items shall be transported in accordance with all local health regulations and/or state laws or regulations. The vehicle shall be maintained in a sanitary condition to prevent cross contamination. Breakdowns shall not relieve the contractor of their responsibility to make scheduled deliveries under this agreement. The contractor’s vehicles shall meet all licensing requirements and be properly insured.

6. Accountability of Articles.

The contractor will be responsible for following Appendix 1, Laundering and Packing Instructions. All bundles will contain only one type of linen. Linen types are not to be mixed, i.e. flat sheets, bundled with fitted sheets, beige towels with white towels, etc. Linen colors are not to be mixed; contractor will keep all colors bundled separately.

6.1. Items being turned into the contractor will be weighed by the Quality Assurance Personnel (QAP) and the Contractor before the weights are permanently affixed to the laundry ticket.

6.2. 19 FSS will weigh soiled linen and attach weight to the delivery ticket. They will provide soiled weight linen and all articles to be serviced. The contractor and the QAP shall sign the laundry ticket when items are being picked up and delivered. The contractor shall furnish the original copy of laundry tickets to QAP with a duplicate copy to the customer. The contractor shall be liable for the return of all articles turned in for service under this contract.

7. Delivery. The contractor shall record weight delivered on the delivery ticket and the government representative shall verify this amount. The contractor shall provide the delivery tickets and furnish the government representative at each location the applicable original delivery ticket.

8. State and Local Department of Health Requirements. Compliance with any certification or labeling requirements arising from state or local department of health regulations is the responsibility of the contractor.

9. Workmanship and Sanitation. The Contractor shall comply with federal, state, local and Department of Health regulations, and the Center for Disease Control (CDC) and the Healthcare Infection Control Practices Advisory Committee (HICPAC) regulations. Delivered articles shall conform to the generally accepted industry standards of quality of cleanliness, finish, and appearance. All work performed shall be done under sanitary conditions. The items shall be clean, free of soil, bacteria, fungi, and other microorganisms, including pathogens or disease-producing organisms. A test piece report should be made available at the beginning of the contract to the QAP and annually thereafter.

9.1 In accordance with the Infection control section of the Center for Disease Control (CDC) and the Healthcare Infection Control Practices Advisory Committee (HICPAC) (Linen and laundry subsection) recommendations; Contractor is responsible as a minimum for properly handling contaminated laundry. The contractor’s facility will be subject to inspection of sanitary conditions by the Government at anytime.

10. Contract Manager. The contractor shall provide a Contract Manager who shall be responsible for the performance of the work. The name of the person, and an alternate or alternates who shall act for the contractor when the manager is absent, shall be designated in writing to the Contracting Officer within five (5) business days of award.

10.1 The Contract Manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. The Contract Manager or alternate shall be available during the normal duty hours to meet at a mutually agreed upon time on the installation with government personnel (designated by the Contracting Officer) to discuss problem area; The Contract Manager and Alternate(s) must be able to read, write, speak, and understand English.

11. Removal of Contractor Employees. All personnel employed by the contractor in performance of this contract, or any representative of the contractor entering the Government installation, shall abide by all security regulations of the installation. The government reserves the right to direct the removal of an employee for misconduct or security reasons. This action does not relieve the contractor from total performance of the contract tasks specified herein.

12. Contractor Employees. The contractor shall not employ persons for work of this contract if such employee is identified to the contractor by the Contracting Officer as a potential threat to the health, safety, security, general well being or operational mission of the installation and its population. Contractor personnel shall present a neat appearance and be easily recognized as contractor employees. This may be accomplished by wearing distinctive clothing bearing the name of the organization or by wearing appropriate badges containing the organization name and the employee name. Contractor personnel must wear issued base identification cards at all times while on the installation.

13. Listing of Employees. The contractor shall maintain a current listing of employees that will require access to the installation. The list shall include the employee's name and privacy act information. The list shall be provided to the Contracting Officer prior to the performance start date. An updated listing shall be provided when an employee's status or information changes. All contractor employees requiring access to the installation are subject to a criminal background check.

14. Base Passes. The contractor shall obtain individual and vehicle passes from the 19 SFS/SFOP for all contractor personnel working on the installation.

15. Vehicle Operators. Operators of contractor vehicles shall possess a valid commercial driver’s license for the category of vehicle operated. They shall comply with the installation rules and regulations for motor vehicle use. Additionally, proof of insurance and the State Vehicle Registration are required to be in the vehicle while on the installation.

16. Performance Evaluation Meetings

16.1 The Contracting Officer may require the Contract Manager to meet with Contracting Officer, Contract Administrator, Quality Assurance Person (QAP), and/or other government personnel as deemed necessary.

16.2 The contractor may request a meeting with the Contracting Officer when he or she believes such a meeting is necessary.

16.3 If QAP(s) or any other USAF personnel need to conduct a site visit, a Contracting Officer or Contract Administrator will be notified. Written minutes of any such meeting shall be recorded in the contract and signed by the Contract Administrator/Manager and the Contracting Officer. If the contractor does not concur with any portion of the minutes, such non-concurrence shall be provided in writing to the Contracting Officer within ten calendar days following receipt of the minutes.

17. Recognized Holidays. If a scheduled pick-up or delivery falls on an observed federal holiday, service will be performed on the next workday. The only exception is Thanksgiving, in which case pick-up and delivery will occur on the preceding Wednesday. The following federal legal holidays are observed by this base:

18. Emergency (Contingency) or Special Event Services: Emergency and special situations (i.e., accident and rescue operations, civil disturbances, weather warnings, base alerts, Acts of God, etc.) may require the linen Exchange facility to operate on an extended basis of up to 24 hours per day. The contractor shall provide laundry services when required by the contracting officer, or authorized representative.

Section B Service Summary

1.1. The table below outlines the performance objectives or required services that will be surveillanced by government quality assurance personnel. Government inspection, however, is not limited to the services stated. The Contractor has the responsibility to ensure that all services comply with contract requirements.

1.2. The performance threshold represents the percentage of conformance to or the number of deviations allowed from the performance objective. Acceptable performance is achieved when the contractor does not exceed the performance threshold, within an observation period, as determined by the government.

1.3. Performance will be documented by the QAP. When the contractor’s performance exceeds the performance threshold, the contractor’s representative (driver) will be asked to sign the documentation acknowledging they have been informed of the discrepancies. A service that is not performed in accordance with contract requirements will be considered as unsatisfactory and count against the performance threshold. In accordance with FAR 52.212-4, the contractor will be given opportunity to reperform the service. Reperformance must be completed within a reasonable time period as determined by the government. When customer complaint is the method of surveillance, a valid Customer Complaint Record will be submitted to the contractor representative for review and completion.

2. DEFINITIONS

2.1 STANDARD DEFINITIONS.

a.) Quality Assurance Personnel (QAP). Representative(s) who have been appointed as a QAP to be responsible for providing technical assistance to the Contracting Officer in administration of the contract. The QAP shall be responsible for inspection of materials and services rendered by the contractor.

b.) Quality Control. Those actions taken by the contractor to control the production of outputs to ensure they conform to the contract requirements

2.2 TECHNICAL DEFINITIONS.

a.) Authorization. The quantity actually required of each item to support linen exchange activities.

b.) Clean. Finished articles are dry and free of soil and objectionable odors. Spots and stains are removed where practical to do so without damaging the fabric.

c.) Customer. The people authorized to exchange and receive clean linen items, either for use from linen exchange or the consolidated club. This also applies to individuals or units having organizational equipment/clothing laundered.

d.) Laundering. The process by which soiled fabric items are cleaned and treated. The term includes cleaning by mechanical action in appropriate temperature water with non-alkaline soap, solutions of detergents, and other applicable additives (e.g., water repellent treatment, bleaching, bluing, rinsing chemical, rinse-sour, fabric softener, and agents for controlling the growth of micro-organisms such as molds and bacteria). Laundering also includes starching, ironing, and pressing. The appropriate products and procedures used are to be determined by the contractor for the various types of fabric and nature of soil.

e.) Linen Items. Sheets, pillowcases, mattress covers/pads, blankets, bedspreads, pillows, table cloths, napkins and organizational items.

f.) Minor Maintenance. Normal operator care of equipment recommended by the manufacturer including cleaning, oiling, and greasing equipment, tightening nuts and bolts, and performing other preventive maintenance tasks.

g.) On Hand. Linen Items both clean and dirty, that are in Linen Exchange or in transit to and from commercial laundry.

h.) Repairable Items. Items with rips, tears, holes, open seams, and missing buttons and broken zippers i.) Organizational Items. Item requiring laundering such as field jackets, sleeping bags.

j.) Unserviceable Linen/Salvage. Items that are excessively worn, torn, stained, frayed, or dingy in coloration.

Appendix 3 Pickup and Delivery Points Normal Hours of Operation. The contractor shall perform the pick-up and delivery services required under this agreement during hours of 0800-1100, Monday, Wednesday and Friday, unless otherwise specified. Requests for delivery/pick up for days other than the regularly scheduled days i.e., weekends and holidays, must be approved by the contracting officer.
