STATEMENT OF WORK
FOR
Preventive Maintenance
1. DESCRIPTION OF SERVICES
1.1. Scope of Work. The Contractor shall provide all personnel, labor, and transportation to conduct preventative fitness center equipment maintenance at Little Rock Air Force Base (LRAFB), AR, Building 827. The Contractor agrees to maintain “optimal operational condition” of all equipment listed in this contract. Fitness Center employees may use discretion in determining “optimal operational condition” for any equipment.
1.2. Equipment. The contractor shall furnish all tools and equipment necessary to repair or replace equipment parts in accordance with this Statement of Work. The Contractor will be responsible for performing routine maintenance and service on the types of equipment listed in Table 1.

1.3. Certification: At a minimum, the Contractor shall be certified by the following national companies:
Nautilus Precor Cybex Hammer Strength.
1.4. Service Calls: The Government will notify the contractor within 24 hours of discovering any inoperable equipment. The contractor will address service calls within 24 hours and provide documentation of assessments that have been made. The Contractor will need to annotate all performed maintenance in the Fitness Center’s equipment maintenance log book as soon as a job is accomplished. Additionally, the contractor will provide the government with a remedy to any issue within 48 hours. The Contractor is allowed 5 business days to order the necessary parts needed to accomplish service call(s). The Contractor will log status of job progress/completion, parts ordered and estimated downtime of equipment. In the event that the Contractor cannot perform minor preventive maintenance, the fitness center staff is authorized under AFI 34-266 to perform preventive maintenance on equipment.
1.5. Labor: Labor costs, whether incidental or consequential to the performance of this contract, are covered by the monthly price.
1.6. Parts: Any internal or external parts required for performance of this contract will be submitted with at least three quotes for evaluation by LRAFB Fitness Center staff. Quotes shall contain at a minimum: (1) price of the part; (2) delivery time; and (3) company name. The staff will then make a determination and the part will be ordered.
1.7. Limitations of Liability. In no event shall the Government be liable for any loss of or damage to equipment owned by contractor personnel stored at LRAFB.
1.8. Maintenance Room. The Contractor is responsible for the safe keeping and cleanliness of the maintenance room, in the event it is used to perform required tasks. The room shall be free from dust, walk-ways shall be clear, equipment and parts shall be stored neatly and at no time shall equipment, tools, or any other objects block entrances/exits.
1.9. Hours of Operation. The Fitness Center’s hours of operation are Mon – Fri, 0500-2300; Sat-Sun, 0800-1900. The contractor shall conduct a weekly inspection of equipment and a monthly thorough cleaning of equipment for preventive maintenance. All performed inspections and maintenance will be logged in the Fitness Center’s equipment maintenance log.
1.9.1. Changes. The Contractor shall notify the Fitness Center Director or Section Manager of scheduled days of inspection and monthly cleaning.
2. GOVERNMENT RESPONSIBILITES
2.1. Government. The government shall furnish a clean and safe environment for the Contractor. The areas designated for equipment repair should be clean, safe, and regularly maintained to avoid injury to fitness patrons. The Contractor is allowed to utilize one secure locker in the maintenance room. The Fitness Center is not responsible for tools that are unsecured.
3. GENERAL INFORMATION
3.1 Increased Base Security. In the event of increased security measures (i.e. Exercises or Real World Occurrences) the Fitness Center will notify the contractor of difficulties accessing base facilities. Exercise dates will be announced so the contractor can plan ahead to prevent delays in performance.
3.2 Federal Holidays. Inspections will not be conducted on the days shown in Table 2, Federal Holidays.

3.3 Down Days. Base and Command down days will be announced as soon as the Fitness Center is notified so scheduling issues can be dealt with accordingly.
