1. How many drivers/vehicles do you estimate to be used to service this contract.
Current workload is 3 deliveries per week. Anticipate possible multiple deliveries in one day
(One in the morning and another in the afternoon).
2. What are the vehicle sizes required?
Vehicle size required is a normal passenger vehicle. Most samples will be hand carried.
Sometimes a dolly may be used for a larger box.
3. Will dry ice be needed for specimen transport? If so, is it the responsibility of the contractor to provide dry ice?
Dry ice may or may not be required, however in the rare occasion that it is required it will not be the responsibility of the courier to provide dry ice.
4. Will there be other equipment the driver will be responsible for, such as cooler?
Courier will be receiving a closed package (box) with samples and materials to maintain temp
(dry ice or wet packs).
5. What is the required turnaround time, time of day, location(s), and expected trip volume per year?
Expected turnaround time: Within 2 hours after a call order is made. Within 1 hour for urgent samples.
Expected delivery time: ASAP (no delays between pickup to delivery)
Expected Trip/Year: 75 – 150 trips/year
6. Do the couriers need a hazmat endorsement on their license or will the medical courier training relevant to DOT, Material of Trade Exceptions & OSHA Blood Borne Pathogen Training suffice?

7. What is the expected on-call 24/7 trip volume/count per day, week, month and/or year for each expected pickup location, and/or per year for all locations combined?
The estimated trips per year for each location is listed in the table on the combo.
8. What was the cost of the final full option year of the previous contract?
This requirement is not current on contract.
9. How many drivers/vehicles are currently used to service the current contract?

10. Regarding evaluation factors for the price Volume: we usually like to provide a “Pricing realism” cost break-down, to include SCA minimum per year, the number of drivers, hours and miles that are estimated to be required for performance, and other costs. Is it acceptable that we forward this
Pricing Realism cost-chart with our Technical response?
This is a lowest priced, technically acceptable combo. Pricing shall be firm fixed price and listed in the table on the combo.
