STATEMENT OF WORK

CLEANING OF PORTABLE TOILETS

TRAVIS AFB CA

01 Oct 2019

1.0. SCOPE OF WORK: This contract is for the cleaning and servicing of the portable toilets. The contractor shall provide all personnel, equipment, tools, materials, supervision, quality control, and other items and services necessary to complete this contract. Contractor shall comply with all federal, military, state, and local laws and regulations. Portable toilets are to be compliant with all applicable codes, regulations and industry standards including disposal.

2.0. HOURS OF OPERATION: All work shall be performed/serviced once a week between the hours 0700 and 1600 hours. The exact day of the week will be determined at the beginning of the contract award. The contractor shall report to the

Water Fuel System Management (WFSM) shop before cleaning any portable toilets to receive locations/maps/checklist and/or instruction to follow. The Water Fuel shop shall provide an escort to the secure areas. The WFSM shop shall provide quality control in the secure areas. The WFSM is located in building 176 on First

Street near the fire department. The phone numbers for the WFSM shop are 707-

424-8869 and 707-424-1126. Please contract 60 CES/CEOES/Service with any deviations from the schedule prior to servicing units. Service Contracts is located in building 878 on 241 “V” Street. The POC for any concerns shall go through the

4.0. LOCATION: All work on this contract shall be performed on Travis Air Force

Base (AFB), California.

5.0. DESCRIPTION OF WORK: The following outline of the principal features of work does not in any way limit the responsibility of the contractor to perform all work and furnish all materials required for a complete job, within the intent and scope of the contract documents. The following is a general description of the work requirements:

5.1. Contractor shall clean not to exceed eight (8) units in a week inside, outside, pump waste complete removal of wastes, trash, replacement of chemically treated water and replenish deodorizer water in standard portable toilets and portable toilets with sinks. Make and Model of Portable toilets: Satellite Industries., T-20788-1A and Satellite Industries., 1204

5.2. Contractor shall replenish water for hand washing units, toilet tissue, paper towels, disposable toilet seat covers, hand sanitizer, trash bags and soap in units as needed.

5.3. Contractor shall report all problems with the units to the Water Fuels Shop

Supervisor or the Government Point of Contact.

5.4. Contractor shall submit a weekly report on Fridays to confirm the services completed that week with the amounts and types of units serviced and the location to the Government Point of Contact at 60 CES CEOES/Service Contracts. The contractor shall respond to all emails and phone calls within 24 hours.

6.0. ADDITIONAL SERVICES OUT OF CYCLE:

When notified by the CO and/or Government Point of Contact the contractor shall respond within 24 hours. When notified by the CO and/or Government Point of

Contact the contractor shall perform services as prescribed in Description of Work 4.0.

7.0. SITE CLEAN UP: The contractor shall remove all waste to an off base location at the contractor’s expense. All waste shall be disposed of in an approved disposal site.

8.0. EQUIPMENT: The contractor shall provide all equipment and supplies required to complete the work, except for items specifically identified as being government furnished.

9.0. VEHICLES: The contractor shall provide all vehicles necessary to complete this contract. The contractor shall be able to transport all equipment, materials, supplies, and personnel to the job site and perform the required work.

9.1. Vehicles shall have the contractor’s company name displayed in English on both sides and shall be readable up to a distance of at least 50 feet away.

9.2. All vehicles shall be operated in accordance with base traffic regulations. If equipped with communication devices, equipment shall comply with all base and local frequency regulations.

10.0. WORKING CONDITIONS: Perform all work to minimize interference to existing use(s). The contractor shall comply with all federal, state, and local laws pertaining to safety and fire codes and environmental.

11.0. CONTRACTOR EMPLOYEES: The contractor’s supervisor/foreman or representative shall have the capability of speaking and understanding the English language.

12. CUSTOMER SERVICE AND COMPLAINT PROGRAM

12.1. Customer Service/Complaints. The Contractor shall provide a customer service/complaint program that allows the government, at a minimum, to be able to call into the Contractor by telephone or email. The contractor shall reply to all telephone calls and/or emails. The Contractor shall have a process to log or maintain a physical record of the customer(s) complaint(s), the action taken to satisfy the complaint(s) and have available to the Government any open or closed complaint(s) throughout the life of this contract. Contractor shall respond to customer complaints within twenty-four (24) hours of receipt of the complaint.

13. GENERAL INFORMATION.

13.1. QUALITY CONTROL. The contractor shall develop and maintain a quality control program to ensure that all services are performed IAW federal, state and local requirements and commonly accepted commercial practices and standards.

The contractor shall develop and implement procedures to identify, prevent nonconforming services addressing the areas identified in paragraph 2, Service

Delivery Summary. The government evaluator must have a specific quality control inspector to notify in case of customer complaints. Contractor records shall be kept and made available to the Government throughout the contract performance period.

14. QUALITY ASSURANCE. The government will evaluate the contractor’s performance to ensure services meet contract requirements. When the performance threshold has not been met or contractor performance has not been accomplished, the Contracting Officer will be notified to issue a corrective action report to the contractor.

14.1. GOVERNMENT REMEDIES. The contracting officer shall monitor valid customer complaints and implement corrective action(s) for the contractor’s failure to correct unresolved complaints IAE the inspection of service clause.

SS-1. Perform Cleaning Services:

Restocking toilet paper, paper towels, seat covers, hand sanitizer, trash bags, soap and water for the hand washing sinks.

SS-3. Perform Out of Cycle Service:

Respond within 24 hours when notified the government.

6.0 No deficiencies

SS-4. Perform Site Clean Up:

Remove all waste off base

SS-5. Customer Service/Complaints:

Submit weekly reports and reply to phone calls/email. Respond to customer complaints within 24 hours.

No more than 1 late, incomplete or inaccurate submittals/schedules/reports or customer complaints per month
