Statement of Work

Complete Turnkey Services for

Fall Protection for Hangars 808 & 14

1. SCOPE: To outline the procedures and responsibilities in the performance of this Statement of Work (SOW) of both the requesting agency and the vendor the contract is awarded to. This SOW does not in any way limit the responsibility of the vendor to perform all work and furnish all materials required to do a complete job within the intent and scope.

1.1. The vendor is responsible for supplying material, equipment, and labor necessary to perform all operations and setup in accordance with this SOW. The contractor will be responsible for all installation and training for fully operational fall protection system in Hangars 808 and 14.

3. LOCATION: 60 MXS

Hangars 808 & 14, Travis AFB, CA 94535

4. FALL RESTRAINT SPECIFICATIONS:

A. Contractor will provide complete engineering layout by qualified trained engineers** as required by Occupational Safety and Health Administration (OSHA) regulations/meets American National Standards Institute (ANSI) standards **

1. Provide fall protection solution(s) best suited for users based upon information gathered from client and site visit.

2. Provide written proposal identifying scope of work to be performed

3. Contractor will provide complete set of engineered layout drawings illustrating system(s) for client review and approval.

B. Contractor will provide complete fabrication and procurement of Materials for system(s) to be installed.

C. Contractor will install and certify system(s) with qualified trained and certified installers as required by OSHA regulations/meets ANSI standards **

D. Contractor will perma-swage and properly tension all lifeline end connections.

E. Contractor will provide final engineering certification of:

1. Systems

2. Structural Steel Installation

3. Horizontal Lifeline Installation

4. Horizontal Lifeline End Swaging and Pre-Tension Setting

F. Contractor will provide complete Fall Protection Liability coverage of all items noted above.

G. Contractor will provide initial training to Travis AFB and/or its contractors utilizing this system.

H. Approved installer will be one that has been in the business of Fall Protection for no less than 5 years and is familiar with working in Aircraft Hangars.

I. Contractor will maintain a full (5) year warranty

J. Contractor will provide annual re-certification of the systems installed at an additional cost to be determined at the time of the re-cert. Re-certification will be paid by government purchase card as required.

K. Existing System or Special Condition Description

1. Both hangars have overhead cranes that run the length of the hangars

2. Fall protection systems will be indoors in Hangars 808 and 14.

3. Sufficient structural steel to attach systems to (clamps, no welding or drilling)

4. Both hangars are very open, hardly any obstructions

5. C-5/C-17/KC-10, Aircraft will be out of the hangar during installation of the systems.

6. Travis AFB to provide PDF Structural Drawings of Repair Hangars 808 and 14,

7. No special atmospheric considerations; corrosive, or toxic environment such as airborne contaminants or asbestos roofing etc.

8. No known deterioration of existing structural elements.

9. Down Days/Federal Holidays per year. No work will be performed on 5-10 Air Force

Command (AMC) Family or Down Days per year and the following Federal Holidays:

New year’s Day, Martin Luther King Day, President’s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran’s Day, Thanksgiving Day, Christmas Day Standard work hours: Monday – Friday from 0700 to 1700 hrs. Any change in work hours must be approved by the government.

4. SCOPE OF SERVICES

ITEM I. New Fall Arrest Horizontal Lifeline System Turnkey Project

Fall Protection System Scope of Work

1. Contractor to install (8) Uni-lined Uni-8 Horizontal Lifeline Systems or equal at designated area where C-5; C-17; KC-10 and KC-46 aircraft is worked on in repair Hangar 808 and 14.

Systems will be installed above Overhead Crane, so as not to interfere with crane. When utilizing the fall arrest system, overhead crane will need to be locked and tagged-out per

OSHA.

2. All fall protection components will include 8mm cable intermediate brackets, swag tubes, termination toggles and tensioner devices will be fabricated from stainless steel

3. All supplementary steel will confirm to the following plates and angle stock per American Society for Testing and Materials (ASTM).

ASTM A36 rolled shapes ASTM rolled shapes ASTM A992 Grade 50 Round square rectangular tube ASTM A500 Grade B Structured steel will be furnish primed and painted grey Bolts will be ASTM ½ diameter 9/16 holes unless noted otherwise All welds will be in accordance with AWS unless otherwise noted

4. Each Unlined Uni-8 Horizontal Lifeline System or equal will be designed for (2) workers attached to the system at one time.

5. (2) Systems will be over each of the wings, and (4) systems will be located over the fuselage or spine of the plane for a total of (8) systems per building. This will allow for (4) workers to be on either wing or fuselage at one time.

6. Each system will have (2) By-pass trolleys on the cable that will allow the worker to have continuous connection while attached to the system. When the trolley comes to an intermediate the trolley will be able to by-pass it without disconnecting from system.

7. Due to the height of systems above the C-5; C-17; and KC-10 aircraft, the systems will need to utilize DBI/SALAS®motorized Taglines (or equal) to bring the Self Retracting Lifelines (SRL's) down to the workers. Workers will be able to control the motorized taglines with a remote control. There will be a total of (8) motorized taglines to allow workers to transfer from spine of plane to the wings of the plane, and to allow for hookup to system while inside of aircraft.

8. Fabricate all materials as engineered, all fabrications to be completed by AWS certified welders as required by OSHA regulations.

9. Complete system installation performed by qualified Installers as required by OSHA regulations

10. FALL ARREST SYSTEM Designed for (2) 310 lbs. User(s)- at a minimum working height of 25 feet 0 inches

a. All DFP systems shall be designed for their structural integrity using a minimum factor of safety of (2) for LOADING per OSHA requirements for engineered systems for the total number of users specified on each system. All systems are designed to accommodate workers with a total weight of 310 lbs. which includes body weight and all clothing and equipment on the user.

b. Removal and reinstallation of bird netting will be the responsibility of the contractor

L. On-Site Training Session to be conducted during the last day of system installation during the installation phase.

One hour class-room training

c. Training on Fall Protection Basics

d. Training on the use of the actual system installed

e. Training to cover multiple shifts or multiple days can be added at an additional cost.

(1) Hard Copy of Instructional Manuals & Maintenance Records

(1) CD of Instructional Manuals & Maintenance Records

(2) Sets of "As-Built Drawings (AutoCAD)

(2) "Fall Protection Basics" DVDs Handout Outline of Training
