PERFORMANCE WORK STATEMENT (PWS)

WET CHEMICAL FIRE SUPPRESSION SYSTEMS

1. DESCRIPTIONS OF SERVICES. The Contractor shall provide all management, tools, supplies, equipment, material, transportation, labor and accessories necessary to inspect, maintain, test and repair wet chemical fire suppression systems at Altus AFB, Oklahoma in a manner that will ensure continuous and safe operation. Systems and their locations are listed in Appendix A.

1.1. REFERENCES.

1.2. SCHEDULED INSPECTION AND MAINTENANCE SERVICES. The Contractor shall perform semi-annual inspections, preventive maintenance, testing and repairs for each fire extinguisher system identified in Appendix A in accordance with (IAW) the references listed in paragraph 1.1. Scheduled services shall be accomplished during the months of January and July.

1.2.1. The Contractor shall adjust, lubricate, repair, replace, inspect and test extinguishing systems and their associated parts. The Contractor shall perform recurring maintenance and service as required by the manufacturer’s listed procedures and the references listed in paragraph

1.1. The Contractor shall ensure that each system is in a safe and reliable operating condition. A discharge of the chemical is normally not a part of testing.

1.2.2. The Contractor shall clean, weigh, inspect, test (including hydrostatic testing as required), and recharge all system containers as required. Recharge the system per the manufacturer’s instructions.

1.3. SAFETY TESTING: The Contractor shall inspect and test all safety devices, alarms, activators, etc, semi-annually. All tests shall be accomplished IAW NFPA directives and in the presence of Fire Prevention Office (FPO) personnel. Provide proper dust covers to prevent contamination of foods and adjacent area during testing.

1.4. UNSCHEDULED SERVICE CALLS.

1.4.1. The Contractor shall respond to unscheduled service calls during normal operation hours.

The Quality Assurance Office (QAO) shall contact the Contractor to schedule service at a

M Latest Edition mutually acceptable time within five (5) business days. Unscheduled service calls include travel, per diem and the first eight (8) hours of labor for service on site.

1.4.2. Once the Contractor arrives at the job site, the Contractor shall perform diagnosis/ troubleshooting of the problem and provide a written estimate of repair parts needed, shipping/handling fees and estimated hours to troubleshoot and repair the equipment. This must be approved by the QAO, Contract Administrator (CA) or Contracting Officer (CO) prior to the start of repair work. Any changes to estimated pricing while performing repairs (including number of labor hours), must be approved by the QAO prior to continuing work.

1.4.3. If unscheduled maintenance/repair labor hours are to exceed eight (8) hours, the Contractor shall submit a written estimate for the additional labor hours required to complete the repair. Only the approved additional labor hours will be reimbursed.

1.5. REPLACEMENT PARTS. The Contractor shall furnish and use only original manufacturer or approved equal material/parts.

1.5.1. Minor Repair. The Contractor shall repair equipment at Contractor’s expense if the total material cost is equal to or less than $100 per system. The Contractor shall absorb the labor expenditure.

1.5.2. Major Repair. If repair work exceeds $100.00 in materials per system, the Contractor shall provide a written repair cost estimate to the QAO within two (2) business days of finding the deficiency. Estimate shall require the approval of the QAO prior to ordering the replacement part.

The QAO may authorize individual parts up to $3000. Individual parts over $3000 shall be approved by the CA or CO.

1.5.3. The estimate shall include, at a minimum, estimated material costs, estimated labor hours, estimated freight cost and estimated repair/recertification completion date. The Contractor shall not proceed with the repairs without the authorization of the QAO, CA or CO. At no time shall work in excess of $100 be completed without approval. The Contractor shall only be financially reimbursed for the material, freight and labor costs if the authorization is granted.

1.6. COORDINATION OF WORK. Work to be performed is in occupied facilities. The Contractor shall coordinate all down-times of cooking equipment to ensure minimal impact/disruption to occupants and mission. The Contractor is responsible for setting dates/times of inspection/maintenance with the facility managers. The Contractor will notify the QAO (580- 481-5674/7756) and FPO (580-481-5845/5848) no less than one (1) week prior to proposed start of scheduled work. Phone, e-mail and fax notification is acceptable.

1.7. REPORTS. All reports shall include, at a minimum: date of inspection, building number, type of equipment, manufacturer, model number, serial number, work performed, date of certification, repairs accomplished, listing of parts used, overall condition of the equipment and name of technician. All reports shall be submitted to the QAO within fourteen (14) calendar days after completion of service. E-mailed or faxed reports are acceptable.

2. SERVICE SUMMARY (SS). The contract service requirements are summarized in performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimally acceptable levels of service required for each requirement. The Services Summary (SS) and the Contractor’s Quality Control Plan (QCP) provide information on contract requirements, the expected level of Contractor performance and the expected method of Government validation and confirmation of services provided. These thresholds are critical to mission success. Procedures as set forth in the FAR 52.212-4 (a), Contract Terms and Conditions

- Commercial Items, Inspection/Acceptance, will be used to remedy all deficiencies.

2.2. QUALITY ASSURANCE. The QAO and Fire Protection offices will assess the Contractor’s performance. The QAO shall use 100% surveillance, periodic surveillance, random sampling and/or customer complaints as dictated by the QASP. If inspection reveals unacceptable performance, the QAO shall notify the Contract Administrator (CA). The CA will then notify the Contractor and require re-performance of the service (referred to as “call back”).

The Contractor shall be allotted time as agreed to by all parties, but no longer than 48 hours, to return to Altus AFB and accomplish corrective call back work.

3. GOVERNMENT FURNISHED PROPERTYAND SERVICES.

3.1. GOVERNMENT FURNISHED PROPERTY. The Government will furnish without cost to the Contractor a reasonable amount of electricity in connection with the contract performance. The Contractor shall practice energy conservation in the Government provided facilities.

3.2. GOVERNMENT FURNISHED SERVICES. The Government will provide police and fire protection while on Altus AFB. In the event of a medical emergency, call “911” for transportation of an injured employee by civilian ambulance to a local hospital. The Government shall provide emergency medical treatment for life-threatening emergencies only until civilian ambulance arrives.

4. GENERAL INFORMATION.

4.1. CONTRACTOR PERSONNEL. Contractor personnel shall present a neat appearance and be easily recognized as Contractor employees. This shall be accomplished through the wear of distinctive clothing, overcoats or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection. The management and control of Contractor employees in the performance of this contract shall be the responsibility and prerogative of the Contractor.

However, the Contractor shall comply with the contract requirements and Altus AFB regulations concerning conduct of employees.

4.2. CONTRACTOR QUALIFICATIONS. Contractor shall provide only qualified and knowledgeable personnel to perform the tasks required/specified in this PWS. Contractor personnel shall be certified by appropriate federal and state regulatory agencies to meet federal and local requirements for servicing wet chemical fire suppression systems.

4.3. SECURITY REQUIREMENTS. The Contractor shall comply with Altus AFB security measures. These regulations are available for review in the contracting office. Security and base access requirements are contained in clause 5352.242-9000 “Contractor Access to Air Force Installations.” Contract employees will be required to obtain and display identification badges.

4.3.1. Contractor employees shall provide information to assist in the completion of background checks for employees requiring access to Altus AFB. This information shall consist of, but is not limited to: full name (first, middle, last), social security number, date of birth, address, driver’s license number and state of issue, number of years residency in the state of Oklahoma and Jackson County. This information will be protected under the procedures listed in the Privacy Act of 1974. A background check must be completed prior to unescorted access while on Altus AFB. If the background check reveals a Contractor employee or prospective employee with an adverse criminal history or as a potential threat to the health, safety, operational mission of the installation, then the Government reserves the right to restrict or deny access to Altus AFB for that individual.

4.3.2. Contractor Badges. The Contractor is responsible for tracking and retrieving badges from employees upon their separation, termination and/or completion of business on Altus AFB, OK. Badges from employees who are no longer employed on the contract must be retrieved and turned in to the COR within 30 days of the individual’s separation or termination. Employees who abuse their badge privileges by attempting base access beyond the dates, times, locations or specified work and other limitations may be subject to revocation of base access. Businesses may lose base pass privileges if they fail to control use of the badges or do not return them upon completion of use. Failure to control or turn in badges may result in contract termination or a decision not to exercise the option periods of this contract.

4.3.3. Anti-Terrorism. Altus AFB has an antiterrorism program that is responsible for helping secure and protect the base and its personnel from terrorist attacks. As a Contractor working on Altus AFB, you and your sub-contractors are required to obey all orders from Security Forces and adhere to all security measures implemented as a result of Force Protection Condition changes

(security posture changes). Additionally, you and your sub-contractors are expected to report to base security forces (481-7444) any suspicious activities, packages or items you see while conducting work on Altus AFB. Suspicious activities could include personnel conducting surveillance of the installation, unauthorized personnel requesting access to the installation or somebody asking a lot of questions about the base. In an effort to help familiarize you, your employees and sub-contractors about antiterrorism, Altus AFB has an antiterrorism binder with all of the pertinent information for awareness training available for review at the Pass and Registration Office located at the south gate (building #428) of Altus AFB, Oklahoma.

4.3.4. Vehicles. The Contractor shall provide and maintain vehicles for transportation to meet the requirements of this contract. All Contractor vehicles used in the performance of this contract shall be readily identified as property of the company and be maintained to present a professional appearance at all times. Contractor personnel shall operate vehicles in compliance with base traffic regulations. Any violations of base traffic regulations are considered federal offenses and are adjudicated at federal court in Oklahoma City, OK. The current policy for Contractor personnel receiving a Driving Under the Influence (DUI) charge, on base, shall result in that person having their driving privileges on base revoked for a minimum of one year. See clause FAR 5352.242-9000, entitled “Contractor access to Air Force Installations.” All vehicles shall be registered, insured, and operated by a licensed operator at all times.

4.4. SAFETY: The Contractor shall comply with all safety and health requirements necessary for the protection of personnel, facilities and equipment including, but not limited to, all applicable Occupational Safety and Health Administration (OSHA) standards.

4.4.1. Provide a verbal report to the CO or COR as soon as possible of each occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. Provide a completed copy of required Accident Investigation Reports to the CO or COR within five calendar days of each occurrence.

4.5. ENVIRONMENTAL AND HAZARDOUS MATERIAL/WASTE (HM/W)

REQUIREMENTS. Hazardous Materials (HM) are defined in 49 CFR 172.101 and 40 CFR 301-304. Absolutely no HM shall be brought onto Altus AFB until that material is authorized for use by the Civil Engineer Environmental Branch at (580) 481-7156. The Contractor shall submit a list of chemicals with attached Material Safety Data Sheets (MSDS) and other supporting documentation for each HM prior to their use to the Environmental Branch and QAO. The Contractor shall take appropriate actions to comply with waste minimization and pollution prevention practices and policies.

4.5.1. Spill Control. The Contractor shall maintain spill control material on hand at all times sufficient to contain a worst-case spill, both volume and hazard level. Contractor is responsible for disposal of all material used during spill clean-up. Materials must be disposed of IAW local, state and/or federal regulations. If the Contractor spills or releases any hazardous substances/hazardous waste into the environment, the Contractor shall immediately report the incident to the Fire Department and the QAO/CO. The Contractor shall be liable for the costs of clean up and remediation of any spills or the release of such substances into the environment.

4.5.2. Hazardous Materials Management. The Contractor shall remove all hazardous materials by the end of each workday, unless permission is granted by CE Environmental Branch.

If permission is granted, hazardous materials shall be stored in locked cabinets in the storage yard of building 501. Contractor shall not conduct process treatments (e.g. paint thinner solvent recycling) on base. Contractor is responsible for proper storage, labeling and security of each cabinet.

4.5.3. Contractor Environmental Awareness Training. The Contractor shall ensure all employees that work on Altus AFB complete the Altus AFB Environmental Management System (EMS) Awareness training IAW AFI 32-7001, paragraph 5.6, Environmental Management within 30 days of starting work. The EMS Contractor Awareness training can be accessed at http://www.altus.af.mil/altusafbenvironmentalnews.asp. The Contractor shall submit a list of the employee names that have completed this training to the QAO within 30 days of employee starts date. This is a one-time requirement for all current and new Contractor/contract employees working on Altus AFB.

4.6. WARRANTY OF WORK. The Contractor warrants that work performed under this contract, including work performed by any subcontractor, shall conform to the contract requirements and shall be free of defects for 30 days after completion of work. The Contractor shall remedy, at the Contractor’s expense, any damage to Government owned or controlled real or personal property. The terms of this warranty shall continue for (30) days after the completion of this contract.

4.7. HOURS OF OPERATION. The Contractor shall perform services during normal duty hours which are 0700 to1700, Mondays through Fridays, excluding federal holidays. The Contractor must request/coordinate to work weekends or holidays through the QAO and/or FPO.

4.7.1. Recognized Holidays. The Contractor is not required to provide service on federal holidays, except in emergency situations. The holidays are:

If these holidays fall on Saturday, the preceding Friday will be observed. If these holidays fall on Sunday, the following Monday will be observed. If a holiday falls on a scheduled service day, the Contractor will be responsible for rescheduling services for the first day post the holiday observance.

4.7.2. Base Closures. Work scheduled but not accomplished because of base closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening the base.

4.7.3. Performance of Services During Crisis OR Heightened Security. Services at Altus AFB are not considered mission essential and service will not continue in the event of a crisis or heightened security caused by a national emergency, natural disaster or other cause, as necessary, in support of the Air Force mission. Contractor employees with approved background checks will be allowed access to the base for performance of work, except during “Charlie” conditions. If the “Charlie” condition exists for an extended duration, some services may be required. The CO may direct that services continue on a case-by-case basis.

4.8. CONTRACT MODIFICATIONS. Periodically, during the contract performance period, the Government may equipment decommissioned from use or new equipment installed. If this occurs, a modification to the contract shall be initiated to incorporate changes to requirements, if warranted.
