PERFORMANCE WORK STATEMENT (PWS)

1. DESCRIPTION OF SERVICES. The Contractor shall furnish all personnel, supervision, labor, equipment, supplies, tools and other items to complete operations necessary to perform recurring maintenance, repair, testing and certification of cranes, hoists and monorail telescoping platform systems at Altus Air Force Base (AFB), Oklahoma, as listed in Exhibit 1.

1.1. SCHEDULED MAINTAINANCE OF CRANES, HOISTS AND PLATFORMS.

1.1.1. Scheduled inspections shall include annual weight capacity testing (load test) on all equipment included in this contract. The Contractor shall also perform non-destructive inspection testing (NDI Test) for all hooks, latches, chains and wire ropes. The Contractor shall complete all annual load testing and NDI testing during the month of May.

1.1.2. Scheduled maintenance shall include, but is not limited to, testing, cleaning, greasing, lubrication, and adjustment of motors, bearings, rollers, switches, brakes (discs) and clutches.

Contractor shall provide supply consumable items such as grease, oil, hydraulic fluid, etc., at no additional cost to the Government. Scheduled maintenance will also include tightening/replacing nuts, bolts, screws, inspection/repair of cables, end stops, repair/replacement of all electrical connections, loose wires, strobe lights, horns, lights and fuses.

1.1.2.1. Scheduled Maintenance for Cranes and Hoists. The Contractor shall inspect, maintain and repair all cranes and hoists at semi-annual intervals. The Contractor may divide the equipment listed in Exhibit 1 over the 12 month period, ensuring that each crane/hoist is serviced every 6 months. The Contractor shall also clean equipment thoroughly to remove any accumulation of dirt. Remove all debris from catwalks and cabs. Any rust spots shall be cleaned, prepped, primed and touch-up painted to match existing colors.

1.1.2.2. Scheduled Maintenance for Monorail Platforms. The Contractor shall perform monthly, quarterly and semi-annual inspections and maintain/repair on three (3) TC/American telescoping monorail platform lifts in Hangar 517. These lifts are comprised of two (2) wing platforms (right and left) and one (1) tail-stand platform. Platforms are utilized in the maintenance and washing of military aircraft. The Contractor shall clean equipment thoroughly to remove any accumulation of dirt/debris from catwalks and cabs. Any rust spots shall be cleaned, prepped, primed and touch-up painted to match existing colors.

1.1.3. Testing. Testing shall be accomplished prior to the expiration date of the previous certification. Upon certification/recertification, each unit shall be labeled with the appropriate certification tag.

1.1.3.1. Inspection and certification testing shall be done in accordance with (IAW) the following references:

- 29 CFR 1910.179, Overhead Crane Safety

- Crane Manufacturers Association of America (CMAA)

- ASME/ANSI B30.5, Safety Requirements for Mobile and Locomotive Cranes

- ASME/ANSI B30.11, Monorails and Under-hung Cranes

- ASME/ANSI B30.16, Overhead Hoist Inspection

- Air Force Instruction (AFI) 91-203, Air Force Consolidated Occupational Safety

- Other current publications relating to weight testing standards.

1.1.3.2. Observed defects that require testing be stopped shall be reported immediately to the Service Contract Office (SCO) and testing shall be suspended until the deficiency is corrected.

Once repairs are made, the inspection/testing shall be re-performed at no additional cost to the Government.

1.2. REPAIRS ON EQUIPMENT SCHEDULED FOR MAINTENANCE.

1.2.1. Scheduled repairs are those that are identified during scheduled maintenance inspections that can be accomplished during the scheduled maintenance visit. The Contractor has one (1) working day to notify the SCO, in writing, of any deficiencies to the inspected equipment that decertify the equipment or that make the equipment inoperable. The Contractor shall perform diagnosis of the problem and provide a written estimate of repair parts, shipping/handling fees and estimated hours to repair.

1.2.2. The SCO must approve the proposal prior to start of repair work. Any changes to estimated pricing while performing repairs (including number of labor hours), must be approved by the SCO prior to continuing work.

1.2.3. If necessary, after repairs or maintenance, the Contractor shall retest and recertify the equipment at no additional cost to the Government.

1.2.4. Travel and per diem will not be reimbursed for repairs on equipment programmed for the scheduled maintenance visit. Approved labor hours exceeding the eight (8) initial hours of service on the equipment shall be reimbursed.

1.2.5. If the repair cannot be completed during the scheduled maintenance visit (i.e. parts must be ordered), then the repair will be accomplished at a later date as an unscheduled service call.

1.3. UNSCHEDULED SERVICE CALLS. The Contractor shall provide unscheduled maintenance/repair services on equipment items listed in Exhibit 1, as necessary, between regularly scheduled maintenance and inspection visits. Unscheduled service calls include travel, per diem and the first eight (8) hours of labor for service on site.

1.3.1. Upon notification of service requirement, the Contractor shall call the SCO within eight (8) duty hours to schedule service at a mutually acceptable time. If the Contractor cannot respond due to severe weather conditions, the Contractor shall contact the SCO to schedule an alternate arrival date.

1.3.2. Once the Contractor arrives at the job site, the Contractor shall perform diagnosis/ troubleshooting of the problem and provide a written estimate of repair parts needed, shipping/handling fees and estimated hours to troubleshoot and repair the equipment. This must be approved by the SCO, Contract Administrator (CA) or Contracting Officer (CO) prior to the start of repair work. Any changes to estimated pricing while performing repairs (including number of labor hours), must be approved by the SCO prior to continuing work.

1.3.3. If unscheduled maintenance/repair labor hours are to exceed eight (8) hours, the Contractor shall submit a written estimate for the additional labor hours required to complete the repair. Only the approved additional labor hours will be reimbursed.

1.4. ADDITIONAL UNSCHEDULED MAINTENANE CALL WHILE ON ALTUS AFB.

If the Contractor is called to perform an additional unscheduled maintenance while on Altus AFB, the Contractor shall provide a written estimate of repair parts needed, shipping/ handling fees and estimated hours to troubleshoot and repair the equipment.

1.4.1. Once approval is given by the SCO, the Contractor may order parts and begin repair.

1.4.2. Travel and per diem will not be reimbursed for the additional unscheduled repair while already on Altus AFB for another service. Approved labor hours shall be reimbursed.

1.5. REPLACEMENT PARTS. The Contractor shall furnish and use only original manufacturer or approved equal material/parts.

1.5.1. Minor Repair. The Contractor shall repair equipment at Contractor’s expense if the total material cost is equal to or less than $100 per crane or hoist. The Contractor shall absorb the labor expenditure.

1.5.2. Major Repairs. If repair work exceeds $100.00 in materials per crane or hoist, the Contractor shall provide a written repair cost estimate to the SCO within two (2) business days of finding the deficiency. Estimate shall require the approval of the SCO prior to ordering the replacement part. The SCO may authorize individual parts up to $3000.00. Individual parts over $3000.00 shall be approved by the CA or CO.

1.5.2.1. The estimate shall include, at a minimum: estimated material costs, estimated labor hours, estimated freight cost and estimated repair/recertification completion date. The Contractor shall not proceed with the repairs without the authorization of the SCO, CA or CO. At no time shall work in excess of $100 be completed without approval. The Contractor shall only be financially reimbursed for the material, freight and labor costs if the authorization is granted.

1.6. COORDINATION OF WORK. All work is to be performed when hangars are free of aircraft, except as noted below. Locations and work times are to be coordinated through the SCO.

Contractor will notify the SCO no less than one (1) week prior to proposed start of scheduled work.

1.6.1. Aircraft in Hangar Coordination. In the event that work must be performed while an aircraft is in the hangar, the Contractor employee will be escorted by a Government worker. The hangar facility custodian shall assign a Government worker with a restricted area badge to be present during service by the Contractor. If an escort is not available, the Contractor will not perform service in the hangar until free of aircraft. If necessary, the Contractor will make alternate arrangements with the SCO to complete the service.

1.7. REPORTS AND DOCUMENTATION. All reports shall include, at a minimum: date of inspection, building number, type of equipment, manufacturer, model number, serial number, work performed, date of certification, repairs accomplished, listing of parts used, overall condition of the equipment and name of technician. All reports shall be submitted to the SCO within fourteen (14) calendar days after completion of service. E-mailed or faxed reports are acceptable.

1.7.1. Scheduled Maintenance Reports. The Contractor shall submit written reports and documentation for the preventive maintenance, inspection, testing, repair, load testing and certification of each crane and hoist serviced.

1.7.2. Certificates. Load test and NDI test certificate documentation shall be maintained by the Contractor and a copy shall be provided to the SCO.

1.7.3. Unscheduled Maintenance Reports. Reports for all unscheduled maintenance/repair calls shall be submitted in writing.

2. SERVICE SUMMARY (SS). The Contractor service requirements are summarized into performance thresholds that relate directly to mission essential items. The stated performance thresholds are the minimum acceptable levels of service for each stated requirement. the SS provides information on contract requirements, the expected level of Contractor performance and the expected method of Government validation and confirmation of services provided. These thresholds are critical to mission success. Procedures as set forth in the FAR 52.212-4 (a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance, will be used to remedy all deficiencies.

2.1. QUALITY ASSURANCE. The Contracting Officer Representative (COR) from the SCO will assess the Contractor’s performance and adherence to standards using the SS table. The COR shall use 100% surveillance, periodic surveillance, random sampling and/or customer complaints as dictated by the SS. If inspection reveals unacceptable performance, the COR shall notify the CA. The CA will then notify the Contractor and require re-performance of the service (referred to as “call back”). The Contractor shall be allotted time as agreed to by all parties, but no longer than 72 hours, to return to Altus AFB and accomplish corrective call back work.

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES.

3.1. GOVERNMENT FURNISHED PROPERTY. The Government will furnish, without cost to the Contractor, all reasonable amounts of electricity in connection with the contract performance. The Contractor shall practice energy conservation in Government facilities.

3.2. GOVERNMENT FURNISHED SERVICES. The Government will provide police and fire protection while on Altus AFB. In the event of a medical emergency, call “911” for transportation of an injured employee by civilian ambulance to a local hospital. The Government shall provide emergency medical treatment for life-threatening emergencies only until civilian ambulance arrives.

4. GENERAL INFORMATION.

4.1. CONTRACTOR QUALIFICATIONS. Contractor employees shall be certified to inspect all hoists, cranes and monorail platforms in accordance with the references list in paragraph 1.1.3. In addition, Contractor employees shall be certified or licensed technicians familiar with the particular crane and hoist system(s). Copies of technician certification or licensure shall be submitted upon request.

4.1.1. Contractor must have the capability to access the Programmable Logic Center (PLC) of the TC/American Monorail Platforms in order to successfully diagnose system malfunctions.

4.2. CONTRACTOR PERSONNEL. Contractor personnel shall present a neat appearance and be easily recognized as Contractor employees. This shall be accomplished through the wear of distinctive clothing, overcoats or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection. The management and control of Contractor employees in the performance of this contract shall be the responsibility and prerogative of the Contractor.

However, the Contractor shall comply with the contract requirements and Altus AFB regulations concerning conduct of employees.

4.3. SECURITY REQUIRMENTS. The Contractor shall comply with Altus AFB security measures. These regulations are available for review in the contracting office. Security and base access requirements are contained in clause 5352.242-9000 “Contractor Access to Air Force Installations.” Contract employees will be required to obtain and display identification badges.

4.3.1. Contractor employees shall provide information to assist in the completion of background checks for employees requiring access to Altus AFB. This information shall consist of, but is not limited to: full name (first, middle, last), social security number, date of birth, address, driver’s license number and state of issue, number of years residency in the state of Oklahoma and Jackson County. This information will be protected under the procedures listed in the Privacy Act of 1974.

A background check must be completed prior to unescorted access while on Altus AFB. If the background check reveals a Contractor employee or prospective employee with an adverse criminal history or as a potential threat to the health, safety, operational mission of the installation, then the Government reserves the right to restrict or deny access to Altus AFB for that individual.

4.3.2. Contractor Badges. The Contractor is responsible for tracking and retrieving badges from employees upon their separation, termination and/or completion of business on Altus AFB, OK. Badges from employees who are no longer employed on the contract must be retrieved and turned in to the COR within 30 days of the individual’s separation or termination. Employees who abuse their badge privileges by attempting base access beyond the dates, times, locations or specified work and other limitations may be subject to revocation of base access. Businesses may lose base pass privileges if they fail to control use of the badges or do not return them upon completion of use. Failure to control or turn in badges may result in contract termination or a decision not to exercise the option periods of this contract.

4.3.3. Anti-Terrorism. Altus AFB has an antiterrorism program that is responsible for helping secure and protect the base and its personnel from terrorist attacks. As a Contractor working on Altus AFB, you and your sub-contractors are required to obey all orders from Security Forces and adhere to all security measures implemented as a result of Force Protection Condition changes (security posture changes). Additionally, you and your sub-contractors are expected to report to base security forces (481-7444) any suspicious activities, packages or items you see while conducting work on Altus AFB. Suspicious activities could include personnel conducting surveillance of the installation, unauthorized personnel requesting access to the installation or somebody asking a lot of questions about the base. In an effort to help familiarize you, your employees and sub-contractors about antiterrorism, Altus AFB has an antiterrorism binder with all of the pertinent information for awareness training available for review at the Pass and Registration Office located at the south gate (building #428) of Altus AFB, Oklahoma.

4.3.4. Vehicles. The Contractor shall provide and maintain vehicles for transportation to meet the requirements of this contract. All Contractor vehicles used in the performance of this contract shall be readily identified as property of the company and be maintained to present a professional appearance at all times. Contractor personnel shall operate vehicles in compliance with base traffic regulations. Any violations of base traffic regulations are considered federal offenses and are adjudicated at federal court in Oklahoma City, OK. The current policy for Contractor personnel receiving a Driving Under the Influence (DUI) charge, on base, shall result in that person having their driving privileges on base revoked for a minimum of one year. See clause FAR 5352.242-9000, entitled “Contractor access to Air Force Installations.” All vehicles shall be registered, insured, and operated by a licensed operator at all times.

4.4. SAFETY. The Contractor shall comply with all safety and health requirements necessary for the protection of personnel, facilities and equipment including, but not limited to, all applicable Occupational Safety and Health Administration (OSHA) standards.

4.4.1. The Contractor shall provide and place signage as needed to safely cordon areas and clear zones where cranes and hoists are being serviced.

4.4.2. The Contractor shall notify the SCO if any hoist, crane or platform that when used would violate fire or safety regulations or that would pose a risk to life or property. The SCO shall locked-out and tag-out the equipment. The tag shall indicate the problem, corrective action required, date condition was noted and the anticipated repair date. The Contractor shall immediately notify the SCO in bldg. 358, (580) 481-7756/5674/5675, the operations office and the respective facility manager of the defective and tagged-out equipment.

4.4.3. The Contractor shall obtain required open flame and welding permits from the Base Fire Department, Bldg. 288, (580) 481-5845/5846 before commencing with any welding, torch cutting or burning operations.

4.4.4. The Contractor shall notify the SCO (580-481-7756/5674/5675) of any crane, hoist or platform that, when used, would violate fire or safety regulations or that would pose a risk to life or property. The SCO shall lock-out and tag-out the equipment. The tag shall indicate the problem, corrective action required, date condition was noted and the anticipated repair date.

When the Contractor returns to the site for repairs, they shall notify the SCO to unlock the equipment.

4.4.5. Provide a verbal report to the CO or COR as soon as possible of each occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. Provide a completed copy of required Accident Investigation Reports to the CO or COR within five calendar days of each occurrence.

4.5. ENVIRONMENTAL AND HAZARDOUS MATERIAL/WASTE (HM/W)

REQUIREMENTS. Hazardous Materials (HM) are defined in 49 CFR 172.101 and 40 CFR 301-304. Absolutely no HM shall be brought onto Altus AFB until that material is authorized for use by the Civil Engineer Environmental Branch at (580) 481-7156. The Contractor shall submit a list of chemicals with attached Material Safety Data Sheets (MSDS) and other supporting documentation for each HM prior to their use to the Environmental Element and SCO. The Contractor shall take appropriate actions to comply with waste minimization and pollution prevention practices and policies.

4.5.1. Spill Control. The Contractor shall maintain spill control material on hand at all times sufficient to contain a worst-case spill, both volume and hazard level. Contractor is responsible for disposal of all material used during spill clean-up. Materials must be disposed of IAW local, state and/or federal regulations. If the Contractor spills or releases any hazardous substances/hazardous waste into the environment, the Contractor shall immediately report the incident to the Fire Department and the SCO/CO. The Contractor shall be liable for the costs of clean up and remediation of any spills or the release of such substances into the environment.

4.5.2. Hazardous Materials Management. The Contractor shall remove all hazardous materials by the end of each workday, unless permission is granted by CE Environmental Branch. If permission is granted, hazardous materials shall be stored in locked cabinets in the storage yard of building 501. Contractor shall not conduct process treatments (e.g. paint thinner solvent recycling) on base. Contractor is responsible for proper storage, labeling and security of each cabinet.

4.5.3. Contractor Environmental Awareness Training. The Contractor shall ensure all employees that work on Altus AFB complete the Altus AFB Environmental Management System (EMS) Awareness training IAW AFI 32-7001, paragraph 5.6, Environmental Management within 30 days of starting work. The EMS Contractor Awareness training can be accessed at http://www.altus.af.mil/About-Us/Environmental-Information.aspx. The Contractor shall submit a list of the employee names that have completed this training to the SCO within 30 days of employee starts date. This is a one-time requirement for all current and new Contractor/contract employees working on Altus AFB.

4.6. WARRANTY OF WORK. The Contractor warrants that work performed under this contract, including work performed by any subcontractor, shall conform to the contract requirements and shall be free of defects for 30 days after completion of work. The Contractor shall remedy, at the Contractor’s expense, any damage to Government owned or controlled real or personal property. The terms of this warranty shall continue for (30) days after the completion of this contract.

4.7. CLEAN-UP. The Contractor shall keep the work area orderly and in a hazard-free condition while job site is left unoccupied during breaks, etc. The Contractor shall be responsible for clean-up of the job site on a daily basis including removal of all waste from site areas. All debris and waste shall be properly disposed of at an off base property.

4.8. HOURS OF OPERATION. The Contractor shall perform services during normal duty hours which are 0700 to1700, Mondays through Fridays, excluding federal holidays. The Contractor must request/coordinate to work weekends or holidays through the SCO.

4.8.1. Recognized Holidays. The Contractor is not required to provide service on federal holidays, except in emergency situations. The holidays are:

If these holidays fall on Saturday, the preceding Friday will be observed. If these holidays fall on Sunday, the following Monday will be observed. If a holiday falls on a scheduled service day, the http://www.altus.af.mil/About-Us/Environmental-Information.aspx

Contractor will be responsible for rescheduling services for the first day post the holiday observance.

4.8.2. Base Closures. Work scheduled but not accomplished because of base closure due to weather, exercises or actual alert will be accomplished as soon as possible after reopening the base.

4.8.3. Performance of Service During Crisis or Heightened Security. In the event of crisis or heightened security caused by a national emergency, natural disaster or other causes, continued performance is necessary in support of the Air Force mission. The contract price and delivery schedule may be adjusted to reflect any change of work that may be directed by the CO.

4.9. INVOICING. The Contractor shall invoice through Wide Area Work Flow (WAWF).

Invoicing should occur every month to keep the contract up-to-date. Contractor shall bill using the following CLINs:

CLIN X001 – Scheduled Maintenance - Semi-Annual Inspections (includes all cranes and hoists except 3 monorail platforms in bldg 517) (Paragraph 1.1.)

CLIN X002 – Scheduled Maintenance - Monthly, Quarterly, Semi-Annual Inspections (for 3 monorails in bldg 517) (Paragraph 1.1.)

CLIN X003 – Unscheduled Maintenance Service Calls (Includes initial 8 hours labor, travel and per diem) (Paragraph 1.3.)

CLIN X004 – Parts Reimbursement (approved parts and freight) (Paragraph 1.5.)

CLIN X005 – Labor Hours (must be approved by the SCO or CO for reimbursement) (Paragraph 12.4., 1.3.3. and 1.4.2.) Examples include:

- Scheduled Repair labor hours over the eight (8) initial hours of work on the unit.

- Additional labor hours for Unscheduled Maintenance Calls over the initial (8) hours of work on the unit.

- Labor hours for an additional Unscheduled Maintenance Call while on Altus AFB performing other services.

4.10. CONTRACT MODIFICATIONS. Periodically, during the contract performance period, the Government may have equipment decommissioned from use or new equipment installed. If this occurs, a modification to the contract shall be initiated to incorporate changes to requirements, if warranted.
