PERFORMANCE WORK STATEMENT (PWS)
ALTUS AIR FORCE BASE (AFB) CHAPEL
CATHOLIC MASS MUSICIAN Page 1 of 4
30 September 2018 to 29 September 2019
1. Contract Position Title: Catholic Mass Musician
2. Performance Location: Altus Air Force Base (AFB) Chapel, Bldg 301, or alternate location determined by the Designated Chaplain.
3. Scope of Responsibilities: The Catholic Mass Musician (“Contractor”) will, in coordination with the
Designated Chaplain, have responsibility for providing music for the Catholic Mass.
4. Core Working Hours: The Designated Chaplain and Contractor will determine rehearsal times, and core working hours will not exceed 120 weekly rehearsals and worship services for all 52 weeks of contractual year. Worship services will be held on Sundays from 9:00 AM to 10:00 AM. (Contractor will be notified at least seven (7) in advance of exceptions to dates and times of rehearsals and worship services.)
5. Education and Experience: The Contractor must:
5.1. Be at least 21 years of age.
5.2. Possess a minimum of three (3) years’ experience within the last five (5) as a Musician with the ability to read music and play a lead instrument (i.e., guitar, piano, and/or keyboard) to accompany the Catholic Mass Music Director.
5.3. Be positive, self-directed, motivated, and able to interface well within the interfaith Air Force chapel environment with chaplains and volunteers from various religious backgrounds. Possess an ability to maintain healthy, functional interpersonal relationships with staff and congregants, and also maintain professional standards of conduct and public decorum, including but not limited to dress and grooming.
5.4. Display active membership/attendance at a church/chapel with a letter of endorsement from their current pastor/chaplain.
5.5. Provide the following: (a) a video segment performing a minimum of a five (5) traditional and contemporary-style songs; and (b) a complete resume with academic and at least three (3) work related references including phone numbers and points of contact. Previous work performance and/or letters of recommendation may also serve to supplement a resume as factors in the applicant selection process.
6. General Responsibilities. The Contractor will:
6.1. Provide musical selections for the Catholic Mass as coordinated with Designated Chaplain and the Music Director.
6.2. In support of the Music Director, provide direction to additional musicians who may play a variety of instruments to include, but not limited to, piano, organ, guitar (acoustic, electric, and bass), drums and woodwinds.

6.3. Coordinate weekly music rehearsals on an agreed upon date with the Music Director and Designated Chaplain.
6.4. At a minimum, meet twice monthly or as reasonably needed with the Designated Chaplain and
Music Director to access the quality of the music program. This meeting will also be used to plan future musical performances.
6.5. Be knowledgeable in areas of Air Force Instructions (AFIs) relevant to chapel functions and/or willing to gain that knowledge to include financial procedures necessary for compliance with Air Force directives dealing with security and conflict of interest issues. Knowledge of scheduling policies is also needed to prevent conflicts with other users of chapel facility and resources.
6.6. Be able to successfully obtain a favorable background check if required. Failure to accomplish a favorable background check IAW DoDI 1402.5, Criminal History Background Checks will result in an immediate termination of the contract in accordance with FAR 52.212-4. The Contractor will upon request complete (IRC), AF Form 2583, items 1-7 only, Volunteer Questionnaire, Acknowledgement of Rights/Consent Release Records, and Statement of Understanding.
Additionally, an SF 85c, Questionnaire for Non-Sensitive Positions will be accomplished. These background checks will be accomplished at the expense of the chapel. Contractor will not be allowed to work with children under the age of 18 alone until background check is complete.
6.7. Be accountable for security and orderliness of all facilities and equipment utilized in conjunction with their programs. At completion of each service and rehearsal, ensures stage is returned to a neutral status and left in a neat and orderly fashion as directed by the Chapel Staff. Secures the building following rehearsals and services as required.
7. Specific Responsibilities. The Contractor will:
7.1. Provide music in support of the Music Director. All music performance/rehearsal schedules will be as follows:
7.1.1. Conduct one (1) worship service per week or as requested by the Designated Chaplain;
7.1.2. Conduct one (1) rehearsal per weekday in support of the Music Director as requested by the Designated Chaplain; and
7.1.3. Conduct Special Worship Service or rehearsal(s) as requested by the Designated Chaplain.
7.2. Arrive a minimum of 30 minutes prior to any rehearsal or worship service to insure that proper preparations of the facility, sound system setup (including microphones, music stands, etc), music and any necessary coordination with the Designated Chaplain has been made. The Contractor will also make sure all facilities and equipment are returned to their original position/condition per the direction of the Chapel Staff and relevant procedures.
7.3. Be accountable for paying any Social Security payments, Federal or Local Income Taxes, and
State Unemployment Insurance premiums for themselves, as well as any individual employed or subcontracted by them to provide services under this contract.

7.4. Personally perform the services described above or coordinate performance by a substitute of equal or higher proficiency who has been approved in advance by the Designated Chaplain. Submit names and qualifications or proposed Subcontractors to the Designated Chaplain at least 14 days prior to the proposed absence. Subcontractors must meet the requirements for the position as outlined in this PWS. Contractor is responsible for payment to Subcontractor.
8. The Designated Chaplain.
8.1. Designated Chaplain will be appointed by the Wing chaplain and will provide all necessary supplies and equipment needed for the Contractor to meet the terms of this PWS.
8.2. Provides building keys necessary to perform all programs. The Contractor shall be responsible for keys provided by the government and for security of facilities and property made available by the government in conjunction with the performance of this contract. Keys shall be safeguarded and will not be duplicated. All keys will be returned on revocation of this contract. The Contractor will report any duplicated, lost or misplaced keys to the government within one (1) hour of the discovery of the occurrence. The Contractor will reimburse the government for replacement of all locks or re-keying required as a result of keys being duplicated, lost or misplaced by the Contractor or while under the responsibility of the Contractor.
9. Installation Access and Background Checks: This check is conducted to allow for the issuing of a
Contractor Identification/Common Access Card (CAC). Contractor must be able to meet Altus AFB installation access requirements:
9.1. Altus AFB has an antiterrorism program that is responsible for helping secure and protect the base and its personnel from terrorist attacks. As a Contractor working on Altus AFB, you and your sub-Contractors are required to obey all orders from Security Forces, and adhere to all security measures implemented as a result of Force Protection Condition changes (security posture changes). Additionally, you and your sub-Contractors are expected to report to base security forces any suspicious activities, packages, or items you see while conducting surveillance of the installation, unauthorized personnel requesting access to the installation, or somebody asking a lot of questions about the base. In an effort to help familiarize you, your employees, and sub- Contractors about antiterrorism, Altus AFB has an antiterrorism binder with all of the pertinent information for awareness training available for review at the Pass and Registration Office located at the south gate (building #428) of Altus AFB, Oklahoma.
9.2. Pass and Identification Items: The Contractor shall ensure the following pass and identification items required for contract performance are obtained for employees and non-government owned vehicles requiring access to Altus AFB to perform contract requirements:
9.3. The Contractor is responsible for providing information to assist in completing background checks for those employees requiring access to Altus AFB. This information shall consist of, but not limited to, the following: full name to include first, middle, last, social security number, date of birth, address, driver's license number and state of issue, number of years resident of the State of Oklahoma and Jackson County. This information will be protected under the procedures listed under the Privacy Act of 1974. A background check must be completed prior to unescorted access.
Those Contractor employees whose background checks reveal adverse criminal history may be denied access to Altus AFB.

9.4. The Contractor is responsible for tracking and retrieving badges from employees upon separation, termination and/or completion of business on Altus AFB. Badges from employees no longer working on the contract must be turned in to the Contracting Office within 30 days of leaving.
9.5. Contractors may lose base pass privileges if they fail to control use of badges or do not return them upon completion of use. Excessive abuse may result in termination or a decision not to exercise option periods for this contract. Employees who abuse their badge privileges by attempting base access beyond the dates, times, locations or specified work and other limitations may be subject to revocation of base access.
10. Other.
10.1. Working Hours and Extended Period of Absence. Work hours must ensure successful accomplishment of contract requirements. The Contractor is required to give the Designated Chaplain a two (2) week notice of any extended period of absence. Extended period of absence is defined as an absence for more than three (3) days. The Contractor will provide a Subcontractor to perform services in their absence. This Subcontractor must be of equal or greater capability, have a successfully completed background check, and be approved by the Designated Chaplain prior to performance of work. Contractor shall be responsible for payment to any Subcontractor.
10.2. Compliance. The Designated Chaplain will monitor and inspect the Contractor’s performance.
The Contractor will be notified of any and all noncompliance, and will be given an opportunity to correct, if possible, nonconforming work. The Contractor will also be given an opportunity to respond to any notice of noncompliance.
10.3. Registration. Contractor must be registered in System for Award Management (SAM) before providing a quote on contract.

10.4. Payment. Rate will be based on a competitive quote. Quotes submitted will contain the individual’s name and price quote per month. The Contractor will be responsible for filing any applicable social security, federal, or state taxes. Contractor will be required to submit monthly invoices through Wide Area Workflow (WAWF) before payment is made. The Defense Finance Accounting Service (DFAS) will make payment within 30 days of receipt of a valid invoice.
10.5. Local Installation Environmental Management System (EMS) Policy: Altus AFB has implemented an EMS under the guidelines of AFI 32-7001 and ISO14001. As a result of this every contract employee that will physically work on the installation is required to take a 10-15 minute on-line EMS awareness training session. The Contractor shall ensure all employees, prior to their performance of any contract work on the installation, review the slide presentation titled "Altus AFB Contractor EMS Awareness" on the link provided below.
