PERFORMANCE WORK STATEMENT (PWS)

FOR

ELEVATOR (VTE) MAINTENANCE, REPAIR AND INSPECTION SERVICES

1. DESCRIPTION OF SERVICES. The Contractor shall provide all management, tools, supplies, equipment, parts and labor necessary to maintain, repair and inspect the real property vertical transportation equipment (VTE) at Altus AFB, OK in a manner that will ensure continuous and safe operation (full maintenance and repair with the exception of the exclusions listed in paragraph 1.3. below). The elevators (VTE) and their locations are listed in Appendix A.

1.1. FULL MAINTENANCE SERVICES. This full maintenance contract signifies the elevator Contractor takes total service responsibility for the equipment identified in the contract.

Except those services covered in paragraph 1.3. and subparagraphs below, the monthly fixed price includes all inspections, testing, maintenance, repairs, part replacements, routine and emergency service calls. This contract allows the Government to budget total yearly costs and eliminates concerns relating to individual parts repair or replacement invoicing. The Contractor assumes all responsibility and determines the amount of service visits and types of service required to keep the elevators operating safely 95% of the time and within performance objectives of paragraph 2. Calculation of VTE availability is as follows:

1.2. REFERENCES. Work conducted on all elevators shall be in accordance with (IAW) the American Society of Mechanical Engineers (ASME) A17.1-2016 (or applicable code for year VTE put in service); Safety Code for Elevators and Escalators (ASME A17.1); American National Standards Institute (ANSI)/ASME A17.2-2010, Guide for Inspection of Elevators, Escalators and Moving Walks (applicable to equipment); ANSI/ASME A17.3-2015, Safety Code for Existing Elevators and Escalators (applicable to equipment); National Fire Protection Association Standards (NFPA); Unified Facilities Criteria (UFC) 3-600-01, Fire Protection Engineering for Facilities; Occupational Safety and Health Administration (OSHA) Standards and the recommendations of the original equipment manufacturer (OEM).

1.3. EXCLUDED SERVICES. The following services are not covered under this full maintenance service contract and are considered over and above services. These services shall be paid on an over and above Contract Line Item Number (CLIN) or as a stand-alone contract for repair.

1.3.1. Alterations (a.k.a. modernizations) are not covered in the full maintenance service contract. (See Paragraph 1.4.2.)

1.3.2. Acts by parties other than the Contractor of vandalism, abuse, negligence and damage resulting from rescue and recovery operations or acts of God (including damage resulting from emergency power generator power spikes and low quality power) are not included in the contract.

1.3.3. Additional items excluded are cosmetic, construction and ancillary components of the elevator systems including the repair, construction, refinishing or replacement of the following:

the cab enclosure; car and hoistway enclosures; ceiling frames; car and hoistway door panels;

door frames and sills; rams and cylinders; car flooring and floor covering; main line power switches and power breaker(s); hydraulic elevator jack outer housing; magnetic traction systems, buried pipe or conduit; smoke/fire sensors and fire service systems; power wiring and communication feeders to controller; security systems; car light fixtures, tubes and ballasts;

batteries for emergency lighting and lowering that are not solely dedicated to the elevator, air conditioners and heaters.

1.3.4. Routine cleaning and refinishing of the interior of cars and the exterior of the hoistway doors and frames is not included in the full maintenance service.

1.3.5. In the event that the Contractor encounters an item of work included in paragraph 1.3.1 through 1.3.4., the Contractor will report the situation to the Contracting Officer Representative (COR). The report will include the work location, a detailed scope of the required work, justification for Contractor determination that the work was not included in the contract and an itemized cost estimate (including time to repair, all required parts and a breakdown of labor hours). No repairs shall begin until the COR verifies funding and gives notice to proceed.

NOTE: Any part, component or assembly unavailable from the manufacturer due to obsolescence, remains the Contractor’s responsibility to obtain an engineered equivalent product from a commercial source.

1.4. DEFINITIONS.

1.4.1. Maintenance, Repair and Replacement. Maintenance, repair and replacement are on existing elevator (VTE) and included in the contract scope of services with the exception of the items listed in paragraph 1.3 and subparagraphs. Maintenance, repair and replacement are actions to restore equipment to a state in which it may safely perform its required operations as installed IAW the manufacturer’s recommendations. See ASME A17.1 for requirements.

1.4.2. Machine Room-Less (MR-L). An elevator (usually new construction) that eliminates the use of a traditional elevator machine room – when the code will allow it.

1.4.3. Alteration. An alteration is on existing VTE, but is not included under the full maintenance service arrangement. Alteration/modernization will be covered on an over and above CLIN or it may result in a modification to the contract. The typical alteration results in a betterment to the safe operation of the equipment. In the case of elevators, when an alteration is made, all affected safety requirements must be complied with. All work as part of an alteration is required to comply with specific requirements of the current ASME A17.1 Code edition.

1.5. MAINTENANCE, INSPECTION AND TESTING SERVICES.

1.5.1. The Contractor, with the COR, shall determine the working order and condition of all elevators listed in Appendix B within thirty (30) calendar days of the contract award. The

Government shall request repair of all items not in working order or serviceable condition (not previously covered under contract by the Contractor). These repairs shall require an itemized estimate including elevator location, time to repair, all required parts and a breakdown of labor hours to be submitted to the COR. No repairs shall be started until funding has been verified by the COR and notice to proceed has been given. All repair work shall be IAW standard commercial practices using only new parts of equal quality as specified by the VTE manufacturer in effecting repairs.

1.5.2. The Contractor shall perform preventive maintenance, inspections and testing IAW the Contractor’s written Maintenance Control Program (MCP) to ensure reliable and continuous safe operation (paragraph 2.2). The maintenance work shall be IAW commercial practices or manufacturer’s specifications.

1.5.3. Cleaning of equipment spaces and daily cleanup of job sites in conjunction with maintenance, inspections, tests and repairs are required.

1.5.4. The Contractor shall perform necessary inspections and tests as required under ASME A17.1, Appendix N, using a Qualified Elevator Inspector (QEI). The first annual test shall include the five-(5)-year test for all traction and roped hydraulic elevators regardless of due date.

The three-(3)-year test shall be scheduled in conjunction with the annual test two (2) years later from the first annual test.

1.5.5. The Contractor shall submit a written service ticket to the COR prior to leaving Altus AFB. The service ticket shall identify the elevator repaired, the location, maintenance work performed, any repairs performed, any repairs needed that are not covered under the full maintenance contract, date of inspection, name of inspector and overall condition of the elevator.

The Contractor shall submit copies of inspection certificates to the COR upon request and shall ensure the inspection certificate is displayed inside the cab of the elevator.

1.5.6. The Government may perform inspections of the VTE, at no cost to the Contractor, for purposes of capital asset management and quality assurance. If discrepancies are discovered during these inspections, the Contractor will be notified in writing of any determination and may be responsible for the corrective actions.

1.6. REPAIR SERVICE CALLS. The Contractor shall provide service calls (routine and emergency) under the full maintenance service agreement at no additional charge. All repair work shall be done IAW standard commercial practices. The Contractor shall repair and replace components of the elevator at no additional cost to the Government (except as noted in paragraph

1.3. and subparagraphs). The Contractor shall ensure that work is completed to reduce repeats of the same problems.

1.6.1. Replacement Parts. When a component in a labeled product is replaced, it must be replaced with an identical component manufactured under the original labeling service (certifying agency). Substitution of a different component is only permitted where it is equivalent to that which was tested, as determined by the certifying agency. The parts used for replacement are required to comply with all the requirements that the old parts originally

FA441918Q0004 Elevator Maintenance complied with. The Unified Facilities Guide Specifications (UFGS) and the UFC will be used to benchmark acceptable replacement components. See UFGS 14 21 00. 00 20, Electric Traction Elevators; UFGS 14 21 13, Electric Traction Freight Elevators; UFGS 14 21 23, Electric Traction Passenger Elevators and UFGS 14 2 00, Hydraulic Elevators for guidance.

1.6.2. Routine Calls. Service calls will be classified as routine when the work, repairs or adjustments do not qualify as an emergency call.

1.6.2.1. The COR shall contact the Contractor to request routine service calls. The Contractor shall make scheduling arrangements with the COR to service elevator within four (4) duty days or at a time mutually beneficial to the COR and elevator user.

1.6.2.2. The Contractor shall respond in-person to the elevator and begin work on routine service calls at no additional cost to the Government during regular work hours. If the repair is not covered by the contract, see paragraph 1.3.

1.6.2.3. The Contractor shall submit a written service ticket to the COR prior to leaving Altus AFB. The service ticket shall identify the elevator repaired, the location, maintenance work performed, any repairs performed, any repairs needed that are not covered under the full maintenance contract, date of inspection, name of inspector and overall condition of the elevator.

1.6.3. Emergency Calls. Emergency calls are defined as calls made for services when a VTE system fails and constitutes a danger to personnel; threatens to damage properties or threatens to disrupt activity, operations and/or training missions. Altus AFB does not request emergency service, but mitigates safety and damage issues as described below.

1.6.3.1. The base fire department shall respond to all after hours “rescue” calls.

1.6.3.2. The COR shall lock-out/tag-out the elevator and contact the Contractor to request a routine call. The Contractor shall make scheduling arrangements with the COR to service elevator within four (4) duty days or at a time mutually beneficial to the COR and elevator user.

1.6.3.3. The Contractor shall respond in person to the COR (building 358) upon arrival at Altus AFB so that the COR can un-lock the elevator for service. The Contractor shall then begin work on the routine service call.

1.6.3.4. The Contractor shall submit a written service ticket to the COR prior to leaving Altus AFB. The service ticket shall identify the elevator repaired, the location, maintenance work performed, any repairs performed, repairs needed that are not covered under the full maintenance contract, date of inspection, name of inspector and overall condition of the elevator.

1.6.4. Repairs Not Covered Under Contract. If a repair is not covered under the full maintenance contract, the Contractor shall report to the work location, survey the elevator and note required repairs. Within two (2) business days after preforming the survey of the elevator, the Contractor shall submit a written itemized estimate including elevator location, time to

FA441918Q0004 Elevator Maintenance repair, all required parts and a breakdown of labor hours to the COR. These services shall be paid on an over and above contract CLIN or as a stand-alone contract for repair.

1.6.4.1. The Contractor shall commence repair work only after the COR verifies funds availability.

1.7. REPORTS. All reports shall include, at a minimum: date of inspection, building number, elevator manufacturer, model number, serial number, work performed, date of certification (if performed), repairs accomplished, listing of parts used, number of labor hours expended (for unscheduled calls), overall condition of the equipment and name of technician.

All reports shall be submitted to the COR not later than two (2) business days after completion of service. E-mailed or faxed reports are acceptable (please contact COR for information).

1.7.1. Scheduled Maintenance Reports/Service Tickets. The Contractor shall submit written reports and documentation for the preventive maintenance, inspection, testing and repair of each elevator serviced to the COR within . Service tickets may be accepted as reports if all required information is annotated on the ticket.

1.7.2. Unscheduled Maintenance Reports/Service Tickets. Reports for all unscheduled maintenance/repair calls shall be submitted in writing to the COR. Service tickets may be accepted as reports if all required information is annotated on the ticket.

1.7.3. Elevator Certification. The Contractor shall post the current elevator inspection certification inside the elevator car. If the certficicate is not available at the time of the service, the Contractor shall forward a copy to the COR within two (2) business days of the service. The COR shall place these certificates in the elevator car.

2. SERVICE SUMMARY (SS). The Contractor service requirements are summarized into performance thresholds that relate directly to mission essential items. The stated performance thresholds are the minimum acceptable levels of service for each stated requirement. All aspects of the Contractor’s performance are subject to COR surveillance and absence of any contract requirement from the SS Table below shall not detract from its enforceability nor limit the rights or remedies of the Government under contract clauses.

2.2. Submit to CO and

2.1. QUALITY ASSURANCE. The COR will assess the Contractor’s performance and adherence to standards using the SS table. The COR shall use 100% surveillance, periodic surveillance, random sampling and/or customer complaints as dictated by the SS table. If inspection reveals unacceptable performance, the COR shall notify the Contractor. If unacceptable re-performance is noted, the COR shall notify the Contracting Officer (CO). The CA will then notify the Contractor and require re-performance of the service (referred to as “call back”). The Contractor shall be allotted time as agreed to by all parties, but no longer than 72 hours, to return to Altus AFB and accomplish corrective call back work.

2.2. QUALITY CONTROL. The Contractor shall develop a writeen Maintenace Control Plan (MCP) with procedures that address the areas identified in the SS Table and conform to the requirements of the references and codes listed in paragraph 1.2. The MCP shall be submitted to the CO and COR within the first thirty (30) calendar days after contract award.. Develop a plan to state the required specific maintenance and inspections to be performed for each VTE to ensure compliance with commercial standard practices and manufacturer’s specifications. The plan should include procedures to identify, prevent and ensure non-performance and repeat of defective service does not occur. The plan shall specifically address the Contractor’s strategy to provide quality workmanship, continual process improvement and for correcting deficiencies as required. Any updates that are required shall be submitted to the CO and COR in writing. The Contractor shall perform inspections and maintenance IAW the MCP and ensure compliance with all minimum code requirements.

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES.

3.1. UTILITIES. The Government will furnish electricity as necessary for the accomplishment of work IAW this contract. The Contractor shall practice energy conservation in the Government facilities.

3.2. SECURITY, FIRE AND MEDIAL SERVICES. The Government will provide police and fire protection while on base. In the event of a medical emergency, call “911” for civilian transportation of an injured employee to the local hospital. The Government will provide emergency medical treatment on site for life-threatening emergencies only until a civilian ambulance arrives.

4. GENERAL INFORMATION.

4.1. CONTRACTOR PERSONNEL. Contractor personnel shall present a neat appearance and be easily recognized as Contractor employees. This shall be accomplished through the wear of distinctive clothing, overcoats or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection. The management and control of Contractor employees in the performance of this contract shall be the responsibility and prerogative of the Contractor.

However, the Contractor shall comply with the contract requirements and Altus AFB regulations concerning conduct of employees.

4.2. CONTRACTOR QUALIFICATIONS. The Contractor will be licensed by the State Oklahoma to provide the services specified in this contract. All work will be performed by personnel who are trained and qualified for the systems and equipment. Contractor personnel shall be certified by appropriate federal and state regulatory agencies to meet federal and local certification requirements (documentation provided to CO/COR upon request) in maintenance of all elevators. The Contractor shall provide copies of each employees licensure to the CO/COR.

4.3. SCHEDULE COORDINATION. The Contractor shall be responsible for coordinating all phases of his/her operations with the appropriate base personnel through the COR. The facilities shall remain in operation while the Contractor is working, and it is up to the Contractor to coordinate around the normal activities of the facility.

4.4. SECURITY REQUIREMENTS. The Contractor shall comply with Altus AFB security measures. These regulations are available for review in the contracting office. Security and base access requirements are contained in clause 5352.242-9000 “Contractor Access to Air Force Installations.” Contract employees shall be required to obtain and display identification badges.

4.4.1. Contractor employees shall provide information to assist in the completion of background checks for employees requiring access to Altus AFB. This information shall consist of, but is not limited to: full name (first, middle, last), social security number, date of birth, address, driver’s license number and state of issue, number of years residency in the state of Oklahoma and Jackson County. This information will be protected under the procedures listed in the Privacy Act of 1974. A background check must be completed prior to unescorted access while on Altus

AFB. If the background check reveals a Contractor employee or prospective employee with an adverse criminal history or as a potential threat to the health, safety, operational mission of the installation, then the Government reserves the right to restrict or deny access to Altus AFB for that individual.

4.4.2. Contractor Badges. The Contractor is responsible for tracking and retrieving badges from employees upon their separation, termination and/or completion of business on Altus AFB, OK. Badges from employees who are no longer employed on the contract must be retrieved and turned in to the COR within 30 days of the individual’s separation or termination. Employees who abuse their badge privileges by attempting base access beyond the dates, times, locations or specified work and other limitations may be subject to revocation of base access. Businesses may lose base pass privileges if they fail to control use of the badges or do not return them upon completion of use. Failure to control or turn in badges may result in contract termination or a decision not to exercise the option periods of this contract.

4.4.3. Anti-Terrorism. Altus AFB has an antiterrorism program that is responsible for helping secure and protect the base and its personnel from terrorist attacks. As a Contractor working on Altus AFB, you and your sub-contractors are required to obey all orders from Security Forces and adhere to all security measures implemented as a result of Force Protection Condition changes (security posture changes). Additionally, you and your sub-contractors are expected to report to base security forces (481-7444) any suspicious activities, packages or items you see while conducting work on Altus AFB. Suspicious activities could include personnel conducting surveillance of the installation, unauthorized personnel requesting access to the installation or somebody asking a lot of questions about the base. In an effort to help familiarize you, your employees and sub-contractors about antiterrorism, Altus AFB has an antiterrorism binder with all of the pertinent information for awareness training available for review at the Pass and Registration Office located at the south gate (building #428) of Altus AFB, Oklahoma.

4.4.4. Vehicles. The Contractor shall provide and maintain vehicles for transportation to meet the requirements of this contract. All Contractor vehicles used in the performance of this contract shall be readily identified as property of the company and be maintained to present a professional appearance at all times. Contractor personnel shall operate vehicles in compliance with base traffic regulations. Any violations of base traffic regulations are considered federal offenses and are adjudicated at federal court in Oklahoma City, OK. The current policy for Contractor personnel receiving a Driving Under the Influence (DUI) charge, on base, shall result in that person having their driving privileges on base revoked for a minimum of one year. See clause FAR 5352.242-9000, entitled “Contractor access to Air Force Installations.” All vehicles shall be registered, insured, and operated by a licensed operator at all times.

4.5. SAFETY REQUIREMENTS AND REPORTS. The Contractor shall comply with all safety and health requirements necessary for the protection of personnel, facilities and equipment including, but not limited to, all applicable Occupational Safety and Health Administration (OSHA) standards.

4.5.1. Provide a verbal report to the CO or COR as soon as possible of each occurrence of damage to Government property or an accident resulting in death, injury, occupational disease or

FA441918Q0004 Elevator Maintenance adverse environmental impact. Provide a completed copy of required Accident Investigation Reports (provided by the Government) to the CO and COR within five (5) calendar days of each occurrence.

4.6. ENVIRONMENTAL AND HAZARDOUS MATERIAL/WASTE (HM/W)

REQUIREMENTS. Hazardous Materials (HM) are defined in 49 CFR 172.101 and 40 CFR 301-304. Absolutely no HM shall be brought onto Altus AFB until that material is authorized for use by the Civil Engineer Environmental Branch at (580) 481-7156. The Contractor shall submit a list of chemicals with attached Safety Data Sheets (SDS) and other supporting documentation for each HM prior to their use to the COR. The COR shall forward the documentation to the Environmental Flight. The Contractor shall take appropriate actions to comply with waste minimization and pollution prevention practices and policies.

4.6.1. Spill Control. The Contractor shall maintain spill control material on hand at all times sufficient to contain a worst-case spill, both volume and hazard level. Contractor is responsible for disposal of all material used during spill clean-up. Materials must be disposed of IAW local, state and/or federal regulations. If the Contractor spills or releases any hazardous substances/hazardous waste into the environment, the Contractor shall immediately report the incident to the Fire Department and the COR/CO. The Contractor shall be liable for the costs of clean up and remediation of any spills or the release of such substances into the environment.

4.6.2. Hazardous Materials Management. The Contractor shall remove all hazardous materials by the end of each workday, unless permission is granted by CE Environmental Flight.

Contractor shall not conduct process treatments (e.g. paint thinner solvent recycling) on base.

Contractor is responsible for proper storage, labeling and security of each cabinet.

4.6.3. Contractor Environmental Awareness Training. The Contractor shall ensure all employees that work on Altus AFB complete the Altus AFB Environmental Management System (EMS) Awareness training IAW AFI 32-7001, paragraph 5.6, Environmental Management within 30 days of starting work . The EMS Contractor Awareness training can be accessed at http://www.altus.af.mil/altusafbenvironmentalnews.asp. The Contractor shall submit a list of the employee names that have completed this training to the COR within 30 days of employee starts date. This is a one-time requirement for all current and new Contractor/contract employees working on Altus AFB.

4.6.4. Asbestos. The Contractor shall comply with Section 29 CFR 1910.1001(j)(7)(iv). VTE may be located in facilities that may contain ACM or PACM. The Contractor (not the Government) shall determine if any exposure to ACM may result in performing work under the contract.

4.7. HOURS OF OPERATION. The Contractor shall perform services during normal duty hours which are 0700 to 1600, Mondays through Fridays, excluding federal holidays. The Contractor must request/coordinate to work weekends or holidays through the COR. The recycling center shall provide 8 hours of customer service during the hours of operation.

4.7.1. Recognized Holidays. The Contractor is not required to provide service on federal holidays. The holidays are:

New Year’s Day - 1 January Martin Luther King Day - 3rd Monday in January President’s Day - 3rd Monday in February Memorial Day - last Monday in May Independence Day - 4 July Labor Day - 1st Monday in September Columbus Day - 2nd Monday in October Veteran’s Day - 11 November Thanksgiving Day - 4th Thursday in November Christmas Day - 25 December

If these holidays fall on Saturday, the preceding Friday will be observed. If these holidays fall on Sunday, the following Monday will be observed. If a holiday falls on a scheduled service day, the Contractor will be responsible for rescheduling services for the first day after the holiday observance.

4.7.2. Base Closures. Work scheduled but not accomplished because of base closure due to weather, exercises or actual alert, will be accomplished as soon as possible after reopening the base.

4.7.3. Performance of Service During Crisis or Heightened Security. Services at Altus AFB are considered mission essential and limited service will continue in the event of a crisis or heightened security caused by a national emergency, natural disaster or other cause, as necessary, in support of the Air Force mission. Contractor employees with approved background checks will be allowed access to the base for performance of work, except during “Charlie” conditions.

If the “Charlie” condition exists for an extended duration, some services may be required. The CO may direct that services continue on a case-by-case basis.

4.8. CONTRACT MODIFICATIONS. Periodically, during the contract performance period, the Government may have equipment decommissioned from use or new equipment installed. A new elevator is expected to be installed during construction of an addition to building 174 during this contract performance period. If this occurs, a modification to the contract shall be initiated to incorporate changes to requirements, if warranted.
