Site Visit Request Information

Altus Air Force Base has a requirement for purchase and installation of HVLS fan systems in hangars 285, 514, 517, 509 and 435 under solicitation FA4419-16-Q-0036. See below for information regarding the request for a site survey/walk through. The offeror should review the Performance Work Statement, and submit questions in writing if applicable. Understand the government will not reimburse or be liable for any expenses incurred by the attendee to include but not limited to travel expense, lodging, and per diem.

SITE VISIT REQUEST DUE DATES:

Date: August 22, 2016 @ 0900 (9:00am CST)

IMPORTANT INFORMATION:

1) To gain access to the government installation, advance notice MUST be received by Contractor on or before August 22, 2016 at 9:00 am. The following information MUST be provided for any persons requesting access:

‐ Name (as indicated on driver’s license) ‐ Date of Birth & Social Security Number* ‐ Company Represented (name and address)

*The use of the Social Security Number and Date of Birth is for a background check that is needed for access to Altus Air Force Base.

2) Failure to submit required information by the date indicated above, will result in denial of access to the installation. Please email your information to Michael L.
