PERFORMANCE WORK STATEMENT (PWS)

FOR

ALTUS AIR FORCE BASE CHAPEL

11 July 16

1. Contract Position Title. Catholic Music Director

2. Scope of Responsibilities. This a non-personal contract for a Catholic Music Director at Altus Air Force Base Chapel. The Catholic Music Director will, in coordination with the Catholic Chaplain or Wing Chaplain, have overall responsibility for the congregational and choir music program for the Catholic Mass. The contract expectations are described below.

3. Education and Experience. The contractor must:

3.1. Be able to work within an interfaith environment with chaplains and volunteers from many various religious backgrounds.

3.2. Be positive, self-directed, motivated, and able to interface well within the religiously pluralistic environment of the Air Force Chaplaincy. They must possess an ability to maintain healthy, functional interpersonal relationships with staff and parishioners, and maintain professional standards of conduct and public decorum, including but not limited to dress and grooming.

3.3. Prepare and lead various forms of music commonly found in Catholic worship.

3.4. Be able to read music and have knowledge of vocal harmony, have the ability to play lead instrument (i.e. piano/keyboard and/or guitar), and be able to provide accompaniment for volunteer praise team and solo vocalists as needed.

3.5. Provide direction to musicians, who may play a variety of instruments to include, but not limited to, piano, organ, guitar (acoustic, electric, and bass), drums and woodwinds.

3.6. Recruit and direct volunteers for praise teams, choirs, choral groups and other appropriate musical groups.

3.7. Have at least two years of experience leading or playing music in a chapel or church.

3.8. Provide a complete resume with academic and work related references. Previous experience as a music director will be verified. Include phone number and point of contract for references. Please provide at least three references that can validate ability to perform duties outlined in this document. The contractor must be at least 18 years of age. Previous work performance and/or letters of recommendation may also serve to supplement a resume as factors in the applicant selection process.

3.9. Contractor must comply with all environmental requirements to include completion of required General EMS Awareness training. Contact 97 CES/CEAN for training accessibility.

4. General Responsibilities. The contractor will:

4.1. Provide musical selections for preludes, postludes, congregational singing, and other music requirements as coordinated with the chaplain responsible for the service. They will also prepare any needed PowerPoint slides with song lyrics before each worship service.

4.2. Rehearse all members/volunteers of the music program completely so that they are able to perform music that has been selected for worship presentation and that enhances the worship experience.

Rehearsals are to be held weekly on a date agreed to with the chaplain responsible for the service.

4.3. The contractor must act in accordance with Air Force Instructions relevant to chapel functions. The contractor will comply with AF financial procedures necessary for compliance with AF directives dealing with security and conflict of interest issues. In addition, the contractor will follow agency scheduling policies to prevent conflicts with other users of chapel facility and resources.

4.4. Be able to obtain a favorable background check if required. Failure to accomplish a favorable background check IAW DoDI 1402.5, Criminal History Background Checks will result in an immediate termination of the contract in accordance with FAR 52.212-4. The contractor will upon request complete AF Form 2583, items 1-7 only, Volunteer Questionnaire, Acknowledgement of Rights/Consent Release Records, and Statement of Understanding. Additionally, an SF 85, Questionnaire for Non-Sensitive Positions will be accomplished. These background checks will be accomplished at the expense of the chapel. Contractor will not be allowed to work with children under the age of 18 alone until background check is complete.

4.5. Meet on a regular basis with the Catholic Chaplain or Wing Chaplain, to review the music program and how it is meeting the needs of the worshipping community. This meeting will also be used to plan future music events. As appropriate, the contractor will, in consultation with the Catholic Chaplain or Wing Chaplain, develop an annual calendar of events and financial budget. This information is submitted as required by local Chaplain Corps operating procedures. Altus Chapel Tithes & Offerings Funds (CTOF) will be the primary source for purchasing.

4.6. Be accountable for security and orderliness of all facilities utilized in conjunction with their programs. At completion of each service and rehearsal, ensures stage is returned to a neutral status and left in a neat and orderly fashion as directed by the Catholic Chaplain or Wing Chaplain.

5. Specific Responsibilities. The contractor will:

5.1. Provide music in support of the Catholic Worship Program. This includes weekend Mass, Special/Holiday Masses, funerals/memorials, and Holy Day of Obligation Masses. For each Mass, a rehearsal is required. All music performance/rehearsal schedules will be as follows:

Catholic Mass Music Performances Conducted: 1 per mass/Holy Day/Special Service

Rehearsals Conducted: 1 per weekday per supported Mass/Holy Day/Special Service

Performance Location: Altus Air Force Base Chapel, Bldg 301 or alternate location determined by the Catholic Chaplain or Wing Chaplain

Hours of Performance for Mass and Rehearsals:

Sunday Mass Rehearsal Determined through coordination with Catholic Chaplain or Wing Chaplain

Sunday Mass 9:00 a.m. to 10:00 a.m. (or time scheduled by

Catholic Chaplain or Wing Chaplain)

Holy Day Rehearsal/Mass Determined through coordination with Catholic

Special Service Rehearsal/Mass Determined through coordination with Catholic

5.2. Arrive a minimum of 20 minutes prior to any rehearsal or worship service to insure that proper preparations of the facility, sound system setup (including microphones, music stands, etc.), music and any necessary coordination with the Catholic Chaplain has been made. The Contractor will also make sure all facilities and equipment are returned to their original position/condition per the direction of the Chapel Staff and relevant procedures.

5.3. Select music to be purchased from catalogs or other venues approved by the Catholic Chaplain.

Music, supplies and equipment will be purchased from Catholic Chapel Tithes and Offering Funds or appropriated funds, following guidelines established by the Wing Chaplain and the availability of funds.

The contractor coordinates all support requirements with the Catholic Chaplain or a designated representative.

5.4. Attend Catholic Parish Advisory Council meetings as scheduled, or provide a written or verbal recorded report to the Catholic Chaplain to present at the meeting. The report will consist of any Choir news, special needs and choir attendance figures.

5.5. Be accountable for paying any Social Security payments, Federal or Local Income Taxes, and State Unemployment Insurance premiums for themselves, as well as any individual employed or subcontracted by them to provide services under this contract.

5.6. The contractor will personally perform the services described above or coordinate performance by a substitute of equal or higher proficiency who has been approved in advance by the Catholic Chaplain or Wing Chaplain. Submit names and qualifications of proposed subcontractors to the Catholic Chaplain or Wing Chaplain at least 14 days prior to the proposed absence. Subcontractors must meet the requirements for the position as outlined in this statement of work (SOW). Contractor is responsible for payment to subcontractor.

6. The Wing Chaplain.

6.1. Provides all necessary supplies and equipment needed for the contractor to meet the terms of this Statement of Work. The Catholic Chaplain or designated representative is the point of contact for supplies and support requirements.

6.2. Provides building keys necessary to perform all programs. The contractor shall be responsible for keys provided by the government and for security of facilities and property made available by the government in conjunction with the performance of this contract. Keys shall be safeguarded and will not be duplicated. All keys will be returned on revocation of this contract. The contractor will report any duplicated, lost or misplaced keys to the government within one (1) hour of the discovery of the occurrence. The contractor will reimburse the government for replacement of all locks or re-keying required as a result of keys being duplicated, lost or misplaced by the contractor or while under the responsibility of the contractor.

7. Installation Access and Background Checks: The chapel will conduct a background check as is consistent with Altus AFB Wing policies and procedures per DoDI 1402.5. This check is conducted to allow for the issuing of a Contractor Identification/Common Access Card (CAC). Contractor must be able to meet Altus AFB installation access requirements:

7.1. Altus Air Force Base has an antiterrorism program that is responsible for helping secure and protect the base and its personnel from terrorist attacks. As a contractor working on Altus Air Force Base, you and your sub-contractors are required to obey all orders from Security Forces, and adhere to all security measures implemented as a result of Force Protection Condition changes (security posture changes).

Additionally, you and your sub-contractors are expected to report to base security forces any suspicious activities, packages, or items you see while conducting surveillance of the installation, unauthorized personnel requesting access to the installation, or somebody asking a lot of questions about the base. In an effort to help familiarize you, your employees, and sub-contractors about antiterrorism, Altus Air Force Base has an antiterrorism binder with all of the pertinent information for awareness training available for review at the Pass and Registration Office located at the south gate (building #428) of Altus Air Force Base, Oklahoma.

7.2. Pass and Identification Items. The contractor shall ensure the following pass and identification items required for contract performance are obtained for employees and non-government owned vehicles requiring access to Altus AFB to perform contract requirements:

7.2.1. The contractor is responsible for providing information to assist in completing background checks for those employees requiring access to Altus AFB. This information shall consist of, but not limited to, the following: full name to include first, middle, last, social security number, date of birth, address, driver's license number and state of issue, number of years resident of the State of Oklahoma and Jackson County. This information will be protected under the procedures listed under the Privacy Act of 1974. A background check must be completed prior to unescorted access. Those contractor employees whose background checks reveal adverse criminal history may be denied access to Altus

AFB.

7.2.2. The contractor is responsible for tracking and retrieving badges from employees upon separation, termination and/or completion of business on Altus AFB, Oklahoma. Badges from employees no longer working on the contract must be turned in to the Contracting office within 30 days of leaving.

7.2.3. Businesses may lose base pass privileges if they fail to control use of badges or do not return them upon completion of use. Excessive abuse may result in termination or a decision not to exercise option periods for this contract. Employees who abuse their badge privileges by attempting base access beyond the dates, times, locations or specified work and other limitations may be subject to revocation of base access.

8. Other.

8.1. Working Hours and Extended Period of Absence. Work hours must ensure successful accomplishment of contract requirements. The contractor is required to give the Catholic Chaplain or their designated representative a two-week notice of any extended period of absence. Extended period of absence is defined as an absence for more than three days. The contractor will provide a subcontractor to perform services in their absence. This subcontractor must be of equal or greater capability, have a successful background check completed, and be approved by the Wing Chaplain prior to performance of work. Contractor shall be responsible for payment to any subcontractor.

8.2. Compliance. The Catholic Chaplain or their designee will monitor and inspect the contractor’s performance. The contractor will be notified of any and all noncompliance, and will be given an opportunity to correct, if possible, nonconforming work. The contractor will also be given an opportunity to respond to any notice of noncompliance.

8.3. Registration. Contractor must be registered in System for Award Management (SAM) before providing a quote on contract. Contractor my register. SAM may be accessed at www.sam.gov.

9. NON-PERSONAL SERVICES AND/OR INHERENTLY GOVERNMENTAL SERVICES.

9.1. The Government will neither supervise contractor employees nor control the method by which the contractor performs the required tasks. It shall be the responsibility of the contractor to guard against any actions that are of the nature of personal services, or give the perception of personal services. If the contractor feels that any actions constitute, or are perceived to constitute personal services, it shall be the contractor's responsibility to notify the contracting officer immediately. These services shall not be used to perform work of a policy/decision making or management nature, (e.g., inherently governmental functions). All decisions relative to programs supported by the contractor shall be the sole responsibility of the Government.

10. QUALITY ASSURANCE.

10.1. The Contracting Officer's Representative (COR) is the authorized government representative(s) who will perform assessments of the contractor's performance. Subsequent to contract award, the identity of the COR, with a letter defining their duties and authority will be promptly furnished to the successful bidder/offeror.

10.2. The COR(s) will inform the contractor in person when discrepancies occur and will request corrective action. The COR(s) will make a notation of the discrepancy on their surveillance checklist with the date and time the discrepancy was noted and will request the contractor to initial the entry on the checklist.

10.3. Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the Contracting Officer (CO) and not to the COR(s).

10.4. The services to be performed by the contractor during the period of this contract shall at all times and places be subject to review by the Contracting Officer or authorized representative(s).

11. CONTRACTOR MANPOWER REPORTING.

11.1. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Air Force (or applicable service) via a secure data collection site. The contractor is required to completely fill in all required data fields at http://www.ecmra.mil.

11.2. Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY) which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the CMRA help desk.
