PERFORMANCE WORK STATEMENT (PWS)

FOR

ALTUS AIR FORCE BASE CHAPEL

6 July 2016

1. Contract Position Title. Catholic Religious Education Coordinator

2. Scope of Responsibilities. This is a nonpersonal services contract to provide Catholic religious education opportunities for Altus Air Force Base. Performance is in accordance with accepted guidelines provided by Archdiocese for the Military Services (AMS) USA, 52 series of Air Force Instructions (AFI), MAJCOM supplements and Altus Air Force Base Chapel Operating Instructions.

3. Education, Experience and Other Requirements: The contractor must:

3.1. Age: Be at least 18 years of age.

3.2. General Education: Must possess, at least, a High School diploma OR equivalent.

3.3. AMS Certification: Contractor must substantiate at least one of the following:

3.3.1. Must have completed at least one required course for AMS Certification Classes for Catholic Religious Education Coordinator Certifications Level 1 and Level II in Catechesis.

3.3.2. Be an AMS Certified Catechist who has completed the basic minimum catechist certification.

3.3.3. Copy of any and all AMS Course Completion Certificates towards the Level I and Level II certification in Catechesis is required.

3.3.4. Link to required courses and certification requirements listed in paragraph 11.1.

3.5. Youth Protection Training: Contractor must submit a Certificate of Youth Protection Training from either the Boy Scouts of America or the Dallas Charter. Link to required certification requirements listed in paragraph 11.2.

3.6. Microsoft Office Program: Have at least 3 years’ experience using Microsoft Office programs (Excel, Word, and PowerPoint), the Internet, and E-mail as part of previous employment (Demonstrate this in resume).

3.9. Resume: Submit a resume with the following:

3.8.1. Evidence of one-year minimum past performance as a Catholic Religious Education Coordinator/Catechist in a Chapel

3.8.2. Three academic and/or work related references related to Catholic Religious Education Coordinator/Catechist services.

3.9. Diversity: Be able to work in a religiously pluralistic environment.

3.10. Be able to obtain a favorable National Agency Check if required in accordance with HC OI 52-2. Failure to accomplish a favorable background check IAW DODI 1402.5 and HC OI 52-2 will result in an immediate termination of this contract in accordance with FAR 52.212-4. The contractor will upon request complete Installation Records Check (IRC), AF Form 2583, items 1-7 only, Volunteer Questionnaire, Acknowledgement of Rights/Consent Release Records, and Statement of Understanding. Additionally, accomplish an SF 85, Questionnaire for Non-Sensitive Positions. The Chapel is responsible for the expense of the background check.

3.11. Contractor must comply with all environmental requirements to include completion of required General EMS Awareness training. Contact 97 CES/CEAN for training accessibility.

4. Responsibilities. The contractor shall:

4.1. In coordination with the Catholic Chaplain, the contractor will have overall responsibility for Catholic Youth of the Chapel (CYOC), Summer School and camps, Adult RE, programs and preparations for Baptism and First Holy Communion and confirmation, research of all Catholic curriculum, and volunteer appreciation events.

Contractor files all sacramental records with the Archdiocese for Military Services. In connection with their coordinator duties, they will:

4.1.1. Arrange for teachers and volunteers as needed.

4.1.2. Coordinate purchase of religious education supplies through approved purchasing policies outlined in the 52 series of Air Force Instructions.

4.1.3. Ensure facility is orderly and restored to its original condition after each function.

4.1.4. Coordinate teacher(s) requests for support of Religious Education materials.

4.2. Be able to work effectively with chaplains, chapel staff, teachers, and parishioners of all faith groups. The contractor must be sensitive to the beliefs and religious observances of other faith groups. The contractor will work with the Catholic Chaplain in preparing all educational services, policies and procedures.

4.3. Register and train CYOC and Summer School students, teachers, and new families when required. The contractor will assist in the recruitment and training of all Religious Education teachers, assistants, helpers and substitutes from the start of the new Religious Education School Year and periodically throughout the year.

4.4. Ensure all new teachers/volunteers working with children will have an appropriate background check accomplished per Chapel Operating Instructions.

4.5. Maintain attendance and training records on students and teachers in all Catholic Religious Education.

Ensure attendance figures are provided the following Monday to Chapel staff members as identified by the Wing Chaplain.

4.6. Maintain a continuity binder in case of absence.

4.7. Maintain a supply of expendable items and coordinate with the Catholic Chaplain for the purchase of additional supplies through the Chapel Tithes & Offerings Fund (CTOF).

4.8. Prepare publicity needed for the religious education program including, but not limited to: bulletin/pulpit announcements, worship aids, program calendars, flyers, permission slips, etc.

4.9. Perform the following duties weekly:

4.9.1. Track religious education attendance and report numbers to Chapel staff members as identified by the Wing Chaplain.

4.9.2. Assign rooms as needed to each teacher.

4.9.3. Issue supplies and curriculum material.

4.9.4. Arrange for substitute volunteer teachers as needed.

4.9.5. Coordinate provision of refreshments as needed.

4.10. Attend monthly parish advisory council meetings in order to brief the council on status of the RE programs.

The contractor may be required to attend infrequent briefings/interviews for chapel visitors, such as Command Chaplain visits, inspectors and other distinguished visitors; also other meetings as needed to accomplish job responsibilities.

4.11. Meet with Catholic Chaplain quarterly to discuss and review status of Catholic RE and exchanges feedback for the contract period remaining.

4.12. Coordinate and/or accomplish:

4.12.1. Submit curriculum orders.

4.12.2. Develop a yearly calendar of events for all the RE and Pastoral programs.

4.12.3. Create and award certificates of appreciation to teachers, volunteers and exceptional students.

4.12.4. Develop annual budgetary projections for education and pastoral programs through CTOF.

4.12.5. Prepare facility use and CTOF purchase requests necessary for effective operation and support of the RE and Pastoral Programs.

4.12.6. Maintain both a hard copy and electronic continuity file of all RE events.

4.12.7. Assist the Catholic Chaplain in the preparation and distribution of publicity for the comprehensive Religious Education.

4.12.8. Plan and coordinate, as appropriate, special seasonal programs to involve the Religious Education program in the celebration of Holy Days and other seasonal religious events and activities.

4.12.9. Assist in the preparation of special program booklets as needed by the Catholic Chaplain.

4.12.10. Visit all classes and activities regularly to help maintain quality control in the classrooms.

4.12.11. Inspect and prepare the classrooms to ensure they are ready and set up properly prior to classes. Ensures the classrooms and facilities are orderly for the next use, and lock all appropriate doors at the end of each event.

4.12.12. Identify curriculum needs of the program. After coordination with the Catholic Chaplain, will submit an order for texts and instructional materials to support the curriculum needs of the teachers. Uses only approved curriculum sources to order materials. Coordinates all purchase requests for proposed curriculum, materials and supplies through the Catholic Chaplain. Identify final selections to the chaplain in writing for proper processing, through either chaplain funds or other funding sources.

4.12.13. Work with the Catholic Chaplain to plan, organize, and execute Ecumenical programs with the Protestant community, as needed, i.e. Vacation Bible School, Harvest Festival, Base Christmas Tree Lighting Ceremony, Airmen Ministry Center events, etc.

4.12.14. Accomplish all required Air Force training applicable to the contracted position.

4.12.15. Accomplish all required Archdiocese for the Military Service training applicable.

5. The Wing Chaplain.

5.1. Provides all necessary supplies and equipment needed for the contractor to meet the terms of this Statement of Work. The Catholic Chaplain or Wing Chaplain is the point of contact for supplies and support requirements.

All services will be performed at Building 116 (Religious Education Building/Airmen Resilience Center) or 301 (Base Chapel), unless otherwise directed and coordinated by the Catholic Chaplain.

5.2. Provides building keys necessary to perform all programs. The contractor shall be responsible for keys provided by the government and for security of facilities and property made available by the government in conjunction with the performance of this contract. The contractor shall also employ accountability methods consistent with Chapel Operating Instructions. Safeguard keys to prevent duplication. Return all keys at the revocation of this contract. The contractor will report any duplicated, lost or misplaced keys to the government within one (1) hour of the discovery of the occurrence. The contractor will reimburse the government for replacement of all locks or re-keying required due to key duplication, lost or misplaced by the contractor or while under the responsibility of the contractor.

6. Installation Access and Background Checks: To issue a Contractor Identification/Common Access Card (CAC), Altus AFB conducts this check to ensure installation access requirements are met, such as:

6.1. Altus Air Force Base has an antiterrorism program, which is responsible for helping secure and protect the base and its personnel from terrorist attacks. As a contractor working on Altus Air Force Base, you and your sub-contractors are required to obey all orders from Security Forces, and adhere to all security measures implemented due to Force Protection Condition changes (security posture changes). Additionally, Altus AFB expects you and your sub-contractors to report to base security forces any suspicious activities, packages, or items you see while conducting surveillance of the installation, unauthorized personnel requesting access to the installation, or somebody asking many questions about the base. In an effort to help familiarize you, your employees, and sub-contractors about antiterrorism, Altus Air Force Base has an antiterrorism binder with all of the pertinent information for awareness training available for review at the Pass and Registration Office located at the south gate (building #428) of Altus Air Force Base, Oklahoma.

6.2. Pass and Identification Items. The contractor shall obtain the following pass and identification items required for contract performance for employees and non-government owned vehicles requiring access to Altus AFB to perform contract requirements:

6.2.1. The contractor is responsible for providing information to assist in completing background checks for those employees requiring access to Altus AFB. This information shall consist of, but not limited to, the following: full name to include first, middle, last, social security number, date of birth, address, driver's license number and state of issue, number of years resident of the State of Oklahoma and Jackson County. This information will be protected under the procedures listed under the Privacy Act of 1974. A background check must be completed prior to unescorted access. Those contractor employees whose background checks reveal adverse criminal history may be denied access to Altus AFB.

6.2.2. The contractor is responsible for tracking and retrieving badges from employees upon separation, termination and/or completion of business on Altus AFB, Oklahoma. Badges from employees no longer working on the contract must be turned in to the Contracting office within 30 days of leaving.

6.2.3. Businesses may lose base pass privileges if they fail to control use of badges or do not return them upon completion of use. Excessive abuse may result in termination or a decision not to exercise option periods for this contract. Employees who abuse their badge privileges by attempting base access beyond the dates, times, locations or specified work and other limitations may be subject to revocation of base access.

7. Other.

7.1. Working Hours and Extended Period of Absence. Work hours must ensure successful accomplishment of contract requirements. The contractor is required to give the Catholic Chaplain or Wing Chaplain a two-week notice of any extended period of absence. Extended period of absence is defined as an absence for more than three days. The contractor will provide a subcontractor to perform services in their absence. This subcontractor must be of equal or greater capability, have a successful background check completed, and be approved by the Catholic Chaplain and Wing Chaplain prior to performance of work. Contractor shall be responsible for payment to any subcontractor.

8. The Government will neither supervise contractor employees nor control the method by which the contractor performs the required tasks. The contractor is responsible for guarding against any actions that are of the nature of personal services, or give the perception of personal services. If the contractor feels any actions constitute, or are perceived to constitute personal services, it shall be the contractor's responsibility to notify the contracting officer immediately. These services shall not be used to perform work of a policy/decision making or management nature, (e.g., inherently governmental functions). All decisions relative to programs supported by the contractor shall be the sole responsibility of the Government.

9. QUALITY ASSURANCE.

9.1. The Contracting Officer's Representative (COR) is the authorized government representative(s) who will perform assessments of the contractor's performance. Subsequent to contract award, the identity of the COR, with a letter defining their duties and authority will be promptly furnished to the successful bidder/offeror.

9.2. The COR(s) will inform the contractor in person when discrepancies occur and will request corrective action.

The COR(s) will make a notation of the discrepancy on their surveillance checklist with the date and time the discrepancy was noted and will request the contractor to initial the entry on the checklist.

9.3. Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the Contracting Officer (CO) and not to the COR(s).

9.4. The services to be performed by the contractor during the period of this contract shall at all times and places be subject to review by the Contracting Officer or authorized representative(s).

10. CONTRACTOR MANPOWER REPORTING.

10.1. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Air Force (or applicable service) via a secure data collection site. The contractor is required to completely fill in all required data fields at http://www.ecmra.mil.

10.2. Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY) which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the CMRA help desk.
