SCOPE OF WORK
MEDICAL/DENTAL CLINIC BUILDING 91020
HURLBURT FIELD, FLORIDA
12/28/2010
AUTOMATED DOOR OPENNING, FOR MAIN ENTRANCE
AND THE EAST AND NORTH ENTRANCES TO
BLDG 91020
1.0 GENERAL
This scope of work is intended to provide support for the requirement being submitted to have three (3) separate automated door systems installed in bldg. 91020. One (1) system will involve having an exterior double door and an interior double door system in the main entrance, one (1) exterior single door and an interior single door system on the east entrance and a single door system on the north entrance.
2.0 DESCRIPTION
2.1 Work Required: The objective of this work requirement is for the contractor to provide all labor, materials, equipment, and supervision to accomplish the installation of the (3) separate automated door systems The work requirement is located at the 1st Special Operations Medical Group (1SOMDG), building 91020, Hurlburt Field, Florida. The work involved, is to have (3) separate automated door systems be installed and have the door systems linked too and communicate to our Access Control system
THIS PROJECT WILL BE TURN-KEY. The contractor will be responsible for all required demolition, construction, and installation work necessary to provide a fully functional product to the customer.

2.2 Work Plan: All materials and equipment will be delivered to the jobsite, prior to the beginning of work. The contractor will coordinate all personnel activities and work assignment scheduling prior to the beginning of work. The purpose of this coordination is to minimize the impact of the contractor’s work on the daily operations, and the health care processes occurring at the 1SOMDG. The contractor will review their work plan with the FSI Contract Maintenance Manager (CMM) and the 1SOMDG Facility Manager (FM) prior to start of work.
2.3 Conditions and Stipulations: All work will be performed in accordance with Federal, State, and Hurlburt Field design codes, laws, regulations, and standards. Work will be accomplished in accordance with Industry standards, codes, references, and regulations. Work will comply with 1SOMDG Infection Control Risk Assessment (ICRA) and Interim Life Safety Measures (ILSM), policy. 1SOMDG facilities will remain fully functional at all times during normal business hours. The normal business hours of the 1SOMDG is from 7:00 AM until 5:00 PM Monday through Friday. All other hours are considered non-business hours when the 1SOMDG facilities are normally closed.
2.4 Scheduling: The required work can be scheduled for after normal business hours, aprox. 5:00 Monday-Friday and 0700-1600 weekends.
2.5 Utility Outages: Any requirement to shut down a utility system or service must be coordinated in advance. A minimum notice of 48 hours is required, however, as much advance notice as possible will allow for best planning of work activities.
2.6 Burn Permits: Any “Hot Work” will require issuance of a burn permit. Hot Work consist of any work activity requiring a source of ignition by means of flame or heat. Any work creating flame, heat and/or sparks will also require a burn permit. Burn permits must be coordinated 48 hours in advance with the Base Fire Department. The phone number for the Base Fire Department is 850-884-6360. The contractor must provide their own fire extinguisher. At a minimum, the fire extinguisher must be at least 5-lbs, type ABC dry chemical, and must have a current inspection tag.
2.7 Dig Permits: Any work activities that will require digging or excavation will require issuance of a dig permit. Dig permits must be coordinated 48 hours in advance with Base Civil Engineering. The phone number for the Base Civil Engineering office 850-884-6683.
2.8 Ceiling Permits: Any work required above ceiling surfaces will require issuance of a ceiling permit. Ceiling permits must be coordinated in advance with the 1SOMDG FM. The contractor will attach a diagram showing all locations where walls are to be penetrated.
2.9 Interim Life Safety Measures (ILSM): Any work activity interfering with life safety egress or operation of the fire protection systems may require the implementation of ILSMs. ILSMs must be identified, approved, and implemented prior to the start of work. The contractor is responsible for adherence to all active ILSMs.
2.10 Infection Control Risk Assessment (ICRA): If any work activity will be invasive to walls, ceilings or floors located in or adjacent to patient care activities; an ICRA may need to be implemented prior to the start of work. The contractor is responsible for adherence to all active ICRAs.
3.0 SAFETY
3.1 The contractor is required to designate an on-site supervisor for all work, and the on-site supervisor must be present to oversee and enforce jobsite safety. The designated on-site supervisor will identified in writing as the Competent Person (CP). The CP and all employees will receive a Safety Orientation briefing from the FSI CMM prior to start of any work.
3.2 All work spaces will be maintained in a reasonably professional, uncluttered, and orderly fashion. All paths of egress will remain free of debris throughout the work process. All debris will be removed daily, and is the responsibility of the contractor for disposal.
3.3 The contractor will comply with all applicable industry, government, state, and local safety policy and regulations. Specifically, OSHA, NFPA, UFC, Army Corp of Engineers EM 385, Air Force Instructions, Hurlburt Field Safety policy, and 1SOMDG Safety policy.
3.4 The contractor will document and submit an Activity Hazard Analysis (AHA) per Army Corp of Engineering EM 385 prior to the start of work. Upon completion of the safety orientation briefing the CP will review the AHA with all employees, and each employee will sign the AHA
3.5. The 1SOMDG FM and the FSI CMM reserve the right to stop work “time out”, if unsafe conditions or work practices are observed without penalty from the contractor.
4.0 BID SUBMITTALS
4.1 Follow up questions for clarification of the required work will be addressed by email to the CMM. Answers to all submitted questions will be provided to all contractors interested in submitting a bid.
CONTRACTOR WILL VERIFY ALL EXISTING CONDITIONS PRIOR TO BID!
4.1 The contractor’s quote must be Firm Fixed Price (FFP). The FFP will include all associated cost; materials, labor, shipping, handling, fees, and taxes, etc. The quote must be broken out to show the cost of material, labor, and taxes.
4.3 Quotes must be valid for a minimum of 30 days. However, preference is 90 days to allow time for Government review and approval process.
4.4 Proposed Work Plan. The contractor will prepare a proposed work plan as part of their bid proposal. The proposed work plan will include as a minimum; a work schedule in sufficient detail to accurately depict the work to be accomplished. This will include estimated man-hours and number of work days. It should also identify any anticipated utility outages and permit requirements.
4.5 Equipment and Material Submittal. The contractor will provide a submittal of proposed equipment, materials, colors, textures, etc.
5.0 DELIVERABLES
5.1 Finalized Work Plan. The contractor will provide a finalized work plan in sufficient detail to allow scheduling of work; to address planned utility outages; and to coordinate required permits. The contractor will meet with the CMM and the FM to address issues and concerns prior to submitting the finalized work plan. The finalized work plan will be submitted for review prior to start of work.
5.2 Safety Data Sheets (SDS). The contractor will submit a SDS for each material considered under the Hazardous Communications Program as hazardous materials based on the Globally Harmonized System of Classification and Labeling of Chemicals (GHS). All SDS’s will be submitted prior to the start of work. The contractor will maintain copies of the SDS in close proximity of the job site; such that the SDS is readily available to the contractor’s employees working at the job site.
5.3 Safety Plans. The contractor will submit any safety plans associated with the required work as an attachment to the AHA. Specifically, Fall protection plans, Lockout/Tagout (LOTO) plans, confined space plans, respiratory protection plans, etc.
5.4 Warranty Statement for all labor, parts, and materials. A documented warranty statement will be provided to the CMM indicating the warranty start date and end date.
5.5 Manufacturer’s Equipment Data. The contractor will provide a summary identifying the manufacturer’s specification data. The summary will include manufacturer name, make, model, serial, and technical specification data such as volts, amps, RPM, HP, GPM, etc. The contractor will provide two copies of the manufacturer’s Operations and Maintenance (O&M) manuals. These manuals will typically consist of wiring diagrams, parts lists, preventive maintenance instructions, technical theory of operation, etc. Note: An operator’s manual in and of itself does not constitute an O&M manual; as it typically only addresses minimal user maintenance and equipment operation only.
5.6 Manufacturer’s Material Data. The contractor will provide a summary identifying the manufacturer’s specification data. The summary will include manufacturer name, make, model, color, texture, construction material type, catalog number, lot number, etc. The contractor will provide two copies of the manufacturer’s specification sheet.
5.7 User Operator and Maintenance Training. The contractor will provide user operator training, and user maintenance training for new equipment installations.
5.9 Invoicing. Upon the completion of all work; the contractor will coordinate an on-site acceptance inspection with the CMM and the FM to review the completed work. Any deficiency identified during the acceptance inspection will be corrected by the contractor and re-inspected. Upon acceptance of all work by the FM; the contractor will submit an invoice for the full FFP amount.
6.0 SITE VISITS
6.1 Initial Site Visit. An initial site visit date will be coordinated and scheduled by the FM for all contractors interested in providing a bid, to walk the work site and review the work requirements.
6.2 Follow Up Site Visit. The contractor may request a follow up site visit; if they need to clarify issues, or gather additional information needed for the development of their bid. Follow up site visits are typically completed within five work days of the original site visit.
