1. The 1st Special Operations Contracting Squadron at Hurlburt Field Air Base, Florida intends to establish a Firm Fixed Price contract for Medical Courier Service.

3. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
Additionally, the Government will utilize simplified procedures in accordance with FAR Part 13.
4. This combined synopsis/solicitation is issued as a Request for Quotation (RFQ) with solicitation number FA441721QA011.
5. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-07 effective 10 Sept 2021, Defense Acquisition Circular, and Air Force Acquisition Circular.
6. In accordance with FAR 19.502-2, a Total Small Business Set-Aside, North American Industry Classification System (NAICS) code for this solicitation is 492110 – Couriers and Express Delivery Services with a size standard of 1,500 Employees.
7. The contractor shall provide Medical Courier Services in accordance with the Performance Work Statement (PWS). The following table contains the description of requirements for the Contract Line Item Numbers (CLIN) items to be acquired and the date and place of the period of performance.

Part I – Price Submission
1. Submit pricing and company information on separate attachment.
2. Complete the necessary fill-ins and certifications in the Contract Clauses section. The provisions FAR 52.212-3 and FAR 52.204-24 shall be returned along with the quote. If the quoter has completed all representations and certifications contained in this solicitation in their SAM registration, the quoter shall state so.
Part II – Technical Submission Quoters shall provide the following information in their technical submission:
1. Qualifications and certifications of technicians performing the work Note: .zip files are not an acceptable format for the Air Force Network and will not go through our email system.

9. The provision at FAR 52.212-2, Evaluation – Commercial Items (Oct 2014) and addendum applies to this acquisition. Pursuant to FAR 12.602 contract award will be made using Simplified Acquisition Procedures in accordance with FAR 13.106. Award will be made to the lowest priced responsible quoter whose submission is determined to be technically acceptable and whose price is determined to be fair and reasonable.
ADDENDUM TO 52.212-2 – Evaluation – Commercial Items (OCT 2014) Methodology. This acquisition will be awarded to the most advantageous offer that conforms to the following evaluation criteria: the Government intends to award to an offeror with a technically acceptable quote at the lowest overall total evaluated price. The Government reserves the right to end evaluations after comparison of Total Evaluated Price (TEP) of all quotes and receipt of a technically acceptable quotation. Technical evaluations will continue until an acceptable technical quote is determined based on order of TEP.

a.Total Evaluated Price = (Total Evaluated Price for Base Period) + (Total Evaluated Price for Option Period 1 - 4) b.Total Evaluated Price for Base Period and Option Periods 1 through 4: Quantities identified will be multiplied by the unit price to confirm the extended price and then the extended price for all CLINs will be added together for a total for that specific period.
Part II – Technical Submission Offerors shall propose a technical solution that meets the governments needs IAW the Performance Work Statement. The following information shall be included in the technical submission:
1. Proposed list of government approved materials and products to be used, including Safety Data Sheets for all products.
2. Proposed schedule, including a start date and estimated time to complete the project.
3. Qualifications and certifications of technicians performing the work.

The clause at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items (Oct 2018), applies to this acquisition with the following Addendum: Paragraph (c) of this clause is tailored as follows: Changes in the terms and conditions of this contract may be made only by written agreement of the parties with the exception of administrative changes, such as changes in the paying office, appropriations data, etc., which may be changed unilaterally by the Government.

FAR 52.217-8 Option to Extend Services (Nov 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days.

FAR 52.217-9 Option to Extend the Term of the Contract (Mar 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 30 Days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 66 months.
