STATEMENT OF WORK (SOW)

FOR

REPAIR OF 2003 ROADTEC MILLING MACHINE

1. DESCRIPTION OF SERVICES. The contractor shall provide all personnel, management, supervision, supplies, materials, equipment, tools, and all other items necessary to perform maintenance and repair services required on one (1) RX 60 model Roadtec Milling Machine. All work shall be accomplished at 633 Independence Road, Hurlburt Field, Florida, and shall be IAW Roadtec specifications and all applicable OSHA regulations and AFTO 36-1-191, all applicable commercial standards, local, state, and federal laws, instructions, and regulations.

1.1. The contractor shall provide all parts, equipment, tools and associated labor to replace the Rotory Encoder Retrofit Kit. The contractor shall replace; Rear Encoder Mounting Bracket (part# 207330-03), Control Arm (part# 207457-03), Guide Rod Linkage (part# 207456-03) x2, Control Arm Assembly (part# 207396-03), 3/8” – 24 Nylok Nut (part# 136479) x4, Rotary Encoder (part# 149101) x2, Front Encoder Linkage WLD’T, Rotary Position Sensor Wall Mount (part# 207344-03), Swivel Pivot Linkage Plate (part# 207345-04), Wire Harness: Front Rotary Encoder (part# 207452-02), Wire Harness: Rear Rotary Encoder (part# 207453-02), 1/4” -20 Set Screw (part# 143041) x4, #10 – 32 x 1” Large Slotted Head Screw (part# 135807-04) x8, #10 Hex Nut (part# 90102) x8, #10 Flat Washer (part# 92552) x16, #10 Lock Washer (part# 90103) x8, Copy of 60C151, 24V Solenoid Valve (part# 207885-04), 1/4” x 12` W/6-4ST & 6-4 90` Hose (part# 130207), 1/4 NPT x 6 JIC SW (part# 86622-04), 8 SAE x 1/4 NPT Fitting (part# 86136-04), 8 SAE x 6 JIC (part# 83469-04), 6 JIC RUN TEE (part# 84636-04) x2, 0.031 ORIFICE (part #135409-04) x2, Double Pole Single Throw Switch (part# 97678), 1/2 x 116” LG W/ 8-8 ST Both Ends (part# 201290S) and any other associated parts. The contractor will then calibrate and adjust the steering system. The vehicle repairs shall render the vehicle 100% functional IAW the manufactures’ technical specifications.

1.1.1. The contractor shall provide and install OEM approved parts and shall repair the units IAW the manufacturers’ technical specifications.

1.1.2. The contractor shall in strict accordance with manufacturers’ technical data and approved procedures, perform a steering system calibration and operator’s check to determine proper installation and operation of the machine IAW manufactures’ specifications.

1.2.1. The contractor shall perform an operator’s check on all components of the repair. If additional work not listed in this SOW is determined to be needed during this operator check or throughout the process of the repair, the contractor shall notify and obtain approval from the Contracting Officer prior to completion of additional work.

2. Deliverables.

2.1 Parts and Repair Documentation. The contractor shall provide to the customer, written documentation identifying all repairs completed and any applicable warranties provided.

2.2. All repairs on the Roadtec model RX60 Vehicle Identification Number (VIN) # RX60C154 will be made at the 823 REDHORSE Sq, 633 Independence Road, Hurlburt Field, Florida, 32544.

GENERAL INFORMATION

3.1. Contractor Personnel.

3.1.1. Contract Manager. The contractor shall provide a contract manager who shall be responsible for the performance of the work.

3.2. Physical Security. The contractor shall ensure no government owned or controlled real or personal property is damaged during any service provided under the contract. The contractor will be responsible for the repair or replacement of any government property damaged by the contractor at no expense to the government.

3.3. Hours of Operation: Normal operation is Monday through Friday, 0630 to 1530, except for federal holidays. There may be mission situations that may require the contractor to work other than normal hours. Some of these situations are military exercises, contingency operations, or weather emergencies.

3.4. Federal Holidays. The following is a list of federal holidays observed by this installation.

Repairs should not be coordinated on these dates unless coordinated with the customer in advance.

New Year's Day, January 1 or the Friday preceding or Monday following Martin Luther King's Birthday, 3rd Monday in January President's Day, 3rd Monday in February Memorial Day, last Monday in May Independence Day, July 4 or the Friday preceding or Monday following Labor Day, first Monday in September Columbus Day, 2nd Monday in October Veterans Day, November 11 or the Friday preceding or Monday following Thanksgiving Day, 4th Thursday in November Christmas Day, December 25 or the Friday preceding or Monday following

3.5. Contractor/Employee Base Pass and Identification, Special Clearances, and Vehicle Passes.

3.5.1. The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished contractor identification badges while visiting or performing work on the installation.

3.5.2. The Contractor shall be required to complete the following forms (to be provided at the pre-performance conference) in order for employees to gain access to the installation:

a. Contractor Employee Listing

b. Hurlburt Field Contractor Access Badge Affidavit (CAB)

c. Department of Homeland Security Form I-9 (DHS I-9)

3.5.3. The Contractor Employee List. This form shall list every employee requiring access.

Every time employees change, it shall be updated to reflect additions and/or deletions. Identify those individuals who require escort authority by including the word “escort” next to their names. Limit the number of individuals designated as escorts to a reasonable number (i.e. one or two). The Contracting Officer will then prepare and attach a cover letter, and forward the Employee List to the Visitor Control Center.

3.5.4. Upon notification that the Employee List has been forwarded to the Visitor Control Center, the other two forms (DHS I-9 & CAB) shall be completed and hand carried to the Visitor Control Center by the employee. If everything is in order, they will receive a contractor badge.

If an employee will be driving on Hurlburt Field they are required to provide a valid driver’s license, vehicle registration, and proof of insurance. NOTE: The section of the Visitor Control Center administrating this function is closed after 1400 (2:00 p.m.), therefore, employees must arrive prior to this time in order for their documentation to be processed.

3.5.5 Upon completion or termination of the contract or expiration of the identification passes, the contractor shall ensure that all base identification passes issued to employees are returned to the issuing office.

3.5.6. Failure to comply with these requirements may result in withholding of final payment.
