STATEMENT OF WORK (SOW)

VDYD 17-1121 Repair (SUS), Replace Carpet, Systems Control; B61

SCOTT AIR FORCE BASE, ILLINOIS

28 August 2017

1. Description of Work: The work consists of furnishing all labor, materials and equipment necessary to accomplish, but not limited to, the following as stated in this SOW:

a. Carpet Tile Removal and Replacement East Wing Basement (see attached floor plans)

1) Remove, dispose, and install new carpet tile (Apx. 1,040 square yards (SY)) for the areas indicated on floor plans. Replace carpet tile with 24”x24” Tandus Centiva Esparta 04119, color Tincture 45211 or equal

2) Install new cove base (Apx. 1700 linear feet) with Johnsonite 168 Thunder, Traditional Wall Base at areas where carpet tile was removed.

a) Contractor shall coordinate with paint contractor(s) and establish a phased installation plan to ensure new carpet and base is installed after paint application. (All construction coordination shall be conducted by the facility manager, project manager, contracting administrator, and contractor collectively.)

3) Install new conductive resistant rubber tile flooring (Apx. 25 SY) for the areas indicated on the floor plans with Roppe .080 Gauge Hammered Profile, R480 esd rubber static control tile.

4) Clean cut and fill gap in concrete subfloor (Apx 40 cubic feet). New fill to be flush with like texture and finish as surrounding material. Appropriate joining to ensure moisture control and minimal movement is to be utilized.

1. Systems furniture and all other equipment to be lifted/moved for installation of materials. Comm and electric through systems furniture shall be left in place or, if installation of above requirements dictates, be reconnected.

a) Secure safes are used in work area and must be moved for the installation of new carpet. Heavy equipment moving will be required.

2. Field Conditions: The Contractor shall be responsible for all field measurements to accomplish work. Quantity and sizes of materials shall be determined from site visits and/or drawings. The contractor shall carefully examine the entire site and building and shall make all necessary investigations to become thoroughly aware of the existing conditions and all difficulties involved in the completion of the work in accordance with the contract documents.

a. Contractor shall site verify any and all dimensions and existing site conditions prior to the commencement of work. Commencement of work shall begin and be completed within the agreed upon period of performance (PoP) stated in the contract issued by the Contracting Officer (CO).

b. Contractor shall brief all sub-contractors on all safety and fire prevention regulations.

c. All documents related to the disposal of hazardous material must be supplied to this office and to the 375th Civil Engineer Squadron, Natural Resources (375 CES/CEI),

618.256.2092. The Contractor must obtain the proper documents from 375 CES/CEI to ensure all materials leaving Scott AFB can be tracked and accounted for, in accordance with all governing laws.

3. Accomplishment of Work: The work areas of the existing building will be in occupied rooms.

The contractor must ensure that all accesses to the existing building remain clear for other building occupants and emergency access at all times during the construction period. All temporary closures, barriers, and protective procedures shall be coordinated with the CO. All work shall be accomplished between the hours of 5:00 P.M. to 4:30 A.M., Sunday through Saturday.

a. The Contractor shall submit material/equipment submittals to 375 CONS and 375 CES and allow up to twenty-one (21) calendar days to review and comment. Construction may be authorized by the CO upon acceptance and approval of the schedule and equipment/material submittal approvals.

4. Work Conditions: The contractor shall coordinate work with the CO. The Contractor shall notify the CO or the inspection representative the day he/she moves onto the work site to commence work. The Contractor shall again provide notification the day he moves back to the work site after an interruption of work of two or more day’s duration. The Contractor shall also provide an original work schedule and updates if required. This schedule shall describe major work items and projected completion dates.

5. Protection of Building Property: The Contractor shall protect from damage all building property located in or adjacent to the building and shall repair, replace, or otherwise be held liable for all such damage. Building property includes, but is not limited to, furniture, vehicles, or items of supplies. Exercise control measures of all dust and toxic fumes associated with certain construction operations, in order to prevent hazards or illness to base and Contractor personnel.

6. Cleanup: The contractor shall cleanup each work site at the end of each work day to the satisfaction of the CO. Upon completion of the work, all construction equipment and debris shall be removed from the site and the area shall be left in a neat and completely acceptable condition as required by the General Provisions. Contractor must always recycle materials, such as, but not limited to cardboard and paper. The Contractor will use whatever means necessary to store all recyclable materials and responsible for turning in recyclable materials to the Base Recycling Center. Location of the Base Recycling Center may be obtained from the CO.

7. Patching and Repairing: After the work has been completed, holes, cuts, and damages made in the finished surfaces of the work site and adjacent areas shall be properly repaired and restored to match the existing finishes. Any abrasion or disfigurement of the finished work or any portion of the building where any such abrasion or disfigurement is caused by the activities of the Contractor shall be repaired and neatly refinished to match the adjacent work to the satisfaction of the CO.

8. Products: All products shall conform to their respective manufacturing and performance publications. All materials and equipment used in the work area shall be the products of manufacturers regularly engaged in the manufacture of construction products.

9. Specifications: Work shall be performed in strict accordance with this contract’s general specifications, as well as all applicable local, state, national Building and Life Safety codes, American Disability Act (ADA) regulations, American Society for Testing and Materials (ASTM) specifications and National Electric Codes (NEC). Safety issues must adhere to all applicable Occupational Safety and Health Act (OSHA) regulations.
