STATEMENT OF WORK FOR

BETM 130000

DEMOLISH MARS FACILITY Building 500

AT

SCOTT AIR FORCE BASE PLUM HILL ANNEX, ILLINOIS

375TH CIVIL ENGINEER SQUADRON

11 September 2017

I. Description of Work: The work consists of furnishing all labor, materials, equipment, transportation, and supervision necessary to demolish Building 500 and surrounding area to provide a clean site at the Plum Hill Annex.

A. Contractor shall submit an abatement plan for review and approval, which will subsequently be submitted to the Illinois Environmental Protection Agency for notification purposes. A 10-day notification period is required prior to starting any abatement work.

1. Contractor shall comply with all federal, state and local regulations for the abatement and disposal of hazardous materials.

2. All documents related to the disposal of hazardous material must be supplied to the contracting office and to the 375th Civil Engineer Squadron, Installation

Management Flight, Environmental Compliance Section (375 CES/CEIEC), 618-256-

2092. The Contractor must obtain the proper documents from 375 CES/CEIEC to ensure all materials leaving Scott AFB Annex (Plum Hill Site) can be tracked and accounted for, in accordance with all governing laws. The contractor shall supply 375

CES/CEIEC with weights of materials disposed of at landfill and a list of materials with weights recycled, including those taken to the base’s recycling center.

B. Demolition

1. The contractor shall be responsible for the complete demolition of Building 500, the MARS Facility. The building is approximately 3,519 square feet, 1 story, CMU structure with a 5-ply, metal deck roof with bar joists. Work includes demolition and disposal of structure, foundation, slab and fixtures.

2. Removal of 18 utility poles and associated guy wires. Some poles have been preserved with creosote. Accordingly, all poles must be removed to their full depth.

3. Cutting creosote-preserved poles at ground level and abandoning the buried portion is not acceptable.

4. Removal of approximately 54 utility pole anchors

5. Removal of detached shed.

6. Removal of all concrete and blacktop pads.

7. Removal of all mechanical equipment and transformer.

8. Removal of existing brush on property and along fence line.

9. Fence, gate, and blacktop drive are to remain. The fabric may be replaced as long as none of the poles are removed. They identify the boundries of the government site and eliminating any of them could cause boundry disputes with the local farmers.

(submit as an alternate).

C. Well Abandonment

1. The contractor shall be responsible for the complete abandonment of a drinking water well on the Plum Hill Annex property. Well abandonment will be performed in accordance with state and local laws and regulations.

2. Monitoring wells will be abandoned in accordance with 77 IAC Part

920.120. Water wells must be sealed within 30 days after abandonment and must be sealed by a licensed water well driller pursuant to the Water Well and Pump

Installation Contractor's License Act. The submersible pump, drop pipe, pressure tank, and associated electrical components shall be removed from the well. The well casing or liner shall be removed to at least three feet below final grade. The well shall be sealed in accordance with 77 IAC Part 920.120(b) (3) (A), which applies to unconsolidated formations. (Reference attached Well Construction

Report accordingly on the FBO site.

3. If slurry is used, a mud balance and/or Marsh Funnel will be used to ensure that the density (pounds per gallon) of the abandonment mud mixture conforms to the manufacturer's specification. The grout will be placed by tremie pipe, submerged in the grout at all times. The tremie pipe may be raised as the grout is placed as long as the discharge end remains submerged in the grout. The grout will be placed from the bottom to the top of the hole in one continuous operation. All abandoned monitoring wells will be checked 24 to 48 hours after mud/solid bentonite emplacement to determine whether curing is occurring properly. More specific curing specifications or quality assurance checks may be recommended by the manufacturer and will be followed. Additionally, if significant settling has occurred, a sufficient amount of mud/solid bentonite will be added to attain its initial level. These slurry/solid bentonite curing checks and any addition of mud/solid bentonite will be recorded in the field logs.

4. The Illinois Department of Public Health will be notified by telephone or in writing at least 48 hours prior to the commencement of any work to abandon a well. The individual performing the sealing shall submit a sealing form to the local health department not more than 30 days after the well is sealed.

Information required on the sealing form is provided in 77 IAC Part 120(e).

All fill materials shall be non-contaminated with the proper documentation to demonstrate as such, and meet the requirements of 35 Illinois Administrative Code (IAC) 1100. Proposed amendments to 35 IAC 1100, the Clean Construction or Demolition Debris (CCDD) Fill Operations, can be tracked on the Illinois Pollution Control Board website

2. Furnish and install IDOT specified Class 1 or 1B seed and fertilizer. (IDOT Article 250)

3. Furnish and install straw mulch ground cover. (IDOT Article 251)

4. Any soil disturbed as a result of any aspect of this project shall be containerized and sampled for contaminants. The contractor shall provide results of the sampling to the government. If soil is clean of contaminants, it becomes the property of the contractor. If soil is contaminated, it shall be disposed of at a licensed landfill. The contractor shall provide documentation that contaminated soil was handled and disposed of in accordance with federal, state, and local regulations. (This paragraph only applies to the creosote poles that will be removed and testing at each of the sites to determine the extent of possible contamination to the surrounding soils. Testing at each pole location shall be figured into the total cost and any contaminated removal may be addressed as a modification at a later date). For bidding purposes, due to our inability to know for certain if the soils are contaminated or not, assume that it is clean soil. After award, if there is contaminated soil discovered, it may be handled by a change order.

E. OBTAIN REGULATORY CLOSURE OF LEAKING UNDERGROUND

STORAGE TANK (LUST)

1. The contractor shall be responsible for obtaining a No Further Remediation letter from the Illinois Environmental Management Agency (IEMA) for Scott AFB’s Tank #87 LUST site (Illinois Emergency Management Agency (IEMA) Incident

#930373) IAW 35 Illinois Administrative Code (IAC) 734. (It is the contractors responsibility to close out the tank as provided by the reports that are on the FBO site.)
