8. Removal of existing brush on property and along fence line. Fence and gate to remain.

B. Environmental Cleanup

ATTACHMENT 3 - CLIN 0003 SOW 1 of 19

1. Remove approximately 93 cubic yards +/- 10% of soil from underneath and adjacent to Building 4900. See referenced attachments and figures for previous sampling and testing results and area for excavation. Excavating equipment should be decontaminated off-site prior to use on-site. Contaminated soils should be excavated directly into receiving trucks with the minimal disturbing/spreading of contaminated soils as practical.

2. All documents related to the disposal of contaminated soil must be supplied to the contracting office and to the 375th Civil Engineer Squadron, Installation Management Flight, Environmental Compliance Element (375

CES/CEIEC), ext. 256-2092. The Contractor must obtain the proper documents from 375 CES/CEIEC to ensure all materials leaving Scott AFB can be tracked and accounted for, in accordance with all governing laws. All Industrial Waste

Tracking Receipts or manifests (if required) must be signed by the government as the generator of the waste and the Contractor shall supply 375 CES/CEIEC with weights of materials disposed of at the licensed landfill.

3. The Contractor shall provide clean fill soil over the demolition site to match the existing surrounding grade, and grade any disturbed areas with an emphasis on proper drainage. All fill materials shall be non-contaminated with the proper documentation to demonstrate as such, and meet the requirements of

35 Illinois Administrative Code (IAC) 1100, subpart F at the Illinois Pollution

Control Board website at:

http://www.ipcb.state.il.us/SLR/IPCBandIEPAEnvironmentalRegulations-

Title35.aspx. Analytical data supporting the determination of clean fill must follow the requirements of Section C of this PWS.

C. Confirmation Sampling

1. Collect twenty-eight (28) soil confirmation samples at locations as identified in the field by the government by white flags. Sample name will be denoted on the flag.

2. Samples will be collected using an auger or similar coring device, as mutually agreed upon with the government, 3-5 inches into the existing excavation walls or floor into native and previously undisturbed soil. The sampling device will be cleaned with a dry brush and decontaminated after each use with alconox and water. The Contractor shall also collect a Quality Control

(QC) sample at the work site for an equipment rinsate blank. Contractor is responsible for proper disposal of all wastes generated from the decontamination of the sampling equipment.

ATTACHMENT 3 - CLIN 0003 SOW 2 of 19 http://www.ipcb.state.il.us/SLR/IPCBandIEPAEnvironmentalRegulations-Title35.aspx http://www.ipcb.state.il.us/SLR/IPCBandIEPAEnvironmentalRegulations-Title35.aspx

3. Protective gloves will be worn at all times and fresh gloves will be worn to collect each sample.

4. Twenty-four samples to be analyzed by analytical method SW8270C –

SIM and four samples to be analyzed by analytical method SW6010B for lead.

D. Management of Chemistry Services and Laboratory Selection

1. The Contractor shall be responsible for the quality of all required chemistry services performed. The Contractor shall ensure that all chemistry-related tasks are conducted in accordance with this statement of work. Minimum

Quality Assurance/Quality Control (QA/QC) requirements identified by the current version of the Department of Defense (DoD) Quality Systems Manual

(QSM) shall be followed by the contractor. The Contractor shall select a laboratory that complies with the requirements of the current version of the DoD

QSM (specifically Appendices A and B). The most current version of the DoD

QSM can be found at the following website: http://www.navylabs.navy.mil. A waiver for DOD ELAP certification may be obtained subject to Project Manager

(PM) and CO approval, but the laboratory must be accredited by the National

Environmental Laboratory Accreditation Conference (NELAC). All reporting limits shall be at or below Illinois Tiered Approach to Corrective Action

Objectives (TACO) Tier I Residential levels. The laboratory must also have analytical capabilities sufficient for the methods specified in this statement of work and adequate capacity to handle the project’s analytical workload during the field activities. The Contractor shall ensure that the selected laboratory meets all state and federal requirements including state and federal certification/accreditation programs, where appropriate.

2. The Contractor shall ensure that all hard copy and electronic data deliverables supplied by the laboratory are complete and adequate to support the quality and usability of the data. Raw data packages shall be submitted to Scott

AFB. Data packages shall include all information required to review the data and re-create the analysis, including correspondence with the laboratory regarding

QA/QC exceedances and documentation of corrective actions.

E. GENERAL NPDES PERMIT: Since the construction site is less than one acre in size, storm water discharge from the construction site does not require a Notice of Intent

(NOI) under the National Pollutant Discharge Elimination System (NPDES). The

Contractor shall use Best Management Practices to limit the amount of sediment run-off from the site.

F. Accomplishment of Work:

ATTACHMENT 3 - CLIN 0003 SOW 3 of 19 http://www.navylabs.navy.mil/

1. Contractor shall site verify any and all dimensions and/or existing site conditions prior to the commencement of work. Commencement of work shall begin within ten (10) calendar days upon the receipt of the Notice to Proceed issued by the Contracting Officer after verifying that all communication equipment has been removed.

2. Contractor shall be required to apply for and submit to the Contracting

Officer an AF Form 103, Work Clearance Request, for all work to be accomplished under this delivery order five (5) days prior to the commencement of work. The AF Form 103 can be obtained from the project manager or inspector and approved by the 375th Civil Engineer Squadron, Operations

Support Element (375 CES/CEOE), ext. 256-4710. This requirement will be coordinated with the project manager.

3. Contractor shall brief all sub-contractors on all safety and fire prevention regulations. Because the work site is located at a Scott AFB annex, an AF Form

592, Welding, Cutting, and Brazing Permit, are not required for welding, cutting, or brazing operations. The Contractor must abide by all local fire codes and ordinances. The 375th Civil Engineer Squadron, Fire and Emergency Services

Flight (375 CES/CEF), at ext. 256-5130, is available for fire prevention guidance, yet local fire district officials should also be consulted.

4. All documents related to the disposal of hazardous material must be supplied to the contracting office and to the 375th Civil Engineer Squadron, Asset

Management Flight, Hazardous Waste Management Element (375 CES/CEIEC), ext. 256-2092. The Contractor must obtain the proper documents from 375

CES/CEIEC to ensure all materials leaving Scott AFB can be tracked and accounted for, in accordance with all governing laws. The contractor shall supply

375 CES/CEIEC with weights of materials disposed of at landfill and a list of materials with weights recycled, including those taken to the base’s recycling center.

5. Under the 1990 Amendments to the Clean Air Act, all products used in executing this project must not be related to the Class I Ozone Depleting

Chemicals identified in Air Force Policy. The following products are considered

Class I ODCs: CFC-11, CFC-12, CFC-113, CFC-114, Halon 1211, Halon 1301, Halon 2402, CFC-13, CFC-111, CFC-112, CFC-211, CFC-212, CFC-213, CFC-

214, CFC-215, CFC-216, CFC-217, Carbon Tetrachloride, and Methyl

Chloroform.

6. Field Conditions: The Contractor shall be responsible for all field measurements to accomplish work. Quantity and sizes of materials shall be determined from site visits and drawings. The contractor shall carefully examine

ATTACHMENT 3 - CLIN 0003 SOW 4 of 19 the entire site and building and shall make all necessary investigations to become thoroughly aware of the existing conditions.

7. Work Conditions: The contractor shall coordinate work with the

Contracting Officer. The Contractor shall notify the Contracting Officer or the inspection representative the day he/she moves onto the work site to commence work. The Contractor shall again provide notification the day he/she return to the work site after an interruption of work of two or more day’s duration. The

Contractor shall also provide an original work schedule and updates if required.

This schedule shall describe major work items and projected completion dates.

8. Protection of Private Property: The Contractor shall protect from damage all property not belonging to the government located adjacent to the work site, and the Contractor shall repair, replace or otherwise be held liable for all such damage. Private property includes, but is not limited to, vehicles, farm equipment, agricultural land, or items of supplies. Exercise control measures of all dust and toxic fumes associated with construction operations, in order to prevent hazards or illness to the public.

9. Upon completion of the work, all construction equipment and debris shall be removed from the site and the area shall be left in a neat and completely acceptable condition as required by the General Provisions.
