The 375th Contracting Squadron (LGCB2) at Scott AFB, IL is contemplating an award for the purchase of Base Camera Preventive Maintenance Service. Quotes are for four (4) quarterly
PMI and annual total for base. There is also four option periods to include a 6-month extension of services. The reimbursable CLIN will have an amount added annually that will be used to pay for any broken equipment and does not require you to bid. Please quote per year and provide a grand total for the entirety of the contract.
This is a commercial purchase using FAR Part 12 and Part 13 procedures. The following information, as required per FAR 12.603(c)(2) applies:
(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with
FAR Part 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotations are being requested and a written solicitation will not be issued.
(ii) The solicitation reference number is FA4407-16-Q-2006; it is being issued as a Request for
Quotation (RFQ).
(iii) The solicitation document, incorporated provisions, and clauses are those in effect through the Federal Acquisition Circular 05-78 effective 26 December 2014, and the Defense Federal
Acquisition Regulation Supplement DPN 20150129 Edition.
(iv) This acquisition is being solicited as a 100% Small Business Set Aside on Federal Business
Opportunities webpage Fed Biz Opps utilizing NAICS Code 561621, Security Systems Services
(except locksmith) and a PSC Code of J063, Maintenance and Repair of Alarm, Signal and
Security Detection Systems. A single award will be made as a result of evaluation of quotes received. Please provide a quote for the purchase of the item listed below by: 23 June 2016 at
11:00 a.m. central standard time. If this date is not possible, please contact the above to discuss completion date.

This is a non-personal services contract to provide quarterly Preventive Maintenance Inspection
(PMI) of Scott AFB cameras system to include all other associated equipment such as digital recording devices, camera housing units and cabling. The contractor will also be responsible for service responses for unscheduled camera and associated equipment outages as outlined in the
SOW. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government. (See SOW)

(ix) The provision at 52.212-2, Evaluation—Commercial Items (OCT 2014), applies to this acquisition.
(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, Price and other factors considered. For this acquisition, the following factors shall be used to evaluate offers: Price, Technical capability, and past performance of the item offered and a best value determination to meet the Government requirement. Evaluation of quotes will be conducted using simplified Acquisition procedures in accordance with FAR Part 13.106-2.
(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).

AFFARS 5352.201-9101 OMBUDSMAN (APR 2014)
(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern.
(b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution.
Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).

The following provisions are full text:
52.204-17, Ownership or Control of Offeror (Nov 2014).
(a) Definitions. As used in this provision—
“Commercial and Government Entity (CAGE) code” means--http://www.whitehouse.gov/omb/circulars_a076_a76_incl_tech_correction/
(1) An identifier assigned to entities located in the United States or its outlying areas by the
Defense Logistics Agency (DLA) Contractor and Government Entity (CAGE) Branch to identify a commercial or Government entity, or
(2) An identifier assigned by a member of the North Atlantic Treaty organization (NATO) or by the NATO Support Agency (NSPA) to entities located outside the United States and its outlying areas that the DLA Contractor and Government Entity (CAGE) Branch records and maintains the CAGE master file. This type of code is known as an NCAGE code.
“Highest-level owner” means the entity that owns or controls an immediate owner of the offeror, or that owns or controls one or more entities that control an immediate owner of the offeror. No entity owns or exercises control of the highest level owner.
“Immediate owner” means an entity, other than the offeror, that has direct control of the offeror.
Indicators of control include, but are not limited to one or more of the following: Ownership or interlocking management, identity of interests among family members, shared facilities and equipment, and the common use of employees.
