PART 1.0 – SCOPE OF WORK:

1.1. GENERAL:

The work to be performed under this contract and in accordance with this Statement of Work shall consist of furnishing all necessary parts, labor, tools, transportation, supplies, supervision, equipment, materials, insurance and incidentals necessary to perform grease-trap cleaning service, waste transportation and disposal according to the service frequency on Technical Exhibit 1, statement of work, and all applicable codes, regulations, standards and criteria in effect at the date of solicitation. The work outlined below shall consist of, but not be limited to the following:

1.2. PROJECT DESCRIPTION:

The contractor shall provide all management, tools, supplies, equipment and labor necessary to pump grease traps located at Goodfellow AFB. Work shall comply with government and commercial standards. See “Technical Exhibit 1” for location, size and frequency of cleaning of grease traps.

1.3. LOCATION:

Goodfellow Air Force Base is located in Tom Green County, on the southeast side of San Angelo, TX and is bounded to the north by Highway #388 (Paint Rock Rd.), to the west by Fort McKavitt Rd and Bell Street/Christoval Road, to the south by Highway #1223 (San Antonio Hwy.) and to the east by the eastern city limits. Work locations are in buildings 213, 222, 303, 334, 723, 800, 906, 927, and 3220.

1.4. WORK AND MECHANICS:

The work for this project shall be executed in the best and most workmanlike manner, by qualified and efficient mechanics/tradesmen, skilled in their respective trades. Only certified journeymen in each respective trade, or apprentices under the direct supervision of certified journeymen, shall be permitted to install and/or supervise installation for this project. Individual trade work for this project shall be performed and quality maintained by the applicable trade, only. All trades shall coordinate their work with that of other trades. The Contractor shall coordinate and perform all operations in a manner that will result in a professional and expeditiously completed project. The work shall be in strict accordance with prevailing industry standards and manufacturer’s instructions. Work and materials shall comply with this Statement of Work and the editions in effect at the time of this solicitation for all applicable criteria, regulations, guidelines and codes, all of which are made a part thereof.

1.5. WORK SCHEDULE:

Workdays shall be from 0730 to 1630, Monday thru Friday; except on the Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. Buildings 3220, 303, 334 & 800 will not be scheduled between the hours of 1100 &1300. No weekend work allowed unless approved by the Contracting Officer. If weekend work is required Contractor shall submit request in writing to Contracting Office a minimum of 72 hours in advance. The Contractor shall provide written notification to the Contracting Officer’ Representative seven (7) calendar days lead time for work schedule changes.

1.5.1. Service Notification:

Contractor shall notify the Contracting Officer Representative (COR) of the date of service seven

(7) calendar days in advance.

1.5.2. Recognized Holidays:

The Contractor is not required to provide service on the following respective days:

Holiday New Year’s Day Birthday of Martin Luther King, Jr.

Washington’s Birthday Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Day Christmas Day

If a federal holidays fall on Saturday, the preceding Friday will be observed. If a federal holidays fall on Sunday, the following Monday will be observed. If a federal holiday falls on a scheduled service day, the Contractor will be responsible for rescheduling services for the first day post the holiday observance. Additionally, AETC “Family Days” are not defined as a holiday.

1.6. WORK AREA ACCESS:

Government escorts are not needed in these areas.

1.7. REFERENCES:

All publications listed herein shall be the most current editions in effect at the time of solicitation and form a part of this Statement of Work. The publications are referred to in the text by basic designation only and include the following:

The following list of federal regulations pertaining to liquid wastes is not all-inclusive.

• Resource Conservation and Recovery Act (as amended by the Hazardous and Solid Waste amendments of 1984), subtitle C, “Hazardous Waste Management.”

• Oil Pollution Act of 1990, Section 1001.

• The Clean Water Act, Title 3, “Standards and Enforcement”, and Title 4, “Permits and

Licenses”.

• Title 40 CFR 260-282, “Hazardous Waste”

• Title 40 CFR Parts 104-149 “Water Programs”.

• Title 40 CFR Part 110, “Discharge of Oil”.

• Title 40 CFR Part 112, “Oil Pollution Prevention”.

Contracts are subject to inspections of job sites on base by the Department of Labor. These requirements are additional to and do not replace the standards promulgated by the Department of Labor under Occupational Safety and Health Administration (OSHA). In the event of a conflict between the OSHA standards and these requirements, the most stringent shall apply.

1.8. OSHA Resolution

Resolution of Department of Labor citations for violations of Occupational Safety and Health Standards is a Contractor responsibility and shall provide for no basis of a claim against the Government.

PART 2.0 – EXECUTION:

2.1. GENERAL:

All work shall be performed as shown and in accordance with the manufacturer’s diagrams and instructions, unless otherwise specified. The Contractor shall field verify all dimensions and site conditions. Price increase adjustments to the original contract price will not be issued because the Contractor was not aware of existing conditions. The Contractor shall provide all labor, materials, tools and equipment required to perform all dismantling, repairs and installation as listed in this statement of work.

2.2. COORDINATION AND INSPECTION:

Service shall be performed on all traps as listed in “Technical Exhibit 1”, and shall be accomplished as specified herein. The Contractor shall contact the Government’s inspector at ( 325) 654-3447, 2 weeks prior to accomplishment of work to schedule work. The Contractor shall be accompanied by Government Inspector during the accomplishment of this work. The Contractor shall provide the Government's Inspector a Chain of Custody Affidavit prior to departing.

2.2.1. Contracting Officer (CO)

The CO is the only person authorized to direct changes in any of the requirements under this contract, and, notwithstanding any provisions contained elsewhere in this contract, said authority remains solely in the CO. In the event the Contractor effects any such change at the direction of any person other than the CO, the change will be considered to have been made without authority and solely at the risk of the Contractor. All contract administration will be effected by the CO.

Communications pertaining to contractual administrative matters shall be addressed to the CO.

No changes in or deviation from the terms and conditions shall be affected, without a written modification to the contract, executed by the CO authorizing such changes.

2.2.2. Contracting Officer’s Representative (COR)

The CO shall designate contract level CORs during the term of this contract, under Defense Federal Acquisition Regulation Supplement (DFARS) clause 252.201-7000 “Contracting Officer’s Representative.” The COR will provide assistance in identification and resolution of problems, conflicts in priority, subtask requirement definitions, and other operations type problems. The COR will perform in accordance with the responsibilities and duties identified AFI 63-138.

2.2.3. COR Authority.

A letter of designation issued to the COR by the CO, a copy of which is sent to the Contractor, states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates or changes in delivery dates. The COR is not authorized to change any of the terms and conditions of the resulting order.

2.3. SPILLS:

In the event of a spill on Goodfellow Air Force Base of any materials involved in this contract, the following will apply:

2.3.1. Reporting:

The Contractor must immediately notify the Goodfellow Air Force Base Fire Department by dialing 325-654-3534 or 911 on any on-base phone. The contractor should be prepared to give the following information:

1. His name.

2. Location of spill.

3. Number of injured persons and nature of injuries (if applicable).

4. Substance spilled.

5. Source of spill.

6. Approximate quantity of spill and rate material is currently spilling. Extent spill has traveled.

7. Waters which might be affected.

8. Cause and time of spill.

9. Any action underway to confine or clean up the spill.

2.3.2. Equipment:

All equipment used to perform this contract shall be specifically designed to be used in the emptying of grease traps. No improvised or makeshift equipment will be permitted in performing this contract.

PART 3.0 - DESCRIPTION

3.1. GREASE TRAPS.

Pump out all grease and residues from trap; remove waste build up from trap or pit walls and all associated equipment within the pit. Flush down with water and pump out remaining residue as required by the unique configuration of the grease trap. Residual or recharge water must be higher than lower inlets. Clear obstructions in inlet/outlet pipes to ensure unrestricted flow.

Transport all materials from grease traps to an approved off-base disposal facility.

3.1.1. Service Details

The contractor shall service all grease traps listed in “Technical Exhibit 1” of this contract.

Buildings 334, 800 and 3220 shall be serviced on a monthly basis. All others shall be serviced on a quarterly basis. Additional, buildings 3220, 334 & 303 will not be scheduled for service between the hours of 1100 &1300.

3.2. PERFORMANCE ASSESSMENT

The contractor shall be aware that in the absence of any contract requirement from the Service Summary does not detract from its enforceability nor limit the rights or remedies of the government under any other provision of the contract. The contractor shall ensure the following service summary requirements are accomplished.

PART 4.0 – UTILITY OUTAGES AND SPECIAL CONDITIONS:

4.1. UTILITY OUTAGES:

When a utility outage is necessary to perform the contract work in an occupied facility, regardless of whether the work area is occupied, the outage shall be performed by the Contractor during non-duty hours at no additional cost to the Government, unless otherwise approved by the Contracting Officer. The Contractor shall notify the Contract Inspector of outage requirements to include buildings affected; length of outage; and reasons for outage. The Contractor must allow affected occupants a minimum of two – (2) weeks’ notice prior to an outage. The Contractor is also required to provide the Contracting Office a written notification of the requested outage.

4.2. BASE FIRE REGULATIONS:

The Contractor shall comply with Base Fire Regulations as set forth in the latest edition of GAFB Instruction 32-2001, titled “Base Fire Protection Program”. The Contractor shall use no explosives or fire in performing the work. All work shall be in strict compliance with all National Fire Codes.

4.3. CONFINED SPACE:

In Accordance With Air Force Occupational Safety & Health Standard. 91-25, Ch. 7, the organization shall ensure the following information is included in the SOW (or equivalent contracting tool) when a contractor enters a confined space:

a. Notify the contractor if the space is classified permit or non-permit required.

b. Brief contractor on the contents of the space.

c. Brief contractor on the known hazards of the space.

d. Brief the contractor on what precautions and procedures have been implemented by the organization to protect AF workers.

e. Coordinate operations and procedures and agree on permit system to be used if both AF and contractor personnel will enter the space at the same time.

The fire department will coordinate (document) on the contract if they are supplying a rescue team.

The contractor will follow all requirements outline in OSHA Std. 1910.146.

4.4. LOCKOUT/TAGOUT

Hazardous Energy Control Lockout/Tagout: In addition to the requirements in OSHA Std.

1910.147, if a contractor needs to lock or tag something out, the contractor will ensure that affected employees are notified before and after the locks and tags are used.

4.5. SECURITY REQUIREMENTS.

The contractors shall comply at all times with base law enforcement and security requirements to include base pass requirements. The contractor shall obtain all necessary cards, passes, decals, or other items required for access to Goodfellow AFB. All personnel operating a vehicle shall be a licensed driver and all vehicles shall be licensed and insured. Upon employee termination or completion/termination of the contract, the Contractor shall surrender any employee/vehicle identification to the Security Forces.

4.5.1. Work Area Restrictions

The Contractor’s employees shall be restricted to those areas necessary to perform work under this contract as well as direct routes to and from the work site as approved by the Contracting Officer.

4.5.2. Contractor Installation Access Pass

Before arrival, a government identification card-holding person from the sponsoring agency, base contracting office or applicable local project manager will submit a Request for Base Access Pass/Change to Pass and Registration in person or via a ".mil" email account for each person requesting access to the installation. The BAL will include the individual's pertinent information utilizing Exhibit 1, reason for entry, frequency of entry, destination, times each day requiring entry, and duration of request. For installation access on non-duty hours or down days

4.5.2.1 Identify which workers need access on weekends, federal holidays/family days, and down days.

4.5.2.1.1. Ensure only workers that are already vetted are on the request (no new personnel).

New personnel require a new BAL and formal vetting.

4.5.2.1.2. Complete the BAL for workers requiring down day access to the installation.

4.5.2.1.3. Identify the vehicle requirements. If operations cannot support the search procedures for large vehicles/special purpose equipment, the request may be declined unless arrangements are made to deliver the vehicle/equipment prior, during normal duty hours.

4.5.2.1.4. During the above-mentioned days, the project manager, a representative from the requirement owner or sponsoring ID cardholder is required to be present during hours of the work request.

4.5.2.1.5. All BALs will be (re)accomplished every 6 months or upon a change to personnel regardless of length of DBIDS pass.

4.5.2.1.6. All DBIDS passes for employees will be for the duration of the Purchase Order (PO)/Delivery Order (DO)/Contract if the Period of Performance (PoP) is <180 days.

4.5.2.1.7. All DBIDS passes for employees working on a PO/DO/Task Order (TO)/Contract with a PoP >180 days will be issued DBIDS pass for no longer than 180 days. Passes will be re-issued when the new BAL is submitted.

4.5.2.1.8. Contracting Officers (CO)/Contract Administrators (CA) are to remind your contract managers that BALs are to be re-accomplished when employees change.

4.5.2.1.9. When an employee (regardless of position) is no longer employed by the contractor or subcontractor all DBIDS passes are to be returned to the CA or to the Visitor Control Center.

Failure to return DBIDS passes may result in withholding of funds.

4.5.2.2 Contractor Initial (and periodic) Installation Access Screening. Prior to being allowed access, a minimum of a background/National Crime Information Center (NCIC) check will be completed on all contractors, requesting unescorted access for official business. This screening process will validate the contractor's suitability to visit Goodfellow and certify that the contractor does not pose an increased threat to the base populace. If it is determined a contractor requesting access has been convicted of a felony or plead guilty to a felony charge within the past 10 years, is considered not fit under AFI 31-113 (para 4.12.2.), or based on the information obtained during the identity vetting indicating the individual may present a threat to the good order, discipline and morale of the installation, the VCC clerk will deny entry and inform 17 SFS/ECC of the finding.

The contractor will be informed of the access denial, issued a Access Denial Letter, and will be informed on how they may appeal this order by submitting a letter to the 17 SFS Commander, within 30 days of being denied access. The letter will be addressed from the contractor through the contract manager or through the on-base sponsor or sponsoring agency, to 17 SFS/CC, 361 Apache Trail, Goodfellow AFB, 76908 (or it may be dropped off at the installation Visitor Control Center). The contractor's appeal letter should discuss all the facts and reasons to support belief that this order should be rescinded. The appeal package will be routed from SFS up to the 17 TRW/CC for approval/disapproval.

4.5.2.3 TRAFFIC LAWS. Contractor and employees shall comply with Texas transportation code, base traffic regulations and posted signs while on the installation.

4.5.2.4 WEAPONS, FIREARMS, AMMUNITION. Contractor employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their contractor-owned vehicle or privately-owned vehicles while on base.

4.5.2.5 REAL ID Act of 2005. State issued driver's licenses and identification cards issued by any state or territory that is not compliant with REAL ID Act of 2005 requirements will not be accepted as a source for identity proofing for unescorted access to AF installations and facilities.

It is the responsibility of the Contract manager to ensure personnel are in possession of authorized credentials, or are in possession of an acceptable alternate identification credential, with a photo, and in compliance with the REAL ID Act.

4.6. PHYSICAL SECURITY.

The service provider shall ensure service provider employees comply with base operations plans/instructions for Force Protection Condition (FPCON) procedures, Random Antiterrorism Measures (RAMS) and local search/identification requirements.

The government will provide/make available AT Level I training/information at the contractor's request. This information can be provided through the 17 TRW contractors AT Level I awareness pamphlets/binders, through computer based training programs, or through the unit/installation Antiterrorism Representative (ATR).

4.7. CONTINUATION OF ESSENTIAL SERVICES

Performance of services during crisis declared by the national command authority or overseas combatant commander IAW DODI 3020.37 (Continuation of Essential DoD Contractor Services During a Crisis), and aforementioned Air Force implementation, unless otherwise directed by an authorized Government representative, it is determined that services to the Child Development Center (CDC), Youth Center, Dining Facilities, and any contingency areas identified by the Contracting Officer are essential for performance during a crisis.

4.8. SPECIAL QUALIFICATIONS.

Contractor personnel shall be certified by the Texas Commission on Environmental Quality (TECQ) in maintenance of grease traps. All personnel performing the tasks required in this contract shall be knowledgeable and experienced in properly emptying/cleaning grease traps as specified herein. Certificates shall be turned in prior to contract start date.

4.8.1. Employees.

The contract manager and on-site employees must be able to read, write, speak, and understand English.

4.8.1.1. The Government has the right to restrict the employment under the contract of any Contractor employee, or prospective Contractor employee, who is identified as a potential threat to the health, safety, security, general wellbeing or operational mission of the installation and its population.

4.8.1.2. Contractor personnel shall present a neat appearance and be easily recognized. This shall be accomplished through the wear of identification badges or distinctive clothing bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection.

PART 5.0 - ENVIRONMENTAL REQUIREMENTS:

5.1. COMPLIANCE WITH LAWS:

All maintenance activities are NOT exempt from air emission, storm water, hazardous waste, and other environmental compliance rules and regulations. The Contractor shall comply and ensure that all Sub-Contractors comply with all applicable federal, state, and local laws, regulations, ordinances and standards related to environmental matters. The Contractor shall also comply and ensure that all Sub-Contractors comply with all specific instructions or directions given to the Contractor by Goodfellow AFB regarding environmental matters.

5.2. CONTRACTOR ENCOUNTERED HAZARDOUS WASTE:

The Contractor shall notify the Contracting Officer’s Representative and BEC upon encountering any material not identified in this Statement of Work thought to be hazardous that could jeopardize the safety of workers or personnel in the area. The Government will be responsible for characterization, transportation, storage and disposal of the waste if necessary.

5.3. NUISANCE AND POLLUTING ACTIVITY PROHIBITED:

Polluting, dumping, or discharging of any harmful, nuisance, or regulated materials (such as but not limited to concrete truck washout, vehicle maintenance fluids, residue from saw cutting operations, solid waste and hazardous substances) into building drains, site drains, streams, waterways, holding ponds or to the ground surface shall not be permitted and the Contractor shall be held responsible for any and all damages which may result. Further, the Contractor shall conduct work activities in such a fashion as to avoid creating any legal nuisance, including but not limited to, suppression of noise and dust, control of erosion, and implementation of other measures as necessary to minimize offsite impacts of work activities.

5.4. RELEASE OF FLUIDS TO THE SANITARY SEWER SYSTEM:

Goodfellow AFB’s sanitary sewer system discharges into the Publicly Owned Treatment Works (POTW) operated by the City of San Angelo, Texas. This POTW has established testing requirements for certain constituents as well as discharge limits of those same constituents.

Accordingly, any Contractor performing work at Goodfellow AFB and contemplating a release of non-hazardous water into the sanitary sewer system shall meet the pretreatment standards and comply with the testing/release requirements established by the City of San Angelo.

Contractor is also responsible for any and all testing, monitoring, measuring, documenting, etc.

to prove compliance with it. Contractor shall not discharge wastewater to base’s sanitary sewer without prior approval of the Government.

5.5. STORM WATER COMPLIANCE

Regardless of the amount of soil disturbed, all non-storm water discharges from Contractor’s site shall conform to TPDES General Permit TXR040000 for Small Municipal Separate Storm Sewer Systems (MS4).

5.6. ENVIRONMENTAL MANAGEMENT SYSTEM:

Contractor’s on site supervisory personnel shall be aware of GAFB EMS policy to follow ISO 14001 principles. EMS is a systematic approach to the management of all of the environmental aspects of operating a business, organization or any entity that has impact on the environment.

EMS is a continuous improvement process that reduces environmental impact. GAFB has taken a proactive approach in EMS, and good management practice is to correct every mistake and preventing reoccurrence. When they cannot be immediately corrected they must become a targeted objective. In coordination with the COR and CO, CEIE shall perform periodic inspections of site work to ensure contractors are meeting the minimum requirements for GAFB

Environmental Policy. For questions or concerns regarding GAFB EMS Policy, please contact GAFB Environmental Management Coordinator at 325-654-5946.

PART 6.0 - SITE MAINTENANCE AND CLEANUP:

6.1. SITE MAINTENANCE:

The Contractor shall protect adjacent property, buildings and their contents from dust, dirt or other materials. Work areas shall be maintained in a neat, clean, safe condition and shall, at a minimum, be cleaned at the end of each shift. All streets and roadways in/or adjacent to the site shall remain free of project generated trash and debris at all times.

6.2. CLEANUP:

The Contractor shall collect any and all trash, debris, refuse, garbage, etc., that is generate and place it in appropriate containers with lids or approved covers on a periodic basis or as directed by the Contracting Officer’s representative. The aforementioned materials shall be hauled from the site by appropriate means on a daily basis, unless otherwise approved by the Contracting Officer’s representative. Disposal shall be outside the limits of Government property. Disposal shall be by sanitary landfill or other approved methods and shall conform to all local, state, and federal guidelines, criteria, and regulations. Upon completion of the work, the Contractor shall leave the work site and storage area(s) in a clean, neat and workmanlike condition satisfactory to the Contracting Officer. It is anticipated that excavation, filling, and plowing of roadways shall be required to restore the area to near natural conditions that will permit the growth of vegetation thereon. Restoration to original contours is required unless otherwise directed by the Contracting Officer.

PART 7.0 - ENERGY CONSERVATION:

7.1. UTILITIES CONSERVATION:

The Contractor shall instruct employees in utilities conservation practices. The Contractor shall be responsible for operating under conditions that preclude the waste of utilities, which shall include: Lights shall be used only in areas where and when work is actually being performed.

The Contractor shall not adjust mechanical equipment controls for heating, ventilation and air conditioning systems. Water faucets or valves shall be turned off after the required usage has been accomplished. The Contractor shall use good judgment in the conservation of Government utilities. Prevailing energy conservation practices shall be adhered to and enforced by the Contractor.

PART 8.0 – RESPONSIBILITY:

The above 1 through 7 summaries do not in any way limit the responsibility of the Contractor to perform all work and furnish all plant, labor, and materials required by this Statement of Work.

PART 9.0 – DAMAGE TO GOVERNMENT PROPERTY

9.1. DAMAGES TO GOVERNMENT PROPERTY.

All damage shall be reported immediately to the INSPECTOR and Building Manager. The Contractor shall be responsible for all damages, which occur as a result of their employee’s actions or inaction. All damage to real property shall be repaired by the Contractor with materials of like type, quality, or finish by skilled craftsman of the trades involved at Contractor expense in a timely manner at no cost to the government. Should the Contractor fail to correct damages, the Government will correct the damages and the cost will be deducted from the Contractor’s payment.

PART 10.0 - CONTRACTOR PAYMENT:

10.1. SUBMISSION

The Contractor shall submit receipts and septic hauler waste chain of custody affidavit signed by the disposal site operator within 5 business days after monthly and quarterly service basis to:

11.1. OPERATIONAL SYSTEMS:

The Contractor shall insure that work for this project is performed in accordance with the criteria herein and that all equipment and systems shall be fully operational at the completion of work for this project.
