PERFORMANCE WORK STATEMENT

GREASE/GRIT REMOVAL SERVICES

COLUMBUS AFB, MISSISSIPPI

20 July 2016

1. DESCRIPTION OF SERVICES. The contractor shall provide all management, labor, transportation, equipment, tools, materials and other items necessary to perform grease and grit removal services as required in this Performance Work Statement (PWS).

2. PERFORMANCE REQUIREMENTS:

2.1. Pump two (2) five hundred (500) gallon grit chambers located at Facility 304 (chamber #1 and chamber #2). Pump one (1) one- thousand (1,000) gallon grit chamber located at Facility

385. Pump and clean two (2) 1,000-gallon grease traps located at Facilities 736 and 944.

2.2. WASTE DISPOSAL PLAN: The Contractor shall maintain a brief plan explaining how the collected waste materials will be disposed and provide a copy of all applicable disposal facility permits to the Government, upon request. Failure to comply with local, state and Federal law is grounds for termination.

2.3. NOTIFICATION: The contractor shall coordinate with the Government Technical Representative (GTR), at least 3 working days in advance of the proposed date for each grease removal operation. The contractor shall notify the Government Technical Representative of the agreed upon date before removal takes place. The GTR for this contract will be provided upon award.

2.4. OPERATIONS: The contractor shall pump and clean the grease traps and grit chambers as scheduled within this contract so that no solids remain in the identified units. This may require the Contractor to agitate the solids and/or scrape accumulated grease off walls. The service schedule is included as Appendix A.

2.5. CLEAN UP: All debris, spillage, or other waste material resulting from this service contract shall be cleaned up and disposed of by the Contractor off base. Site cleanup shall be completed each time services are rendered and before the Contractor leaves the site. Disposal shall be in accordance with all federal, state, and local regulations and shall depend on the waste’s characteristics.

2.6. DOCUMENT SUBMISSION: Any documents identified in this PWS that the contractor is required to provide will be submitted in a timely manner. Failure to submit these documents will be reported to the contracting officer for necessary contractual action/remedy.

3. COMPLIANCE AND CONTRACTOR LIABILITY. The Contractor assumes full responsibility and liability for the compliance with all federal, state, regional, and local regulations pertaining to work practices, disposal, and protection of workers and visitors to the work site.

4. REPORTS: The contractor shall provide receipts showing the actual amounts of grease removed by location. The contractor shall also submit to the GTR a validated receipt from the approved disposal location for all grease and debris transported off base during the current invoice period. The contractor shall verify in writing (e.g., landfill receipt from authorized disposal site) that all waste materials transported off base has been properly disposed.

5. WORK SCHEDULE, PLANS AND WORKING HOURS: Perform services in accordance with the work schedule identified in Appendix A of this Performance Work Statement. Work days for this contract will be Monday through Friday between the hours of 0730 and 1615 (7:30 Am and 4:15 PM). The majority of these facilities will be occupied during the hours of work. The contractor shall not begin any work until he/she is 100% material complete. Written permission from the GTR shall be obtained for any deviation from the approved schedule.

6. SAFETY REQUIREMENTS. Safety requirement shall be the applicable sections of EM 385-1-1, Oct 1992 and Occupational Safety and Health Administration (OSHA) Regulations.

OSHA regulations are available for review at the Base Safety Office, Building 724. EM 385-1-1 is available for reference in the Maintenance Engineering Office, Building 385.

7. PAYMENT: Contractor invoices may be submitted at the end of the month after all required work for the period has been completed and all required paperwork has been submitted.

8. ENVIRONMENTAL CONSIDERATIONS

8.1. CONTRACTOR LIABILITIES FOR ENVIRONMENTAL PROTECTION

8.1.1. Compliance. Contractor is responsible for compliance with all federal, state and local environmental laws and regulations.

8.1.2. Inspection by Environmental Agencies. This installation is subject to federal, state, and local inspections to review compliance with environmental protection laws. An inspection by the Environmental Protection Agency (EPA) or Mississippi Department of Environmental Quality (MDEQ), or any other environmental or safety enforcement agency may include questioning and inspection of Contractor personnel, sites and processes.

8.1.3. Enforcement Actions. If a Notice of Violation, (NOV) is issued to the government or Contractor due to the Contractor’s error or omission, the Contractor shall be held liable for damages to include fines, penalties, and/or corrective actions imposed by federal, state, or local agencies.

8.1.4. Non-Hazardous Waste. The Contractor retains all liabilities associated with containerizing, loading, shipping, and disposing of non-hazardous waste.

8.2. SUBMITTALS

8.2.1. Environmental Management System. The Contractor will submit Environmental Management System (EMS) information, including program plan and required training documentation, to the Columbus AFB EMS Manager upon request.

8.2.2. Hazardous Materials. The Contractor shall submit a list of hazardous materials to be used on the job and Material Safety Data Sheets for each to the GTR. The Contractor shall secure authorizations for these items from the Installation Hazardous Materials Manager (IHMM) prior to bringing the materials on the installation in accordance with AFI 32-7086, Hazardous Materials Management. After work has started, the Contractor shall provide consumption reports to the GTR periodically (at least monthly or more frequently as determined by the core Hazardous Materials Management Program (HMMP) team,) or at the end of their contract, whichever occurs first, in the format specified in AFI 32-7086. The Government Technical Representative will forward copies of the consumption reports to the IHMM.

8.2.3. Non-Hazardous Waste. Contractor shall submit a copy of all non-hazardous waste manifests to the GTR no later than close of business the same day waste is transported off base.

Contractor shall provide a closed copy of the non-hazardous waste manifests not later than 30 days after the waste is transported off base or prior to final payment, whichever occurs first. If any hazardous waste is to be generated by the contractor, the Columbus AFB EPA identification number will be used on all hazardous waste manifests, 14 CES/CEV must sign the manifest,

8.3. ADDITIONAL REQUIREMENTS

8.3.1. Environmental Management System (EMS). The contractor shall be aware of the base's Environmental, Safety and Occupational Health (ESOH) Policy as stated on the Columbus AFB website: http://www.columbus.af.mil/. Significant impacts to the environment will be reported to 14 CES/CEV.

8.3.2. Hazardous Materials

8.3.2.1. According to 40 Code of Federal Regulations (CFR) 355 Emergency Planning and Notification, 40 CFR 370, Hazardous Chemical Reporting, 40 CFR 372, Toxic Chemical Release Reporting, and AFI 32-7086 Hazardous Materials, all quantities of hazardous and toxic materials which are stored, used, or brought on a facility shall be reported to federal, state and local agencies. Since Contractors do not report hazardous and toxic chemical information separately to the Environmental Protection Agency, the base must report it for them.

8.3.2.2. Contractors shall provide hazardous materials submittals to the Hazardous Materials Management Program Manager in accordance with 2.2 of this document. In addition, Material Safety Data Sheets shall be available on the work site for all hazardous materials in use on the project.

8.3.2.3. Upon completion of the project, the Contractor shall remove from the installation all hazardous materials that are not used on the project but otherwise in usable condition. The government will not accept unwanted hazardous materials such as paint, thinners, etc. (whether they are usable or not) as they belong to the Contractor.

8.3.3. Non-Hazardous Waste

8.3.3.1. Any non-hazardous or special waste generated shall be handled in accordance with all federal, state, and local laws and regulations, including 40 CFR 239-299 Resource Conservation and Recovery Act (RCRA) requirements for waste management and 49 CFR Transportation requirements for waste transport. The Contractor shall be responsible for obtaining all necessary permits, licenses, and approvals.

8.3.4. Prohibitions

8.3.4.1. Illegal Disposal of Chemicals. Disposal of any chemical or materials into the sanitary sewer system or out in the environment where it can enter the storm system or any natural waterway is prohibited. Disposal of any hazardous material on base is prohibited. In the event of an accidental spill, Contractor shall follow procedures outlined in section 5.0 of this document.

8.3.4.2. Unlabeled Containers. Unmarked containers are prohibited on the base. All drums and/or containers of liquid materials shall be marked to indicate contents. All containers shall be closed when not in use.

8.3.5. Spills and Cleanup

8.3.5.1. Accidental spills may occur during contract work. Spills include releases of materials from ruptured or leaking storage receptacles such as tanks, from transmission lines and/or valves.

Releases include the leak of hazardous gases into the air.

8.3.5.2. In the event of a spill of any amount or type of hazardous materials or waste, including petroleum products, the Contractor shall immediately notify the Government Technical Representative. The Contractor shall provide a description of material spilled (including MSDS), time and locations of spill, estimate of amount spilled, spill surface and path of spill, noting whether the spill is likely to enter a drain inlet, waterway, or come into contact with the soil.

8.3.5.3. Contractor shall take immediate action to contain and clean up spills. If the Contractor is unable to begin cleanup action within 15 minutes of the spill, the government reserves the right to begin cleanup, continue cleanup efforts/actions until the Contractor can properly take over and continue cleanup. Proper contractor action is to ensure the qualified workers and supervision and necessary materials are available and the necessary coordination take place to contain and clean up spills.

8.3.5.4. As determined by the Contracting Officer, all costs incurred by the government until Contractor is capable of taking control shall be the sole responsibility of the Contractor.

Contractor shall ensure equipment, material and trained personnel are available to meet response time and cleanup requirements.

8.3.5.5. Spill cleanup personnel shall obtain proper training and certifications to perform spill cleanup. Contractor shall assume responsibility for the proper characterization and disposal of any waste and cleanup materials generated. All waste and associated cleanup material shall be removed from the base and transported/stored in compliance with applicable regulations until final disposal.

8.3.5.6. The government reserves the right to control spill cleanup activities. The senior ranking Air Force representative on scene will determine best practices for spill cleanup. The

Contracting Officer remains as the only Air Force representative that may amend contract requirements.

8.3.5.7. The Contractor is responsible for restoring a spill site to the condition of the site prior to the spill at no additional cost to the Government.
