STATEMENT OF WORK
Replace Overhead Operator Joint Base San Antonio – Lackland AFB
Amendment 1
NAME OF INSTALLATION:
Joint Base San Antonio – Lackland AFB
PROJECT MANAGER: Gabriel Talamantes
PART 1 – SCOPE OF WORK:
1.1 GENERAL: The work to be performed under this contract and in accordance with this Statement of Work shall consist of furnishing all necessary parts, labor, tools, transportation, supplies, supervision, equipment, materials, and incidentals necessary for providing all work shown in the statement of work, and all applicable codes, regulations, standards, and criteria in effect at the date of solicitation. All commercial installation must be performed by qualified skilled technicians in overhead doors.
1.2 PROJECT DESCRIPTION: Contractor shall provide all labor, materials, to perform full functional replacement of overhead door electric operator for buildings 5160 at Lackland Air Force Base, TX. 78236.
• The following components must be accomplished: Contractor shall remove and replace seven (7) electric operators (which includes, gear boxes, control boxes, 3/4 (2ea) , 1/2 (3ea) and 1/3 (2ea) horsepower, 208V, 3-phase motors, switches, photo eyes and any other components needed to make these overhead doors fully operational. There are a total of 12 switches instead of just seven (7) needed for this requirement, since some door entries require 2 switches to gain access to two (2) overhead door entrances. Overhead doors are located at building 5160 at 1865 Selfridge Blvd. Lackland Air Force Base, TX. 78236.
• Service Required:
o Contractor shall remove and replace (7) electric operators to include the up and down buttons switches. There is one (1) located on each of seven (7) roll-up doors. See paragraph above for the different horsepower motors and the additional five (5) switches needed. Electric Operators shall be properly installed to manufacturer’s specifications and shall work in conjunction with each other. Along with the manual chain assembly.
o Contractor shall conduct full function check of door rising/opening and lowering/closing of all 7 doors. Roll-up door shall raise and lower properly in conjunction with the limit switch properly adjusted.
o All safeties shall work/operate correctly with roll up door function.
o Coordinate with Lackland AFB Civil Engineer Operation Planning office ahead of time.
o Contractor shall always utilize all proper Personal Protective Equipment (PPE) for this project. If using lifts contractors shall use proper fall protections according to OSHA Regulation 1910.28/1910.29. AFOSH DAFMAN 91-203 Chapter 13.6 Fall Protection Plan.
o Contractor shall haul all project waste off government property.
o Upon completion of project, contractor shall call Civil Engineer Project Manager (PM) and
Facility Manager (FM) for inspection.
o All discrepancies identified in the initial inspection shall be addressed/fixed.
o Practice good housekeeping before, during, and after work in progress to ensure safe work area.
o Upon final inspection, contractor is responsible for removing all tools, trash, equipment, and personnel off government installation.
o Provide all specialized equipment to accomplish work to be done at Contractor cost to perform work.
1.3 LOCATION: Building 5160 at 1865 Selfridge Blvd. Lackland Air Force Base, TX. 78236.
1.4 WORK AND MECHANICS: The work for this project shall be executed in the best and most craftsman-like manner, by qualified commercial door maintenance mechanics/tradesmen, skilled in their respective trades.
Journeymen of 3 years’ experience in this respective trade, or apprentices under the direct supervision of certified journeymen, shall be permitted to install and/or supervise installation for this project. Individual trade work for this project shall be performed and quality maintained by the applicable trade only. The Contractor shall assure that all trades coordinate their work with that of other trades. The Contractor shall coordinate and perform all operations in a manner that will result in a professional and expeditiously completed project. The work shall be in strict accordance with prevailing industry standards and manufacturer’s instructions. Work and materials shall comply with this Statement of Work and the editions in effect at the time of this solicitation for all applicable criteria, regulations, guidelines, and codes, all of which are made a part thereof.
1.5 WORKING CONDITIONS: All Contractor operated/owned vehicles, equipment, tools, etc., shall be stored as prescribed in paragraph DELIVERY, STORAGE, AND PARKING. Applicable traffic control signage where required by the contract shall remain overnight. There shall be no power outage in excess of 8 hours.
1.5.1 HOURS OF OPERATION: The contractor shall perform all work during the hours of 8:00am to 5:00pm, Monday through Friday. Work outside of normal business hours shall be requested from and approved by the Contracting Officer. The contractor is not required to perform work during federal holidays. (New Year's Day, Martin Luther King Day, Presidents Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day). The Contractor shall be allotted five (10) working days to coordinate project listed in Section 1.2., Project Description.
1.5.2 WORK AREA ACCESS: The Contractor shall coordinate with the designated Planning Technician, Mr.
Gabriel Talamantes (210) 808-0154, or the Planning Lead, Mr. Victor Reyna at (210) 808-0010.
1.5.3 PROJECT SCHEDULE PLAN: Contractor shall send a progress schedule plan of start date and completion date 7-14 days after it’s been awarded.
1.6 SAFETY: All Contractor operations shall be conducted and performed in accordance with Department of Labor, OSHA requirements found in 29 CFR 1910 (1910.146 and 1910.147) and 29 CFR 1926, and Air Force Occupational Safety & Health (AFOSH) AFOSH standards including 91-25. The Contractor shall also ensure that all work is performed in accordance with project identified national standards, military manuals, instructions, pamphlets, standards, and handbooks, and with the edition in effect on the date of this solicitation of the Corps of Engineers (COE) Safety Manual 385-1-1.
1.6.1 Resolution of Department of Labor citations for violations of Occupational Safety and Health Standards is a Contractor responsibility and shall provide for no basis of a claim against the Government.
1.7 PROTECTION OF GOVERNMENT PROPERTY: The Contractor shall use reasonable care to avoid damaging existing buildings, equipment, and vegetation on the Government installation. If the Contractor fails to use reasonable care and causes damage to any of this property, the Contractor shall replace the damage at no expense to the Government. If the Contractor fails or refuses to make such replacements, the Contractor shall be liable for the cost, which may be deducted from the contract price.
1.8 SITE MAINTENANCE AND CLEANUP:
1.8.1 SITE MAINTENANCE: The Contractor shall protect adjacent property, buildings and their contents from dust, dirt or other materials. Work areas shall be maintained in a neat, clean, and safe condition and shall, at a minimum, be cleaned at the end of each shift. All streets and roadways in/or adjacent to the site shall remain free of project generated trash and debris at all times.
1.8.2 DAILY CLEANING: The Contractor shall keep the site free from debris at all times and shall perform daily cleaning of all areas.
1.8.3 CLEANUP: The Contractor shall collect any and all trash, debris, refuse, garbage, etc., that is generated and place it in appropriate containers with lids or approved covers on a periodic basis. The aforementioned materials shall be hauled from the site by appropriate means on a daily basis, unless otherwise approved by the designated Planning Technician. Disposal shall be outside the limits of Government property. Disposal shall be by sanitary landfill or other approved methods and shall conform to all local, state, and federal guidelines, criteria, and regulations. Upon completion of the work, the Contractor shall leave the work site and storage area(s) in a clean, neat, and workmanlike condition satisfactory.
1.9 WARRANTY: The Contractor shall identify all items being installed that are guaranteed or under warranty for more than a one-year period and provide validated copies of the manufacturer’s warranty. All warranty information shall be filed by the Contractor in the Government’s name. All warranties shall be identified by product with a listing of the name and address of the company and the expiration date of the guarantee or warranty.
The contractor shall guarantee all materials and workmanship for a period of one year from the date of project completion and shall within that period correct all defects in materials/workmanship within twenty-four (24) hours of being notified. Correction of all defects under this warranty shall be at the expense of the contractor.
1.10 DELIVERY, STORAGE, AND PARKING: All equipment delivered and stored shall be protected from the weather, humidity and temperature variation, dirt and dust, and any other contaminants.
1.10.1 CONTRACTOR STORAGE: The Planning Section Lead and/or designated Planning Technician shall designate Contractor storage and parking area.
1.10.2 STORAGE AREAS: Contractor shall coordinate with Project Manager for assigned storage area/space. All project storage areas shall be kept free of debris, leaks, stains, or splashes and kept in a neat, clean, and safe condition.
Any contamination of the storage area by a hazardous substance shall be immediately remediated by the Contractor.
1.11 BASE FIRE REGULATIONS: The Contractor shall comply with Base Fire Regulations as set forth in the latest edition of Garrison Air Force Base Instruction 32-2001, titled “Base Fire Protection Program”. The Contractor shall use no explosives in performing the work. All work shall be in strict compliance with NFPA-101.
1.12 REMOVAL: Items to be included in the removal shall include but not be limited to the careful removal and disposition of materials. The Contractor shall, before commencement of any work, carefully survey the existing site to determine the extent of the work. The Contractor shall take all necessary precautions to ensure against damage to existing work to remain in place, or to remain the property of the Government. Any damage to the aforementioned shall be replaced by the Contractor at no additional cost to the Government. The Contractor shall be responsible for the proper disposal of all materials, meeting all rules pertaining to the disposal of such products.
PART 2 – ENVIRONMENTAL REQUIREMENTS:
2.1 REFERENCES TO MATERIALS, MANUFACTURERS, AND PRODUCTS: Materials and equipment shall be the standard products of a manufacturer regularly engaged in the manufacture of the products and shall essentially duplicate items that have been in satisfactory use for at least 2 years prior to bid opening. Equipment shall be supported by a service organization that is, in the opinion of the Planning Section Lead, reasonably convenient to the site.
2.1.1 VERIFICATION OF DIMENSIONS AND CONDITIONS: The Contractor shall visit the premises to become thoroughly familiar with details of the work and working conditions, field verify all dimensions and advise the Planning Section Lead of any discrepancies before starting the work.
PART 3 – ADDITIONAL INFORMATION:
3.1 GENERAL: All work shall be installed as shown and in accordance with commercial door operator installation procedures, unless otherwise specified. All dimensions are shown for information only. Contractor shall field verify all dimensions and site conditions. The Contractor shall provide all labor, materials, tools and equipment required to perform all dismantling, replacements, and re-installation as listed in this statement of work.
3.2 OPERATIONAL SYSTEMS: The Contractor shall insure that all work is performed in accordance with the criteria herein and demonstrate to the requester/agent a successful function check.
3.3 INSPECTION AND ACCEPTANCE: A representative from 502 CES will conduct daily inspections of work progress. A final inspection of completed work will be conducted by the project manager, contractor and designated 502 CES personnel. All discrepancies identified during inspections shall be corrected prior to final payment.
3.4 MANUALS: The Contractor shall provide all applicable manufacturers recommendations and operating instructions to the designated Planning Technician or Planning Section Lead.
