STATEMENT OF WORK
FOR
PURCHASE AND INSTALLATION OF OFFICE CARPET
367th Cyberspace Operations Squadron
LACKLAND AFB, TEXAS
19 January 2023 Amended 9/1/2023
1. GENERAL DESCRIPTION: The Contractor shall provide management, labor, equipment, tools, materials, delivery, transportation, disposal and incidentals necessary to accomplish the work required within this Statement of Work (SOW). The work required includes but is not limited to the design, purchase of new products, and the installation in building 2058, 367COS spaces (Rooms 345, 351, 352 A-C, 355, 356A-B, 357-360, 362-369,378- 379, 382) located at 250 Hall Blvd, San Antonio TX, 78243. If the contractor is unable to provide any of the items listed in this document, these deficiencies must be identified clearly in contractor’s offer. Specific, detailed installation drawings and specifications are the responsibility of the contractor. All work shall be in conformance with the specifications for installation of the carpet. All materials and accessories shall be installed in accordance with industry standards and manufacturer’s written recommendations.
a) The contractor shall ensure policies and procedures are established that protect the safety and welfare of customers, employees, and the community to minimize or eliminate safety or environmental risks. These policies shall be in compliance with Federal and Installation policies and procedures.
b) The contractor shall provide a contract manager who shall be responsible for the performance of the work. The name of this person and an alternate or alternates who shall act for the contractor when the manager is absent shall be designated in writing to the Contracting Officer (CO) within 10 days after contract award, if required. The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to the daily operations of this contract on the installation.
c) The contractor shall provide a Project Manager (PM) post award with experience installing the contractor’s product. PM will be responsible for project site management, scheduling, coordination and technical assistance to the Air Force representative as required throughout the project.
d) All contractors shall submit a complete list of personnel after award in accordance with installation policy who will be expected to work on the installation. This list will contain Privacy Act information as required as well as the expectation that each individual will be able to present a current and valid state identification, to meet local Air Force security requirements. The contractor shall coordinate with 367 COS Security Office @ 210-977- 5282 to gain facility access. (Please note that many installations require this information be coordinated at least 10 days in advance).
e) The contractor shall not employ persons for work on this contract if such employee is identified to the contractor by the government as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population.
Contractor personnel shall present a neat appearance and be easily recognized as contractor employees (i.e., name tags, badges, uniforms).
f) The contractor shall be an authorized dealer of the Tier 1 carpet-type installed, as identified as one of the Tier 1 carpets identified in Section 2a below.
g) The vendor shall manage ordering, shipment, delivery and installation of all carpet and supplies from the manufacturer to final acceptance by the designated Government Acceptor.
In no case is the Government responsible to store product above and beyond the product required during installation, or to accept direct delivery of freight for the contractor.
h) Vehicle registration, proof of insurance and a valid driver’s license shall be presented to gain base access.
i) The contractor shall be responsible for coordination of an escort through any controlled areas through the assigned Government Acceptor.
j) Lost vehicle and entry passes shall be reported immediately to the Security Police as well as the Government Acceptor in accordance with the local security forces policy.
k) The contractor shall be responsible for storage of all new carpet, components, and anything else needed to install carpet prior to and during installation.
l) The contractor shall provide the necessary resources to manage and administer the contract.
The contractor shall, as a minimum, maintain a single point of contact for this effort.
m) Contractor shall notify the Contracting Officer (502d CONS) in writing of any conditions which shall prevent satisfactory start and completion of carpet installation.
n) Contractor is responsible for daily cleanup of the project area.
2. SCOPE: For this project, the following is required for purchase, delivery, and complete installation:
Demo and dispose of existing carpet tile, wall base, and the install new carpet and wall base.
Period of performance is 60 days after the contract has been awarded.
Once the contract has been awarded, it becomes the responsibility of the contractor to validate the actual site conditions as well as any specific needs of the customer. In no case shall prices exceed the Not-to-Exceed (NTE) amount established in the Tier I contract. It is understood that the actual site parameters or certain customer requirements may deviate from the standard typical layouts; therefore any deviation that causes an increase in the price submitted during the offer process must be documented in writing and submitted to the Contracting Officer for review prior to implementing changes, along with a floor plan reference to show just cause for the increase in price in comparison to original offer.
3. ACCESSORIES AND INCIDENTALS: Contractor shall provide all incidentals necessary, furniture lift equipment and other parts needed to properly install the carpet. All accessories shall be installed in accordance with the manufacturer’s recommendation.
4. WARRANTIES: Contractor shall provide all commercial warranties for product as provided by the manufacturer.
5. LOCATION: Project work is to be performed on Lackland AFB. Project area is in Building 2058, 367COS spaces (Rooms 345, 351, 352 A-C, 355, 356A-B, 357-360, 362-369,378-379, 382). The organization is the 367th Cyberspace Operations Squadron. The building is located at 250 Hall Blvd, San Antonio TX, 78243. The project includes ample surrounding parking and accessibility areas. No cell phones or two-way communication devices will be allowed into the facility. The contractor shall coordinate with the 367th COS Area Manager, 977-4062 to gain base and facility access. Vehicle registration, proof of insurance as wells as names, date of birth and valid driver’s license number for all drivers shall be presented to gain base access.
6. HOURS OF OPERATION: Project work is required between 3:00 p.m. and Midnight, Monday through Friday, excluding holidays. Access to work sites will be restricted to these hours and days, unless approved otherwise by the 502d Contracting Officer.
a) Recognized US Federal Holidays are as follows: New Year’s Day, Martin Luther King
Day, Presidents Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving and Christmas. If the holiday falls on a Saturday, it is observed on Friday. If the holiday falls on Sunday, it is observed on Monday.
b) When the contractor requires to work other than the agreed upon times, a written request shall be submitted to the Government Acceptor for approval. Written approval from the Contracting Officer must be received before proceeding with the work. The request shall be submitted at least five (5) workdays prior to the proposed performance of work, and it shall indicate the day/days, time/amount of work, and reason why performance is necessary during nonstandard hours.
7. INSTALLATION: All related accessories shall be installed in accordance with industry standards, manufacturer’s recommendations, and call order requirements and per the approved design. Carpet will be installed with existing furniture in place. Use of a furniture lift system is recommended. In cases where the use of a furniture lift system isn’t feasible, the vendor is responsible for moving all furniture and appliances necessary to complete installation. Contractor shall coordinate installation start date no less than one calendar week prior to start of installation.
Contractor shall repair any damages caused to government property as a result of installation at no additional cost to the government.
8. CLEAN UP: No government dumpsters are available for use. All debris, trash and refuse shall be removed from the work site on a daily basis. At completion of project, all areas shall be left clean and in a final professional appearance. All Contractor generated refuse and waste shall be disposed of in the proper manners outside the physical boundaries of the installation. The Contractor may use a waste container provided by the Contractor during the Period of Performance. If the contractor chooses to supply a dumpster or otherwise requires a laydown area, this must be requested from the government at least 14 days before work begin.
9. DELIVERY/STORAGE: Delivery address is 1300 Medina Base Rd (bldg. 331) Lackland, Air Force Base, TX 78236. Facility site does not include a loading dock. Product delivery from manufacturer to Contractor’s site is required. The contractor is responsible for transportation to the worksite or shall make arrangements for a laydown area two weeks in advance. Entrance to facility will be coordinated through the Government representative. The Contractor is responsible for transportation of all new products, loading, unloading, storage and all necessary movement from factory to final onsite installation.
a) The contractor Project Manager shall be responsible for on-site coordination of all staging and deliveries of their product, to include access to and operation of any equipment needed for unloading and movement of materials or product.
b) Contractor shall be responsible for determination of the appropriate delivery method and handling, based on site conditions to include the existence loading docks and equipment at any given location.
c) Work shall comply with applicable building standards and codes, specifically: National Electrical Code (National Fire Protection Association (NFPA 70) and National Life Safety Code (NFPA 101).
d) The contractor shall be responsible for following all safety and security guidelines within the area in which work is being performed, to include securing any contractor owned tools or equipment, and any on site storage being utilized.
e) The contractor shall maintain accountability and control of any keys and/or passes provided and shall return them to the Government Acceptor upon completion of work.
f) The contractor shall prevent its personnel from entering any area other than the designated work area to complete the project herein.
g) The contractor shall maintain a means of egress within all designated work areas to comply with fire codes.
h) The contractor shall ensure its personnel eat, drink, or smoke only in designated areas.
10. INSPECTION/ACCEPTANCE: The government shall inspect all work performed by the contractor. Work performance shall be deemed satisfactory and acceptable to the government when the 91COS Area Manager approves and declares the project satisfactory. The government will document all inspections. Validation between the contractor and the Government Acceptor that all product has been installed according to manufacturer guidelines and customer specifications.
a) A post-installation walkthrough shall be coordinated with the contractor’s authorized representative and Government Inspector/Acceptor within 5 business days of installation.
b) All areas requiring correction shall be documented in a punch list by the contractor and provided to the authorized government representative upon completion of a joint walk through by the contractor and authorized government representative.
c) A final walk through will be performed with the contractor and the Government Inspector/Acceptor to ensure all punch list items have been addressed to the satisfaction of the Government within 5 business days of the post-installation walkthrough.
