PERFORMANCE WORK STATEMENT

FOR

ADVISORY AND PROGRAM MANAGEMENT SUPPORT

MEDICAL EDUCATION AND TRAINING CAMPUS (METC)

3 November 2022

Revised: 1 December 2022

1. DESCRIPTION OF SERVICES / GENERAL INFORMATION. The Contractor shall provide all management, tools, supplies, equipment, and labor necessary to provide non-personal services in Advisory and Program Management Support at the Medical Education and Training

Campus (METC), Joint Base San Antonio-Fort Sam Houston, TX.

1.1. DESCRIPTION OF SERVICES. The contractor shall provide Seven (7) qualified and experienced Full-Time Equivalent (FTE) personnel to support, assist, advise, and/or provide training for the efficient and effective management and operation of the METC Standards and

Evaluation Division (SED). The contractor shall provide a variety of management and professional services in the following subcategories as described in this Performance Work

Statement (PWS):

• Accomplish data analysis and lessons learned development

• Draft reports

• Provide reviews, evaluations, and recommendations on subjects of highly specialized areas of concern

• Conduct coordination efforts with program sponsors, management and technical personnel, support organizations and outside agencies

• Supplement METC Faculty and Staff’s knowledge and expertise in the areas of (but not limited to) instructional delivery processes, curriculum development, course evaluation, academic counseling, instructional qualification records, and institutional and programmatic accreditations

NOTE: See Appendix B for a list of required FTE position Education, Experience and/or

Special Qualifications.

1.1.1. QUALITY ASSURANCE AND ACCREDITATION SUPPORT. The contractor shall provide Two (2) FTE Training Technicians to provide comprehensive support to the Quality

Assurance and Accreditation Branches within SED.

1.1.1.1. Collect, analyze, and evaluate accreditation, affiliation, and quality assurance data. Create trend reports concerning data collected and make recommendations based on results. Develop various databases to store and retrieve accreditation, affiliation, and quality assurance data. Assist

SED Division Managers and/or Contracting Officer Representative (COR) personnel with creating metrics, metric presentations, and reports. Examine problem areas and identify solutions. Rely on analytical skills, including mathematics and statistics, to plan and accomplish goals. Develop processes to improve the effectiveness of current SED operational activities. Work with

Information Management/ Information Technology and other METC divisions on processes to streamline data collection.

1.1.1.2. Interact daily with METC academic programs to schedule End of Program (EOP), Quality of Life (QOL), and Anytime Student Feedback Survey (ASFS). Conduct, and report information obtained from surveys to the Quality Assurance (QA) NCOIC and SED Division Managers and/or

COR weekly. Survey data retrieval shall occur within 72 hours of conducting a survey and sent to the QA NCOIC or SED Division Managers and/or COR within 24 hours if a concern exists. EOP and QOL data must be sent to the program within five days unless an area of concern is discovered per the QA Standard Operating Procedure (SOP). Training Technicians shall be responsible for sending survey data to the SED Program Director, Department Chief, and COR. In addition, they may be tasked to communicate any issues discovered to additional individuals (as required and deemed appropriate by METC SED Division Managers and/or COR).

1.1.1.3. Perform data entry using word processing, spreadsheets, or database commands; and format material as required. Prepare graphic exhibits using software for presentations and agendas.

Create data directories/subdirectories for file and report generation retrieval purposes. Maintain paper and digital files and retrieve reports upon request. Interact with departmental and other staff on matters affecting data and make recommendations for improvement or enhancement to processes to ensure data entry collection and retrieval. Support SED Division Manager personnel with acquiring data required for accreditation or affiliation site visits.

1.1.1.4. Facilitate Classroom Standards and Compliance Checklists (CSCC) with SED Division

Managers and/or COR as well as other METC Divisions Programs annually; contact with Program

Directors for scheduling shall be required. Along with analysis of current data on local share drives, reviews education/training from the program instructors, interaction with students, classroom conditions, and other items required for display within classrooms as necessary. Once all information has been received, Training Technicians shall collaborate with METC Faculty

Management and Accreditation (and SED Division Managers and/or COR) to complete their reports for the verification process. CSCC reports shall be returned within 72 hours to the applicable Program for review and all deficiencies shall be identified and reassessed within thirty calendar days.

1.1.1.5. Required to learn functional areas and responsibilities to brief at METC orientations monthly. Training Technicians should be able to explain their role accurately and efficiently within

METC, DHA, and each military service. Brief on the extended function of creating surveys upon request for programs and survey ability to improve training to incoming staff.

1.1.1.6. Assist in conducting Quality Assurance Review (QAR) by coordinating and conducting briefings with scheduled programs, providing data for return and review, arranging appointments for interviews, and finalizing the executive summary (EXSUM) as needed. Training Technicians shall review relevant survey data results before conducting QARs and program checklists to complete the QAR process.

1.1.1.7. Assist SED Division Managers and/or COR with data to develop goals, objectives, and strategic plans. Provide graphical data to demonstrate trends and other statistical data to users at the program level and senior executives. Make recommendations to department personnel and provide continuous process improvement to the SED.

1.1.1.8. Training Technicians shall resolve all substantiated customer complaints within five (5) business days after notice of complaint.

1.1.2. INSTRUCTOR EVALUATION SUPPORT. The contractor shall provide One (1) FTE

Instructor Trainer to provide education and training support to the Faculty Management Branch

(FMB) within SED. Instructor Trainer is primarily responsible for administration of the METC

Instructor Evaluator Program; in addition to managing the faculty database that maintains instructor records, Faculty Tracking database, and Academic Reporting Menu.

1.1.2.1. Administer the Instructor Evaluator Program. Instructor Trainer design and develops policies, procedures and training lesson plans for program administration and training. Identify developmental training needs of evaluator members and provide for needed development and training. Serve as lead program trainer for campus instructional staff. Identify ways to improve production or increase the quality of the instructor evaluation process being performed. Plan, direct, coordinate and manage the scheduling, functions, and activities of the program’s evaluator members. Develop internal policies, methods and procedures and training programs for carrying out evaluator functions, based on guidance and directives SED Division Managers and/or METC executive leadership. Plan, coordinate and conduct Instructor Evaluator meetings. Provide

Instructor Evaluator Program orientation briefings to incoming instructional staff. Based on program manning levels, find ways to improve production and/or increase quality of the evaluation process being directed. Develop and maintain program performance standards. Ensure maximum production and utilization of program staff. Forecast and coordinate utilization program staff to ensure mission accomplishment during temporary manning shortages. Collect completed monthly program evaluations, ensuring proper content, quality, timeliness, and accuracy of all completed evaluations. Report program performance results to SED Division Managers and/or COR for organizational metrics reporting purposes. The Instructor Trainer shall interface with a variety of

METC staff at various levels.

1.1.2.2. Data Analysis. Coordinate, direct and manage all aspects of daily faculty and programmatic data collection for METC academic programs comprised of approximately 1,200 faculty and support staff. Ensure compliance, proper content, quality, timeliness, and accuracy of program data entry for monthly metrics reporting purposes. Conduct detailed trend analysis, monitoring data content and quality from program reporting sources. Ensure analysis/studies are coordinated with the appropriate activities of the METC Project Management Team tasked with utilizing data for preparation of reports and presentation of results to METC executive leadership.

Focal point for ad hoc queries of programmatic information to include faculty qualifications and certification status, faculty utilization, and service component manning ratios. Oversees, manages, and grants academic reporting menu and faculty tracker database access. This is for various levels of program/support level activity ranging from read-only to full control permission levels. Lead trainer for faculty tracker and all academic reporting group menu tabs. Develop and perform various levels of database training from classroom environment to executive leadership functions.

1.1.2.3. Quality Assurance. Studies, analyzes, develops, and implements methods to improve

Evaluator Program. Plans, coordinates, and monitors the Instructor Evaluator Program’s progress.

Perform self-inspection and presents detailed comprehensive reports with any corrective actions taken to supervisor. Follows-up to ensure complete and quality resolution of discrepancies. Assess and revises policies and procedures as needed to find ways to improve quality, timeliness, and efficiency work. Responds to external and internal customer needs by researching records and files, contacting others to obtain and disseminate proper and accurate information. Perform various assignments as a member of the FMB. This may involve the execution of operational policies and procedures; examining concepts, policies, techniques/methods, and their effectiveness to determine a preferred courses of action for METC instructors.

1.1.2.4 Instructor Trainer shall resolve all substantiated customer complaints within five (5) business days after notice of complaint.

1.1.3. FACULTY MANAGEMENT SUPPORT. The contractor shall provide Four (4) FTE

Healthcare Administrators as follows:

• One (1) to serve as Lead Faculty Manager, and

• Three (3) to serve as Faculty Managers.

Individuals provide faculty management support to FMB within the SED. They are primarily responsible to collect, analyze, and evaluate faculty record data, and make sound recommendations based on findings. Administrators shall interface directly with a variety of METC program and

Academic Affairs staff. Administrators shall maintain the instructor records located in the faculty databases.

1.1.3.1. The Lead Faculty Manager shall provide guidance to the Faculty Managers in support of all METC courses. Develops and maintains detailed processes to determine the effectiveness of current operational activities, examines problem areas, and develops solutions. Provide feedback to FMB Chief and SED Division Managers and/or COR on complex matters and takes steps to resolve the problem. Serves as technical expert in instructor qualifications and credentialing requirements and provides record analyses. Establishes priorities, plans, schedules and conduct work to accomplish the mission. Informs FMB Chief and SED Division Managers and/or COR on changes in policy and any potential impact on the METC mission. Relies on analytical skills, to include mathematics and statistics, to plan and accomplish goals utilizing locally developed reporting tools, faculty database, and/or local instructor databases. Initiates and provides in-depth training on the faculty database to METC faculty.

1.1.3.2. In-Process/Out-Process Instructors. Develops and implement efficient processes to add and/or remove new/returning instructors from all METC programs in the faculty database. This will be accomplished within five duty days of instructor arrival for duties and/or removal from instructor duty. Conducts analysis to monitor and verify the completion of in-processing paperwork, in accordance with school, Service and METC policies. Reconciles instructor statuses with program reports and ensure they are added and removed from the faculty database based on monthly faculty tracker reports. Proactively responds to external and internal customer needs by researching records and files, contacting others to obtain and disseminate proper and accurate information. Implements in place processes to ensure transcript requests and degree completion verification letters are completed and submitted according to policy. Conducts out-processing procedures for instructor staff as outlined in METC policy.

1.1.3.3. Monitor Instructors. Tracks and reports instructor qualifications and requirements in faculty database to Branch and Division leadership. Manages the faculty database that maintains instructor records. Conducts thorough review/analysis of Internship Reports, Instructor

Evaluations, Non-Instructor Duty Reports, Subject Matter Testing, Instructor Methodology, Instructor Qualifications Worksheet/Certifications, and other reports as outlined in school and/or

METC policy. Promulgates information to Department Chairs and Program Directors and monitors outcomes and feedback. Disseminates quarterly spreadsheet to executive leadership on instructor qualification progress. Coordinates with other Standards & Evaluation Branches and

Programs for any curriculum changes or course number changes.

1.1.3.4. Trainer. Responsible for the oversight, training, development, and management of METC

Instructor Supervisors in their duties in managing the faculty database. Plans, coordinates, and sets the agenda for the Instructor Supervisors during monthly meetings. Provides monthly training sessions on the faculty database and ensure METC instructor qualification standards are met for all METC instructor staff. Provides METC orientation briefings to incoming personnel. Applies knowledge/application of leadership and team building/coaching skills and techniques. These include group facilitation, coordination, problem solving, interpersonal communication, integration of work processes/products, and obtaining resources.

1.1.3.5. Technical Guidance. Provides monthly metrics and analysis to executive leadership, on compliance with requirements via status slides and reports. Provides briefings to the Branch, Division and/or executive leadership on mission essential areas affecting METC’s ability to maintain qualified instructors. Coordinate with higher echelons; Army, Navy, and Air Force educational activities and METC institutions to meet identified training needs. Reviews the qualification standards of instructors (on commissions of higher education rosters) and reports status quarterly. Provides weekly reports to Department Chairs and Program Directors on significant issues impacting their instructor qualifications. Provide monthly report to the Services and METC leadership regarding instructor qualification status.

1.1.3.6. Quality Assurance. Coordinates and manages all aspects of daily instructional faculty and programmatic data collection for METC programs. Ensures compliance, proper content, quality, timeliness, and accuracy of program data entry for monthly metrics reporting purposes.

Perform self-inspection and presents detailed comprehensive reports with any corrective action taken to appropriate supervisor and SED Division Managers and/or COR. Initiates follow-up actions to ensure complete and quality resolution of discrepancies. Assesses and revises policies and procedures (as needed) to find ways to improve quality, timeliness, and efficient work.

Responds to external and internal customer needs by researching records and files, contacting others to obtain and ensure dissemination of proper and accurate information. Performs various assignments as a member of FMB. Duties may involve the execution of operational policies and procedures; examine concepts, policies, techniques, methods, and their effectiveness to recommend preferred courses of action for instructors at METC. Additional duties may require contractor to perform data entry using word processing, spreadsheets or database commands and formats material.

1.1.3.7. Faculty Managers shall resolve all substantiated customer complaints within five (5) business days after notice of complaint.

1.2. GENERAL INFORMATION.

1.2.1. CONDUCT. The Government reserves the right to restrict the performance on this contract by any individual who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of METC and its population. Contractor employees shall possess and maintain the qualities of integrity and professionalism that demonstrate a positive work ethic in relation to the METC mission. Contractor employees shall keep interactions with the military/DoD civilians, other contractors, and volunteers at a professional level; avoid unprofessional relationships which may create the appearance of favoritism, misuse of position, or reflect poorly on the METC mission. Furthermore, contractor employees shall refrain from disruptive, offensive, or otherwise improper behavior that undermines order and discipline.

Employees shall be neatly groomed and wear appropriate attire while serving in official capacity as determined by the respective METC SED Division/Program Managers and/or COR. The

Contracting Officer may request that any Contractor employee, whose conduct, or appearance in the opinion of the Contracting Officer, interferes with proper order or professionalism correct the identified behavior. If the contractor employee refuses, they may be disqualified from providing services and removed from the installation. The Prime contractor will comply with such requests and provide prompt replacements suitable to the government. Removal under other circumstances will be subsequent to, and at the direction of the Contracting Officer only. If a situation meriting removal occurs, the COR will contact the Contracting Officer and the Contractor representative within twenty-four (24) hours. A meeting may be required with the Contracting Officer, COR, and

Contractor representative to discuss further actions.

1.2.2. Identification of Contractor Employees: All contractor personnel attending meetings, answering government telephones, and working in other situations where their contractor status is not obvious to third parties are required to identify themselves as such to avoid creating an impression in the minds of members of the public that they are government officials. The contractor shall ensure that all documents or reports produced by contractor employees are suitably marked as contractor products or that contractor participation is appropriately disclosed.

1.2.3. Name Tags. The contractor will provide name tags for contractor personnel identifying them as contractor personnel providing services at government facilities under this contract.

Contractor personnel shall wear this name tag in a visible area at all times during work performance. Note: This is at no cost to the Government.

1.2.4. Media Inquiries: Contractor personnel shall not respond to any media inquiries nor provide interviews, comments, or any other responses to the media regarding any subject related to this contract. All inquiries or complaints from the media or other sources shall be immediately relayed to the COR.

1.2.5. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their privately-owned vehicle while on any JBSA installations.

1.3. QUALITY CONTROL. The contractor shall be responsible for quality control for all work accomplished during the performance of the task. The contractor shall provide and maintain a Quality Control Plan (QCP) which ensures the requirements in this PWS are met. The QCP will be considered acceptable and approved unless the contractor is notified by the Contracting Officer

(CO) prior to award. The QCP shall be included in the technical proposal for evaluation and acceptance. As a minimum, the contractor shall develop quality control procedures that address the areas identified in Section 2, Service Summary. In addition, the Quality Control Plan (QCP) shall contain, at a minimum:

• Contractor’s plan to address complaints received from the Government and resolve issues within 15 days after notice.

• The inspection system to cover all services. Description shall include specifics as to the areas to be inspected on a scheduled and unscheduled basis, frequency of inspections, and the title/organizational placement of the inspector(s).

• The methods used for identifying and preventing defects in the quality of services performed.

• Records must document all inspections and corrective, or preventative actions taken.

• Note: Contractor shall define in their Quality Control Plan (QCP), internal procedures to cover fluctuations in staffing, vacancies and/or employee absences.

The Contractor shall also identify any applicable commercial and industry Safety Standards and include them in the QCP. The Contractor shall provide the Government the contact information, to include business telephone, cell phone if necessary, and business email address of a designated

Quality Control inspector, who shall be available to respond to the Government’s request by the next business day after notification. The QCP shall be submitted to the Contracting Officer

Representative (COR) and Contracting Officer (CO) fifteen (15) calendar days after contract award and five (5) days when changes are made thereafter.

1.3.1. QUALITY ASSURANCE. The Government will periodically evaluate the contractor’s performance by appointing a Contracting Officer Representative(s) (COR) to monitor performance to ensure services are received. The COR will evaluate the contractor’s performance in accordance with (IAW) the Quality Assurance Surveillance Plan (QASP) for this requirement.

1.3.2. HOURS OF OPERATION. The contractor is responsible for conducting business during normal duty hours Monday thru Friday except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. Normal duty hours are as follows:

Monday-Friday: From 0700 hours to 1600 hours

The Contractor must at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this PWS when the Government facility is not closed for the above reasons. Any work performed outside of the hours must be coordinated through the COR and approved by the Contracting Officer.

1.3.2.1. Routine or Situational Telework. Contractor shall submit teleworking requests to the

COR with full justification information. The government reserves the right to deny and/or authorize routine or situational telework on a case-by-case basis. Examples include, but are not limited to the following:

a) Employee has a short-term need for uninterrupted time to complete work on a complex project or report.

b) Employee is recovering from illness or an injury and is temporarily unable to report to the traditional office physically.

c) Employee receives word of inclement weather and notifies her/his supervisor that s/he would like to opt for unscheduled telework that day.

d) Employee is directed to work from an alternate location as a result of a local, state, or national event/crisis.

e) Teleworking is a privilege NOT a Right!

The contractor shall be responsible for safeguarding any (i.e., Government Laptops) provided by the government. Prior to commencing telework, the employee must complete fundamental telework employee training and have a suitable workspace, adequate utilities for installing equipment and a general work environment that is free from interruptions and provides reasonable security and protection of government laptops and governmental information.

1.3.3. HOLIDAYS OBSERVED.

Note: When any holiday falls on a non-business day (Saturday or Sunday), the holiday is usually observed on Friday (if the holiday falls on Saturday) or Monday (if the holiday falls on Sunday).

1.3.4. SECURITY REQUIREMENTS. Contractor personnel shall complete, as a minimum, a

Tier 1 (T1) investigation, before operating government furnished workstations. The contractor shall comply with the Department of Defense (DOD) 5200.2-R, Personnel Security Program, Air

Force Manual (AFMAN) 33-152, User Responsibilities and Guidance for Information Systems, and Air Force Instruction (AFI) 33-200, Information Assurance (IA) Management, requirements.

T1 investigation requests are initiated using the Standard Form (SF) 85 and are submitted to the installation Information Protection Office through the using agency's Unit Security Manager. T1 investigations are different from the Wants and Warrants checks and are provided by the government at no additional cost to the contractor.

1.3.4.1. The contractor shall fully adhere with the provisions of the referenced publications by having each of their employees who are performing under this contract submit the appropriate forms and maintain a favorable suitability determination to continue performing under this contract. The contractor shall ensure the following is accomplished at least 14 calendar days prior to the required start or desired start date.

1.3.4.1.1. Submit personnel name, email account, contract number, required start date, and telephone contact number to the COR and Unit Security Manager.

1.3.4.2. Schedule an appointment with the Unit Security Manager where services are provided.

For public trust positions, the appointment shall be arranged for the contractor’s first day of work.

The Contractor personnel requiring a T1 investigation cannot start work earlier than the scheduled appointment date with the Unit Security Manager. Contractor personnel requiring a clearance shall follow local security process and official start date shall depend on local processes.

1.3.4.3. Ensure the contractor completes the SF-85 in a secure website called Electronic

Questionnaires for Investigations Processing (e-QIP) Direct after the Unit Security Manager contacts the contract employee to establish an account in e-QIP Direct, or (in some cases) completes the SF-85 from a downloadable form on the Office of Personnel Management (OPM) website and sends the completed form to the Unit Security Manager. The contractor shall ensure the contract employee completes the SF-85 before the appointment with the Unit Security Manager when provided an e-QIP account or downloadable form on the OPM website. The employee shall need access to documentation requested on the SF-85 (e.g., current, and previous addresses in the past 7 years, zip codes, telephone phone numbers, passport numbers, family member names, SSNs) in order to ensure the SF-85 is completed before the appointment date. The Unit Security Manager may provide instructions with regard to faxing or emailing signature forms for the SF-85 as well as information on completing the FD-258, FBI Fingerprint Card before the appointment date.

1.3.4.4. Contractor personnel may be required to submit an original Birth or Naturalization

Certificate to the Unit Security Manager. Copies of the certificate shall be maintained by the

Government.

1.3.4.5. The appointment with the Unit Security Manager is necessary to ensure:

1.3.4.5.1. The Unit Security Manager is expecting the contracting personnel’s arrival and is available to review and process the SF-85, if not already accomplished through e-QIP.

1.3.4.5.2. The Unit Security Manager obtains or arranges for the fingerprints (FD-258, FBI

Fingerprint Card or electronic submission) for submission to the base-level information protection office, if not already complete. Once the fingerprints are processed, the Government shall open an investigation via the Office of Personnel Management (OPM) NP2 Secure Portal. With an open investigation, the local commander may grant interim access to the base and Government computer systems pending the results of the T1 investigation.

1.3.4.5.3. The Contractor personnel completed all security requirements.

1.3.4.6. The contractor shall advise their contracted personnel that a favorable report, verified through the Joint Personnel Adjudication System (JPAS), is needed as a condition of employment under this contract. The contractor understands that, while the unit commander may allow contracted personnel temporary or interim access to Government systems in non-sensitive positions pending the outcome of the T1 investigation (usually through a 90-day waiver), contract personnel shall be immediately removed from the position if the T1 investigation returns with unfavorable or denied adjudication. In addition, the Government shall immediately suspend contract personnel computer access if the T1 investigation is returned with an adjudication of “no determination made”. The Commander shall determine whether to allow the contract personnel to continue in the public trust position or deny computer access, which requires the contract personnel to be removed from the position.

1.3.5. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE

NATIONAL COMMAND AUTHORITY OR OVERSEAS COMBATANT COMMANDER.

The services required under this PWS are not designated as mission essential.

1.3.6. RECORDS MANAGEMENT. The contractor shall manage all data created for

Government use or legally controlled by the Government, in support of the functional activity or required by Air Force (AF) publication, IAW the AF records management procedures in AFI 33-

1.3.7. AIR FORCE ANTITERRORISM (AT) PROGRAM. The AT program seeks to deter or limit the effects of terrorist acts against the AF by giving guidance on collecting and disseminating timely threat information, providing training to all AF members, developing comprehensive plans to deter and counter terrorist incidents, allocating funds and personnel and implementing AT measures. IAW AFI 10-245, Antiterrorism, paragraphs 2.18 and paragraph

2.25, DOD contractors and sub-contractor personnel shall be offered Level 1 – AT Awareness

Training. AT Awareness Training may be accomplished by either a Level I – qualified instructor, or by completing the "Force Protection" computer based training (CBT) course on the Advanced

Distributed Learning System (ADLS) https://golearn.csd.disa.mil/kc/login/login.asp or the

Defense Technical Information Center (DTIC) at https://atlevel1.dtic.mil/at for individuals without access to Nonsecure Internet Protocol Router (NIPR), possessing a Common Access Card

(CAC), nor enrolled in Defense Enrollment Eligibility Reporting System (DEERS). Compliance with the training will be monitored by the COR.

1.3.8. Installation/Facility Access and General Protection. As prescribed by the AFFAR

5352.242-9000, Contractor access to Air Force installations, the contractor shall comply with the following requirements:

a. The contractor shall obtain installation access passes for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to present the installation access pass while visiting or performing work on the installation.

b. No later than ten working days prior to contract commencement, the contractor shall submit a written request on company letterhead to the COR (or other authorized program manager) listing the following:

1. Contract number,

2. Location of work site,

3. Start and stop dates, and

4. Names, dates of birth, driver’s license number and state of issue for the contractor employees needing access to the base.

5. The COR (or other authorized program manager) will endorse the request and forward it to the issuing installation pass and registration office or security forces (SF) for processing.

6. Contractors will present government (state or federal) issued ID, before being issued a pass to enter the installation. Before being issued a pass to enter the installation, a criminal history check will be conducted for every individual requesting a pass.

Personnel employed by the contractor and operating a motor vehicle on the installation must possess proof of the following:

(a) Liability Insurance

(b) Current License Plates

(c) Current State Inspection Sticker (If required)

(d) Valid State Driver License

c. Traffic Laws. The contractor and their employees shall comply with base traffic regulations.

d. Upon completion or termination of the contract or expiration of the identification passes, the contractor shall ensure that all base identification credentials issued to contractor employees are returned to the issuing office. If a contractor employee has been terminated, the credentials will need to be retrieved and returned to issuing activity so that employee does not have base access. If the credential is not retrieved, then Security Forces will need to be notified so base access is not allowed. NOTE: In addition to turn-in of Base Access credentials, any Contractor personnel terminated for any reason, or upon contract termination, shall turn in their DoD

Common Access Card (CAC), and any other documents issued by the government to the COR.

e. Failure to comply with these requirements may result in withholding of final payment to the prime contractor.

1.3.9. Insurance. The contractor shall provide proof of insurance in accordance with Federal

Acquisition Regulation (FAR) clause 52.228-5, Insurance - Work on a Government Installation.

A copy of the insurance policy and proof of payment shall be submitted to the Contracting Officer within 10 days of contract award and annually thereafter, or at time of policy renewal.

1.3.10. Healthcare. Healthcare provided at the local military treatment facility is available on an emergency reimbursable basis only.

1.3.11. PERIOD OF PERFORMANCE. The period of performance shall be for a base period of twelve (12) months and four (4) 12-month option periods. The period of performance reads as follows:

Base Period 1 February 2023 to 31 January 2024

Option Period 1 1 February 2024 to 31 January 2025

Option Period 2 1 February 2025 to 31 January 2026

Option Period 3 1 February 2026 to 31 January 2027

Option Period 4 1 February 2027 to 31 January 2028

3. GOVERNMENT FURNISHED PROPERTY (GFP) AND SERVICES.

3.1. The Government shall provide the contractor with the Information Technology (IT) resources (e.g., computer workstations, software applications, printers, scanner, etc.) as well as other normal office equipment (e.g., telephones, faxes, etc.) required to perform the specific tasks of this requirement. NOTE: All government (IT) equipment provided the contractor is for contract exclusive work and shall not be removed from the installation or utilized by the contractor for

Prime Contractor company business. EXCEPTION: The Contractor will follow the guidelines to properly sign out the GFE (laptops) if authorized to perform Situational Telework.

3.2. DOCUMENTS. The Government shall furnish or make available to the contractor any documentation/material deemed necessary to accomplish the specific tasks of this requirement.

3.3. LOCAL AREA NETWORK (LAN). The contractor, with a valid T1 investigation through the JPAS roster of validation from the contractor’s security manager, shall be provided access to JBSA– Fort Sam Houston’s unclassified computer network and its inherent capabilities including, but not limited to: Internet access, electronic mail, file and print services and dial-in network access. The contractor shall be aware of and abide with all Government regulations concerning the authorized use of the Government’s computer network.

3.4. WORKSPACE. The Government shall provide the contractor adequate workspace to perform the specific tasks of this requirement and all office supplies necessary to perform the specific tasks of this requirement.
