1. DESCRIPTION OF SERVICES/GENERAL INFORMATION: The purpose of this contract is to provide Auxiliary Catholic Priest services for Joint Base San Antonio.

2. DESCRIPTION OF SERVICES

2.1. Contractor shall provide the religious support services as outlined in Appendix A within JBSA area of responsibility which includes Joint Base San Antonio – Lackland, (JBSA-LAK), Joint Base San Antonio – Fort Sam Houston (JBSA-FSH), Joint Base San Antonio – Randolph (JBSA-RND), and Camp Bullis.

2.1.1. The contractor shall not receive any stipend for services provided IAW this performance work statement.

2.2. Contractor shall meet with the designated Catholic Chaplain or Contracting Officer Representative (COR) on a quarterly basis to review and obtain the schedule of services for that quarter.

2.3. Contractor shall be at the respective places of worship and prepared for each service no later than 15 minutes prior to the designated starting time of any service, and no later than 15 minutes prior to the designated starting time of any Special Services.

2.4. Dress and appearance standards are business casual or appropriate religious garments, anything outside of the dress code outlined in this paragraph will require prior approval by the designated Catholic Chaplain or COR.

2.5. Contractor shall complete the monthly user file for Air Force Chaplain Corps Activity Reporting System (AFCCARS) by the 2nd day of the following month. The file will be forwarded to the base recorder. If unable to gain access to the military computer network to complete the file then a like report shall be provided and completed on a daily basis and given to the designated Catholic Chaplain or COR.

2.6. Submit after action reports for emergency ministry requirements within 72 hours.

2.7. The contractor must understand that privileged communication is protected communication IAW Military Rule of Evidence 503. Under this rule, “a person has a privilege to refuse to disclose and to prevent another from disclosing a confidential communication by the person to a clergyman or to a clergyman’s assistant, if such communication is made either as a formal act of religion or as a matter of conscience.”

2.7.1 Annual training will be completed by the contractor every year, which will be provided by the chapel staff and will cover:

-Privileged communication -Child care criminal history background checks IA W DoDI 1402.5, -Family advocacy program IAW AFI 40-301 and -Sexual assault prevention and response policy IAW AFI 90-6001

2.8. The contractor shall not use the duty title of chaplain while performing services, to include but not limited to printed documents and electronic communications.

2.9. Off-base services are not included in this contract except for special services as listed in Appendix A.

2.10. The Contractor shall designate a Contract Manager and alternate, who is responsible for the contract operations and one of whom, is available to the CO at all times during the normal work day. The Contract Manager /Alternate(s) shall have full authority to act for the Contractor on all matters.

2.11. Contractor shall invoice in Wide Area Workflow (WAWF) as three separate location specific invoices for services completed.

2.12. The contractor shall be required to coordinate efforts with chapel personnel and work in a multi-denominational and pluralistic religious environment to include, but not limited to, Catholic, Protestant, Jewish, Orthodox, and Islamic faiths. The contractor shall be required to provide all services in accordance with (IAW) Catholic, Archdiocese for the Military Services, USA (AMS) faith requirements.

2.13. CONTRACTOR’S REQUIREMENTS

2.13.1. The Contractor and/or any priest providing services must be a Roman Catholic Priest in good standing with their home diocese and the Archdiocese for the Military Services, USA (AMS) and be able to provide a completed DD Form 2088 for each Catholic Priest performing services and meet the educational and endorsement requirements of Air Force Instruction 36-2005.

2.13.1.1. In accordance with Air Force Instruction 52-101

Paragraph 3.7.2. Civilian Clergy. Personnel who serve in Civil Service positions or as contractors. These individuals are not chaplains and will not use the duty title of “chaplain”. (T-

2) They must meet the same educational requirements outlined in AFI 36-2005, Appointment in Commissioned Grades and Designation and Assignment in Professional Categories - Reserve of the Air Force and United States Air Force and provide a valid DD Form 2088, Statement of Ecclesiastical Endorsement from a recognized religious organization. (T-0)

2.13.1.2. In accordance with Air Force Instruction 36-2005:

Paragraph 4.4.1.1.1. Qualification and Requirements. Applicants must possess 120 semester hours of undergraduate credit from an accredited college or university; and possess a Master of Divinity (normally 90 semester hours) or an equivalent theological degree, (usually acquired in 3 years), or have completed 3 full time resident years of graduate level study in theology or related subjects leading to ordination and ecclesiastical endorsement that qualifies the applicant to perform professional functions as a chaplain.

2.13.2. Award is contingent on endorsement by the Archdiocese for the Military Services, USA

(AMS).

2.13.3. Ability to administer both the documentation of services performed and the timely rendering of payment for services (if used) is required.

2.13.4. The Contractor must be able to read, write, speak and understand English fluently. English shall be the only language used with regard to this contract for written correspondence, discussions and other business transactions.

2.13.5. In accordance with Dept. of Defense Instruction (DODI) 1402.5, each individual working with children under the age of 18 in conjunction with this Contract must successfully complete a federally mandated criminal history background check (conducted by the Base Chapel) which will include a National Agency Check w/ Inquiries (CNACI) and an Installation records Check (IRC). Failure to satisfactorily complete the criminal history background check shall result in termination of the Service Agreement.

2.13.6. A résumé shall be included with proposal submissions, past performance reports, work references and all supporting documentation to show that the contractor meets the above requirements.

3. GOVERNMENT/CONTRACTOR FURNISHED PROPERTY

3.1. Government Furnished Supplies and Equipment: All Catholic service supplies and equipment required will be provided by the 502 ABW Chapel. The contractor may be provided government computer access to conduct duties associated with contract performance and to complete monthly reports. This access is dependent on if the contractor completes required training and checks IAW DoD and AF policies. The contractor will be required to use own personal vehicle, and shall not be reimbursed for mileage incurred. The Contractor shall coordinate all support requirements with the chapel designee.

3.2. Government Furnished Building: The Contractor shall be provided the areas and facilities to conduct services. Contractor shall be provided an area for private counseling and to conduct duties associated with contract performance when required.

4. GENERAL AND ADMINISTRATIVE

4.1. RECORDS MANAGEMENT. The Contractor shall manage all data created for Government use or legally controlled by the Government, in support of the functional activity or required by AF publication, IAW with the AF records management procedures in Air Force Instruction (AFI) 33 - 322, Records Management Program, paragraph 10, Air Force Manual (AFMAN) 33-363, paragraph 7.4, Air Force Instruction (AFI) 33-364, and the Air Force Records Disposition Schedule (AF RDS).

4.2. AIR FORCE ANTITERRORISM (AT) PROGRAM

The AT program seeks to deter or limit the effects of terrorist acts against the AF by giving guidance on collecting and disseminating timely threat information, providing training to all AF members, developing comprehensive plans to deter and counter terrorist incidents, allocating funds and personnel and implementing AT measures. DOD contractors and sub-contractor personnel shall be offered Level 1 - AT Awareness Training. AT Awareness Training may be accomplished by either a Level I - qualified instructor, or by completing the "Force Protection" computer based training (CBT) course on the Advanced Distributed Learning System (ADLS) https://golearn.csd.disa.mil/kc/login/login.asp or the Defense Technical Information Center (DTIC) at https://atlevel1.dtic.mil/ at for individuals without access to NIPR, possessing a CAC https://golearn.csd.disa.mil/kc/login/login.asp card, nor enrolled in DEERS. Compliance with the training will be monitored by the contracting officer representatives.

4.3 CONTRACTOR FULL-TIME EQUIVALENT REPORTING.

The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Joint Base San Antonio Chapel via a secure data collection site. The contractor is required to completely fill in all required data fields at https://afcmra.hqda.pentagon.mil/login.aspx.

Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the CMRA help desk.

4.4. EMERGENCY, AFTER NORMAL DUTY HOURS, WEEKENDS, AND HOLIDAYS.

The Contractor may be required to work after normal duty hours, weekends, or holidays to complete all religious support services for Joint Base San Antonio (JBSA). The Contractor will not be paid for any overtime hours. The Contractor shall notify the chapel designee of any work performed after duty hours, weekends, or holiday, for informational purposes only. All such work as described in this paragraph shall not be subject to separate negotiations, but shall be included as part of the basic proposal.

4.5. TRAFFIC LAWS. The Contractor shall comply with all JBSA base traffic rules and regulations at all times while on JBSA installations.

4.6 PHYSICAL SECURITY. The Government will not be responsible in any way for damage to the Contractor’s supplies, materials, vehicles, equipment, and property or to Contractor personnel’s personal belongings that are damaged or destroyed by fire, theft, accident or other disaster, including terrorist attack. Contractor personnel and any representative of the Contractor entering any JBSA Installation shall abide by all security regulations and shall be subject to security checks.

4.7. KEY CONTROL

4.7.1. The Contractor shall be responsible for keys provided to the contractor by the government.

Keys shall not be lost, misplaced, or duplicated by contractor personnel, nor used by unauthorized contractor personnel. The contractor shall develop and implement procedures to insure that keys issued to the contractor by the government are safeguarded.

4.7.2. The contractor shall report any occurrence of duplicated, misplaced, or lost keys to the Contracting Officer within 24 hours after discovery of occurrence, and submit a written report to the Contracting Officer by close of business the next workday. The written report shall provide complete details relating to duplication, misplacement, or loss. In the event a key is duplicated, misplaced, or lost, all locks and keys for that system will be replaced at the discretion of the government. The contractor shall reimburse the government for replacement of locks or rekeying required as a result of keys being duplicated, misplaced, or lost by contractor personnel. The contractor shall also reimburse the government for any government property lost or stolen as a result of unauthorized key duplication, misplaced key, or loss of key by the contractor or contractor personnel.

4.8. TELEPHONE SERVICES. A class “C” telephone line will be provided to the Contractor for local phone calls.

4.9. EMERGENCY MEDICAL TREATMENT. Emergency medical treatment may be provided for the contractor. Contractor personnel are responsible for preparing the necessary paperwork and the Contractor shall also reimburse the Government for the cost of medical treatment and transportation.

4.10. BASE CLOSURE. Work scheduled not accomplished because of base closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening of the military installation.

4.11. INTERFACES. The Contractor shall not unduly interfere with regularly scheduled Government operational activities in the performance of contract requirements. In the event that a Government employee directs the Contractor to stop work, the Contractor shall immediately contact the CO and COR and report the incident, to include the name of the Government personnel involved, date, time, and location.

4.12. SECURITY REQUIREMENTS.

4.12.1. The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on JBSA, TX. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.

4.12.2. During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned subcontractors and for prompt return of credentials and vehicle passes for any subcontractor who no longer requires access to the chapels.

4.13. SAFETY REQUIREMENTS AND REPORTS. The contractor shall perform all work in a safe manner as required by OSHA Safety and Health Requirements and follow all DOD Safety regulations. The Contractor shall provide a verbal and written notification to the CO within twenty four (24) hours of an employee accident resulting in death, injury, occupational disease, etc.

4.14. SAFETY

4.14.1. The contractor shall be knowledgeable of and shall comply with all applicable federal, state, and local laws, permits, Department of Defense (DOD), AF, and base requirements and instructions required for the performance of the duties outlined in this PWS. The contractor shall ensure policies and procedures are established that protect the safety and welfare of customers, employees, and the community to minimize or eliminate any safety or environmental risks. Any safety issues shall be reported immediately to the chapel designee.

4.15. INSPECTION AND ACCEPTANCE. Services performed under this contract are subject to inspection by the chapel designee at the completion of each task. Inspection of the performance of this contract will be conducted by use of a checklist utilizing random sampling, and/or customer feedback. The Contractor shall promptly correct unsatisfactory service at no additional expense to the Government. The office having responsibility for inspection and acceptance is the JBSA Fort Sam Houston Chaplain’s Office.

4.16. FRAUD, WASTE, AND ABUSE. The Contractor shall practice utilities conservation. The Contractor shall be responsible for operating under conditions that prevent the waste of any utilities used. The Contractor shall be responsible for maintaining proper conduct and good discipline within the Contractor occupied work area(s). Contractor personnel shall be encouraged to be alert to report any suspected situations of fraud, waste, and abuse, or other intentionally dishonest conduct against the Government observed during, or in the performance of this contract.

4.17. CONSERVATION OF UTILITIES. Contractor personnel shall practice utilities conservation and shall operate under conditions that preclude waste of Government furnished utilities.

4.18. CONTRACT FILES The Contractor shall maintain complete and accurate files of documentation, records, and reports required under the terms of this contract. The Contractor shall not allow access to the files by any Government agency, non-Governmental agency, or individual agency, or individual unless specifically authorized by the Contracting Officer. Files shall be made available to the Contracting Officer or designated representative upon request. All files will become the property of the Government and shall be turned over the Contracting Officer at the completion or termination of this contract.

4.19. SMOKING. The Contractor shall comply with AFI 40-102 Tobacco Free Living which outlines uniform policies governing use of tobacco products in Government facilities. This also applies to Camp Bullis.

4.20. LAWS. The Contractor shall comply with all local state, and federal laws and regulations that control any work performed. The Contractor shall comply with and keep informed of any changes to these laws and regulations throughout the period of the contract. The Contractor is a recognized professional in their area and is expected to be knowledgeable of all laws and regulations pertaining to their profession.

4.21. PERMITS, LICENSES AND CERTIFICATIONS. Unless otherwise specified in this contract, the Contractor shall obtain all necessary permits, licenses, and certifications; give all required notices; and comply with applicable federal, DOD, Air Force, state, county, and local laws, codes, and regulations in performance of the requirements of this contract. Maintain records of such requirements and make the documentation available to the Government for review.

4.22. CONTRACTING OFFICER AND CONTRACTING OFFICER

REPRESENTATIVE. The CO will appoint a primary and alternate Contracting Officer Representative for management of the day to day activities of the contract. The identity, title, and authority of this representative will be provided in writing to the Contractor after contact award.

4.23. CLEAN-UP/HOUSEKEEPING. The Contractor shall maintain facilities and work sites in a clean, neat, and professional appearance at all times.

4.24. QUARTERLY SCHEDULE The Contractor shall provide the chapel designee with a written schedule which shows the areas being serviced for that quarter. This schedule shall be given to the chapel designee at least fifteen (15) calendar days prior to the quarter the schedule is to begin. When an unforeseen facility closure, inclement weather, or other training event is found to be an acceptable delay by the Contracting Officer, and precludes completion of services on a regularly scheduled work day, if required the Contractor shall reschedule the work to be performed and notify the chapel designee of the new time. The preferred method of notification is e-mail.

4.25. PERIODIC PROGRESS MEETINGS. Quarterly meetings shall be established after contract award. The CO, chapel designee, other Government personnel as appropriate, and the contractor shall periodically meet to discuss the contractor’s performance. The following issues shall be discussed, opportunities to improve the contract, any modifications required of the contract, unsatisfactory inspections and trends against each performance objective observed, positive performance and steps taken by the contractor to prevent unsatisfactory occurrences in the future. The contractor shall provide a summation of unsatisfactory inspections and customer complaints and provide insight into any identified trends. The minutes of these meetings will be reduced to writing, signed by the CO and any other signatures as deemed appropriate, distributed to the functional area and the contractor. Should the contractor not concur with the minutes, the contractor shall provide a written notification to the CO identifying areas of non-concurrence for resolution within 15 days of notification.

4.26. GOVERNMENT LIMITATION OF LIABILITY. The Government shall not be responsible in any way for damage to Contractors' supplies, materials, equipment and contractor personnel's personal belongings that are damaged or destroyed by fire, theft, accident or other disaster.

4.27. CONTRACTOR LIABILITY

4.27.1. The Contractor shall, at own expense, defend any suits, demands, claims, or actions in which the United States might be named as a defendant, arising out of or as the result of the Contractor’s performance of work under this contract, whether or not such suit, demand, claim or action arose out of or was the result of the Contractor’s negligence. This shall not prejudice the right of the United States to appear in such a suit, participate in a defense and take such action as may be necessary to protect the interest of the United States. Nothing in the above provision shall in any way limit other remedies available to the United States as provided by law, or waive rights of the United States as provided by law.

4.27.2. The Contractor is considered an independent contractor, and not an agent of the government. During the performance of these provisions, the contractor will be liable and will hold harmless the government against all actions or claims for damages to persons and property due to the fault, negligence, wrongful act, or wrongful omission of the contractor or their agents.

The government is not responsible for damage to personal supplies or equipment of the contractor.

4.27.3. The Contractor is liable for all damages to government real property, when it has been determined by competent authorities that damages were due to Contractor personnel or sub- Contractors negligence.

4.28. DAMAGE TO GOVERNMENT/PRIVATE PROPERTY. Damages to Government buildings, equipment, or vegetation are addressed in the FAR contract clause 52.237-2

“Protection of Government Buildings, Equipment, and Vegetation.” The Contractor shall be liable for repair/replacement for any Government or private property damaged by Contractor personnel, vehicles, equipment, or negligence. The Contractor shall immediately advise the chapel designee of any damages and provide an action plan for repairs or replacement.

4.29. INDEMNIFICATION: The contractor agrees to indemnify and hold the Government and the Department of the Air Force harmless, whether in tort or in contract, for any and all loss or liability for injury to or death of service provider personnel in transit to, from or during the period of attendance at any of the required locations in support of this production

4.30. DISPUTE PROCESS: Any grievance by the Contractor should be resolved at the lowest level. The reporting levels are as follows: Contracting Officer Representative and then the Contracting Officer.

4.31. CONTRACTOR QUALITY CONTROL. The contractor shall be responsible for quality control for all work accomplished during the performance of the task. The contractor shall provide and maintain a Quality Control Plan (QCP) which ensures the requirements in this PWS are met. The QCP will be considered acceptable and approved unless the contractor is notified by the Contracting Officer (CO) prior to award. The QCP shall be included in the technical proposal for evaluation and acceptance. As a minimum, the contractor shall develop quality control procedures that address the areas identified in the Service Delivery Summary.

4.32. GOVERNMENT QUALITY ASSURANCE. The government will periodically evaluate the contractor’s performance by appointing a contracting officer representative to monitor performance to ensure services are received. The representative will evaluate the contractor’s performance in accordance with the PWS.

4.33. GOVERNMENT REMEDIES. The contracting officer shall follow the requirements of FAR 52.212-4, Contract Terms and Conditions for Commercial Items (Jan 2017), for contractor’s failure to correct nonconforming services.

4.34. HOURS OF OPERATION. Hours of operation will be in accordance with (IAW) the Performance Work Statement (PWS).

4.35. HOLIDAYS OBSERVED. Because of the nature of services being provided, services may be required on a holiday. All federal holidays:

a. New Year's Day, January 1

b. Martin Luther King's birthday, the third Monday in January

c. President's Day, the third Monday in February

d. Memorial Day, the last Monday in May

e. Independence Day, July 4

f. Labor Day, the first Monday in September

g. Columbus Day, the second Monday in October

h. Veteran's Day, November 11

i. Thanksgiving Day, the fourth Thursday in November

j. Christmas Day, December 25

k. Any other day designated by Federal statute, executive order, or Presidential proclamation.

Adverse weather conditions or national emergencies may require the installation to close, for which the government would not pay the contractor additional compensation.

APPENDIX A

PROJECTED SCHEDULE OF SERVICES

A.1 . Services are held in any of the facilities on JBSA, the contractor is expected to provide their own transportation to and from services. Contractor will normally be notified several days in advance of a projected move and/or time change.

A.1.1. Schedule shall be provided to the contractor by the chapel for schedule of services on a quarterly basis.

JBSA-Randolph Contractor Duties and Schedule

JBSA-Randolph Chapel, Building 102 Religious Activities Center (RAC), Building 975 JBSA-Randolph, TX 78150

A.1.3. Contractor maintains a minimum 30-hour work week to complete responsibilities IAW PWS. The 30-hour work week includes but not limited to Masses, Holy Days, Sacraments, Religious Education engagement/oversight, training, office hours, retreats and any administrative duties necessary to complete duties in this PWS.

-Base Year: 30 hours per week.

-Option Years: May be reduced to 20 to 25 hours per week

*Reduction in hours would also include a reduction in services provided (i.e. weekend services, weekday mass, meetings, parish visits). Specific reductions would be outlined in the modification (example is also in the below chart).

A.1.3.1. A primary Catholic Priest will perform at least 75% of the services.

A.1.3.2. Oversee planning for weekly Catholic services and programs.

A.1.3.3. Contractor may attend chapel staff meetings if the meeting takes place during scheduled office hours and is coordinated with the COR or designee; date and time of meeting is subject to change.

A.1.4. Administrative duties may include:

A.1.4.1. Complete required sacramental paperwork and submit to appropriate church of record.

A.1.4.2. Provide written weekly activity report to the JBSA Randolph Deputy Joint Base Chaplain; this can be done via email with prior coordination.

A.1.4.3. Attend Catholic Parish Advisory Council (PAC) meetings and shall provide pastoral guidance; meetings take place monthly but are subject to change.

A.1.4.4. Provide Catholic specific sacramental counseling to include but not limited to marriage prep, marital counseling, child and adult Catholic religious education concerns.

A.1.5. Religious Education duties may include:

A.1.5.1. Oversight of sacramental preparation within the religious education program; coordinate support (i.e. books, retreats, supplies) with the Director of Religious Education and senior JBSA AF Catholic Chaplain.

A.1.5.2. Weekly Parish Visits (pastoral visit) -Visits limited to the Catholic youth group, Catholic Men and Catholic Women groups/Bible studies and the Religious Education (grade/middle school) throughout the year; to also include visits with RCIA (Adult Education).

A.2 Definitions

A.2.1. Weekday Mass - Weekday services are normally held Monday-Friday.

A.2.1.1. At JBSA-RND daily mass is only conducted on Friday.

-Estimated time 45 minutes A.2.2. Weekend Mass - Weekend services are held Saturday and Sunday. If any Baptisms are required they shall be done as part of the Sunday Mass.

-Estimated time 60 minutes

A.2.3. Holy Day Mass/Special Service - Celebrate the Mass according to the current Roman Catholic Rite for the specific Holy Day being celebrated.

A.2.3.1. Holy Day Masses -Solemnity of Mary, the Mother of God -The Ascension of Our Lord -The Assumption of the Blessed Virgin Mary -All Saints Day -The Solemnity of the Immaculate Conception -Christmas

A.2.3.2. Special Service-may include but not limited:

-Ash Wednesday -Holy Thursday

- Good Friday

A.2.4. Seasonal Penance Services during Advent and Lent- Perform the Sacrament of Reconciliation as part of schedule Communal Penance Services.

A.2.4.1. The JBSA-RND Catholic Priest contractor will coordinate additional priest support with the designated JBSA Catholic chaplain.

-Estimated time 90 minutes

A.2.5. Sacrament of Reconciliation/Confessions – Perform the Sacrament of Reconciliation, which is usually performed in conjunction with the designated weekend mass. To also perform the sacrament for 1st Communion candidates, this shall be known as 1st Reconciliation.

A.2.6. Sacraments of Initiation – Perform the below religious ceremonies in accordance with Catholic doctrine.

-Confirmation -1st Communion

-Estimated time 90 minutes

A.2.7. Sacramental Rehearsals – Time required prior to the sacrament or mass to go through the ritual and sacrament with the participants. The below services should require a rehearsal:

-Confirmation -1st Communion -Catechetical Sunday -Easter Vigil

-Estimated time 60 minutes

A.2.8. Training of Lay Personnel – Training of altar servers, Lay Eucharistic Ministers, and Lectors.

A.2.9. Funeral Vigil – Vigil prayer services on the eve prior to the funeral mass.

A.2.10. Funeral Mass – Celebrate the Funeral liturgy for Active Duty and their immediate dependents.

A.2.11. Graveside Services – Celebrate the Graveside liturgy for Active Duty and their dependents.

A.2.12. Emergency Sacrament – To include but not limited to Anointing of the Sick and Confession - In the absence of the assigned Catholic Priest, conduct sacraments with Active Duty personal and their dependents requesting to see a Catholic priest to perform emergency ministrations such as Anointing of the Sick or Confession.

A.2.13. Wedding Ceremony – (JBSA-RND) Celebrate Wedding Ceremony/Con-Validation for Active Duty and their dependents.

ARCHDIOCESE FOR THE MILITARY SERVICES, USA (AMS)

APPLICATION PROCESS

Step 1 Must complete an application for the Archdiocese for the Military Services, USA (AMS) endorsement and faculties.

- Priest Application - Application Narrative

Step 2 Obtain and submit to the Archdiocese for the Military Services, USA (AMS) a letter stating:

a. Permission from the Diocesan Ordinary or religious superior

b. Dallas Charter statement

c. Proof of Youth Protection Training.

d. A letter of approval from the designated Catholic Chaplain at Joint Base

San Antonio, TX.

Step 3 After Archdiocese for the Military Services, USA (AMS) faculties are granted, a

DD2088 must be sent to 502 ABW/HC Org email. In the subject line of the email type: “For the NCOIC Resource Management”.
