1 Performance Work Statement (PWS) PEST and RODENT CONTROL SERVICES

KEESLER AIR FORCE BASE (KAFB) MEDICAL CENTER

1.1 SCOPE: The contractor shall provide all labor, supplies, equipment, superintendence, and transportation necessary for the accomplishment of complete insect, pest and rodent control and preventive maintenance to ensure non-recurrence of rodent and pest infestation in Bldg. 00468, 81st Medical Group, KAFB, MS. The regular hours the medical center is open are from Monday through Friday, 0700 to 1700. The contractor shall execute their preventative maintenance plan and perform “as-required” pest control services during these regular hours as requested by the Medical Group Facility Management Office. The only exception to daylight hours will be the dining hall spraying. This work must be accomplished after regular hours (times to be specified and coordinated with the MDG Facility Management Office). The contractor is required to treat and set mechanical traps to kill pests inside the hospital and its surrounding perimeter. The contractor shall treat rooms and building using products that meet guidelines of federal, state, and local laws. The products that the contractor uses for the interior of the facility must be labeled for use in a hospital; and the contractor shall be responsible for finding and submitting an acceptable substitute if necessary. The guidance for products being labeled for hospital use can be found in the Armed Forces Pest Management Board Technical Guide (AFPM TG20). Information on currently registered products may be obtained from the DoD Pesticide Hotline at (410) 436-3773. The contractor must have and be current on all required state and federal licenses.

a. The work to be executed consists of the following, at locations shown in section 2.3:

(1) Treat/bait for roaches, flying insects, to include larvae, bedbugs, lice, fleas, and other small pests as required.

(2) A proactive treatment program for ants, to include crazy and fire ants that treats the entire exterior of the facility.

Bait interior as required.

(3) A proactive treatment program for mice and rats. Approximately 35 bait boxes will be required for exterior of facility. At a minimum, 20 glue boards will be required for the dining facility serving area. The contractor is responsible for rebaiting boxes/replacing glue boards as required, checking their traps and bait boxes at least monthly, and disposing of pests throughout facility, as required; see section 2.3 for locations.

(4) Proactive treatment for pests in sub-basement. This is a confined space. Confined space certification is required.

(5) Treat perimeter and grounds of Bldg. 00468 for pests and/or rodents.

1.2 SAFETY AND FIRE PREVENTION:

a. The contractor shall be responsible for instructing their employees in appropriate safety measures as specified in base hospital regulations. The contractor shall be required to interrupt their work at any time to allow passage of hospital patients, personnel, equipment, or carts.

b. Flammable materials, equipment, tools, or debris shall not be stored or allowed to remain in or about hospital buildings. All chemical products shall not be stored or remain in hospital buildings unless accepted by Functional Requirements Evaluator Designees (FRED), or authorized representative, after authorization by the installation hazardous material office. Flammable liquids must be stored in UL approved container.

c. The contractor shall at all times provide adequate protection to safeguard persons passing through and around the premises and protect the buildings, grounds, walks, roadways, and vehicles from damage.

d. All contractor personnel working on KAFB shall be subject to the security and safety regulations in existence at KAFB and shall abide by these regulations in every respect.

1.3 COORDINATION OF ACTIVITIES: The contractor shall check in at the Medical Center Facilities Management section before starting work every visit. After completion of work, the Medical Group Facility Manager, or designated representative, will be notified to inspect the work. If completion is after normal duty hours, these actions will be completed at the start of the next normal duty day. The contractor for pest control shall be required to sign in and out with Facility Management, Room 5A144.

1.4 MATERIALS AND EQUIPMENT: The contractor shall furnish all treatments, tools, and equipment necessary to maintain all areas of the Medical Group in regards to pest control.

1.5 MEASUREMENTS: The contractor is responsible for verifying all measurements at the site and shall be responsible for corrections of same.

1.6 PERSONNEL:

a. The contractor shall provide a contract manager who shall be responsible for the performance of the work. The name of this person and an alternate who can act for the contractor when the manager is absent shall be designated in writing to the Contracting Officer (CO). The contract manager shall be qualified in pest control services.

b. The contract manager, or alternate, shall have full authority to act for the contractor on all contract matters relating to daily operations of their contract.

c. The contract manager or alternate shall be available during normal duty hours to meet on the installation with government personnel designated by the CO to discuss problem areas.

d. The contract manager and alternate must be able to read, write, speak, and understand English.

e. Contractor personnel shall present a neat appearance and by easily recognized. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges which contain the company name and employee name.

f. The government will provide the contractor personnel (authorized to work on the installation) with appropriate pass and identification items. An application for these documents will be provided to the contractor at the performance conference. The contractor will escort their personnel to the Pass and Registration Office for final issuance for appropriate passes.

g. The contractor shall not enter the hospital building except on official business to treat or evaluate problem areas.

If after hours, in the dining room, an individual from Food Services will be available for supervision.

2. EXECUTION:

2.1 At a minimum the entire facility shall be serviced weekly. For the purpose of this PWS, "as-required" is defined as a telephone call placed to the contractor by the Medical Group Facility Manager or authorized representative, or a work order presented to the technician during weekly visit.

2.2 The contractor shall begin service not later than 4-hours for emergency request (i.e. animal in trap, bedbugs, lice, fleas, flies in interior of facility), 8-hours for an urgent request (i.e. live animal in ceiling), and 24 hours for a routine request, after receipt of a call from the Medical Group Facility Manager or authorized representative.

2.3. The contractor shall service the entire facility, to include the dining facility, eaves, mechanical rooms (both interior and exterior), mechanical penthouses, patient drop-off overhangs, ambulance bays, interior and exterior stairs and ramps, atriums, loading docks, facility 468 grounds, smoking areas, and planters/beds.

2.4 The contractor shall provide the Facility Manager, or an authorized representative, @ 81 MDSS/SGSMF PHONE 376-5100, with labels and Safety Data Sheet (SDS) for all contract pesticides/treatment materials within 10 days from contract start date. Provide Base Entomology Shop with the list of pesticides/treatments to be used on the contract. Any request during the contract period to use nonstandard pesticides/treatments (those not in the IPMIS) will be forwarded to the MAJCOM PMC for approval prior to use. All treatments and pesticides used on Keesler AFB under this contract will be pre-approved and authorized by the installation environmental compliance office (377-3004/377-1262) located inside the southeast entrance of building 4705.

2.5 The contractor shall provide data on daily pesticide/treatment use to include name of each pesticide/treatment used, EPA # of pesticide/treatment used and amount of pesticide/treatment used, to 81 MDSS/SGSMF monthly and the base Entomology Office by the FIRST of each month for the previous month. This information should have the Date, Size of the treated area, Target Pest, Amount of the Product Concentrate Mixed, Dilution Rate Percent, Employee Name and Mississippi License Number. Entomology Shop will need a copy of the Mississippi Professional services license and a copy of employees Mississippi Commercial Pesticide Certification.

Data should be emailed to the Entomology Shop Supervisor for the Installation Maintenance Contractor, Call 377- 3317 for email address.
