The following have been modified:

ADDENDUMS

ADDENDUM TO FAR 52.212-1 INSTRUCTION TO OFFERORS—COMMERCIAL ITEMS. The following paragraphs of this provision are tailored as follows:

(b) Submission of Offers: All pages requiring fill-in information must be returned with the offeror’s proposal and must be received by the date/time provided in Solicitation FA301018R0027, unless otherwise changed via solicitation amendment. Solicitation amendments must be acknowledged in writing or the proposal may be considered non-responsive. It is the offeror’s responsibility to check fbo.gov for amendments issued against this solicitaion and respond accordingly.

Contractors shall submit questions pertaining to the solicitation package to the CA in no later than 3 Jul 18, 12:00 PM Central.

(b)(6) Price

The Offeror shall submit proposed pricing for all contract line item numbers identified in the solicitation price schedule.

(b)(10) Past Performance Information (PPI)

a. Provide a list of no more than three (3), of the most relevant contracts performed for Federal agencies or commercial customers within the last three (3) years.

Recent contracts are defined as those on which some portion of performance occurred during the last three (3) years from date of receipt of proposal. Relevant performance includes present/past performance effort involving similar scope and magnitude of effort of this solicitation.

The offeror shall submit Attachment 3, Past Performance List of References, with your proposal for contracts that you consider relevant. The offeror shall ensure that the references listed in Attachment 3 are provided the Past Performance Questionnaire (PPQ), Attachment 4 for completion. If a CPAR is available for your references, annotate on Attachment 3 and no PPQ will be required.

Questionnaires shall be sent by the reference completing them directly to the Government Contract Specialist, Katie Fulford via e-mail katie.fulford.1@us.af.mil. This office strongly recommends all offerors request their references to submit their past performance questionnaires immediately to the Contract Specialist listed above.

b. The evaluation of PPI will take into account past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to this solicitation. The type of past performance information (PPI) identified under Attachment 3 will determine which of the remaining paragraphs apply.

(1) For PPI where the offeror was the prime contractor; comply with paragraph c.

(2) For PPI where the offeror was a subcontractor; comply with paragraphs c. & d.

(3) For PPI where the offeror is using a subcontractor’s PPI; comply with paragraph c. & e.

(4) For PPI where the offeror is part of a teaming arrangement; comply with paragraph f.

(5) For PPI where the offeror key personnel; comply with paragraph g.

c. Furnish the following information for each contract listed:

(i) CompanyName

(ii) ContractNumber

(iii) Description of work performed

(iv) Period ofPerformance

(v) Contracting Agency POC/Customer, Verified, up-to-date name, telephone number and email

(ii) What percentage of the total contract value/annual value did the offeror perform on the Atch 3 contract?

(iii) What was the value of the subcontract for each year and total amount on the Atch 3 contract?

e. If using PPI from subcontractors as part of list in Attachment 3, in addition to information requested in paragraph

b. above, furnish the following information:

(i) What function(s)/task(s) did the subcontractor perform on the Atch 3 contract?

(ii) What function(s)/task(s) will the subcontractor perform on this contract?

(iii) What percentage of the total contract value/annual value did the subcontractor perform on the Atch 3 contract?

(iv) What percentage of the total contract value/annual value will the subcontractor perform on this contract?

f. If a teaming arrangement or JV is proposed, the offerors shall provide complete information as to the arrangement, including any relevant and recent past/present performance information on previous teaming or JVs with same partner. In addition to information requested in paragraph (c) above for each team/JV member, furnish the following information:

(i) What function(s)/task(s) will each team or JV member perform on this contract?

(ii) What approximate percentage of the total contract value/annual value will each team or JV member perform on this contract?

g. If using PPI from key personnel as part of list in Attachment 3, in addition to information requested in paragraph

c. above, furnish the following information:

(i) What function(s)/task(s) did the key personnel perform on the Atch 3 contract?

(ii) What function(s)/task(s) would the key personnel perform on this contract?

(iii) What percentage of the total contract value/annual value did the key personnel perform on the Atch 3 contract?

(iv) What percentage of the total contract value/annual value will the key personnel perform

(c) Period of Acceptance of offers. The offeror agrees to hold the prices in its proposal firm for 90 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation.

(e) Deleted.

Reference clause 52.228-5, INSURANCE - WORK ON A GOVERNMENT INSTALLATION The Contractor shall, at its own expense, procure and thereafter maintain the following kinds of insurance with respect to performance under the contract.

(a) Workers’ compensation and employer’s liability. Contractors are required to comply with applicable Federal and State workers’ compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer’s liability section of the insurance policy, except when contract operations are so commingled with a contractor’s commercial operations that it would not be practical to require this coverage. Employer’s liability coverage of at least $100,000 shall be required, except in States with exclusive or monopolistic funds that do not permit workers’ compensation to be written by private carriers. (See 28.305(c) for treatment of contracts subject to the Defense Base Act.)

(b) General liability.

(1) The contracting officer shall require bodily injury liability insurance coverage written on the comprehensive form of policy of at least $500,000 per occurrence.

(2) Property damage liability insurance shall be required only in special circumstances as determined by the agency.

(c) Automobile liability. The contracting officer shall require automobile liability insurance written on the comprehensive form of policy. The policy shall provide for bodily injury and property damage liability covering the operation of all automobiles used in connection with performing the contract. Policies covering automobiles operated in the United States shall provide coverage of at least $200,000 per person and $500,000 per occurrence for bodily injury and $20,000 per occurrence for property damage. The amount of liability coverage on other policies shall be commensurate with any legal requirements of the locality and sufficient to meet normal and customary claims.

(d) Aircraft public and passenger liability. When aircraft are used in connection with performing the contract, the contracting officer shall require aircraft public and passenger liability insurance. Coverage shall be at least $200,000 per person and $500,000 per occurrence for bodily injury, other than passenger liability, and $200,000 per occurrence for property damage. Coverage for passenger liability bodily injury shall be at least $200,000 multiplied by the number of seats or passengers, whichever is greater.

(e) Vessel liability. When contract performance involves use of vessels, the contracting officer shall require, as determined by the agency, vessel collision liability and protection and indemnity liability insurance

REFERENCE CLAUSE 52.237-1, SITE VISIT

A site visit will be held Thursday, 28 Jun 18, 9:00 AM CST. All interested offerors are encouraged to inspect the site prior to submitting a proposal. If you plan on attending the site visit, please email both Kimberley Alvarez at kimberley.alvarez.1@us.af.mil and Katie Fulford at katie.fulford.1@us.af.mil no later than 1200 PM CST on 26 Jun 18 to confirm attendance. Further reporting instructions will be provided upon confirmation. Each vendor is limited to two attendees. The number of attendees, to include name/phone/company name, should be included in the confirmation. Every effort will be made to arrange government transportation to/from the site visit; however, if transportation cannot be arranged or is cancelled in the event of mission requirements, please be prepared to obtain a base pass by bringing required documentation necessary to gain access. Required documentation is all of the following: a valid driver’s license, current vehicle registration, proof of insurance and social security card must be provided for each vehicle. All individuals entering the installation must have picture identification. Technical and Contracting personnel will attend the site visit and all questions/answers received will be posted to www.fbo.gov.

PERFORMANCE CONFERENCE

Offerors are hereby advised if they are awarded a contract as a result of this solicitation, they may be required to appear at the 81st Contracting Squadron, 310 M Street, Bldg 4605, Keesler AFB, MS, to attend a Preperformance Conference prior to commencement of any work on the military installation.

NOTICE TO OFFERORS: Funds are not presently available for this effort. No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date for receipt of proposals. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs
