SUBJECT: Past Performance Questionnaire for Solicitation Number FA3002-13-R-0017

FOR OFFICIAL USE ONLY

The Air Education and Training Command (AETC) Contracting Squadron (CONS) is in the process of selecting a contractor for the “Fixed Wing Air Advisor Training requirement” to include Cessna T182T, Cessna 208 Pilot & Maintenance and King Air 350 Avionics & Maintenance Training. This requirement consists of ground, flight, maintenance and avionics training. In addition, this requirement contains syllabi development.

One of the considerations in proposal evaluation is the verification of the offerors' past and present performance on contracts, which reflect the offeror's ability to perform on the proposed effort. We have referenced your contract number ______________________ with our company as a candidate for providing past/present performance information. AETC CONS is depending on information received from agencies and companies such as yours, which have had first-hand experience with our company, for the evaluation of our performance on the identified contract.

The areas of interest regarding our performance are summarized in the enclosed questionnaire. We sincerely request that you please complete the questionnaire and send it directly to the Contracting Officer (Mr. Robert Miller) for the Fixed Wing Air Advisor Training solicitation by the date set for receipt of proposals, 30 May 13. This schedule will allow AETC CONS sufficient time to analyze the data prior to the start of discussions, if held.

To assist you in preparing your response and expediting your reply, the questionnaire may be filled out either electronically or by hand, and either e-mailed or faxed to the attention of Robert Miller at robert.miller.17@us.af.mil (210) 652-2304 (DSN 487-2304). Please call Mr. Miller at (210)-652-6487 (DSN 487-6487) prior to a fax transmission, if you have any questions, or if receipt of your questionnaire is not acknowledged. Your completed questionnaire will become a part of the official Source Selection records.

Your help is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Source Selection. Thank you.
