STATEMENT OF WORK
Precision Laser Specialist – Laser Equipment Maintenance 11th Medical Group (MDG) Joint Base Andrews-NAF Washington, Maryland
1.0 DESCRIPTION OF SERVICES:
1.1. PRECISION LASER SPECIALIST
1.1.1. Contractor shall provide scheduled preventive maintenance inspections (PMIs), calibration, on-call corrective maintenance and as-needed parts and service for five (5) Laser Systems; Coherent Omni DF, Coherent Aura, Convergent Odyssey 30, Lumenis Ultrapulse Encore and Lumenis Selecta Duet. The period of performance is 01 October 2018 through 30 September 2019 with four one-year options.
1.1.2. Service shall include all travel, labor, parts, materials (excluding user consumable supplies), manufacturer’s authorized field modification instructions (FMIs), individual tools and test equipment required to service and maintain the equipment described in paragraph 1.1.1. All service must be performed onsite, within Malcom Grow Medical Clinics and Surgery Center, at Joint Base Andrews-NAF Washington.
1.1.3. Contractor shall provide all software updates, to this equipment, to include field modifications, safety corrections, revisions designed to correct software deficiencies and equipment modifications normally furnished by the manufacturer. Upgrades and modifications will be furnished at no additional cost under the contract and installed during the next PMI after the manufacturer makes them available. These software updates, upgrades and modifications will not include those designed primarily as performance enhancements or performance upgrades.
1.1.4. Contractor shall ensure that the equipment functions in conformance with the latest published editions of all applicable regulatory guidance i.e.: NFPA, OSHA, CAP, TJC and the FDA Department of Health and Human Services; the manufacturer's maintenance specifications as listed in the technical literature; and the quality control policies and procedures governed by the local activity.
1.1.5. The 11th Medical Group (11 MDG) reserves the right to add or delete items and or their components from this contract on as needed basis to continue to provide high quality healthcare. The contractor will be duly notified and any adjustments required will be agreed upon by both parties but not limited to: price, PMI schedule, etc.
1.1.6. The end customer for this effort is the Malcolm Grow Medical Clinics and Surgery Center, 1060 W. Perimeter Rd, Joint Base Andrews-NAF Washington AFB, MD 20762. The performance period is 12 months from the date of award plus 4 option years.
1.1.7. The contractor shall submit reports and documentation as requested by the QAP.
1.2 PREVENTIVE MAINTENANCE INSPECTIONS (PMIs):
1.2.1 PMI’s are the care and servicing of the equipment (in accordance with manufacturer’s maintenance specifications) for the purpose of retaining the equipment in a serviceable condition. This inspection includes checking for proper operation, detection and correction of incipient failures either before they occur or before they develop into major defects; and includes, but is not limited to actions pertaining to periodic technical inspections, lubrications, adjustments, calibration, interior cleaning, and replacement of worn or deteriorated parts. During preventive maintenance and/or calibration the contractor shall complete a Vendors Verification/Certification work sheet provided by the contractor confirming preventive maintenance checks and calibration accuracy. After calibration, the Contractor shall attach a Verification/Certification Label, provided by either the contractor or the Medical Equipment Repair Center (MERC) that includes date work was completed and the date next scheduled maintenance is due.
1.2.2. PMI’s will be coordinated and scheduled through the Medical Equipment Repair Center (MERC).
1.3 ON-CALL CORRECTIVE MAINTENANCE:
1.3.1.On-call corrective maintenance is the service required to restore equipment to a condition within the manufacturer’s specifications. It includes all service except that which is required as a result of damage to the equipment by accident, neglect, abuse, power surge, power failure, or operation in an environment not compatible with the equipment’s published specifications, unless such is caused by the contractor. Parts and or service not provided by the contractor for the above reasons will only be provided upon the approval of the Contracting Officer or his designated representative.
1.3.2.The contractor will provide 24/7 technical phone support.
1.3.3.The contractor will provide all tools, parts, and labor needed for emergency maintenance and incur all associated costs. Emergency service shall be available within 24 hours following contractor notification of a malfunction. The contractor will respond by telephone to emergency calls within two hours of notification. The contractor will respond on-site to emergency service calls within 24 hours of notification and within two days of notification for routine service calls.
1.3.5. Contractor’s service technician shall continue uninterrupted on all services being performed until the system is 100% operational according to the manufacturer’s specifications, i.e. shall not discontinue work in order to service another customer.
1.3.6. Contractor’s service plan shall demonstrate their ability to provide parts to meet the repair time frame specified in this contract and the orderly elevation of problems beyond the capability of their maintenance personnel. Non-availability of required repair parts shall not be an excusable delay for completing the repair.

1.3.3

3.0. POINT OF CONTACT. The contractor shall provide a point of contact that shall be responsible for the performance of the work. The point of contact shall have full authority to act for the contractor on all matters relating to the daily operation of this contract. The contactor shall designate this individual, in writing, to the Contracting Officer before the contact start date.
3.1. AVAILABILITY. The contactor shall provide a point of contact that shall be available via telephone 24 hours a day seven days a week.
3.2. ARRANGE FOR REPLACEMENT STAFF. The contractor shall have adequate replacement contract employees available in such instances so a lapse in coverage does not occur after 40 consecutive hours. Substitutes shall be as qualified as the primary contract employee.
3.3. CONTINUITY OF SERVICES. The contractor shall ensure qualified backup service is available to provide coverage during scheduled and unscheduled absences to primary service after 40 consecutive hours.
4.0. GENERAL AND ADMINISTRATIVE INFORMATION
4.1. HOURS AND LOCATION OF OPERATIONS. The contractor shall provide service 24 hours per day, 7 days a week, to include emergency and routine maintenance service required to perform clinical laser application. PMI's shall be performed during normal duty hours; (normal duty hours are 8:00 a.m. to 5:00 p.m., five days per week, Monday through Friday coverage, excluding U.S. Holidays).
4.2. DESIGNATED REPRESENTATIVES: The names of the QAP and personnel authorized to place service calls shall be provided to the contractor at the time of contract award. Changes to designated QAP and personnel authorized to place service calls must be submitted/approved by the Medical Equipment Repair Center (MERC) Section Chief.
4.3. ADMINISTRATIVE CONTROL: Contractor's service personnel shall report to the MERC, Building 1050, room CB-7, Joint Base Andrews-NAF Washington AFB, MD prior to reporting to the work-site.
4.4. SERVICE DOCUMENTATION: Contractor's service technician shall prepare a service report for each service and obtain the signature of a designated QAP within the using activity. The service report must be received within five working days. The service report shall include the following minimum information:

| (8) | Name of Service Technician. |
| (9) | Signature of individual acknowledging services. |
| (10) | Physically attach a contractor's or MERC's completed verification/calibration sticker to the unit after completion. |
4.5. SECURITY REQUIREMENTS.
4.5.1. SECURITY REQUIREMENTS. All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the government installation, shall abide by all security regulations of the installation. The contractor shall sign an agreement stipulating the security requirements of this contract.
4.5.2. PHYSICAL SECURITY. The contractor shall be responsible for safeguarding all government property provided for contractor use. At the close of each work period, all government facilities, equipment and materials shall be secured. The contractor is responsible for safeguarding all government property provided for contractor use.
4.5.3. CONDUCT. The contractor shall be responsible for the actions of any of their employees who are working at any location as required by this contract.
4.5.4. WEAPONS. Weapons of any sort are not authorized on JBANAFW, this includes, pistols, shotguns, assault rifles, rifles, knives, swords, nunchuck, brass knuckles, or any other instrument designed with the sole purpose to injure or maim. Combination tools such as Leatherman and small pocket knives with folding blades less than four inches are authorized if used in the performance of duty. If there are any questions concerning an authorization of an item, the contractor shall verify with the Non-Commissioned Officer in Charge at the JBANAFW Visitor Control Center prior to transporting the item on the installation. Those found in violation of directives are subject to apprehension, criminal charges and debarment from the installation.
4.5.5. ILLEGAL/CONTROLLED SUBSTANCE. The U.S. Federal government is the most vocal opponent of the drug industry and it has set the de facto international standards regarding the legality and illegality of different drugs. Illegal controlled substance and narcotics are not authorized on JBANAFW what so ever, controlled substances may not be transported on the installation via vehicle or on an individual person, i.e., marijuana/Cannabis, cocaine, crack cocaine, LSD, Methamphetamine (Meth), Heroin, Anabolic steroids, Psilocybin mushrooms, are some examples of forbidden substances (this list is not all inclusive). Prescription medications are authorized for the person the medication is issued to; however, the medication must not be expired and the label must clearly indicate the name of the person the medication is issued to. Otherwise, the prescription medication may be deemed illegal by law enforcement officials. Those found in violation of directives are subject to apprehension, criminal charges and debarment from the installation.
4.5.6. BASE PASSES. A base identification card/vehicle pass shall be obtained for all employees including any subcontractor. It is the intent of the government that each employee has possession of their pass at all times while working on this contract. The contractor shall be responsible for the return of any pass when employment and/or the contract is terminated. Failure to return all passes will result in non-payment of the final invoice. Lost or stolen passes shall be reported immediately to the contracting officer with a written follow up to the contracting officer within 72 hours.
5.0 QUALITY ASSURANCE: The government shall evaluate the contractor’s performance by appointing a representative(s) to monitor performance to ensure services are received. The government representative will evaluate the contractor’s performance through constant review of lasers performance following all maintenance and/or repairs.
